The relation between standing balance and walking function in children with spastic diplegic cerebral palsy.

To establish and compare the relationship between standing balance and walking performance, eight children with spastic diplegic cerebral palsy (CP) and 16 non-disabled, age- and sex-matched children were studied. The results showed that the children with CP had worse static balance stability in various sensory environments and dynamic balance (rhythmic shifting ability) than the non-disabled children. Moreover, the children with CP walked at a slower speed but at a greater physiological cost than the non-disabled children. In the children with CP, dynamic balance significantly correlated with

A randomized clinical trial to compare selective posterior rhizotomy plus physiotherapy with physiotherapy alone in children with spastic diplegic cerebral palsy.

A randomized controlled single-blind trial was performed to compare lumbo-sacral selective posterior rhizotomy (SPR) followed by intensive physiotherapy, with intensive physiotherapy alone in improving motor function in children with spastic diplegic cerebral palsy. Fifteen patients were randomly assigned to each treatment modality. Patients in the SPR group had rhizotomy within 1 month, followed by intensive outpatient physiotherapy for 9 months. Patients assigned to physiotherapy alone had identical intensive physiotherapy. There was a statistically significant and clinically important diffe

Therapeutic electrical stimulation following selective posterior rhizotomy in children with spastic diplegic cerebral palsy: a randomized clinical trial.

A randomized controlled trial was carried out to determine the effectiveness of therapeutic electrical stimulation (TES) in improving the function of children with spastic cerebral palsy (CP), who had undergone selective posterior lumbosacral rhizotomy more than a year previously. Children were randomly assigned to groups to receive TES for 1 year, or to have no TES. The primary outcome was the change in the Gross Motor Function Measure (GMFM), a quantitative and validated measure for use in children with spastic CP. There was a statistically significant and clinically important improvement in

Associative learning in children with perinatal brain injury.

Associate learning for visual nonverbal and auditory verbal items was examined in 21 children with spastic diplegic cerebral palsy (SDCP) and 28 healthy children using four paired associate tasks. SDCP children showed poorer performance than the comparison group for learning pairs that required visual nonverbal responses, regardless of the stimulus modality. Within the SDCP group, lesion severity was assessed in 17 of the children. Lesion severity was related to the level of performance on paired associate tasks requiring visual nonverbal responses; lesion severity did not reach statistical si

Quantitative clinical measure of spasticity in children with cerebral palsy.

OBJECTIVE: This investigation developed an objective measure to quantify the degree of spasticity. DESIGN: Specifications included a single variable that integrated key elements characterizing spasticity: velocity, range of motion, and resistance to passive motion. A dynamometer at a children's hospital quantified the passive resistance of the hamstrings to knee extension for a range of motion at 4 different speeds for the prospective descriptive investigation. PATIENTS: A convenience sample of six children with able bodies and 17 children with spastic diplegic cerebral palsy volunteered. DA

Changes in cognitive performance in children with spastic diplegic cerebral palsy following selective dorsal rhizotomy.

Children with cerebral palsy who receive selective dorsal rhizotomy (SDR) for treatment of spasticity may show suprasegmental changes in upper limb function and control of speech musculature. Anecdotal reports suggest that supra-segmental effects may extend to cognitive functions such as attention and language. This study examined the performance of 16 children with spastic diplegic cerebral palsy on tests of visual attention and other cognitive functions 1 day prior to and 6 months following SDR. Children undergoing SDR were compared with 9 children with spastic diplegia, matched for severity

Working memory following improvements in articulation rate in children with cerebral palsy.

It has been postulated that rehearsal rate is the primary determinant of working memory capacity for verbal material (Baddeley et al., 1975). A previous study of normal control children and children with spastic diplegic cerebral palsy (SDCP) suggested that covert rather than overt rehearsal rate determines working memory capacity (White et al., 1994). In the current study, a subset of SDCP children who received a surgical treatment to relieve spasticity were retested on measures of articulation rate and memory span. A subset of control children from the original study were also retested. The

Cycling patterns in children with and without cerebral palsy.

Pedaling smoothness and electromyography patterns were quantified in children with spastic diplegic cerebral palsy (CP) and a cohort of children with typical development. Video analysis of the pedaling rhythm yielded equivalent time periods for the control group and irregular time periods in the group with CP, with greater time spent at the bottom of the pedaling cycle. Electromyography patterns of the tibialis anterior, rectus femoris, medial hamstring and lateral gastrocnemius muscle groups yielded greater percentages of muscle activity time and co-contraction time at both the ankle and knee

Gait parameters of children with spastic diplegia: a comparison of effects of posterior and anterior walkers.

The purpose of this study was to compare the effects of posterior and anterior walkers on the gait parameters of five children with spastic diplegic cerebral palsy. Computer-based kinematic analysis was used to investigate differences in gait. Gait laboratory data indicated that use of the posterior walker (1) facilitated more upright posture in these subjects, as seen in decreased trunk and hip flexion during stance phase; (2) decreased double stance time; and (3) increased walking velocity. According to parent report, both the parents and their children preferred the posterior walker. Most p

Hereditary spastic paraplegia.

Hereditary spastic paraplegia (HSP) is a genetically determined condition that is often confused with, and misdiagnosed as, spastic diplegic cerebral palsy. Both autosomal dominant and recessive forms have been reported. We present three index patients in a single family with this disease. The pedigree of three generations strongly supports the genetic basis of an autosomal dominant condition. Awareness of this entity may help in genetic counseling and possibly avoiding some medical litigation related to cerebral palsy patients.

Ankle-foot orthoses: effect on energy expenditure of gait in spastic diplegic children.

This study examined the effect of conventional plastic bilateral ankle-foot orthoses (AFOs) on the energy expenditure of ambulation in spastic diplegic cerebral palsy patients. Eighteen subjects (aged 8.3 +/- 2.83 years) ambulated five minutes at freely chosen velocities with and without their AFOs. Heart rate (beats per minute) and distance (meters) ambulated were recorded for each minute. A physiologic cost index was calculated by determining the ratio of walking heart rate to velocity [(HRamb-HRrest) divided by meters per minute]. The mean physiologic cost indexes for the last three minutes

Rehabilitation management of children with spastic diplegic cerebral palsy.

Spastic diplegia has been recognized as the type of cerebral palsy most frequently associated with prematurity. Due to constantly improving neonatal care in developed countries, more and smaller premature infants are surviving, and the number of spastic diplegic children can be expected to increase. This paper reviews the incidence, pathophysiology, and associated handicaps of patients with this type of cerebral palsy. The role of the physiatrist and aspects of traditional management are discussed. Recent advances in treatment of spasticity and lower extremity bracing are stressed as they seem

The effect of positioning on the hand function of boys with cerebral palsy.

The effect of positioning (sitting and prone standing) on the hand function of 10 boys (mean age = 12.5 years, SD = 1.2 years) with spastic diplegic cerebral palsy was studied. Two groups of subjects were tested twice (Tests 1 and 2) with the Jebsen-Taylor Hand Function Test (Jebsen, Taylor, Treischmann, Trotter, & Howard, 1969; Taylor, Sand, & Jebsen, 1973) to measure rate of manipulation. In addition, a scale modified from Hohlstein (1982) was used to measure quality of grasp on each subtest of the Jebsen-Taylor test. No significant differences between the mean scores of the two groups were

Cerebral palsy: postural-locomotor prognosis in spastic diplegia.

Prognosis of locomotion in children with cerebral palsy (CP) is complicated by the variety of motor and other neurologic deficits in each case. Children presenting with spastic diplegic (SD) CP related to prematurity generally demonstrate more uniform and comparable neuromotor abnormalities. In this study, longitudinal data were collected on the early postural-locomotor development of children with spastic diplegic cerebral palsy (SDCP). The data were then correlated with the level of ambulatory function attained. Fifty spastic pediatric patients, aged 5 to 19 years, were included in the study

Cerebral palsy associated with maple syrup urine disease.

Maple syrup urine disease (MSUD) is a rare heritable enzyme defect associated with mental retardation. A diet deficient in the branched-chain amino acids is essential for survival. Patients with MSUD are at risk of ketoacidotic metabolic crises brought on by catabolic states, including simple infection or fasting. Delayed diagnosis and therapy can predispose these patients to loss of the gag reflex, lethargy, seizures, and feeding problems. Ultimately, this may result in aspiration and respiratory arrest, which, in turn, can cause cerebral palsy. Of seven cases of MSUD reviewed at The Hospital

Effects of electromyographic feedback training on motor control in spastic cerebral palsy.

Electromyographic feedback training may play a special role in increasing active range of motion in spastic cerebral palsied individuals. The effects of electromyographic feedback training were documented on four persons with spastic diplegic cerebral palsy within a single subject design. Subjects received immediate auditory and visual feedback twice daily during ten consecutive days. Electromyographic feedback training resulted in increases in the active range of motion of the ankle joint of from 20 percent to 500 percent in the limbs trained during the first five days with electromyographic

Electromyography before and after surgery for hip deformity in children with cerebral palsy. A comparison of clinical and electromyographic findings.

Twenty-three ambulatory children with spastic diplegic cerebral palsy were evaluated clinically and by electromyography before and after hip-muscle surgery. The stretch tests originally designed to distinguish specific muscle tightness and spasticity were found to be non-specific when tested by electromyography. Ambulatory electromyograms using needle electrodes and telemetry generally showed decreased activity in the released muscles and, on occasion, changes in activity in muscles not operated on. These unanticipated changes after release may explain some of the unpredictability of results o
