The human brain is an energetically demanding organ. Neurons have traditionally been considered to rely predominantly on glucose to sustain neuronal activity under normal physiological conditions. However, recent advances challenge this long-standing view, demonstrating that neurons can oxidise endogenous and exogenous long-chain fatty acids through mitochondrial β-oxidation to create energy. In this mini-review, we discuss the emerging role of neuronal fatty acid β-oxidation and how fatty acid metabolism is integrated with glycolysis, lipid storages, and mitochondrial energy production. We fu

Spinal cord injury after cardiac arrest (CA) can cause devastating neurological deficits, including paraplegia. Although lumbar spinal cord injury is clinically important, regional comparisons remain limited. In this study, we examined region-associated vulnerability of the thoracic and lumbar spinal cord using a rat model of asphyxial CA (ACA). Adult male Sprague-Dawley rats underwent sham surgery or ACA, and thoracic and lumbar spinal cord tissues were collected 24 h after return of spontaneous circulation (ROSC) from ACA survivors with hindlimb paralysis. Histopathological analysis demonstr

The current study aimed to present a diagnostically challenging case of neurosyphilis manifesting with atypical neurological symptoms, emphasizing the importance of clinical vigilance and advanced diagnostics in avoiding misdiagnosis and inappropriate intervention. In the present study, a case of neurosyphilis, initially misdiagnosed and treated as acute cerebral infarction, is reported. Diagnostic analyses included serial brain magnetic resonance imaging (MRI), continuous electroencephalography (EEG) monitoring and metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) for

INTRODUCTION: Cauda equina syndrome (CES) is a rare but devastating spinal surgical emergency, most commonly caused by lumbar disc herniation. Delayed diagnosis or decompression may result in irreversible neurological and sphincter dysfunction.
MATERIALS AND METHODS: This retrospective single-Center case series included eight consecutive patients with acute CES who underwent emergency surgical decompression over 1 week at a tertiary spine care center. Clinical presentation, radiological findings, surgical management, and 6-month post-operative outcomes were evaluated.
RESULTS: The cohort inc

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

BACKGROUND: A single recurrent variant, NM_004974.4(KCNA2):c.881G>A:p.(Arg294His), causes hereditary spastic paraplegia (HSP) through a unique variant-specific mechanism. Data from paralogous variants suggest that this may be a druggable mechanism.
OBJECTIVE: The objective of this work is to give an overview of the clinical spectrum and course of KCNA2-p.(Arg294His)-associated HSP (HSP-KCNA2) and report treatment with eplerenone, a potassium-sparing diuretic.
METHODS: In this retrospective study, we recruited individuals with the variant of interest through international rare disease network

BACKGROUND: Developmental and epileptic encephalopathies (DEEs) are severe, early-onset, frequently drug-resistant epilepsies in which recurrent seizures and epileptiform activity contribute to cognitive impairment beyond the underlying etiology. Ketogenic dietary therapies (KDTs) exert broad, multimodal antiseizure and neuroprotective effects and occupy a distinct position among nonpharmacologic treatments for DEEs.
OBJECTIVE: To synthesize the evidence for KDT across the principal electroclinical DEE syndromes, an expanding spectrum of gene-specific DEEs, and acute presentations such as feb

OBJECTIVE: To investigate the effects of thoracic kinesiology taping (KT) on pulmonary function, respiratory muscle strength, and functional independence in individuals with chronic post-stroke hemiplegia.
METHODS: 30 patients were randomly assigned to a thoracic KT or sham taping group. Both groups received conventional rehabilitation 5 days/week for 4 weeks. The KT group received thoracic taping with 50-75% tension, whereas the sham group received an identical application without tension. Outcomes were assessed at baseline and post-intervention, and included forced vital capacity (FVC), for

INTRODUCTION: This systematic review aimed to investigate neuraxial administration errors, examine their contributing factors, and evaluate clinical outcomes using a novel harm scale.
METHODS: The author searched PubMed, Web of Science, Scopus, EBSCOhost, and Google Scholar for reports from 1981 to 2025 in any language. Exclusions were reviews, overdose incidents, and non-neuraxial administrations. Harm was classified using a novel five-level scale, and contributing factors were identified using the Human and Systemic Factors Analysis Classification System (HFACS).
RESULTS: The search reveal

BACKGROUND: Hereditary spastic paraplegia (HSP) is a group of rare, genetically heterogeneous neurodegenerative conditions characterized by progressive lower limb spasticity. In children, rehabilitation management of HSP is commonly extrapolated from cerebral palsy practice, despite differences in etiology and disease progression. There is limited condition-specific evidence to guide rehabilitation clinical decision-making in pediatric HSP.
METHODS: A retrospective cohort study of children with HSP attending a tertiary hospital rehabilitation service (May 2018 - August 2024). Children were cl

PRIMARY OBJECTIVE: Zolpidem paradoxically restores behavioral responsiveness and communication in select patients with severe disorders of consciousness and profound motor deafferentation. While the anterior forebrain mesocircuit hypothesis attributes this awakening to central thalamic disinhibition, macroscopic network dynamics and lower brainstem gating remain under-characterized in locked-in syndrome (LIS).
METHODS AND PROCEDURES: A 45-year-old male with ventral pontine necrosis initially misdiagnosed as vegetative state/unresponsive wakefulness syndrome (VS/UWS) was formally identified wi

BACKGROUND: Sturge-Weber syndrome (SWS) is a rare congenital neurocutaneous disorder. Timely diagnosis and management remain challenging, particularly when intracranial hemorrhage (IH) occurs as a rare but serious complication.
CASE PRESENTATION: A young child developed IH after minor head trauma and presented with irritability, vomiting, gait instability, and lethargy. His symptoms progressed to recurrent focal motor seizures, status epilepticus, and hemiplegia. Initial CT showed a hematoma in the left frontal lobe, but SWS was not recognized immediately. [Purpose] Individuals with chronic stroke often exhibit low physical activity and prolonged sedentary behavior, possibly due to muscle weakness and impaired postural control. This study investigated associations between trunk and lower limb muscle thickness and physical activity, including sedentary behavior, in ambulatory individuals with chronic post-stroke hemiplegia. [Participants and Methods] Twenty-six ambulatory individuals with chronic post-stroke hemiplegia were included. Physical activity was assessed for 7 consecutive days using a triaxial accelerometer and categorized as sedentary

Spontaneous dissolution of a large renal pelvic stone is extremely rare. We report the case of a 65-year-old man with paraplegia below Th6 who experienced recurrent febrile urinary tract infections beginning in May 2016. Noncontrast computed tomography (CT) in August 2016 revealed a right renal pelvic stone measuring 26 × 13 mm. During long-term follow-up, serial CT imaging demonstrated structural homogenization, central cavitation, progressive rim thinning associated with intrastone gas, and complete disappearance by November 2020. CT attenuation analysis suggested a uric acid-dominant core s

Pathogenic variants in ATP1A3 are classically associated with alternating hemiplegia of childhood, CAPOS syndrome, and rapid-onset dystonia-parkinsonism. However, the phenotypic spectrum of ATP1A3-related disease has expanded considerably in recent years, including atypical presentations with spasticity and hereditary spastic paraplegia-like phenotypes. We describe a 43-year-old woman who developed a progressive upper motor neuron syndrome characterized by spastic paraparesis, generalized hyperreflexia, bilateral Babinski signs, inexhaustible ankle clonus, spastic dysarthria, dysphagia, and mi

BACKGROUND: Spinal epidural abscess (SEA) may cause rapidly progressive neurological deterioration and requires urgent decompression when severe spinal cord compression is present. Unilateral biportal endoscopy (UBE) has been increasingly applied to spinal infection, but its use for extensive thoracic SEA has rarely been reported.
CASE DESCRIPTION: A 49-year-old female with poorly controlled type 2 diabetes mellitus presented with severe thoracic back pain and the acute onset of paraplegia. Thoracic magnetic resonance demonstrated a posterior epidural abscess extending from T7 to T11 resultin
