Dermatomyositis (DM) is a rare idiopathic inflammatory myopathy characterized by proximal muscle weakness and pathognomonic cutaneous manifestations. Recognition in patients with skin of color can be delayed due to atypical presentation of classic rashes. A 59-year-old woman with type 2 diabetes mellitus and hypertension presented with progressive weakness, dyspnea, palpitations, edema, and significant unintentional weight loss. Initial evaluation revealed elevated transaminases and troponin, and she was managed for suspected autoimmune myocarditis. On readmission, she had hyperpigmented and v

AIMS/INTRODUCTION: Obesity is often accompanied by skeletal muscle atrophy, which aggravates insulin resistance and metabolic dysfunction. Chronic inflammation is implicated in this process, but the molecular mediators linking obesity-induced inflammation to muscle wasting have remained unclear. We investigated the role of the chemokine CXCL10 in skeletal muscle inflammation and atrophy induced by a high-fat diet (HFD).
MATERIALS AND METHODS: Male C57BL/6J mice were fed an HFD or normal diet for 2 weeks and received either neutralizing antibodies to CXCL10 or control immunoglobulin G. Muscle

Among the known genetic causes of syndromic autism spectrum disorders (ASDs) are transcription factor deficiencies. In this regard, haploinsufficiency of the zinc finger and broad complex, tramtrack, bric and brac domain-containing protein 20 (ZBTB20) leads to a prototypical clinical picture, referred to as Primrose syndrome, comprising severe ASD symptoms together with intellectual disability. Here, we present a comprehensive behavioral and phenotypical characterization of Zbtb20+/- mice, a construct valid model of this thus far untreatable human condition. Zbtb20+/- mice exhibited diminished

BACKGROUND: Rotator cuff impingement syndrome is a prevalent cause of supraspinatus tendinopathy, resulting in considerable pain and functional impairment. Excessive apoptosis of tenocytes has been recognized as a critical pathological mechanism in both supraspinatus tendinopathy and rotator cuff tears. Furthermore, mitochondrial degeneration and endoplasmic reticulum (ER) stress-induced apoptosis are significant factors in chronic multisystem diseases. However, it remains unclear whether mitochondrial damage and ER stress can also cause excessive apoptosis of tendon cells and contribute to th

This study characterizes a rare phenotype of Amyotrophic Lateral Sclerosis (ALS) presenting with predominant proximal lower limb weakness at onset, a presentation often mimicking myopathy. We retrospectively reviewed 1980 patients, identifying 15 (0.75%) with this atypical onset. The majority were males (73%) with a median age of onset of 58.7 years. Approximately half presented with symmetric proximal lower limb weakness. Nine of the 11 tested patients had higher CK. Follow-up (median 53.7 months) revealed that 6 patients maintained isolated lower limb weakness for a median of 60.1 months, wh

BACKGROUND: While increasing moderate-to-vigorous physical activity (MVPA) enhances muscular health through metabolic and vascular adaptations, its association with muscle strength improvement remains unclear, especially in real-world contexts. This study therefore aimed to investigate the longitudinal relationship between changes in MVPA and handgrip strength (HGS) to inform lifestyle guidelines and promote healthy ageing.
METHODS: A pragmatic intervention study enrolled 4344 Chinese community-dwelling adults aged rld contexts. This study therefore aimed to investigate the longitudinal physi

The Cardiometabolic Index (CMI) reflects visceral adiposity and lipid dysregulation, while sarcopenia indicates skeletal muscle depletion, both representing metabolic and functional decline in chronic kidney disease (CKD). This study aimed to investigate the impact of CMI and sarcopenia, as well as their integration into a metabolico-muscular composite risk model, on all-cause and cardiovascular mortality in CKD patients. Based on data from the National Health and Nutrition Examination Survey spanning 1999-2006 and 2011-2018, a total of 1,886 CKD patients were included. Kaplan-Meier survival a

Inborn errors of metabolism comprise a clinically diverse group of conditions that arise from the decreased activity of an enzyme or metabolite transporter and subsequent blockade in a metabolic pathway. These disorders are typically considered in the differential diagnosis of critically ill neonates or young children presenting with hypoglycaemia, metabolic acidosis or hyperammonaemia. However, beyond these classic presentations, a broader group of inborn errors of metabolism can manifest more subtly, with progressive articular and multi-systemic involvement that mimics or overlaps with typic

Calcific tendinitis of the medial collateral ligament (MCL) is an exceedingly rare cause of knee pain. We report an exceptional case of massive, refractory MCL calcific tendinitis, with a unique presentation potentially linked to an underlying malignancy. A 61-year-old female patient presented with a 3-month history of persistent left medial knee pain, unresponsive to conservative therapy. Physical examination revealed medial joint line tenderness and limited range of motion. Imaging-including radiography, 3-dimensional computed tomography, and magnetic resonance imaging-demonstrated an unusua

AIM: Dynapenia-loss of muscle strength despite preserved muscle mass-is a clinical concern linked to functional decline in older adults. Diabetes may contribute to dynapenia; however, few studies have examined whether this association varies by sex and age. Clarifying these differences may help inform targeted prevention strategies. This study investigated the association between diabetes and dynapenia in community-dwelling older adults in Japan, focusing on sex- and age-specific differences.
METHODS: We conducted a cross-sectional analysis using pooled data from four geriatric cohorts in Jap

Lactylation serves as a vital link between cellular metabolism and epigenetic regulation and plays a pivotal role in muscle biology. Muscle tissue is the primary site of lactate production; its unique metabolic environment confers dynamism, specificity and functional diversity for lactylation. Under physiological conditions, lactylation regulates myocyte energy metabolism, proliferation, differentiation, and exercise adaptation through a dynamic "writer-eraser-reader" mechanism. In pathological states, lactate imbalance directly contributes to the progression of various muscular disorders. For

Tripterygium wilfordii Hook F. (TwHF) is a traditional Chinese medicine that has been used for centuries. Tripterygium glycoside (TG), a herbal agent extracted from the TwHF rhizome, is extensively used for the treatment of systemic lupus erythematosus, rheumatoid arthritis, nephrotic syndrome, and atopic eczema. However, its clinical application is hindered by its multi-target toxicity. Current literature highlights multiple TG-related complications, including acute liver and kidney injury, rhabdomyolysis, infertility, and lymphocyte suppression. Contrarily, concurrent severe rhabdomyolysis,

Myopathies are a group of disorders that affect the skeletal muscles, manifesting as weakness, fatigue, or pain. Generally, they are classified as acquired or inherited. It is important to note that while most acquired myopathies are treatable, in the advent of targeted molecular and gene-based therapy, some inherited forms now have effective therapeutic options. These myopathies can be classified based on their underlying causes, including infections, autoimmune disorders leading to muscle inflammation, metabolic abnormalities within muscle cells, skeletal muscle channel dysfunctions, critica

The cGAS-STING signaling pathway is a central component of the innate immune system. Skeletal muscle, the body's largest metabolic and endocrine organ, is essential for overall health, and maintaining its homeostasis is critically important. This review systematically elaborates on the central position and "double-edged sword" role of the cGAS-STING pathway in skeletal muscle pathophysiology. We detail how, under various pathological stimuli-such as metabolic stress, physical injury, aging, toxin exposure, and systemic diseases-cytoplasmic DNA accumulation aberrantly activates the cGAS-STING p

With the continuous rise in the global prevalence of type II diabetes mellitus (T2DM), the associated skeletal muscle complications, including insulin resistance, muscle atrophy, and physical frailty, have garnered increasing attention. Skeletal muscle plays a vital role as a metabolic and endocrine organ and is considered a key factor in the pathological mechanisms of T2DM. Despite significant advances in understanding, the intrinsic molecular mechanisms underlying skeletal muscle dysfunction remain incompletely elucidated. Recent studies have highlighted a significant cellular stress respons

Given the intimate link between muscle function and the skeleton's ability to adapt to mechanical loads, it is no surprise that individuals with neuromuscular disorders (NMDs) are at risk for low-trauma (i.e., osteoporotic) fragility fractures. In this review, we highlight the interdependent relationship between muscle and bone strength and the need to assess individuals with NMD who demonstrate muscle weakness that interferes with weight bearing and typical activities of daily living. This review also emphasizes the importance of the clinical context when evaluating the risk of bone fragility

Mitochondria are essential organelles responsible for energy production, autophagy, and apoptosis, and mitochondrial dysfunction has been implicated in various diseases affecting the heart, liver, and kidneys. Mitochondrial transplantation, wherein isolated mitochondria are administered into cells or tissues, has recently emerged as a promising therapeutic approach for restoring cellular functions by enhancing ATP generation and reducing oxidative stress. However, the characteristics and functional diversity of the mitochondria isolated from different cell types remain poorly understood. Here,

BACKGROUND: Obesity is a major global health concern, leading to increased risk of metabolic diseases and mortality. Swimming, as a low-impact exercise, may provide metabolic benefits. However, the influence of water temperature on the metabolic and physiological responses remains underexplored. This study investigated the effects of water temperatures on muscle and adipose tissue adaptations in high-fat diet-induced obese mice.
METHODS: Obese mice were subjected to swimming at different water temperatures: 15°C, 25°C, or 32°C. Changes in body and tissue weight, grip strength, exhaustive swim

BACKGROUND: Recent studies describe an association between sertraline and acquired late-onset multiple acyl coenzyme A dehydrogenase deficiency (MADD), a rare disorder affecting fatty acid metabolism. MADD was previously considered solely a genetic disorder caused by recessive pathogenic variants mainly in the ETFDH gene.
METHODS: A nationwide population-based registry search was conducted to identify adult patients with sertraline-associated MADD in Sweden 2014-2024. Patients with muscle biopsy reports of lipid storage myopathy or biochemical investigations showing acylcarnitine profile sugg
