The optimality concept developed by Prechtl was adopted to investigate a series of 108 dyskinetic cerebral palsy (CP) cases. The results were compared to those from a control series of 229 newborns. The dyskinetic cases, as compared to the controls, had significantly more reduced optimality scores. Reduced optimality was especially pronounced in the partum and postpartum periods, but was to a highly significant degree also found in the prepartum period. Dystonics and choreoathetotics differed in their pattern of reduced optimality. Dystonics had more reduced optimality in items related to defi

The optimality concept of Prechtl was adopted for the investigation of a series of 229 newborn infants, selected by paired sampling as controls to 116 dyskinetic cerebral palsy cases. A list of 34 conditions, subgrouped into 14 prepartum, 10 partum and 10 postpartum items, was designed and criterias for optimality were defined. When a condition fell short of criteria for optimality, the infant concerned was given a non-optimal point. The sum of non-optimal points for each infant was defined as the reduced optimality score and the proportion of non-optimal points as the reduced optimality rate.

Maple syrup urine disease (MSUD) is a rare heritable enzyme defect associated with mental retardation. A diet deficient in the branched-chain amino acids is essential for survival. Patients with MSUD are at risk of ketoacidotic metabolic crises brought on by catabolic states, including simple infection or fasting. Delayed diagnosis and therapy can predispose these patients to loss of the gag reflex, lethargy, seizures, and feeding problems. Ultimately, this may result in aspiration and respiratory arrest, which, in turn, can cause cerebral palsy. Of seven cases of MSUD reviewed at The Hospital

In an unselected series of 116 dyskinetic cerebral palsy cases, 35 hyperkinetic and 81 dystonic, 21% of the cases were considered to derive from the prenatal period, 67% from the perinatal, and 7% from the postnatal period, while 5% were untraceable. Isolated risk factors were found in 24% of the hyperkinetic and 24% of the dystonic cases and various combinations in 68% and 72%, respectively. Preterm appropriate-for-gestational-age cases with hyperbilirubinemia were found significantly more often among the hyperkinetics (47%) than among the dystonics (11%). Term appropriate-for-gestational-age

An unselected series of 116 dyskinetic cerebral palsy cases born 1959-70 was delineated and subgrouped according to neurological criteria into 35 hyperkinetic (30%) and 81 dystonic (70%) cases. Of the hyperkinetics, 80% had a mild motor disability, while 90% of the dystonics had moderate or severe disability. Subordinated spastic signs were found in 9% of the hyperkinetic and in 44% of the dystonic cases. IQ was greater than 90 in 69% of the hyperkinetics and 25% of the dystonics and less than 50 in 11% and 41%, respectively. Two or more additional neurological abnormalities (spasticity, menta

The purpose of this study was to determine the consistency of reflex responses of subjects in varying behavioral states. The following selected primitive reflexes were examined in children with spastic quadriplegic cerebral palsy: tonic labyrinthine, asymmetrical tonic neck, symmetrical tonic neck, crossed extension, and Moro. Our hypothesis was that a subject's reflex responses would vary as a function of his behavioral state. Ten children, ages 3 to 6 years, participated in the study. Reflex responses were elicited using a specified protocol. A behavioral-state scale was defined, and the sub

The article reviews the collected knowledge concerning the non-progressive syndromes of ataxic cerebral palsy. Three syndromes are described: (1) simple ataxia (signs of dyssynergia); (2) ataxic diplegia (ataxia with added diplegic spasticity); and (3) dysequilibrium syndrome (defective postural control and equilibrium). The probable aetiological factors in each syndrome are discussed. In simple ataxia prenatal factors dominate; a high proportion of cases have an untraceable aetiology. Genetic factors play a large rôle, and birthweight is usually normal. With ataxic diplegia the aetiology is a

Electromyographic feedback training may play a special role in increasing active range of motion in spastic cerebral palsied individuals. The effects of electromyographic feedback training were documented on four persons with spastic diplegic cerebral palsy within a single subject design. Subjects received immediate auditory and visual feedback twice daily during ten consecutive days. Electromyographic feedback training resulted in increases in the active range of motion of the ankle joint of from 20 percent to 500 percent in the limbs trained during the first five days with electromyographic

Twenty-three ambulatory children with spastic diplegic cerebral palsy were evaluated clinically and by electromyography before and after hip-muscle surgery. The stretch tests originally designed to distinguish specific muscle tightness and spasticity were found to be non-specific when tested by electromyography. Ambulatory electromyograms using needle electrodes and telemetry generally showed decreased activity in the released muscles and, on occasion, changes in activity in muscles not operated on. These unanticipated changes after release may explain some of the unpredictability of results o
