Rare case of intracranial histiocytic sarcoma: A case report and literature review.

BACKGROUND: Histiocytic sarcoma (HS) is a rare malignant neoplasm derived from mature histiocytes, typically affecting lymph nodes, the gastrointestinal tract, spleen, soft tissues, and skin. Intracranial occurrence is extremely rare and poses significant diagnostic challenges due to its histopathological resemblance to other aggressive tumors such as anaplastic meningioma. CASE DESCRIPTION: A 70-year-old man presented with acute right hemiplegia and was found to have two extra-axial lesions with intracerebral hemorrhage in the left frontotemporal region. Initial imaging and immunohistochemis

Disseminated histiocytic sarcoma in 2 FeLV-positive cats in Brazil.

Histiocytic sarcomas (HSs) are uncommon malignant neoplasms in cats, typically affecting older animals and not previously associated with feline leukemia virus (FeLV). Here, we describe 2 cases of disseminated HS in young FeLV-positive domestic shorthair cats that clinically and grossly mimicked lymphoma. Case 1, a 1-y-old female cat, was presented with hemiplegia and dysuria and was euthanized because of the clinical suspicion of spinal lymphoma. Case 2, a 4-y-old male cat, had hyporexia, ocular abnormalities, and bilateral renal enlargement and was euthanized because of the clinical suspicio

Acute hemiplegia as initial presentation in FIP1L1-PDGFRA-rearranged myeloid neoplasm with eosinophilia: a case report.

BACKGROUND: Myeloid neoplasms harboring the FIP1L1-PDGFRA (F/P) fusion gene infrequently manifest as acute cerebral infarction. The F/P fusion gene induces proliferation within the eosinophilic lineage, resulting in a clonal hypereosinophilic syndrome that may lead to cerebral infarction. This condition demonstrates a high responsiveness to imatinib therapy. CASE PRESENTATION: A 27-year-old man presented with acute hemiplegia and was found to have significant eosinophilia. Neuroimaging revealed acute-subacute infarcts with concurrent focal stenosis of the right middle cerebral artery. He was

Real-world mortality and the effect of comorbidities on survival in glioblastoma: A database study using computed phenotypes for rapid, reliable patient identification.

INTRODUCTION: Glioblastoma (GBM) continues to have poor prognosis with minimal improvement since the introduction of the Stupp protocol. While clinical data are often homogenous and not generalizable, real-world data can be used to identify more specific risk factors for poor outcomes. METHODS: Utilizing our proprietary computerized phenotype, we identified adult GBM patients at the University of Florida (UF) and the OneFlorida+ CRN database from 2011 to 2021. The chi-square test was used to compare demographic variables between the 2 groups. Additionally, the effect of medical comorbidities

Surgical Treatment after Radiotherapy for Solitary Plasmacytoma of the Cervical Spine: A Case Report.

Solitary plasmacytoma is a hematologic malignancy in which about 50% of cases progress to multiple myeloma within 10 years. Local radiation therapy is effective for solitary plasmacytoma treatment, but there are cases of spinal cord paralysis due to osteolytic pathological fractures in the spine. Here, we report a 63-year-old male with spinal cord injury caused by pathologic fracture progression after radiotherapy. A 63-year-old male presented with neck pain and left hemiplegia. Imaging studies revealed a pathological fracture of the C5 vertebral body, and biopsy confirmed the diagnosis of sol

The New Wave of Gene and Cell Therapies Across Diseases.

Recent years have seen rapid progress in biological treatments for genetic diseases, as well as conditions like type 1 diabetes that lack an obvious genetic component. The authors sought to explain why this progress has emerged at this particular moment. The best way to illustrate this is by showcasing a wide range of therapies targeting diverse diseases. This progress has been driven by technological advances in genetically modified CAR-T and CAR-NK cells (e.g., using CRISPR or transgenes), which have led to significant improvements in cancer therapy. A key trend now is the emergence of "off-

Tofacitinib treatment in a girl with localized scleroderma of the head and face: a case-based review.

En coup de sabre scleroderma (ECDS) and progressive hemifacial atrophy (PHA) are classified as forms of linear scleroderma of the head and face, a subset of juvenile localized scleroderma (JLoS). In ECDS, sclerotic lesions primarily localize to the forehead and frontoparietal scalp, typically unilaterally. PHA involves the atrophy of facial soft tissues and adjacent cartilage-bone structures on one side, leading to significant deformities. Both forms may involve the eye and central nervous system (CNS), potentially presenting with hemiplegia in PHA. These conditions can coexist and require imm

Gradual Occlusion of an Anterior Choroidal Artery Aneurysm Using Overlapping Flow Diversion and Short-term Anticoagulation: A Case Report.

Anterior choroidal artery aneurysms, in which the anterior choroidal artery arises from the aneurysmal sac, pose significant therapeutic challenges, as treatment risks include branch occlusion and devastating ischemic complications, including hemiplegia and hemianopia. We report a patient in their 70s with an incidentally diagnosed 12.2 mm unruptured right anterior choroidal artery aneurysm, in which three-dimensional angiography demonstrated that the anterior choroidal artery originated from the center of the aneurysm dome. During conservative observation, the aneurysm enlarged to 14.1 mm, ne

ATP1A3-related syndromes: our case-series unveiling a dynamic, fever-triggered and overlapping array of neurological phenotypes.

INTRODUCTION: ATP1A3-related neurological disorders show a broad spectrum of manifestations, usually with autosomal dominant transmission. Classical phenotypes include alternating hemiplegia of childhood (AHC), rapid-onset dystonia-parkinsonism (RDP), and syndrome characterized by cerebellar ataxia, areflexia, pes cavus, optic atrophy and sensorineural hearing loss (CAPOS). Additional rarer forms include childhood-onset-schizophrenia (COS), encephalopathy with MRI abnormalities without hemiplegia (D-DEMØ), fever-induced paroxysmal weakness and encephalopathy (FIPWE), and relapsing encephalopat

Prolonged coma and cerebral oedema in a patient with an ATP1A2 variant.

ATP1A2 (OMIM 182340) encodes the α2 subunit of Na+/K+-ATPase. Variation in this gene has been associated with a spectrum of clinical phenotypes, including familial hemiplegic migraine type 2 (FHM2), epilepsy and intellectual disability. A 22-year-old woman with intellectual disability, hemiplegic migraine and epilepsy presented with persistent decreased consciousness, unexplained by initial investigations. Two weeks later, repeat imaging showed new, marked cerebral oedema with no identified cause; this eventually resolved. A year later, she had a further milder episode. An epilepsy gene panel

Cerebral Embolism Caused by Pulmonary Vein Stump Compression Due to Postoperative Exacerbation of Hiatal Hernia After Left Lower Lobectomy: A Case Report.

Cerebral infarction after lung cancer surgery is a critical complication typically associated with thrombus formation in the long pulmonary vein stump, particularly after left upper lobectomy (LUL). Conversely, the risk of thrombus formation after left lower lobectomy (LLL) is low because of the anatomically shorter stump. A man in his 70s underwent video-assisted thoracoscopic surgery (VATS) left lower lobectomy for lung cancer. On postoperative day 1, the patient developed sudden right hemiplegia, global aphasia, and left conjugate deviation. Magnetic resonance imaging (MRI) revealed an acut

Ethylene glycol toxicity presenting as a rapidly deteriorating focal neurological exam and reversible cerebral herniation syndrome.

Ethylene glycol toxicity is an exceedingly rare toxic encephalopathy. Ethylene glycol is the primary toxin in antifreeze and exhibits its toxicologic effects via metabolites glycolic and oxalic acids, driving a profound metabolic acidemia. Classically, ethylene glycol toxicity presents as inebriation without focal neurological deficits. Here we describe a case of a healthy older adult who ingested antifreeze and presented with 1 h of dysarthria, left-sided facial droop, and left-sided hemiplegia and rapidly progressed to a clinical cerebral herniation syndrome from which he ultimately fully re

Assessment of Neuro-Musculo-Vascular Activity in Hemiplegic Shoulder Pain: A Multimodal Study With HD-sEMG, Ultrasonography, and Laser Speckle Imaging.

Hemiplegic shoulder pain (HSP) is a common complication following stroke, significantly affecting upper limb recovery and quality of life. However, the underlying pathophysiological mechanisms of HSP remain poorly understood, which poses a major obstacle to the development of effective therapeutic strategies. This study aims to investigate the underlying mechanism of HSP by evaluating neuro-musculo-vascular activities using high-density surface electromyography (HD-sEMG), musculoskeletal ultrasound, and laser speckle contrast imaging (LSCI). A total of 12 HSP patients and 12 hemiplegic control

Novel lower limb robot-assisted gait phase stimulation enhances walking and balance in stroke patients: a pilot trial.

BACKGROUND: Hemiplegic patients often experience weakness in key muscle groups during gait, significantly impairing walking and balance. To address this, we developed a novel gait phase-specific functional electrical stimulation (FES) delivered through a lower limb rehabilitation robot. This pilot study investigated the effects of combining gait phase-specific FES with robot-assisted gait training (RAGT) in subacute stroke patients. METHODS: Fifty-one hemiplegic stroke patients were randomized into experimental, control, and placebo groups (17 each). The experimental group received convention

Intentional Self-Inflicted Transcranial Screwdriver Injury: A Case Report.

Transcranial screwdriver injuries are rare forms of low-velocity penetrating cranial trauma that are most often described after interpersonal violence and may result in neurologic impairment or death. These injuries can be difficult to recognize when external wounds are small and may be incompletely characterized on initial imaging, particularly when retained metal produces artifact. Plain radiography and computed tomography (CT) are useful for defining foreign-body trajectory and guiding operative planning, but the full severity of injury may only become apparent through serial imaging and su

Lower limb functional electrical stimulation improves stair stepping in an individual with stroke: a case report.

This case report details the potential benefits of an implanted neural stimulation system for walking to assist stair climbing after stroke. A 12-channel stimulation system was implanted in a stroke survivor for walking. Despite biweekly gait training with functional electrical stimulation (FES) that yielded locomotor improvements, the participant did not feel comfortable attempting stair climbing due to the lack of stability and fear of falling. During physical therapy, he previously limited his attempts at stair climbing to a step-to strategy with the paretic limb. The participant agreed to
