Twenty-six nonoperated spastic hemiplegic children underwent three-dimensional kinetic gait analysis. We identified five patterns based on sagittal plane kinetics. Group I had a minimal gait disturbance, a drop foot pattern, and normal kinetics, Group II had a flexed knee but normal hip extension, Group III a flexed knee and hip, Group IV knee hyperextension and tibial arrest, and Group V knee hyperextension and persistent ankle dorsiflexion. We evaluated 35 patients with cerebral palsy on the basis of MR imaging findings in the brain. The types of palsy were spastic quadriplegia (n = 11), spastic diplegia (n = 9), spastic hemiplegia (n = 2), double hemiplegia (n = 1), athetosis (n = 10) and mixed (n = 2). Fourteen children with spastic hemiplegia of various etiology: cerebral palsy (seven cases, five with porencephalic cyst); stroke with small deep infarcts (two); hemiconvulsion-hemiplegia-epilepsy syndrome (three); traumatic brain injury (two), were investigated by 99mTc-HMPAO SPECT. In this report we describe a male patient with the interesting combination of a large congenital scalp defect, distal limb reduction anomalies, right spastic hemiplegia and hypoplasia of the left arteria cerebri media. Follow-up data from birth up to the age of 18 years revealed positive evolution with low to normal intelligence. The findings in the present patient, i.e. an Adams-Oliver syndrome associated with a severe neurological deficit, are best explained as resulting from interruption of the early embryonic blood supply.

To assess the impact of neurologic involvement on bone mineralization, dual-energy X-ray absorptiometry was used to quantitate bone mineral content (BMC), bone mineral density (BMD), fat and lean muscle mass in the limbs of 19 children with spastic hemiplegic cerebral palsy. Four autopsy cases of pure motor hemiparesis due to medullary pyramid infarcts have been previously reported. The deficits that were described included overt limb weakness and "slight facial weakness". Hereditary protein S deficiency (HSPD) is a predisposing factor to recurrent venous thrombosis but is not currently associated with stroke. A 36 year old diabetic man developed a pure motor hemiplegia (PMH) associated with an ipsilateral lingual palsy. Magnetic resonance imaging revealed a pontine infarct. Lingual palsies have never been reported in patients with PMH so far, but may be associated with other lacunar syndromes such as the "dysarthria-clumsy hand syndrome". This observation supports the hypothesis that corticohypoglossal pathways may have bilateral and assymetrical projections.

Between 1.6.1991 and 31.5.1995, 62 patients underwent heart valve replacement with Sorin Bicarbon bileaflet prosthetic valve, age 16-83 years (mean 60.5). The valve disease was rheumatic in 37 cases, degenerative in 17, congenital in 4 and miscellaneous etiologies in the other 4.
