Neurological disease progression in adults with ataxia telangiectasia: a longitudinal study.

Ataxia-telangiectasia (AT) is a rare, multisystem neurodegenerative disorder with marked clinical heterogeneity. Longitudinal data on neurological progression remain limited, restricting understanding of disease mechanisms and therapeutic trial design. We analysed longitudinal Scale for the Assessment and Rating of Ataxia (SARA) data from the largest published adult AT cohort to date (n = 82, age > 16 years). Linear mixed-effects models were used to estimate severity at the earliest available neurological evaluation (baseline) and annual progression rates (points/year) in classic and variant A

Severe hyperglycemic hyperosmolar state complicated by diabetic ketoacidosis and rhabdomyolysis in an obese adolescent patient: a case report.

BACKGROUND: In the pediatric population, glucose metabolism-associated disorders most commonly manifest as diabetic ketoacidosis (DKA). Hyperglycemic hyperosmolar state (HHS) is a rare yet potentially grave condition in children that may coexist with DKA. Early identification is paramount to the effective management of both conditions. Consequently, each documented case contributes meaningfully to expanding the understanding and vigilance of pediatricians regarding acute diabetic complications. CASE PRESENTATION: We describe an obese 13-year-old Chinese girl presenting with moderate DKA, char

Dental Management of a Child With Mitochondrial DNA Deletion Syndrome (Kearns-Sayre Syndrome): A Case Report.

BACKGROUND: Kearns-Sayre syndrome (KSS) is a rare mitochondrial DNA deletion disorder characterized by progressive external ophthalmoplegia, pigmentary retinopathy, and cardiac conduction abnormalities. Dental management may be complicated by developmental enamel defects, high caries burden, limited cooperation, and anesthetic considerations related to multisystem mitochondrial disease. Published dental case reports focused specifically on pediatric KSS remain extremely sparse. CASE DESCRIPTION: We describe the staged dental management of a 10-year-old girl with genetically confirmed KSS and

Beyond the pump: a narrative review of the acute molecular, metabolic, and neuropsychological responses to a bout of resistance exercise.

BACKGROUND: Resistance training is widely recognized for its chronic adaptations, including strength, muscle hypertrophy, and improvements in musculoskeletal health. However, a single bout of resistance exercise also elicits a range of acute physiological and psychological responses across multiple systems. These immediate responses have generally been examined within individual physiological domains, with less attention given to their coexistence and temporal overlap following a single session. METHODS: This narrative review synthesizes evidence relating to the acute systemic effects of a si

Ultrasonographic Features of Amyloid Arthropathy in Light Chain Amyloidosis: A Case Report.

BACKGROUND: Amyloid arthropathy is characterized by the deposition of misfolded proteins in the joints and soft tissues. It is often a manifestation of light chain amyloidosis. The ultrasonographic features of amyloid arthropathy are solely reported in the literature. CASE PRESENTATION: Herein, we present the case of a 70-year-old patient who was diagnosed with light chain amyloidosis. He reported chronic joint pain, bilateral carpal tunnel syndrome, and an inguinal mass. Ultrasound examination revealed tenosynovitis of the flexor digitorum tendons, the extensor carpi ulnaris tendon, and the

Biallelic variants in COX18 cause a mitochondrial disorder primarily manifesting as peripheral neuropathy.

Defects in mitochondrial dynamics are a common cause of Charcot-Marie-Tooth disease (CMT), whereas primary deficiencies in the mitochondrial respiratory chain (MRC) are rare and atypical for this aetiology. This study aims to report COX18 as a novel CMT-causing gene. This gene encodes an assembly factor of mitochondrial Complex IV that translocates the C-terminal tail of MTCO2 across the mitochondrial inner membrane. Exome sequencing was performed in four affected individuals from three families. The patients and available family members underwent thorough neurological and electrophysiological

Autophagy impairment is associated with enhanced satellite cell activation in muscle biopsies from younger late-onset Pompe disease patients.

Late-onset Pompe disease (LOPD) is caused by α-glucosidase (GAA) deficiency, leading to glycogen accumulation resulting in progressive muscular weakness and respiratory insufficiency. Glycogen overload, vacuolation, and autophagic accumulation are the histological hallmarks of the disease. However, markers capable of tracking the progression of LOPD across different disease stages remain insufficiently characterized. We performed a comprehensive myopathologic analysis of eleven LOPD muscle biopsies from untreated patients (age range 7-69 years) and compared them to eleven biopsies from histolo

Preclinical and first-in-human evidence of 4-hydroxybenzoic acid for mitochondrial COQ2 deficiency.

Primary coenzyme Q (CoQ) deficiency is a mitochondrial disorder with variable clinical presentation and limited response to standard CoQ10 supplementation. Recent studies suggest that 4-hydroxybenzoic acid (4-HBA), a biosynthetic precursor of CoQ, may serve as a substrate enhancement treatment in cases caused by pathogenic variants in COQ2, a gene encoding a key enzyme in CoQ biosynthesis. However, it remains unclear whether 4-HBA is required throughout life to maintain health, whether it offers advantages over CoQ10 treatment, and whether these findings are translatable to humans. Here, we de

Electrical muscle stimulation: a multidisciplinary narrative review.

Electrical muscle stimulation (EMS) is one of the most popular and efficient complementary techniques that mimics the action potential for adjacent muscles and maintains muscle health. Aside from muscular rehabilitation, a promising role for EMS has been reported in a wide range of clinical conditions, from cognitive disorders to metabolic diseases and pain management. However, the clinical response and effectiveness of this method remain variable. In this article, we reviewed the role of EMS in different health-related conditions in order to provide a comprehensive view of this matter.

Loss of adenylosuccinate synthetase 1 in mice recapitulates features of ADSS1 myopathy.

ADSS1 myopathy is an ultrarare congenital myopathy characterized by progressive cardiac and skeletal muscle degeneration with childhood to adolescent onset. This autosomal recessive disease is caused by mutations in the ADSS1 gene, encoding the enzyme adenylosuccinate synthetase (AdSS1). AdSS1 plays a critical role in the adenine nucleotide cycle, which is important for energy metabolism in muscle cells. Enzymatic defects, engendered by loss-of-function mutations in ADSS1, lead to a bottleneck in the adenine nucleotide cycle, causing metabolic dysfunction that ultimately results in progressive

Translational pig models for human diseases.

The translation of novel discoveries in biomedical research into clinical applications is a lengthy and resource-intensive process, with variable success. Predictive model organisms for non-clinical research are a key asset in this process. Rodents remain the most widely used models, but they have limitations in recapitulating human pathophysiology and disease phenotypes. Over the past two decades, pigs have played an increasingly important role in bridging this translational gap. Given their close anatomical and physiological similarity to humans, most vividly illustrated by successful pig-to

Mönckeberg medial calcific sclerosis mimicking temporomandibular disorder and giant cell arteritis: a case report.

This report describes a case of Mönckeberg medial calcific sclerosis (MCS) presenting with symptoms mimicking temporomandibular disorder (TMD) and giant cell arteritis in a patient, including jaw claudication and headache on mouth opening. Initial evaluation suggested TMD, but temporal artery biopsy revealed fibrous intimal thickening and medial calcification consistent with MCS. This case highlights the diagnostic challenge in older patients and emphasizes that combining Diagnostic Criteria for Temporomandibular Disorders (DC/TMD)-based assessment with systemic evaluation facilitates early an

Glucagon-Like Peptide 1 Receptor Agonists, Sodium-Glucose Cotransporter 2 Inhibitors, and Risk of Autoimmune Rheumatic Diseases.

OBJECTIVE: Glucagon-like peptide 1 receptor agonists (GLP-1RAs) and sodium-glucose cotransporter 2 inhibitors (SGLT2i) facilitate weight loss and exhibit immunomodulatory effects, but their impact on the risk of developing autoimmune rheumatic diseases (ARDs) is unclear. We compared ARD incidence following initiation of GLP-1RAs or SGLT2i versus a weight-neutral comparator (dipeptidyl peptidase 4 inhibitors [DPP4i]). METHODS: We performed a population-based cohort study using administrative health data from a Canadian province with universal health care. We included adults with type 2 diabete

Sodium-Glucose Cotransporter 2 Inhibitors and Reduced Fibromyalgia Risk in Patients With Diabetes: A Target Trial Emulation Study.

BACKGROUND/AIM: Emerging evidence suggests that sodium-glucose co-transporter 2 inhibitors (SGLT2i) and glucagon-like peptide-1 receptor agonists (GLP-1RA) may offer neuroprotective effects. This study aimed to compare the risk of new-onset fibromyalgia between diabetic patients starting SGLT2i versus those using GLP-1RA. PATIENTS AND METHODS: A target trial emulation was conducted using the TriNetX Global Collaborative Network. Adult patients with type 2 diabetes mellitus starting use of SGLT2i or GLP-1RA were included, excluding those with prior fibromyalgia or psychiatric disorders. Propen

Hereditary transthyretin amyloidosis with hand weakness and bulbar involvement.

A 76-year-old man developed progressive motor weakness, bulbar symptoms and hand muscle atrophy, initially suspected to be due to motor neurone disease. Unexpected findings on cardiological evaluation identified amyloidosis, and genetic testing confirmed the TTR p.Val50Met mutation, indicating late-onset hereditary transthyretin amyloidosis with a mixed neuropathic and cardiac phenotype. The diagnosis was delayed and complicated by minimal sensory symptoms and the atypical presentation.

Megalencephalic leukoencephalopathy with subcortical cysts: a multicenter Italian experience.

BACKGROUND: Megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a rare inherited white matter disorder. Initially, a "classic" phenotype has been characterized, presenting early-onset macrocephaly, cerebellar ataxia, mild spasticity, and a distinctive neuroimaging pattern of diffuse white matter abnormalities with subcortical cysts. An "improving" phenotype has also been described, featuring milder or absent neurological signs and a remitting pattern on neuroimaging. Mutations in four genes, MLC1, HEPACAM, GPRC5B and AQP4 have been associated with MLC. We describe clinical and

Ganoderma lucidum polysaccharide ameliorates cancer cachexia with enhanced efficacy in aged group by reprogramming arginine metabolism to modulate Treg formation.

Cancer cachexia is a syndrome characterized by profound weight loss, muscle wasting, and metabolic disturbance in cancer patients. Cancer-associated cachexia represents significant biological and clinical challenges that profoundly affect human health. Approximately 20 % of cancer patients succumb directly to cachexia-related complications. Aging is characterized by a gradual decline in physiological functions and an increased vulnerability to chronic diseases. However, whether aging inherently increases vulnerability to cachexia or exacerbates its progression remains unclear, impeding the dev
