Anti-VGCC negative and Anti-SOX1 positive Lambert-Eaton myasthenic syndrome as the initial presentation of small cell lung cancer: a case report.

Lambert-Eaton myasthenic syndrome (LEMS) is a rare autoimmune disorder characterized by impaired acetylcholine release at the neuromuscular junction, leading to proximal muscle weakness, autonomic dysfunction, and diminished or absent tendon reflexes. Here, we present a rare case of a 73-year-old male smoker with small cell lung cancer (SCLC), whose initial manifestation was LEMS. Notably, the patient tested seronegative for anti-voltage-gated calcium channel (VGCC) antibodies but was seropositive for anti-SOX1 antibodies. The diagnosis of LEMS was supported by characteristic clinical symptoms

Autopsy-Confirmed Non-Paraneoplastic Lambert-Eaton Myasthenic Syndrome with Cerebellar Degeneration: A Case Report.

Background and Clinical Significance: Lambert-Eaton myasthenic syndrome (LEMS) is mediated by antibodies against P/Q-type voltage-gated calcium channels (VGCCs) and is classified as paraneoplastic (T-LEMS) or non-paraneoplastic (NT-LEMS). Cerebellar degeneration is recognized in T-LEMS, but pathological confirmation in NT-LEMS has not been reported. Case Presentation: A 79-year-old man developed progressive ataxic gait and dysarthria at age 76 and was diagnosed with LEMS based on repetitive nerve stimulation findings and anti-P/Q-type VGCC antibodies. No malignancy was identified during more t

Clinical approach to neurological paraneoplastic syndromes.

Neurological paraneoplastic syndromes are immune-mediated disorders. The diagnosis of paraneoplastic syndromes is often difficult because they are rare and the onset of symptoms usually precedes detection of the underlying malignancy. Accurate diagnosis depends on recognition of the clinical phenotypes likely to be associated with a paraneoplastic aetiology and detection of an anti-neuronal antibody. Clinical phenotypes associated with a high risk of a paraneoplastic cause are limbic encephalitis, encephalomyelitis, rapidly progressive cerebellar syndrome, opsoclonus-myoclonus syndrome, subacu

Exploring the Value of Quantitative Muscle Ultrasound in Neuromuscular Junction Disorders: A Pilot Study.

INTRODUCTION/AIMS: Quantitative muscle ultrasound (QMUS) is a validated technique for assessing muscle pathology, yet its role in disorders primarily affecting the neuromuscular junction (NMJ) remains unclear. This pilot study aimed to explore whether QMUS is associated with structural muscle changes in patients with acquired or genetic NMJ disorders. METHODS: We conducted a retrospective study of 18 patients with confirmed NMJ disorders who underwent electrodiagnostic testing (EDx), including repetitive nerve stimulation (RNS) and/or stimulated single-fiber electromyography (SFEMG), combined

Longitudinal overview of symptomatic and immunosuppressive drugs in LEMS.

Lambert-Eaton myasthenic syndrome (LEMS) is a rare autoimmune neuromuscular junction disorder characterized by muscle weakness and autonomic dysfunction. While amifampridine is the established first-line symptomatic treatment, longitudinal data regarding the evolution of therapeutic regimens over time and the use of immunosuppressive therapies remain limited. This retrospective study analyzed 70 LEMS patients treated at the Leiden University Medical Center (LUMC) between 1992 and 2023. Treatment was evaluated and a four-point "leg score" was developed and used to retrospectively quantify funct

Early Induction of IVIg Improved the Performance Status and Enabled Chemotherapy Initiation in PCD-LEMS: A Case Report.

A 73-year-old man developed progressive gait disturbance and diplopia over two weeks concomitant with rapidly enlarging pulmonary lesions. Examination revealed proximal lower limb weakness, diminished deep tendon reflexes, ataxic dysarthria, limb and truncal ataxia, and orthostatic hypotension. Nerve conduction studies revealed reduced compound muscle action potentials with postexercise facilitation. Elevated anti-P/Q-type voltage-gated calcium channel antibodies confirmed paraneoplastic cerebellar degeneration (PCD) with Lambert-Eaton myasthenic syndrome (LEMS). Bronchoscopy revealed small-ce

Plasma Exchange-Responsive Bulbar Dysfunction in Thymoma-Associated Myasthenia Gravis with Concomitant Lambert-Eaton Myasthenic Syndrome.

The coexistence of thymoma-associated myasthenia gravis (MG) and Lambert-Eaton myasthenic syndrome (LEMS) is rare, and no established therapeutic strategy exists for this condition. We herein report a case of MG-LEMS overlap syndrome (MLOS) in which plasma exchange (PE) effectively treated severe bulbar dysfunction refractory to intravenous immunoglobulin (IVIg). Sequential electrophysiological testing demonstrated an improvement in the low compound muscle action potential amplitudes and post-exercise increments, reflecting LEMS activity. This case highlights that PE may be more efficacious th

Decrement on Low-Frequency Repetitive Nerve Stimulation Is Not Synonymous With Myasthenia Gravis.

Lambert-Eaton myasthenic syndrome (LEMS) is a rare presynaptic neuromuscular junction disorder that may be mistaken for peripheral neuropathy, radiculopathy, inflammatory myopathy, or myasthenia gravis. We report a female in her early 40s with two months of progressive, predominantly proximal lower-limb weakness, areflexia, and dry mouth, without ocular or bulbar symptoms. Routine laboratory testing and creatine kinase were unremarkable, while spinal imaging showed degenerative abnormalities that did not explain the clinical pattern. Nerve conduction studies demonstrated low compound muscle ac

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

Iatrogenic botulism: a risk for botulinum toxin's medical use?

Botulinum toxin (BT) is infamous for its extreme toxicity. If it enters the bloodstream, it can cause botulism presenting with a typical pattern of motor and autonomic dysfunction. An international expert panel organised by IAB-Interdisciplinary Working Group for Movement Disorders explored iatrogenic botulism after BT's medical use (IB), reached conclusions and formulated recommendations. When injected into its target tissue, BT binds to gangliosides on cholinergic nerve terminals before it is internalised permanently. Small amounts of BT, however, are circulating within the bloodstream. When

Electrodiagnostic Approach to Defects of Neuromuscular Transmission.

Neuromuscular junction (NMJ) disorders such as myasthenia gravis, Lambert-Eaton myasthenic syndrome, and botulism are characterized by impaired synaptic transmission leading to weakness. This review examines the electrodiagnostic evaluation of these conditions, emphasizing the importance of techniques such as repetitive nerve stimulation (RNS) and single-fiber electromyography (SFEMG) for confirming the diagnosis and distinguishing presynaptic from postsynaptic defects. The reduced safety factor of neuromuscular transmission (NMT) in postsynaptic disorders produces a decrement in compound musc

Fluctuating, but Not Forgotten: Recognizing Neuromuscular Junction Disorders.

The neuromuscular junction (NMJ), the connection between nerve and muscle, is an essential anatomic site that warrants careful consideration. Dysfunction at this junction often leads to skeletal muscle weakness with a multitude of clinical manifestations. Disorders of the NMJ primarily encompass myasthenia gravis, Lambert-Eaton syndrome, botulism, and organophosphate poisoning. This article will review diseases and disorders of the NMJ, their clinical presentations, and approach to management.

[Botulism in Japan].

Botulism is a paralytic disease caused by the neurotoxin mainly produced by Clostridium botulinum. Botulism is classified as infant, foodborne, wound, adult intestinal toxemia, iatrogenic, and inhalation botulism. In Japan, type E botulism associated with the consumption of "izushi," a traditional fermented fish product, was prevalent before 2000. Since the 2000s, type A botulism has become more common. In some cases, foodborne botulism was suspected, but no incriminating food or sporadic cases were identified. Therefore, foodborne botulism should be considered even in the absence of a clear h

Neuromuscular Junction Disorders in Children: Approach to Diagnosis and Management.

Neuromuscular junction disorders in children present with fatigable weakness and encompass genetic, autoimmune, and toxin-mediated subtypes. Timely and correct diagnosis is crucial as many subtypes respond to specific treatments. Congenital myasthenic syndromes may show therapeutic responses to targeted medications, but they can be misdiagnosed as congenital myopathy, neuropathy, or muscular dystrophy. Furthermore, the treatment landscape for autoimmune myasthenia gravis is rapidly expanding with complement inhibitors and neonatal fragment crystallizable receptor antagonists. In this review, w

Rectal cancer with solitary hepatic metastasis and Lambert-Eaton myasthenic syndrome.

Lambert-Eaton myasthenic syndrome (LEMS) is a rare autoimmune neuromuscular disease mediated by antibodies to voltage-gated calcium channels (VGCCs) at the neuromuscular junction. LEMS often presents as a paraneoplastic disease. Between 40% to 62% of patients diagnosed with LEMS are confirmed to have small cell lung cancer (SCLC). There are few concerned reports on Gastrointestinal carcinomas. This article reports the treatment process of a young woman with rectal cancer and liver metastasis who associated with LEMS to summarize relevant clinical experience and reduce the rate of clinical misd
