Pathogenesis of Shoulder Calcific Tendinopathy.

Shoulder calcific tendinopathy is a common condition affecting adults and is has a higher incidence in women. This condition is due to a multifactorial process and is characterized by the deposition of hydroxyapatite crystals in the rotator cuff tendons. The disease shows a phenotypic transformation of tenocytes into chondrocyte-like cells, likely caused by metabolic and inflammatory changes and mechanical stress. Risk factors promoting this pathology include hyperlipidemia, advanced age, diabetes, female gender, and thyroid dysfunction. Recent studies highlight that metalloproteinases, oxidat

Intensive Care Unit Acquired Weakness as a Modifiable Organ Dysfunction? A Narrative Review of Evolving Diagnostic and Therapeutic Concepts.

Intensive Care Unit Acquired Weakness (ICUAW) is a highly prevalent neuromuscular complication affecting around 40% of critically ill patients, rising to over 80% in high-risk cohorts. It is independently associated with prolonged mechanical ventilation, increased intensive care unit (ICU) and hospital length of stay, elevated mortality (in-hospital, 1-year, and 5-year), higher healthcare costs, and long-term functional impairment. ICUAW is clinically defined by symmetric flaccid tetraparesis, frequently involving respiratory muscles, and exhibits significant pathobiological heterogeneity. Fur

Modifying muscle metabolic dysregulation in inclusion body myositis with pioglitazone: a single-arm trial.

This single-arm, open-label phase 1 trial evaluated the PPARγ agonist pioglitazone in patients with inclusion body myositis (IBM). After a 16-week observation (lead-in) period, participants received pioglitazone 45 mg daily for 32 weeks. The primary outcome was the change in PPARGC1A expression and related metabolic pathways in muscle after 16 weeks of treatment compared with the lead-in period. Of the 16 enrolled participants, 13 initiated pioglitazone and completed at least one on-treatment assessment; the trial was terminated early due to the COVID-19 pandemic. At baseline, muscle metabolom

27-Hydroxycholesterol inhibits muscle cell viability via mitochondrial dysfunction: Protective role of ROS-induced HIF-1α.

The oxysterol 27-hydroxycholesterol (27OHC), which is widely distributed in various tissues and circulation, plays a notable role in pathological processes, such as including breast cancer, atherosclerosis, and neurodegenerative diseases. Although these processes are closely linked to muscle pathophysiology, the effects of 27OHC on metabolic changes associated with muscular atrophy and sarcopenia remain poorly understood. In this study, we demonstrated that 27OHC decreased skeletal muscle viability by activating pro-apoptotic signaling pathways. RNA sequencing revealed that 767 and 989 genes w

HMGB1: a key molecule linking chronic inflammation to complications in type 2 diabetes mellitus and a target for exercise intervention.

The pathological process of type 2 diabetes mellitus (T2DM) is closely associated with chronic low-grade inflammation. High mobility group box 1 (HMGB1), a key damage-associated molecular pattern (DAMP), is frequently dysregulated in T2DM and is implicated in promoting insulin resistance (IR), β cell dysfunction, and the progression of multiple complications-including cardiovascular disease, nephropathy, cognitive impairment, myopathy, and dyslipidemia-primarily through activating signaling pathways such as RAGE/TLR4-NF-κB. Exercise, a cornerstone non-pharmacological intervention, effectively

Beyond breastfeeding: a One Health Decalogue for nurturing the infant microbiota.

BACKGROUND: Early-life nutrition is a key determinant of infant gut microbiota development, immune maturation, and long-term health outcomes. Although breastfeeding is widely recognized as the optimal feeding strategy, many mothers are unable to breastfeed, underscoring the need for practical, evidence-based guidance to support infant health beyond breastfeeding. A One Health approach enables the integration of nutritional, microbial, clinical, environmental, and socio-cultural factors that influence maternal-infant dyads. METHODS: A narrative review of the literature was conducted using PubM

Mapping lung function in late-onset Pompe disease using label-free functional MRI.

Pompe disease is a life-limiting metabolic myopathy characterized by proximal muscle weakness. In late-onset Pompe disease (LOPD), respiratory failure due to respiratory muscle weakness is the leading cause of death. Monitoring relies on spirometry, which is often not feasible in pediatric or severely affected patients, highlighting the need for effort-independent assessments. Phase-resolved functional lung (PREFUL) MRI enables non-invasive, label-free evaluation of lung function during free breathing without active cooperation. In this prospective pilot study, ten LOPD patients and ten age- a

KIF5A downregulation in spinal muscular atrophy links axonal regeneration defects with ALS.

Spinal muscular atrophy (SMA) is a devastating neuromuscular disorder caused by mutations in the survival motor neuron 1 (SMN1) gene leading to decreased SMN protein levels and motor neuron dysfunction. SMN-restoring therapies offer clinical benefit, but the downstream molecular consequences of SMN reduction remain incompletely understood. SMN deficiency resulted in downregulation of kinesin heavy chain isoform 5A (KIF5A) in human neurons and in a mouse model of SMA. SMN associated with KIF5A mRNA and contributed to its stability. Reduced SMN levels impaired axon regeneration, which was rescue

Analysis of a serious adverse reaction of rhabdomyolysis precipitated by a possible interaction of methylprednisolone with atorvastatin: a case report.

A man in his 80s had been taking atorvastatin for 2 months for hyperlipidaemia. Due to the recurrence of nephrotic syndrome, methylprednisolone, torasemide, benidipine and atorvastatin were prescribed after admission to hospital. While his dyspnoea and oedema improved with diuretic therapy, the patient reported significant muscle soreness 8 days after starting the combined atorvastatin and methylprednisolone. Simultaneously, creatine kinase and myoglobin levels increased significantly. The temporal correlation between the drug combination and symptom onset, coupled with rapid clinical and bioc

Exercise in a Pill: Emerging Therapeutics for Age-Related Orthopedic Diseases.

SIGNIFICANCE: The global aging population has led to an increasing burden of age-related orthopedic diseases, seriously imposing social and economic pressures. Although exercise has been proven to have a protective effect on the musculoskeletal system, diseases themselves often restrict patients from engaging in physical activity. Exercise mimetics, as a pharmacological intervention that can simulate or enhance the benefits of exercise, offer new treatment options for such patients. RECENT ADVANCES: Exercise mimetics modulate oxidative stress, inflammatory responses, and apoptosis to enhance

Beyond local injury: pathogenic roles of metabolic, endocrine, and aging-associated systemic factors in tendinopathy and novel therapeutic strategies.

Tendinopathy, characterized by persistent pain and impaired function, is a globally prevalent condition imposing heavy socioeconomic burdens. Long regarded as an isolated local lesion, it is increasingly recognized as a regional manifestation of systemic dysregulation driven by the metabolic-endocrine-aging axis-not merely a peripheral "background factor." This review systematically analyzes four key pathogenic pathways: hyperglycemia-induced advanced glycation end-product (AGE) deposition, dyslipidemia-driven metabolic inflammation, sex hormone fluctuation-mediated collagen homeostasis disrup

UNLABELLED: We evaluated fracture and osteoporosis prevalence in a large cohort of children with DMD undergoing routine spine imaging. Osteoporosis is common and worsens with age and glucocorticoid use. Low total body bone mineral density may herald clinically detectable osteoporosis. Our findings emphasize the need for better detection and prevention. INTRODUCTION: Osteoporosis is a major health concern in individuals with Duchenne muscular dystrophy (DMD) due to long-term glucocorticoid (GC) therapy, muscle weakness and restricted mobility. Osteoporosis results in long bone and vertebral fr
