Hypokalemic periodic paralysis (HPP) is a rare but significant manifestation of Sjögren's syndrome, often associated with distal renal tubular acidosis (dRTA). We report a case of a 60-year-old man with recurrent HPP due to severe hypokalemia. Despite the absence of sicca symptoms, serological testing and glandular function assessment confirmed the diagnosis of Sjögren's syndrome. Laboratory findings revealed marked hypokalemia, metabolic acidosis, and excessive urinary potassium excretion, consistent with dRTA. Renal biopsy showed a combination of hypokalemia-induced tubular damage and autoim

Andersen-Tawil syndrome (ATS) is an ion channelopathy with variable penetrance for the triad of periodic paralysis, arrhythmia, and dysmorphia. Dominant-negative mutations of KCNJ2 encoding the Kir2.1 potassium channel subunit are found in 60% of ATS families. As with most channelopathies, episodic attacks in ATS are frequently triggered by environmental stresses: exercise for periodic paralysis or stress with adrenergic stimulation for arrhythmia. Fluctuations in K+, either low or high, are potent triggers for attacks of weakness in other variants of periodic paralysis (hypokalemic periodic p

Quetiapine-induced hypokalemic periodic paralysis (QIHPP) is a rare condition. Herein, we present the case of a 31-year-old pregnant Malay woman diagnosed with bipolar II disorder and QIHPP. She presented to the casualty department with a 2-day history of bilateral lower limb weakness and numbness. Her renal function tests showed moderate hypokalemia (2.5 mmol/L), whereas other investigations were normal. Quetiapine was suspected to be the cause, prompting a psychiatric referral to manage her acute condition. Balancing the risks of untreated QIHPP against the potential relapse of bipolar sympt

BACKGROUND: Hyperkalemic Periodic Paralysis (hyperPP) is an autosomal dominant genetic disorder where high extracellular potassium is associated with skeletal muscle depolarization and both flaccid muscle weakness as well as delayed muscle relaxation that can lead to myotonia and myopathy. Interventions have typically relied on avoidance of triggers, low potassium diets, and diuretics like acetazolamide and diclofenamide with limited success.
CASE PRESENTATION: The patient is a 48 year old man with hyperPP from a familial autosomal dominant sodium channel point mutation in the SCN4A gene at p

Hypokalemic periodic paralysis is a hereditary or acquired temporary flaccid paralysis of skeletal muscles, affecting proximal musculature more than distal, as a result of hypokalemia. In this report, we describe a unique case of amphetamine-induced hypokalemic periodic paralysis that has only been described twice in the current literature. A 31-year-old male with a history of substance abuse presented with altered mental status and acute tetraparesis after inhalation of methamphetamine. Labs were significant for marked hypokalemia (1.8 mmol/L), elevated creatine kinase and a positive urine dr

BACKGROUND: Hypokalemic Periodic Paralysis (HypoPP) is a rare genetic neuromuscular disorder characterized by attacks of skeletal muscle weakness or paralysis with spontaneous recovery. The attacks are frequently triggered by specific factors, and previous studies have identified certain associated lifestyle factors, such as dietary intake and physical activity. However, there is currently no in-dept knowledge on how patients experience and self-manage triggering factors in their everyday life.
OBJECTIVE: In this study, we aimed to explore patients' experiences with dietary intake and physica

ObjectiveThere is crosstalk between the thyroid hormone and the renin-angiotensin-aldosterone system, but the clinical application is lacking. Our study aimed to verify the correlation between renin and other thyroid-related biomarkers and their association with treatment prognosis.MethodsThis retrospective study recruited eighty consecutive patients treated for hyperthyroidism in a medical center between 2021 and 2023. Among participants, 53 were newly diagnosed, and 27 were recurrent cases. Laboratory measurements, including renin, aldosterone, and other thyroid-related biomarkers, were asse

Thyrotoxic periodic paralysis (TPP) is a rare complication of hyperthyroidism that manifests as recurrent episodes of flaccid paralysis, hypokalemia, and thyrotoxicosis. It can occur in patients with or without a prior diagnosis of hyperthyroidism, and its diagnosis is relatively straightforward once low serum potassium levels and elevated thyroid hormones are evident. However, due to the rarity of the disease, it is seldom the primary diagnosis, and a differential diagnosis must be made with other neurological diseases that resemble the paralysis picture, including Guillain-Barre syndrome. Background: Thyrotoxic periodic paralysis (TPP) is a rare, life-threatening, reversible condition, in which episodic muscle weakness occurs due to low serum potassium in the setting of thyrotoxicosis. In addition to treating thyrotoxicosis, prompt restoration of potassium levels is essential while monitoring for rebound hyperkalemia. Purpose: To emphasize the importance of recognizing and appropriately managing TPP, with a focus on the potential dangers of potassium overcorrection. Case Report: We present a patient with TPP whose potassium replacement therapy caused dangerous hyperkalemia, req

We present the case of a 23-year-old male with no past medical history who presented to the emergency department via emergency medical services (EMS) with the complaint of weakness starting in his legs that progressed to generalized weakness, inability to move the bilateral lower extremities, and body aches. Initial concerns were for Guillain-Barré or atickborne illness. During his workup the patient spontaneously developed ventricular tachycardia that quickly degenerated into ventricular fibrillation, and a prolonged resuscitation effort ensued. After over an hour of CPR sustained ROSC was ob

The perioperative management of patients with hypokalemic periodic paralysis is challenging due to the risk of postoperative muscle weakness. We describe a patient with hypokalemic periodic paralysis who presented for wisdom teeth extraction. Our perioperative goals included minimizing exposure to weakness triggers such as hypokalemia, pain, anxiety, hypothermia, and neuromuscular blockade. The patient tolerated the procedure under deep sedation with intravenous agents, local anesthesia, and a natural airway with spontaneous ventilation. He experienced mild upper extremity weakness postoperati

BACKGROUND: Hypokalemic periodic paralysis (HPP) is characterized by transient episodes of skeletal muscle weakness due to hypokalemia. It can arise from primary or secondary causes, with renal tubular acidosis (RTA) being a notable secondary etiology. Here, we present a case of HPP associated with RTA in a patient with an underlying autoimmune disorder, Sjögren syndrome.
CASE DESCRIPTION: A 29-year-old female presented with dry eyes and progressive weakness in both upper and lower limbs, leading to immobilization. Clinical examination revealed hypokalemia, normal anion gap metabolic acidosis

Thyrotoxic periodic paralysis (TPP) is a potentially life-threatening complication of hyperthyroidism. It is characterized by hypokalemia-induced muscle weakness that typically begins in the proximal lower limbs and may progress to paralysis of all four extremities and involvement of the respiratory muscles. We present a case of a 37-year-old man with a history of TPP, presenting with acute muscle weakness and hypokalemia. The patient reported acute-onset bilateral lower extremity weakness from the previous day. Physical examination revealed normal deep tendon reflexes, but marked muscle weakn

This study describes five families (14 individuals) with hypokalemic periodic paralysis carrying a heterozygous pathogenic variant NM_000069.3:c.2690G>A (p.Arg897Lys) in the Calcium Voltage-Gated Channel Subunit Alpha1 S (CACNA1S) gene. The clinical exam showed pelvic weakness was common (10/14, with three being too young to exclude this age-dependent myopathy). Electromyography showed myogenic changes, and the long exercise test did not reveal a significant reduction of compound muscle action potential amplitude. Muscle MRI in three patients demonstrated involvement of axial musculature, the

OBJECTIVE: Hyperkalemic periodic paralysis (hyperKPP) is characterized by attacks of transient weakness. A subset of hyperKPP patients suffers from transient involuntary contraction of muscle (myotonia). The goal of this study was to determine mechanisms causing myotonia in hyperKPP.
METHODS: Intracellular electrophysiology, single-fiber Ca2+ imaging, and whole muscle contractility studies were performed in a mouse model of hyperKPP.
RESULTS: Myotonia in hyperkPP was caused by both involuntary myogenic action potentials (AP myotonia) lasting less than 5 min and action potential-independent m
