Hybrid pin penetration open stent technique: Retrospective study of a new approach to frozen elephant trunk procedure.

Total arch replacement using the frozen elephant trunk technique for acute type A aortic dissection involving the aortic arch remains technically demanding because of complex supra-aortic branch reconstruction and prolonged circulatory arrest. We developed a modified fenestrated open stent technique, termed the hybrid pin-penetration open stent (HIPPOS) technique, to simplify branch reconstruction while minimizing supra-aortic vessel manipulation. We retrospectively reviewed 15 consecutive patients who underwent total arch replacement using HIPPOS between May 2024 and May 2026 at a single inst

Pruritus-dominant herpes zoster with concurrent contralateral upper limb pain in a patient with complete spinal cord injury: A case report.

CONTEXT: Sensory impairment after spinal cord injury (SCI) may complicate herpes zoster (HZ) recognition. FINDINGS: A 76-year-old man with complete T4* paraplegia developed pruritus and vesicles in the right T4 dermatome without eruption-site pain. The asterisk denotes adjustment for stroke-related left upper-limb weakness. Empiric intravenous acyclovir preceded dermatology assessment. Contralateral shoulder and hand pain developed the day after eruption and was absent on the final treatment day. CONCLUSION/CLINICAL RELEVANCE: Focal pruritus warrants skin examination. When HZ is strongly sus

Primary isolated thoracic vertebral hydatid disease with multicompartment extension: A case report.

Vertebral hydatid disease is a rare manifestation of cystic echinococcosis, accounting for less than 1% of all cases, while primary isolated vertebral involvement is exceptionally uncommon. We report the case of a 33-year-old woman presenting with progressive spastic paraplegia. Magnetic resonance imaging revealed a multilobulated left thoracic costovertebral lesion centered at the T6-T7 level, composed of multicystic and enhancing solid components. The lesion involved the T6 and T7 vertebral bodies and posterior elements, with extensive intraspinal, pleural, and paravertebral soft-tissue exte

Assist-as-Needed Hip Exoskeleton Control for Gait Asymmetry Correction via Human-in-the-Loop Optimization.

Gait asymmetry is a significant clinical characteristic of hemiplegic gait that most stroke survivors suffer, leading to limited mobility and long-term negative impacts on their quality of life. Although a variety of exoskeleton controls have been developed for robot-aided gait rehabilitation, little attention has been paid to correcting the gait asymmetry of stroke patients, and it remains challenging to properly share control between the exoskeleton and patients with partial motor control. In view of this, an assist-as-needed (AAN) hip exoskeleton control with human-in-the-loop optimization

Pharmacological basis of Suhexiang Wan in ischemic stroke: identification and functional characterization of piperlongumine as a brain-distributed bioactive constituent associated with STAT3-mediated blood-brain barrier protection.

ETHNOPHARMACOLOGICAL RELEVANCE: Suhexiang Wan (SHXW) is a classical traditional Chinese medicine (TCM) formula that has been traditionally used for stroke-related disorders, including sudden loss of consciousness, neurological dysfunction, and hemiplegia. Although modern pharmacological studies have suggested its neuroprotective and anti-inflammatory properties, the brain-distributed constituents potentially associated with its therapeutic effects in ischemic stroke (IS) and the underlying molecular mechanisms remain unclear. AIM OF THE STUDY: This study aimed to elucidate the pharmacological

Community-Acquired Klebsiella pneumoniae Meningitis With Liver Abscess and Neurological Sequelae in an Immunocompetent Adult: A Case Report.

Klebsiella pneumoniae meningitis (KPM) is a rare but severe form of community-acquired bacterial meningitis that is often associated with high mortality and neurological sequelae. We report a 46-year-old previously healthy male who presented with acute altered mental status and high-grade fever. Cerebrospinal fluid analysis was consistent with bacterial meningitis, and cultures from the cerebrospinal fluid and blood showed Klebsiella pneumoniae (K. pneumoniae) growth. The clinical course of the patient was complicated with seizures and acute right-sided hemiplegia. Persistent fever (despite ap

Case Report: Fatal varicella-zoster virus meningoencephalitis with Froin-like cerebrospinal fluid and hydrocephalus following pembrolizumab-containing chemoimmunotherapy.

BACKGROUND: In patients receiving immune checkpoint inhibitors (ICIs), central nervous system (CNS) infections may mimic or coexist with neurological immune-related adverse events (irAEs). ICI-driven immune dysregulation has been proposed to amplify inflammation against latent pathogens ("immunotherapy infections due to dysregulated immunity", ITI-DI), but this remains a hypothesis-generating framework rather than a validated diagnosis. CASE PRESENTATION: A 54-year-old man with stage IIIC lung adenocarcinoma developed a herpes zoster eruption and lumbosacral pain 7 days after the first infusi

CNS Neoscytalidium dimidiatum: A Rare Cause of Fungal Brain Abscess.

Neoscytalidium dimidiatum is a dematiaceous fungus that primarily affects plants but can rarely act as an opportunistic pathogen in humans, particularly in immunocompromised patients. It typically causes superficial skin and nail infections in tropical regions; however, central nervous system (CNS) infections are extremely rare. The neurotropism of this fungus is not well understood, but hematogenous spread from cutaneous or pulmonary foci has been hypothesized. A 60-year-old male with chronic lymphocytic leukemia (CLL) treated with acalabrutinib, a second-generation Bruton's tyrosine kinase (

Using the behaviour change wheel to co-design an intervention targeting sedentary behaviour in individuals with spinal cord injury.

ObjectiveThere is a lack of interventions targeting sedentary behaviour in individuals with spinal cord injury. Interventions designed for non-disabled individuals are unlikely to be generalisable to wheelchair-users with paraplegia, necessitating a tailored approach. This study aimed to co-design an intervention targeting sedentary behaviour using the Behaviour Change Wheel for manual wheelchair-users with paraplegia.DesignAn iterative co-design workshop approach was employed following the Behaviour Change Wheel. Initial workshops focused on identifying barriers and facilitators to reducing a

Double trouble in hereditary spastic paraplegia: revisiting the classical inheritance model.

Hereditary spastic paraplegia (HSP) comprises a genetically heterogeneous group of neurodegenerative disorders characterized by progressive lower-limb spasticity and weakness. Although HSP genes have traditionally been classified according to a single mode of inheritance, increasing evidence indicates that several genes can cause disease through both autosomal dominant and autosomal recessive inheritance, complicating molecular diagnosis, variant interpretation, and genetic counseling. Here, we present the first comprehensive review of dual inheritance in HSP by systematically evaluating the p

Imaging-Enhanced Prediction of Upper Limb Motor Function After Stroke Using Automated Segmentation and Cluster Analysis.

BACKGROUND: Stroke is a leading cause of long-term disability, and early prediction of motor outcomes is critical for guiding individualized rehabilitation strategies. Although neuroimaging offers prognostic insights, its clinical integration remains limited. This study aimed to evaluate whether explainable imaging-derived features can improve the prediction of upper limb motor impairment and to identify distinct recovery profiles through cluster analysis. METHODS: We retrospectively analyzed 242 patients with acute ischemic stroke who underwent brain magnetic resonance imaging and motor func

Tuberculous spinal epidural abscess: a cautionary case report of permanent paraplegia due to delayed diagnosis and management.

BACKGROUND: Spinal epidural abscess caused by tuberculosis is a rare but serious complication of spinal tuberculosis. Its atypical presentation predisposes to diagnostic delay and a poor prognosis. This case not only documents the clinical and radiologic evolution of tuberculous spinal epidural abscess but also suggests that delays in diagnosis and treatment may jointly contribute to serious neurological outcomes. CASE PRESENTATION: A 39-year-old man initially underwent acupotomy for recurrent low back pain with recent exacerbation. Subsequent imaging revealed a paraspinal abscess within the

Transcutaneous spinal cord stimulation for functional recovery in individuals with hereditary spastic paraplegia: Protocol for a pilot clinical trial.

BACKGROUND: Hereditary spastic paraplegia (HSP) is a rare genetic neurodegenerative disorder characterized by progressive lower-extremity spasticity, weakness, and gait impairment resulting from corticospinal tract degeneration. With no disease-modifying therapies available, novel strategies to improve motor function are needed. Transcutaneous spinal cord stimulation (tSCS) is a non-invasive neuromodulation approach that may improve motor function but has not been prospectively evaluated in HSP. This study will assess the safety, feasibility, and preliminary efficacy of repeated tSCS in adults

Innovation and practices on case-based genetics teaching focusing on hereditary spastic paraplegia.

Medical genetics serves as a bridge between basic medical science and clinical courses. With its complex curriculum and abstract concepts, traditional lecture-based teaching often leads to difficulties in comprehension, diminishes students' interest in learning, and results in poor teaching outcomes. Incorporating typical case studies into instruction can make genetic theories more tangible, enhance students' intuitive understanding, facilitate their grasp of abstract concepts, and boost their interest in learning. Hereditary spastic paraplegia (HSP) is a group of hereditary neurological disor
