Associative learning in children with perinatal brain injury.

Associate learning for visual nonverbal and auditory verbal items was examined in 21 children with spastic diplegic cerebral palsy (SDCP) and 28 healthy children using four paired associate tasks. SDCP children showed poorer performance than the comparison group for learning pairs that required visual nonverbal responses, regardless of the stimulus modality. Within the SDCP group, lesion severity was assessed in 17 of the children. Lesion severity was related to the level of performance on paired associate tasks requiring visual nonverbal responses; lesion severity did not reach statistical si

Quantitative clinical measure of spasticity in children with cerebral palsy.

OBJECTIVE: This investigation developed an objective measure to quantify the degree of spasticity. DESIGN: Specifications included a single variable that integrated key elements characterizing spasticity: velocity, range of motion, and resistance to passive motion. A dynamometer at a children's hospital quantified the passive resistance of the hamstrings to knee extension for a range of motion at 4 different speeds for the prospective descriptive investigation. PATIENTS: A convenience sample of six children with able bodies and 17 children with spastic diplegic cerebral palsy volunteered. DA

Dyskinetic cerebral palsy: a clinical and genetic study.

The clinical features and family histories of 20 adults with dyskinetic cerebral palsy from 20 families were studied. The majority of the patients showed progressive neurological deterioration in adult life. In only three did the condition stabilise by 10 years of age and in seven there was deterioration after the age of 30. Two patients developed a secondary cervical spondylotic myelopathy. Four patients had affected relatives and there were similar proportions of affected parents and siblings. The family data suggest genetic heterogeneity with autosomal recessive and dominant variants. Self-Optimization of Walking in Nondisabled Children and Children With Spastic Hemiplegic Cerebral Palsy.

Children voluntarily adopt a frequency and movement pattern for walking. The force-driven harmonic oscillator (FDHO) model was used in this study for accurate prediction of the preferred walking frequency of nondisabled children and children with spastic hemiplegic cerebral palsy. Four potential optimality criteria with which the preferred walking pattern was forced to comply were examined: minimization of physiological costs, maximization of mechanical energy conservation, minimization of asymmetry in lower limb movements and minimization of variability of interlimb and intralimb coordination

Bone mineralization in the affected extremities of children with spastic hemiplegia.

To assess the impact of neurologic involvement on bone mineralization, dual-energy X-ray absorptiometry was used to quantitate bone mineral content (BMC), bone mineral density (BMD), fat and lean muscle mass in the limbs of 19 children with spastic hemiplegic cerebral palsy. The BMC in the affected limb was on average 26.5% lower (upper limbs) and 15.6% lower (lower limbs) than in the corresponding uninvolved limb. Lean muscle mass was reduced by 15% and BMD by 6% in the involved limbs and did not differ significantly between upper and lower limbs. The fat content of involved and uninvolved li

Role of disease in energy balance in children.

Nutritional stress can be caused by increased energy demands or inadequate caloric intake. In the absence of food shortage, disease is often a significant contributory factor in promoting nutritional stress. In children, the interaction of nutrition and disease is more difficult to assess than in adults because deficits in growth and development have both a determining and consequential role in defining energy needs. This paper presents a framework for examining the role of disease in energy balance in children and gives three examples of disease-energy balance interactions. Resting and total

Energy expenditure of children and adolescents with severe disabilities: a cerebral palsy model.

Spastic quadriplegic cerebral palsy (SQCP) is a severe disability that is associated with abnormal physical activity, body composition, and food intake and with frequent malnutrition. This study examined the pattern of dietary intake, anthropometry, and energy expenditure in a group of subjects with SQCP aged 2-18 y and a normal control group. The energy expenditure pattern was determined from resting energy expenditure (REE, n = 61 SQCP; n = 37 control group) by using indirect calorimetry and from total energy expenditure (TEE, n = 32 SQCP; n = 32 control group) by using the doubly labeled wa

Energy expenditure and body composition in children with spastic quadriplegic cerebral palsy.

OBJECTIVE: To determine the relationship between resting energy expenditure and body cell mass in a group of children with spastic quadriplegic cerebral palsy (SQCP) in comparison with a group of healthy volunteers. SUBJECTS AND METHODS: Children with SQCP (n = 13) and healthy control subjects (n = 21) participated in the study. Resting energy expenditure (REE) by indirect calorimetry, as well as body composition measurements were obtained. Those included skinfold measurements, isotope dilution methods for total body water and extracellular water (2H2O or H2(18)O and NaBr, respectively), and

Changes in cognitive performance in children with spastic diplegic cerebral palsy following selective dorsal rhizotomy.

Children with cerebral palsy who receive selective dorsal rhizotomy (SDR) for treatment of spasticity may show suprasegmental changes in upper limb function and control of speech musculature. Anecdotal reports suggest that supra-segmental effects may extend to cognitive functions such as attention and language. This study examined the performance of 16 children with spastic diplegic cerebral palsy on tests of visual attention and other cognitive functions 1 day prior to and 6 months following SDR. Children undergoing SDR were compared with 9 children with spastic diplegia, matched for severity

Working memory following improvements in articulation rate in children with cerebral palsy.

It has been postulated that rehearsal rate is the primary determinant of working memory capacity for verbal material (Baddeley et al., 1975). A previous study of normal control children and children with spastic diplegic cerebral palsy (SDCP) suggested that covert rather than overt rehearsal rate determines working memory capacity (White et al., 1994). In the current study, a subset of SDCP children who received a surgical treatment to relieve spasticity were retested on measures of articulation rate and memory span. A subset of control children from the original study were also retested. The

Cycling patterns in children with and without cerebral palsy.

Pedaling smoothness and electromyography patterns were quantified in children with spastic diplegic cerebral palsy (CP) and a cohort of children with typical development. Video analysis of the pedaling rhythm yielded equivalent time periods for the control group and irregular time periods in the group with CP, with greater time spent at the bottom of the pedaling cycle. Electromyography patterns of the tibialis anterior, rectus femoris, medial hamstring and lateral gastrocnemius muscle groups yielded greater percentages of muscle activity time and co-contraction time at both the ankle and knee

A case of dyskinetic cerebral palsy resembling post-anoxic action myoclonus.

Myoclonus after brain anoxia is more commonly observed in adults. Perinatal anoxia, however, on rare occasions causes myoclonus as the main neurologic abnormality. In this paper we described a case of cerebral palsy complicated by chorea and tremulous movements of the foot in which there were no risk factors except perinatal asphyxia. The patient had unique symptoms, action myoclonus associated with the movements of hyperkinésie volitionnelle. This case illustrated two points. One is that abnormal involuntary movements attributed to perinatal brain insults had changed as the brain matured. The

Changes in hip migration after selective dorsal rhizotomy for spastic quadriplegia in cerebral palsy.

Selective dorsal rhizotomy is increasingly used for management of spastic quadriplegic cerebral palsy but rates of hip stability following the operation have not been reported. Determining hip stability by radiographic measurement of lateral migration of the femoral head beyond a lateral edge of the acetabulum after dorsal rhizotomy allows an objective assessment of the outcome of the operation. This prospective study examined the effect of selective dorsal rhizotomy on lateral migration of the femoral head in 45 children with spastic quadriplegic cerebral palsy. The children ranged in age fro

Dyskinetic cerebral palsy and birth asphyxia.

Of 115 children with dyskinetic cerebral palsy (CP) in the Mersey region, 17 were born at term and at an appropriate weight for age, and have preserved cognitive abilities. 10 of these are likely to have sustained intrapartum asphyxial brain-damage as the cause of their CP. In such circumstances, a characteristic pattern is usually seen of severe fetal distress occurring late in labour, severe but short-lived birth asphyxia and only mild or moderate hypoxic-ischaemic encephalopathy. This clinical picture should be distinguished from the pattern of birth asphyxia which precedes the development

Temporomandibular joint contracture in spastic quadriplegia: effect on oral-motor skills.

This study assessed the temporomandibular joint of 34 children with spastic quadriplegic cerebral palsy for contractures to determine whether joint abnormalities were related to oral-motor dysfunction. Subjects were found to have restrictions in mandibular range of motion, increased overjet and decreased overbite. Increased overjet corresponds to retrognathia and decreased overbite relates to fixed posterior rotation of the mandible. The severity of many of these abnormalities correlated with the severity of oral-motor dysfunction. Abnormalities in overbite were particularly significant. The a

Dose response of succinylcholine at the adductor pollicis of children with cerebral palsy during propofol and nitrous oxide anesthesia.

Children with cerebral palsy may be resistant to paralysis induced by nondepolarizing neuromuscular blocking drugs. Potency of a bolus of succinylcholine in children with cerebral palsy has not been studied previously. Therefore, we measured the response of the adductor pollicis to succinylcholine in children with cerebral palsy anesthetized with propofol and nitrous oxide. Forty children between the ages of 2 and 10.2 yr with spastic quadriplegic cerebral palsy were randomly assigned to receive 100, 175, 250, or 375 micrograms/kg of succinylcholine during anesthesia with propofol and nitrous

Gait parameters of children with spastic diplegia: a comparison of effects of posterior and anterior walkers.

The purpose of this study was to compare the effects of posterior and anterior walkers on the gait parameters of five children with spastic diplegic cerebral palsy. Computer-based kinematic analysis was used to investigate differences in gait. Gait laboratory data indicated that use of the posterior walker (1) facilitated more upright posture in these subjects, as seen in decreased trunk and hip flexion during stance phase; (2) decreased double stance time; and (3) increased walking velocity. According to parent report, both the parents and their children preferred the posterior walker. Most p

Treatment of a large infected thoracic spine wound using high voltage pulsed monophasic current.

This case report describes the use of electrical stimulation with high voltage pulsed monophasic current for treatment of a large, infected wound of the thoracic spine, following a surgical debridement procedure. The patient was a 21-year-old man with spastic quadriplegic cerebral palsy who was dependent for all self-care and was severely mentally retarded. The initial wound size was as follows: length = 17 cm, top width = 7.5 cm, middle width = 5.5 cm, bottom width = 2 cm, and depth = 5 cm. The wound was infected with Staphylococcus aureus. The initial treatment consisted of 60 minutes of ele
