Association between glucagon-like peptide-1 receptor agonist use and ischemic stroke severity.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) reduce stroke incidence, but their impact on the severity of acute ischemic stroke (AIS) when events occur remains unclear. The aim of this study is to evaluate whether pre-stroke GLP-1RA use is associated with reduced ischemic stroke severity and improved post-stroke outcomes. METHOD: We conducted a retrospective cohort study using the 2016-2024 data from the TriNetX US Collaborative Network. Patients were categorized based on documented use of GLP-1RAs for at least six months prior to the index stroke event. After, propensity

Temporal trajectory and predictors of hemiplegic shoulder pain after stroke: a 12-month longitudinal clinical-ultrasound study.

BACKGROUND: Hemiplegic shoulder pain (HSP) is a frequent complication after stroke. Although HSP is considered a multifactorial condition, the temporal sequence through which different mechanisms contribute to pain remains poorly understood, mainly due to the lack of longitudinal studies starting from the acute phase and integrating imaging data. AIM: To describe the 12-month temporal trajectory of hemiplegic shoulder pain after stroke and to identify time-specific clinical and ultrasonographic predictors of pain intensity. DESIGN: Prospective Longitudinal Observational Study. SETTING: Inpa

A Musculoskeletal-Finite Element Study of Knee Joint Loading and Cartilage Mechanics in Children with Cerebral Palsy.

PURPOSE: Cerebral palsy (CP) is typically associated with abnormal gait that may alter knee joint loading and accelerate joint degeneration. We developed an integrated musculoskeletal-finite element (MSK-FE) framework to quantify differences in knee joint biomechanics between children with CP and typically developing (TD) peers. METHODS: Gait data from 13 children (TD, n = 5; hemiplegia [HP], n = 4; diplegia [DP], n = 4) were analyzed using OpenSim to obtain knee kinematics and compressive knee joint reaction forces (KJRFs). These were applied to scaled 3D FE knee models, incorporating biphas

Combining Pulsed Radiofrequency With Transcranial Magnetic Stimulation to Manage Hemiplegic Shoulder Pain and Somatosensory Nerve Transmissions.

BACKGROUND: Hemiplegic shoulder pain (HSP) is a major cause of disability that is still challenging to manage in clinical practices. Both pulsed radiofrequency (PRF) and repetitive transcranial magnetic stimulation (rTMS) could reduce poststroke HSP to certain extents. However, it remains unknown whether combined PRF and rTMS treatment could increase analgesic efficacy for poststroke HSP. OBJECTIVES: To evaluate the therapeutic efficacy of combined PRF and transcranial magnetic stimulation in treating HSP. STUDY DESIGN: This was a randomized, assessor-blinded pilot study. SETTING: The rehab

Surface EMG-Validated Multi-DoF Wheelchair-Based Rehabilitation Device.

Rehabilitation is a critical component in the recovery of patients with either complete or partial loss of motor movements. Repeated and slow limb movements are usually advised by practitioners. Advanced robotic systems can help to configure monotonous movements and accelerate the recovery process as an alternative to therapist-assisted motions, especially during the later phase of recovery. In this work, robotic-assisted human limb movements are engineered and augmented with a novel electromyography (EMG) signal to characterize the movements. The proposed lower- and upper-limb assistive syste

Key factors for successful gait acquisition in individuals with severe hemiplegic subacute stroke using a single-legged gait-training assistance robot: a retrospective cohort study.

BACKGROUND: Robot-assisted gait training (RAGT) has been explored using various devices. Most studies have focused on bilateral-legged devices; however, few have evaluated single-legged devices. Therefore, we aimed to identify key factors associated with successful responders among individuals with severe hemiplegic subacute stroke who had ambulatory deficits and underwent gait training using a single-legged assistance robot. METHODS: We retrospectively analyzed patients with stroke who received RAGT at Okayama Rehabilitation Hospital between February 2018 and September 2024. Eligible patients

Case Report: Trousseau syndrome presenting as multifocal cerebral infarctions in advanced gastric signet-ring cell carcinoma: tumor burden-driven hypercoagulability.

BACKGROUND: Malignancy-associated hypercoagulability is a well-recognized driver of thromboembolic events in patients with advanced cancer. Arterial complications, including ischemic stroke, can develop despite appropriate anticoagulation and are often accompanied by markedly elevated D-dimer levels, reflecting sustained coagulation activation in the setting of advanced malignancy. CASE PRESENTATION: A 76-year-old man presented with gastrointestinal bleeding and was subsequently diagnosed with advanced gastric signet-ring cell carcinoma with extensive hepatic and retroperitoneal metastases. Differences in pre-swing posture between successful and tripping steps in individuals with stroke.

BACKGROUND: Tripping is a major cause of falls in individuals with stroke. While prior studies have examined externally induced perturbations or distal joint kinematics, the mechanisms underlying intrinsic tripping-related near-falls during walking remain unclear, particularly during the pre-swing phase preceding swing initiation. RESEARCH QUESTION: Does altered thoracic and pelvic posture during the pre-swing phase contribute to tripping-related near-falls in individuals with stroke, independent of lower limb joint kinematics? METHODS: Tripping-related near-fall trials were retrospectively

Spatio-temporal graph convolutional networks with transfer learning for continuous ground reaction force estimation in hemiparetic gait.

Three-dimensional ground reaction force (GRF) is an important biomechanical indicator of weight-bearing, propulsion, and bilateral asymmetry in hemiplegic gait. However, conventional GRF measurement relies on laboratory-based force plates, limiting its use in continuous rehabilitation assessment. A specific methodological challenge is to estimate continuous three-dimensional GRF in post-stroke hemiplegic gait without using force-plate signals as model input, while still preserving whole-body kinematic coordination and affected-unaffected side asymmetry. This study proposed a force-plate-indepe

A Pilot Study on the Short-Term Effects of an Electric Knee-Ankle-Foot Orthosis on Gait Performance and Physiological Cost Index in Patients With Hemiplegia: Influence of Initial Balance Ability Assessed by the Berg Balance Scale.

Gait impairment is one of the most disabling consequences of stroke and substantially limits functional independence. While traditional orthoses provide joint stabilization, they often restrict natural motion and lack adaptive support. This study is aimed at evaluating the short-term effects of an electric knee-ankle-foot orthosis (E-KAFO) on gait performance and energy efficiency in patients with hemiplegia and at assessing how baseline balance ability, measured using the Berg Balance Scale (BBS), influences outcomes. This pre-post intervention, nonrandomized study enrolled poststroke survivo

Chemical Profiling and Virtual Pharmacological Analysis of Asplenium ruprechtii Sa. Kurata.

RATIONALE: Asplenium ruprechtii Sa. Kurata ("Guo shan jue"), a traditional Chinese folk medicine, is widely used to treat thromboangiitis obliterans, hemiplegia, traumatic bleeding, neurodermatitis, and lower limb ulcers. However, its chemical composition remains incompletely characterized-previous studies have identified 80 major components, while trace-level constituents (critical for the "multicomponent, multitarget" efficacy of traditional Chinese medicine) remain unexploited. METHODS: In this paper, a high-performance liquid chromatography coupled with quadrupole time-of-flight tandem ma

Median sensory distal latency as a practical screening parameter for carpal tunnel syndrome during routine nerve conduction studies.

Objective: Carpal tunnel syndrome (CTS) is typically evaluated using several nerve conduction parameters. However, in routine clinical practice, particularly in resource-limited settings, a single simple reliable screening parameter would be highly valuable. Materials and Methods: We retrospectively reviewed 369 consecutive patients who underwent routine nerve conduction studies (NCS) followed by CTS-specific comparative tests. CTS was diagnosed electrophysiologically by using established comparative techniques as reference standards. The screening performance of the median sensory distal late

Effect of constraint standing training on balance and gait in hemiparetic stroke patients: a single-blind randomized controlled trial protocol.

BACKGROUND: Modified Constraint-Induced Movement Therapy (mCIMT) has demonstrated significant potential in rehabilitating lower extremity motor impairments following stroke. However, mCIMT's application is often limited by the need for specialized devices to constrain the non-affected extremity and longer training duration. Constraint Standing Training (CST) is a novel variation of mCIMT that addresses these limitations by eliminating the need for constraining devices and reducing training time, thereby enhancing its feasibility for clinical implementation. Despite these advantages, the specif
