Primary periodic paralysis (PPP) comprises inherited ion channelopathies, marked by episodic or sustained muscle weakness. Dichlorphenamide has been investigated as a treatment option, however existing studies report inconsistent and variable findings regarding its efficacy. To clarify these inconsistencies, we conducted a meta-analysis and systematic review evaluating its effectiveness in managing PPP. A comprehensive literature exploration was undertaken across multiple databases encompassing studies disseminated up to July 2025, assessing efficacy of dichlorphenamide in PPP. Primary outcome

Hypokalaemic periodic paralysis (HPP) is a rare neuromuscular disorder characterized by recurrent episodes of transient muscle weakness associated with hypokalaemia. The condition typically presents in childhood or adolescence. In this case, a 17-year-old male presented with acute-onset limb weakness upon waking. Examination revealed marked truncal and proximal muscle weakness with preserved distal strength. Serum potassium was 2.7 mmol/L. His symptoms fully resolved following intravenous potassium replacement. HPP is often underdiagnosed, and diagnostic delays can leave patients vulnerable to

Hypokalemic periodic paralysis type 1 (PPHy-1) is a rare autosomal dominant disorder caused by mutations in the CACNA1S gene, leading to recurrent muscle weakness associated with hypokalemia. We describe a 19-year-old male presenting with recurrent episodes of muscle weakness lasting from 2 to 48 hours. During an attack, clinical examination revealed tetraparesis with axial motor deficits, but no bulbar or facial involvement. Serum potassium levels were 2.5 mmol/L, and nerve conduction studies (NCS) showed asymmetrical decrement in motor amplitudes. A long exercise test (LET) post-attack revea

OBJECTIVE: This article describes the clinical approach to rhabdomyolysis and the diagnosis and management of episodic disorders of skeletal muscle including skeletal muscle channelopathies.
LATEST DEVELOPMENTS: New gene variants that cause periodic paralysis have been identified. While these are exceedingly rare, they are now included in relevant genetic testing panels. Dantrolene is emerging as an additional option for the treatment of severe muscle stiffness, along with typical sodium channel blockers in sodium channel myotonia. Deep phenotyping in Andersen-Tawil syndrome shows significant

BACKGROUND: Thyrotoxic periodic paralysis (TPP) is a rare but potentially life-threatening cause of acute flaccid paralysis, characterized by hypokalemia in the setting of thyrotoxicosis and often misdiagnosed in emergency departments.
METHODS: We conducted a narrative literature review of TPP in adults using major databases up to 31 October 2025, with a focus on clinical presentation, precipitants, diagnostic pitfalls, and management, and we synthesized Middle Eastern case data alongside global experience.
RESULTS: TPP typically affects younger males and may be the first manifestation of hy

Periodic paralysis is a rare genetic disorder characterized by recurrent episodes of sudden skeletal muscle weakness. Various factors can trigger this condition, including cold temperatures, abnormal potassium levels, physical activity, and consumption of certain foods. Periodic paralysis is considered benign and usually manifests as muscle weakness. Adult patients with hypokalemic periodic paralysis often experience symptoms such as myalgia and fatigue, whereas myalgia and fatigue are less common in children. We encountered a pediatric patient with hypokalemic periodic paralysis who presented

Andersen-Tawil syndrome (ATS) is a rare genetic disorder characterized by a triad of periodic paralysis, cardiac arrhythmias, and dysmorphic features, typically presenting in the first two decades of life. A 45-year-old man presented with acute lower motor neuron-type quadriparesis. He reported similar self-resolving episodes over the past 2 years, triggered by rest after exercise. There were no cardiac symptoms, dysmorphic features, or relevant family history. Biochemical evaluation revealed hypokalemia. Genetic testing confirmed a heterozygous nonsense mutation in the KCNJ2 gene (c.13C>T, p.

BACKGROUND/OBJECTIVE: Levothyroxine-induced thyrotoxic hypokalemic periodic paralysis belongs to a rare group of periodic paralysis that lead to sudden onset of weakness in proximal muscles. Thyrotoxic periodic paralysis (TPP) is a common manifestation among East Asian patients with hyperthyroidism but is infrequent among the South Asian population. To highlight levothyroxine as a major contributor to periodic paralysis and the importance of frequent monitoring of serum potassium levels in patients taking levothyroxine, necessary to prevent life-threatening complications such as neuromuscular

BACKGROUND/OBJECTIVE: A 33-year-old female presented with recurrent fainting. There was no hyperpigmentation, hirsutism, virilization, or oligomenorrhoea. She had not used antifungal medications, mitotane, metyrapone, or exogenous steroids. Her blood pressure was 135/83 mmHg when supine and 84/62 mmHg on standing. She was afebrile (36.8 °C) and her pulse rate was 81/min. Investigations showed: serum potassium 3.9 mmol/L (3.3 - 5.1), bicarbonate 25 mmol/L (21 - 32 mmol/L), peak cortisol on 250 mcg adrenocorticotropic hormone stimulation test 36 nmol/L, adrenocorticotropic hormone 29.7 pmol/L (1

Acute flaccid paralysis can obscure the underlying endocrine cause in thyrotoxic periodic paralysis (TPP), a rare but potentially life-threatening presentation of Graves' disease. We describe a 23-year-old Cantonese man who presented with sudden bilateral proximal lower limb weakness and severe hypokalaemia after a high-carbohydrate meal. Subsequent investigations revealed thyrotoxicosis consistent with Graves' disease. Management involved prompt potassium replacement, propranolol, and cautious initiation of carbimazole to avoid overtreatment. The patient later underwent radioactive iodine abl

CACNA1S variants can alter the structure and function of the calcium channel, resulting in abnormal calcium influx and homeostasis. It is well established that pathogenic variants in CACNA1S can lead to hypokalemic periodic paralysis, malignant hyperthermia, and congenital myopathy. Nevertheless, the clinical presentations and disease progression of exertional myalgia and weakness associated with CACNA1S variants remain elusive. In this study, four affected individuals from an autosomal-dominant family were described, exhibiting symptoms of severe exertional myalgia, followed by flaccid weakne

CACNA1S gene variants are associated with congenital myopathies (CMyo) with triad dysfunction (triadopathies), malignant hyperthermia susceptibility, hypokalemic periodic paralysis and thyrotoxic periodic paralysis. Here, we generated three iPSC lines derived from patients with CMyo linked to both autosomal dominant and recessive CACNA1S variants (CACNA1S-CMyo). The three lines displayed typical iPSC morphology, uniform expression of markers of the undifferentiated state, trilineage differentiation potential and normal karyotypes. As CACNA1S-CMyo are ultra-rare disorders, these lines enable a

Thyrotoxic hypokalemic periodic paralysis (THPP) is a rare but potentially fatal complication of thyrotoxicosis, characterized by transient episodes of muscle weakness in the setting of hypokalemia and underlying hyperthyroidism. Although thyrotoxicosis is more common in females, THPP predominantly affects males, especially in Asian populations, in which its prevalence is notably higher. Early recognition is essential to prevent serious complications such as cardiac arrhythmias and respiratory failure; however, THPP is frequently misdiagnosed, particularly in Western countries, due to clinical

Store-operated Ca2+ entry (SOCE) is a ubiquitous mechanism controlling Ca2+ homeostasis and relies on the reticular Ca2+ sensor STIM1 and the plasma membrane Ca2+ channel ORAI1. STIM1 and ORAI1 gain-of-function mutations induce excessive Ca2+ influx through SOCE overactivation and cause tubular aggregate myopathy (TAM) and Stormorken syndrome (STRMK), two overlapping disorders characterised by muscle weakness and additional signs such as short stature, thrombocytopenia and hyposplenism. Most patients carry missense mutations in the STIM1 Ca2+-sensing EF-hands or in the CC1 domain implicated in

Hyperthyroidism can sometimes mimic Guillain-Barré syndrome (GBS). Polyneuropathy, paraplegia, and thyrotoxic periodic paralysis presenting as hypokalemia can be the presentation of hyperthyroidism. This case highlights the similar presentation of two clinical conditions that can occur simultaneously, or one of them may precipitate the other. A high index of suspicion is essential for the diagnosis.

Thyrotoxic periodic paralysis is a rare but potentially life-threatening complication of thyrotoxicosis characterised by hypokalaemia-induced flaccid paralysis due to an intracellular potassium shift rather than total body depletion. We report a male in his early 40s of Filipino origin who awoke with acute bilateral lower-limb weakness after he had consumed a large carbohydrate meal and performed strenuous exercise within the preceding 12 hours. Venous blood gas was normal; initial serum potassium was 1.7 mmol/L with phosphate 1.07 mmol/L. The ECG showed sinus tachycardia, first-degree atriove

BACKGROUND: Hypokalemic paralysis is a rare but potentially life-threatening neuromuscular emergency characterized by the sudden onset of flaccid weakness due to reduced serum potassium. While inherited channelopathies represent primary causes, secondary etiologies-particularly endocrine, renal, and metabolic disorders-are more frequently encountered and require timely recognition to avoid complications.
CASE SUMMARY: We report four patients presenting with acute flaccid paralysis associated with documented hypokalemia (serum potassium <3.5 mmol/L). The first case involved a pregnant woman wi

Thiamine (vitamin B1) deficiency may present with diverse symptoms and is often triggered by chronic alcoholism, severe malnutrition or hyperemesis gravidarum. While typically diagnosed clinically, atypical presentations may delay recognition and treatment. We report a 26-year-old pregnant woman in her second trimester with prolonged nausea and vomiting since early pregnancy. She developed ascending lower limb paralysis, dysarthria, horizontal nystagmus, and lagophthalmos. Initially managed as hypokalaemia periodic paralysis with thyrotoxicosis and later Guillain-Barré syndrome (GBS), her symp

Hereditary spastic paraplegia (HSP) is a rare and genetically heterogeneous neurodegenerative disorder, primarily defined by progressive lower-limb spasticity and weakness. Among the numerous genes implicated, pathogenic variants in the spastic paraplegia 7 (SPG7) gene represent one of the most common causes of HSP, whereas mutations in SCN4A, a skeletal muscle ion channel gene, are typically associated with a diverse spectrum of phenotypes, including hyperkalemic and hypokalemic periodic paralysis, potassium-aggravated myotonia, and congenital paramyotonia. To date, however, the coexistence o

Distal renal tubular acidosis (RTA) presenting as hypokalemia, metabolic acidosis, and hypokalemic periodic paralysis is a common manifestation of tubulointerstitial nephritis (TIN) in Sjögren's syndrome. Sjögren's syndrome is a chronic, progressive, systemic autoimmune inflammatory disease characterized by lymphocytic infiltration of exocrine glands, which results in sicca syndrome, i.e., xerostomia and keratoconjunctivitis sicca. Some patients can express sicca symptoms with other autoimmune diseases. It is mostly seen in females, and middle-aged women are more likely to be affected. Some pa
