Sjögren's syndrome is a long-standing autoimmune condition that predominantly affects the body's exocrine glands, most notably the eyes and mouth, leading to ocular and oral dryness. Neurological involvement, particularly affecting the central nervous system, is infrequently encountered, and the occurrence of stroke as an initial clinical manifestation remains exceptionally uncommon. We report a 42-year-old diabetic male who presented with an acute onset of right hemiplegia and aphasia, and was subsequently diagnosed with primary Sjögren's syndrome. Evaluation revealed multiple acute infarcts

PurposeHemiplegia in the distal upper extremity (dUE) is a common post-stroke impairment that severely limits daily activities and functional recovery. Although upper-extremity rehabilitation typically focuses on unilateral training (UT), bilateral training (BT) is gaining recognition for its effectiveness in promoting post-stroke motor recovery. Few studies have characterized the effect of BT on dUE; therefore, the degree to which the effectiveness of BT can be predicted by clinical factors, such as patient characteristics and intervention settings, remains unclear. This study investigated th

Early rehabilitation following stroke has emerged as a pivotal strategy to accelerate recovery and enhance functional outcomes in hemiplegic patients. Despite supporting evidence, the integration of structured rehabilitation protocols in critical care settings remains inconsistent, particularly in resource-limited environments. This study aimed to evaluate the effectiveness of early rehabilitation therapy on stroke severity, cognitive function, and motor performance among hemiplegic stroke patients admitted to a critical care unit. A quasi-experimental design was employed. Fifty-two participan

Hemiplegic migraine is a rare subtype of migraine characterized by reversible hemiplegia/hemiparesis as an aura symptom. Visual, sensory, and/or speech symptoms typically accompany the motor aura, followed by throbbing headache. Because motor symptoms may last for up to several weeks, this unique clinical entity can markedly impair quality of life. Familial hemiplegic migraine is caused by autosomal dominant mutations in the CACNA1A, ATP1A2, SCN1A, and PRRT2 genes. These mutations are thought to induce neuronal hyperexcitability with resultant enhanced synaptic transmission and increased susce

Stroke is one of the leading causes of disability worldwide with a disproportionately high burden in low and middle-income countries. In such countries, limited access to rehabilitation centres, shortage of trained physiotherapists and socioeconomic constraints impede continuous post-stroke care resulting in poorer recovery. Telerehabilitation has emerged as a scalable and cost-effective solution enabling remote monitoring and therapy personalization. Robust vision-based rehabilitation exercise assessment systems can play a significant role in such telerehabilitation programs. This article pre

Alternating hemiplegia of childhood (AHC) represents a severe and complex pediatric neurodevelopmental disorder, predominantly characterized by the occurrence of paroxysmal episodes of transient unilateral or bilateral paresis prior to 18 months of age. It belongs to a group of ultra-rare neurological disorders with a prevalence from 1:100,000 to 1:1,000,000. Even though the majority of AHC patients harbor pathogenic variants in the ATP1A3 gene, recent studies have pinpointed to other causative genes in ATP1A3-negative patients, including RHOBTB2. In this review, we report the case of a patien

Background: Familial hemiplegic migraine (FHM) is a rare and severe form of migraine disorder featuring aura symptoms that include hemiplegia during attacks. While pathogenic missense variants in CACNA1A, ATP1A2, and SCN1A can cause FHM or its sporadic form, they explain less than 20% of suspected hemiplegic migraine cases, suggesting the involvement of other genes or genetic variations, potentially including copy number variations (CNVs). PPRT2 gene variants including CNVs have also been implicated in hemiplegic migraine. Methods: Multiplex ligation-dependent probe amplification (MLPA) assays

Hemiplegia following stroke is characterized by disrupted neuromuscular interactions, yet the central-peripheral dynamics remain unclear. This study investigated dynamic causal interactions between electroencephalography (EEG) and electromyography (EMG) using the adaptive directed transfer function (ADTF) during a thumb-pressing task in hemiplegic patients and explored the central-peripheral balance between central motor commands and peripheral sensory feedback. Results suggested that patients with better motor functions may exhibit a dynamic transition from relatively balanced bidirectional i

This case report describes a rare case of a giant right atrial aneurysm (RAA) in a 7-month-old male infant admitted to Children's Hospital Affiliated to Zhengzhou University in July 2025. A RAA was detected during a physical examination at 2 months of age with no clinical symptoms present. Follow-up reexamination at 7 months of age revealed progressive enlargement of the RAA, and the infant thus underwent aneurysm resection plus atrial septal defect repair under cardiopulmonary bypass. Postoperative histopathological examination confirmed the diagnosis of RAA. On the second postoperative day,

BACKGROUND: Stroke increases the risk of developing depression and anxiety. While psychological therapies are a recommended first-line treatment, outcomes vary between stroke survivors. Identifying those at risk of poorer treatment outcomes could improve treatment selection and patient outcomes. This study aimed to identify classes of stroke survivors based on stroke-related clinical characteristics and examine their psychological treatment outcomes.
METHODS: NHS Talking Therapies for anxiety and depression (NHS TTad) data, linked to secondary care health records, were used to identify stroke

We report a rare case of Nocardia farcinica brain abscess in the basal ganglia, detailing its diagnosis, management, and rehabilitation. Diagnosing brain abscess based solely on clinical and imaging findings remains extremely challenging. Fortunately, metagenomic next-generation sequencing (mNGS) proved valuable in this case by rapidly identifying the pathogen, thereby facilitating targeted antibiotic therapy. This case highlights the importance of differentiating brain abscess from ischemic stroke and intracranial tumors. After completing a full course of anti-infective therapy and comprehens

BACKGROUND AND PURPOSE: Hemiplegic shoulder pain (HSP) represents a common and disabling complication following stroke, which has multiple causes and important functional consequences. Physical modalities are frequently used to manage HSP, but evidence on their effectiveness remains inconsistent. Our aim is to systematically review the evidence from randomized controlled trials (RCTs) on the efficacy and safety of physical modalities, compared with sham or standard care, for improving pain and function in adults with poststroke HSP.
METHODS: This systematic review was conducted following the

BACKGROUND: Hemiplegia is a common motor impairment following ischemic stroke. Post-stroke recovery involves extensive reorganization of structural and functional brain networks, which correlates with clinical deficits and recovery. While acupuncture has demonstrated safety and efficacy in improving motor function, robust neuroimaging evidence elucidating its central mechanisms remains limited. This study aimed to investigate the specific neuroplastic effects of acupuncture by focusing on its modulation of cortical gray matter volume (GMV) and multilayer dynamic functional network topology.

BACKGROUND: Tenecteplase (TNK) is a novel thrombolytic agent with improved fibrin specificity and longer half-life than alteplase. It is increasingly being adopted worldwide for the treatment of acute ischemic stroke (AIS). Although well tolerated, rare adverse reactions can occur, such as angioedema, which can rapidly progress and potentially cause airway compromise.
CASE DESCRIPTION: We report a 60-year-old female presenting to the neurology emergency department with acute right-sided hemiplegia and motor aphasia who received intravenous TNK approximately 2 h after symptom onset. Shortly af

Hemodialysis is a life-sustaining but procedurally complex intervention. Pneumocephalus, a rare complication of hemodialysis defined as the presence of air within the cranial vault, can range from small asymptomatic collections to life-threatening tension pneumocephalus. We report a case of acute pneumocephalus in a 65-year-old woman undergoing routine hemodialysis for end stage renal disease secondary to polycystic kidney disease. The patient collapsed within minutes of initiating dialysis with sudden alteration of consciousness. Emergency non-contrast brain CT revealed pneumocephalus. She wa
