Case Report: Efgartigimod in refractory non-tumor Lambert-Eaton myasthenic syndrome.

Lambert-Eaton myasthenic syndrome (LEMS) is a rare autoimmune disorder, associated with neoplastic disease in 50-60% of cases. In severe cases, currently available immunomodulatory therapies frequently fail to achieve adequate control. We report a 20-year-old woman with non-tumor LEMS refractory to 3,4-diaminopyridine, sequential immunosuppression with prednisone, azathioprine, mycophenolate mofetil and rituximab and plasma exchange, who remained severely disabled after two years of progressive disease. Off-label efgartigimod (1000mg subcutaneously weekly for four weeks, with the interval betw

Infections and the neuromuscular junction: From direct neurotoxicity to immune-mediated dysfunction.

Infectious diseases and disorders of the neuromuscular junction (NMJ) are linked by complex and heterogeneous mechanisms that extend beyond direct pathogen-mediated injury. While some infectious agents can directly impair neuromuscular transmission, others may act as triggers of autoimmunity or contribute to the exacerbation of pre-existing NMJ disorders. However, the mechanisms underlying these interactions remain incompletely understood. In this narrative review, we summarize the current evidence regarding infections associated with NMJ dysfunction. Available evidence suggests that infection

Immune Checkpoint Inhibitor Therapy Triggers Autoimmune Cerebellar Ataxia with Anti-P/Q-type VGCC Antibody and Anti-mGluR5 Antibodies: A Case Report.

Autoimmune cerebellar ataxia (ACA) is an immune-mediated cerebellar syndrome that is often associated with specific autoantibodies. We report a case of a 61-year-old man with small cell lung cancer who developed subacute progressive cerebellar ataxia after durvalumab therapy. Anti-P/Q-type voltage-gated calcium channel (VGCC) and anti-metabotropic glutamate receptor 5 (mGluR5) antibodies were detected. The patient improved substantially with early immunotherapy and the discontinuation of immune checkpoint inhibitor (ICIs) therapy. In this case, anti-VGCC antibodies may have contributed to the

Chimeric antigen receptor T cell therapy in autoimmune neurological disease: current status of efficacy and safety.

Autoimmune neurological diseases affecting the central nervous system, peripheral nervous system and neuromuscular junction are frequently refractory to conventional B-cell-depleting therapies such as CD20-directed monoclonal antibodies, highlighting a need for more effective treatment approaches. Chimeric antigen receptor T (CAR-T) cell therapy, initially developed for haematological malignancy, has been repurposed to more comprehensively deplete pathogenic B-cell and plasma cell populations in autoimmune disease, raising the possibility of durable, treatment-free remission. This scoping revi

Phylogenetic analysis of the genus Taphrina: proposal of one new genus and seven new species (Taphrinales, Taphrinomycetes) from China.

AIMS: Lambert-Eaton myasthenic syndrome (LEMS) is a kind of autoimmune disease of the neuromuscular junction that is often misdiagnosed as a peripheral nerve disease or myopathy. For some difficult cases, muscle biopsy examination is useful for differential diagnosis. However, studies about the pathological findings of LEMS patients are rare, especially of patients who were diagnosed with small cell lung carcinoma (SCLC). This study aimed to describe several pathological muscle features in patients with LEMS associated with SCLC. MATERIALS AND METHODS: The 5 patients enrolled in this study we

Coexisting cerebellar ataxia and Lambert-Eaton myasthenic syndrome without malignancy: insights from a case and systematic literature review.

BACKGROUND: Lambert-Eaton myasthenic syndrome (LEMS) is a rare autoimmune disorder often associated with small-cell lung carcinoma (SCLC). Cerebellar degeneration can coexist with LEMS, although their simultaneous presentation in the non-neoplastic setting is uncommon. METHODS: We describe a 75-year-old man with progressive cerebellar ataxia followed by bilateral ptosis, ophthalmoparesis, and proximal weakness. LEMS was confirmed by positive P/Q-type VGCC antibodies and electrophysiology. Extensive malignancy screening, including PET-CT, was negative, and immunotherapy produced marked neuromus

Immunotherapies in autoimmune neuromuscular junction disorders: Acute and chronic management.

Myasthenia gravis (MG) is the most common disease of the neuromuscular junction. Conventional immunotherapy based on corticosteroids and immunosuppressants has been in use for several decades and has greatly contributed to the improvement of the disease prognosis. In recent years, several new agents, mostly monoclonal antibodies, have proved effective in randomized controlled trials and are in clinical use. Other biologics are currently under evaluation. Novel therapies, that offer the advantage of more selective effects on the immune system, are not devoid of potentially serious adverse effec

Clinical Spectrum and Outcomes of SOX1 Antibody-Associated Paraneoplastic Neurological Syndromes: A Chinese Cohort Study.

BACKGROUND: SOX1 antibody-positive paraneoplastic neurological syndromes (PNS) exhibit significant population-specific clinical heterogeneity. While Western cohorts predominantly manifest Lambert-Eaton myasthenic syndrome (65%-80%), comprehensive clinical characterization and treatment response data in Asian populations remain critically limited. METHODS: We conducted a single-center retrospective case series analyzing 13 consecutive patients with SOX1 antibody-positive PNS treated at Guangdong Sanjiu Brain Hospital from January 2019 to December 2024. SOX1 antibodies were confirmed using comm

[Myasthenic syndromes: retrospective analysis of two patient cohorts].

Myasthenia gravis (MG) and Lambert-Eaton myasthenic syndrome (LEMS) are rare autoimmune-mediated neuromuscular diseases characterized by muscle weakness. Although various treatment options are available for MG, a subset of patients may be considered treatment-refractory, with differing definitions of this status present in the scientific literature. This monocentric, retrospective cohort study investigated 200 patients with MG and 15 patients with LEMS. Data on clinical characteristics, antibody status, treatment response, and concomitant diseases were collected. MG patients who received at le

[How to Diagnose Lambert-Eaton Myasthenic Syndrome Patients Early: A Systematic Review of Japanese Case Reports].

A systematic review was conducted on case reports of Lambert-Eaton myasthenic syndrome (LEMS) in Japanese patients published prior to 2023, focusing on clinical features. Particular attention was paid to the interval from LEMS onset to diagnosis, examining the clinical question (CQ): 'How can the time to diagnosis be shortened?' Patient profiles showed a male predominance, with 76 males and 21 females among the total 97 cases, yielding a male-to-female ratio of 3.6:1. The median age of onset (mean±standard deviation) across all cases was 62.0 (59.3±11.7) years, with the age distribution peakin

Hidden diagnoses among patients with double seronegative myasthenia gravis.

INTRODUCTION: Double seronegative myasthenia gravis (dSnMG) is defined as myasthenia gravis (MG) without detectable antibodies to acetylcholine receptor (AChR) and muscle-specific kinase (MuSK). Absence of a disease-specific biomarker and clinical heterogeneity can significantly complicate diagnostic pathway. This study aimed to identify cases misdiagnosed as dSnMG. METHODOLOGY: The study included 33 patients [64% females, median age at onset 30 (22.5-40) years, median age at testing 46 (34-58) years] previously diagnosed with dSnMG. Disease severity was assessed using MG-ADL and QMG at testi

Efficacy and Safety of Amifampridine in Myasthenia Gravis: A Randomized, Double-Blind, Placebo-Controlled Crossover Trial.

BACKGROUND AND OBJECTIVES: Amifampridine, a short-acting potassium-channel blocker, is the first-line treatment for Lambert-Eaton myasthenic syndrome. Its benefit as add-on to pyridostigmine in acetylcholine receptor (AChR)-positive myasthenia gravis (MG) has been proposed, but no randomized controlled trials have been conducted. We aimed to evaluate the efficacy and safety of amifampridine in AChR-MG with insufficient symptom control on pyridostigmine. METHODS: A randomized, double-blind, placebo-controlled crossover trial was conducted at Leiden University Medical Center, a tertiary center

Cellular therapies in autoimmune neuromuscular disorders: the new frontier.

Antibody-mediated humoral immunity plays a vital role in a number of autoimmune neuromuscular disorders (AINMD), such as myasthenia gravis, Lambert-Eaton myasthenic syndrome (LEMS), stiff-person syndrome (SPS), and autoimmune myositides. Although biologics based B cell and plasma cell depletion (BCDT) therapy effectively treats some AINMDs, its variable success highlights the limitations of this therapeutic modality. B cell directed cellular therapy (BCDcT) targeting a wide array of B cell lineages, including short-lived plasma cells, has emerged as an effective treatment modality in hematolog

Low-Frequency Repetitive Nerve Stimulation in Myasthenia Gravis and Lambert-Eaton Myasthenic Syndrome.

OBJECTIVE: Lambert-Eaton myasthenic syndrome (LEMS) and myasthenia gravis (MG) exhibit distinct decrement patterns in low-frequency repetitive nerve stimulation. While a delayed nadir in LEMS is well-documented, we identified additional unique patterns. This study systematically compared low-frequency repetitive nerve stimulation features between the two conditions using prolonged protocols. METHODS: We retrospectively analyzed 20 patients with LEMS (March 2019-August 2025) and 34 age-matched patients with MG (April 2023-August 2025). All subjects underwent low-frequency repetitive nerve stim

Presynaptic Congenital Myasthenic Syndromes.

Presynaptic congenital myasthenic syndromes (CMS) encompass a large number of rare neurologic disorders caused by impaired release of acetylcholine (ACh) from motor nerve terminals. There are two main groups of presynaptic CMS: one in which the amount of ACh in synaptic vesicles (SV) is diminished and another in which the mechanism of synaptic vesicle release is impaired. The latter is often referred to as a congenital Lambert-Eaton myasthenic syndrome (LEMS). Presynaptic CMS, like other forms of CMS, is primarily characterized by muscle weakness and fatigue, but is often associated with addit
