Subtle hypometabolism on [18F]FDG-PET, yet absent [123I]FP-CIT SPECT binding in a patient with spastic paraplegia type 7.

Nuclear medicine neuroimaging is becoming increasingly important for the diagnosis and differential diagnosis of Parkinson's disease, atypical parkinsonian syndromes and more rare neurological (movement) disorders. We report on a 46-year-old woman presented with frequent falls, a gradually progressive bipyramidal syndrome with gait ataxia, dysphagia, urinary urgency and -incontinence, low mood with disturbed emotional regulation and a mild asymmetric hypokinetic rigid syndrome. Additionally, she had a blurry vision, asymmetrical ptosis and abnormal vertical saccades. [18F]FDG PET/CT of the bra

Percutaneous Endoscopic Interlaminar Lumbar Discectomy for Treating Acute Cauda Equina Syndrome: A Case Series of 25 Patients.

BACKGROUND: Cauda equina syndrome (CES) is caused by compression of nerves in the lumbar spine below the conus medullaris. Symptoms include progressive weakness in the lower extremities, back pain with sciatica, saddle anesthesia, and urinary or bowel dysfunction. These symptoms vary in intensity and onset, and can sometimes be an emergency. The standard surgical procedure for CES typically involves a microscope-assisted interlaminar decompression with laminotomy and sequestrectomy, with or without partial, rather than complete, discectomy. However, endoscopic spine surgery was introduced as a

Alternating Hemiplegia of Childhood: Development of a Disease-Specific Clinical Rating Scale.

BACKGROUND AND OBJECTIVES: Alternating hemiplegia of childhood (AHC) is a rare neurodevelopmental disorder, primarily caused by pathogenic variants in ATP1A3 that account for approximately 80% of cases. It is characterized by both paroxysmal and chronic neurologic features. Currently, no validated, disease-specific clinical outcome measures exist for AHC, limiting standardized assessment and longitudinal tracking. We aimed to develop and validate the AHC Rating Scale (AHC-RS), a tool applicable across all age groups. We hypothesized that the AHC-RS would demonstrate strong inter-rater reliabil

Case Report: Successful treatment of mixed-phenotype acute leukemia with cauda equina syndrome as the initial manifestation.

BACKGROUND: Mixed-phenotype acute leukemia (MPAL) has a poor prognosis, and initial presentation with cauda equina syndrome (CES) is extremely rare. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) may be the only potentially curative modality. To our knowledge, this is the first reported case of MPAL presenting with CES at disease onset. METHODS: We reviewed the clinical data of a 21-year-old man with high-risk myeloid/B-lymphoid MPAL presenting with CES as the initial manifestation. The patient initially presented with lumbosacral radicular pain and lower limb numbness, which

Synergistic effects of action observation and object affordance on cortical excitability during BCI-triggered motor imagery in stroke: a randomized, within-subject, 2 × 2 factorial study protocol.

BACKGROUND: Brain-computer interfaces (BCI) using motor imagery (MI) offer promising rehabilitation potential for upper limb function in stroke survivors. Conventional paradigms rely on abstract visual feedback, which may lack ecological validity and could limit neural recruitment. Action observation (AO) and object affordance (OA) independently modulate motor cortical excitability via the mirror neuron system and canonical neural pathways. However, their potential synergistic effects in enhancing BCI performance have not been systematically investigated. We propose a novel "Phygital" interven

Synergistic Effects of Constraint-Induced Movement Therapy and Mirror Therapy on Neuroplasticity and Functional Recovery in Rats With Hemiplegic Cerebral Palsy.

IMPORTANCE: The mechanisms underlying the combined effects of constraint-induced movement therapy (CIMT) and mirror therapy (MT) on neuroplasticity in hemiplegic cerebral palsy (HCP) remain unclear. OBJECTIVE: This study investigated whether combined CIMT and MT improves functional recovery more than either therapy alone and examined accompanying motor cortical neuroplastic changes. DESIGN: HCP rats were randomly assigned to HCP, HCP+CIMT, HCP+MT, and HCP+CIMT+MT groups (n = 13 per group), with a separate control group. SETTING: The study was conducted at the Experimental Animal Center, Gua

Hereditary Congenital Cataract With Spastic Paraplegia Associated With a Missense PAX6 Variant (p.Arg26Trp).

To date, several genetic factors have been reported in cases presenting both congenital cataract (CC) and spastic paraplegia (SPG), but the overall genetic landscape of this condition remains insufficiently understood. Here, we describe the clinical, genetic, and functional findings in a single Japanese family presenting with hereditary CC and SPG. Affected individuals exhibited severe CC and mild SPG symptoms. Whole exome sequencing combined with stringent filtering revealed no pathogenic variants in the three known genes. Instead, we identified a heterozygous missense variant in PAX6 (NM_001

A rare case of hereditary spastic paraplegia with homozygous mutation in the ZFYVE26 gene.

Hereditary spastic paraplegia (HSP) refers to a group of rare, progressive neurodegenerative disorders characterized by spasticity and weakness predominantly affecting the lower limbs, with significant clinical and genetic heterogeneity. Clinically, HSP is classified into "pure" and "complicated" forms. SPG15 (ZFYVE26) is a rare autosomal recessive subtype typically presenting in childhood with cognitive decline, followed by the development of gait disturbances and spasticity, often accompanied by thinning of the corpus callosum. Here, we report the case of a 19-year-old female patient, born t

Understanding in conditions of neutrality: a closer look at its objects.

Studies in vitro have shown that phosphorylated translation initiation factor 2 alpha (TIF 2 alpha) may have several functions, including regulation of protein synthesis, control of cell death and procurement of resistance to oxidative stress in nerve cells. These properties may have implications in certain human neurodegenerative diseases, such as Alzheimer's disease (AD) and Creutzfeldt-Jakob's disease (CJD), in which oxidative stress appears to be involved in the process of neurodegeneration and neurone death. Single and double-labelling immunohistochemistry to phosphorylated TIF 2 alpha, p

Gut-liver axis mediates the ameliorative effects of North Hawthorn polysaccharide on liver inflammation and oxidative stress in hyperlipidemic mice via Nrf2 signaling pathway.

Elderly patients are at increased risk of developing lower respiratory tract infections compared with younger patients. In this population, pneumonia is a serious illness with high rates of hospitalisation and mortality, especially in patients requiring admission to intensive care units (ICUs). A wide range of pathogens may be involved depending on different settings of acquisition and patient's health status. Streptococcus pneumoniae is the most common bacterial isolate in community-acquired pneumonia, followed by Haemophilus influenzae, Moraxella catarrhalis and atypical pathogens such as Ch

Enhanced clinical trial stratification in spinal cord injury: the value of electrophysiology.

There are no approved interventional therapies, aside from neurorehabilitation, that enhance neurological recovery after acute traumatic spinal cord injury. A key challenge is the lack of biomarkers surpassing clinical standards for optimal stratification. We evaluated electrophysiological markers of preserved neuronal function to improve enrichment strategies over clinical measures. We hypothesized that participants with preserved electrophysiological markers would achieve greater neurological and functional recovery in response to a plasticity-inducing intervention. We conducted a retrospect

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Non-invasive closed-loop spinal stimulation restores leg stepping control in humans with paraplegia.

Gait disturbance in individuals with spinal cord injury (SCI) at levels rostral to the lumbar locomotor centre results from disconnection between the supraspinal system and the spinal locomotor centre. Here, we present a non-invasive volition-controlled spinal stimulation paradigm that empowers paraplegic individuals to regain stepping control in their impaired legs. Using hand muscle-controlled magnetic stimulation targeting the lumbar spinal motor circuits in the preserved lumber cord, individuals with chronic SCI achieved control of start-stop motion, step length and cadence of bilateral cy
