Relationship between glucagon-like peptide-1 receptor agonist use and incidence of trigger finger in patients with type 2 diabetes.

PURPOSE: The relationship between glucagon-like peptide-1 receptor agonist (GLP-1 RA) use and trigger finger (TF) remains unknown. This study aims to compare rates of TF, TF release, and complications of TF release between diabetic patients with and without a history of GLP-1 RA use. METHODS: This study utilized the TriNetX database to identify adult patients with Type II diabetes mellitus. The primary analysis included patients with and without a history of GLP-1 RA use. The secondary analysis included patients with a history of TF separated by the presence or absence of GLP-1 RA use, respec

Gliomas with succinate dehydrogenase deficiency: A case series and brief review of the literature of oncometabolite-driven gliomagenesis.

Oncometabolite production plays a key role in the development and progression of isocitrate dehydrogenase-mutant gliomas. The aberrant gain-of-function activity of the mutant isocitrate dehydrogenase protein results in the production of the oncometabolite D-2-hydroxyglutarate, which in turn promotes DNA hypermethylation and gliomagenesis through several mechanisms. Rare gliomas in patients with fumarate hydratase deficiency syndrome share many morphologic and molecular features with isocitrate dehydrogenase-mutant gliomas, with the oncometabolites succinate and 2-succinocysteine similarly thou

Direct AMPK Activation Confers Mutation-Independent Therapeutic Benefit in Duchenne Muscular Dystrophy.

BACKGROUND: Duchenne muscular dystrophy (DMD) is a severe, life-limiting neuromuscular disorder (NMD) characterized by progressive muscle wasting and mitochondrial dysfunction. Although gene therapies offer promise, even those already approved by regulatory agencies, their use remains constrained by mutation specificity, delivery challenges and durability. Pharmacologically targeting AMPK has shown potential to ameliorate dystrophic pathology, but prior strategies have been hindered by inadequate efficacy and off-target effects. METHODS: Comparative transcriptomic analyses were conducted to a

Basics in clinical sports nutrition: Physical activity, muscle and clinical nutrition.

Physical activity and nutrition are inextricably linked in maintaining muscle and overall health, enhancing metabolic resilience, and preventing age-related muscle loss. Exercise acts as a powerful physiological stressor, inducing acute metabolic, endocrine, and immunological responses, while long-term training elicits structural and functional adaptations at the muscular, cardiovascular, and systemic levels. Proper nutritional strategies, aligned with the type, intensity, and duration of physical activity, provide the necessary substrates to fuel exercise, optimize recovery, and support muscl

Attenuated lamin A-prohibitin2 interaction leads to mitochondrial dysfunction in LMNA 289 A>G-mediated dilated cardiomyopathy.

Lamins are critical in maintaining nuclear homeostasis, chromosome positioning, and modulating mechanotransduction. Recent studies indicated the involvement of lamin A in mitochondrial homeostasis and the regulation of superoxide. Missense mutations in LMNA are linked to a spectrum of diseases known as laminopathies, which include conditions, such as dilated cardiomyopathy (DCM), muscular dystrophy, and progeria. K97E is one such mutation, which leads to DCM with severe phenotypes. In this study, we established direct reduction of interaction between lamin A K97E and prohibitin 2. As a sequel,

[Digestive tract reconstruction after proximal gastrectomy-current applications and future perspectives of esophagogastrostomy].

In recent years, proximal gastrectomy (PG) has attracted increasing attention because it enables radical tumor resection while preserving gastric function and improving nutritional status and quality of life. However, how to effectively prevent reflux and anastomotic stricture during reconstruction remains a key challenge in functional reconstruction. This review summarizes the main types of gastroesophageal anastomosis after PG and categorizes the major reconstructive concepts into two groups. The first is the "residual stomach reshaping" approach, which rebuilds the anti-reflux structure by

Loss of Myonuclei and Transcriptional Activity During Diaphragm Atrophy in Critically Ill Patients.

BACKGROUND: Diaphragm weakness frequently develops in critically ill patients and is explained by a combination of atrophy and myofiber dysfunction. Myofibers are large syncytial cells maintained by a population of myonuclei, which provide gene transcripts to a finite fiber volume, termed the myonuclear domain. Myonuclear number is a determinant of transcriptional capacity and therefore critical for diaphragm and peripheral muscle regeneration after critical illness. Changes in myonuclear number in myofibers undergoing atrophy have not been investigated in mechanically ventilated ICU patients,

Colocalization and functional analyses identify GBE1 as a gene linking muscle strength and cardiometabolic fitness.

Handgrip strength is a proxy for muscular fitness, an indicator for general health status, and is associated with cardiometabolic health. The mechanisms connecting handgrip strength to skeletal muscle function are incompletely understood. We applied integrated linkage-disequilibrium-adjusted colocalization analysis of genome-wide association study summary statistics for handgrip strength, combined with expression and splicing quantitative trait loci from skeletal muscle, and identified glycogen branching enzyme 1 (GBE1) as a candidate gene for handgrip strength. CRISPR-interference knockdown o

Vacuolar myopathy caused by CASQ1 p.Asp244His: pathogenic evidence from two unrelated Chinese families.

Calsequestrin-1 (CASQ1)-related myopathy is a rare skeletal muscle disorder caused by mutations in CASQ1 gene, which encodes a major calcium-buffering protein of the sarcoplasmic reticulum (SR). It is characterized histopathologically by tubular aggregates or optically empty vacuoles, predominantly affecting type II muscle fibers. In this study, we report two unrelated Chinese patients presenting with late-onset, slowly progressive muscle weakness, fatigue, and myalgia. Both had mildly to moderately elevated serum creatine kinase levels. Muscle biopsies revealed typical optically empty vacuole

An inducible Flad1 knockout mouse model establishes its essential role in energy metabolism, muscle function and adult survival.

The FLAD1 gene encodes the key enzyme in the biosynthesis of flavin adenine dinucleotide (FAD). As an essential cofactor of various enzymatic reactions, FAD is potentially involved in a series of crucial biological processes. Mutations in the FLAD1 gene are implicated in the pathogenesis of severe metabolic disorders, including lipid storage myopathy (LSM) and multiple acyl-CoA dehydrogenase deficiency (MADD). However, the in vivo pathophysiological consequences of systemic FLAD1 deficiency remain poorly understood. To address this, we generated a tamoxifen-inducible Flad1 knockout mouse model

Update of guideline for diagnosis and treatment of subacromial pain syndrome: a multidisciplinary review by the Dutch Orthopedic Association Part 2: Operative considerations and treatment of various conditions related to subacromial pain syndrome.

BACKGROUND AND PURPOSE: In 2013, the first clinical practice guideline for subacromial pain syndrome (SAPS) was developed in the Netherlands to support healthcare professionals. SAPS refers to non-traumatic, non-rheumatologic shoulder complaints that are particularly painful during arm elevation. It includes conditions such as supraspinatus tendinosis, calcific tendinitis, and degenerative supraspinatus tears. Over 50,000 patients annually consult orthopedic surgeons for these issues. In response to new evidence and clinical needs, an updated guideline was developed. Part 2 focuses on supraspi

Erythematous rash and proximal muscle weakness in a 59-year-old woman: Delayed diagnosis of dermatomyositis in a patient with skin of color.

Dermatomyositis (DM) is a rare idiopathic inflammatory myopathy characterized by proximal muscle weakness and pathognomonic cutaneous manifestations. Recognition in patients with skin of color can be delayed due to atypical presentation of classic rashes. A 59-year-old woman with type 2 diabetes mellitus and hypertension presented with progressive weakness, dyspnea, palpitations, edema, and significant unintentional weight loss. Initial evaluation revealed elevated transaminases and troponin, and she was managed for suspected autoimmune myocarditis. On readmission, she had hyperpigmented and v

The role of CXCL10 in high-fat diet-induced skeletal muscle inflammation and atrophy.

AIMS/INTRODUCTION: Obesity is often accompanied by skeletal muscle atrophy, which aggravates insulin resistance and metabolic dysfunction. Chronic inflammation is implicated in this process, but the molecular mediators linking obesity-induced inflammation to muscle wasting have remained unclear. We investigated the role of the chemokine CXCL10 in skeletal muscle inflammation and atrophy induced by a high-fat diet (HFD). MATERIALS AND METHODS: Male C57BL/6J mice were fed an HFD or normal diet for 2 weeks and received either neutralizing antibodies to CXCL10 or control immunoglobulin G. Muscle

Erythropoietin alleviates syndrome-associated intellectual disability and autism-like behavior in Zbtb20-haploinsufficient Primrose syndrome mouse model.

Among the known genetic causes of syndromic autism spectrum disorders (ASDs) are transcription factor deficiencies. In this regard, haploinsufficiency of the zinc finger and broad complex, tramtrack, bric and brac domain-containing protein 20 (ZBTB20) leads to a prototypical clinical picture, referred to as Primrose syndrome, comprising severe ASD symptoms together with intellectual disability. Here, we present a comprehensive behavioral and phenotypical characterization of Zbtb20+/- mice, a construct valid model of this thus far untreatable human condition. Zbtb20+/- mice exhibited diminished

Rotator Cuff Impingement Damages the Organelles of Tenocytes, Resulting in Excessive Tenocyte Apoptosis and Tendinopathy.

BACKGROUND: Rotator cuff impingement syndrome is a prevalent cause of supraspinatus tendinopathy, resulting in considerable pain and functional impairment. Excessive apoptosis of tenocytes has been recognized as a critical pathological mechanism in both supraspinatus tendinopathy and rotator cuff tears. Furthermore, mitochondrial degeneration and endoplasmic reticulum (ER) stress-induced apoptosis are significant factors in chronic multisystem diseases. However, it remains unclear whether mitochondrial damage and ER stress can also cause excessive apoptosis of tendon cells and contribute to th

Clinical characterization of the proximal lower-limb ALS phenotype: a retrospective cohort study.

This study characterizes a rare phenotype of Amyotrophic Lateral Sclerosis (ALS) presenting with predominant proximal lower limb weakness at onset, a presentation often mimicking myopathy. We retrospectively reviewed 1980 patients, identifying 15 (0.75%) with this atypical onset. The majority were males (73%) with a median age of onset of 58.7 years. Approximately half presented with symmetric proximal lower limb weakness. Nine of the 11 tested patients had higher CK. Follow-up (median 53.7 months) revealed that 6 patients maintained isolated lower limb weakness for a median of 60.1 months, wh
