Transcutaneous spinal cord stimulation for functional recovery in individuals with hereditary spastic paraplegia: Protocol for a pilot clinical trial.

BACKGROUND: Hereditary spastic paraplegia (HSP) is a rare genetic neurodegenerative disorder characterized by progressive lower-extremity spasticity, weakness, and gait impairment resulting from corticospinal tract degeneration. With no disease-modifying therapies available, novel strategies to improve motor function are needed. Transcutaneous spinal cord stimulation (tSCS) is a non-invasive neuromodulation approach that may improve motor function but has not been prospectively evaluated in HSP. This study will assess the safety, feasibility, and preliminary efficacy of repeated tSCS in adults

Innovation and practices on case-based genetics teaching focusing on hereditary spastic paraplegia.

Medical genetics serves as a bridge between basic medical science and clinical courses. With its complex curriculum and abstract concepts, traditional lecture-based teaching often leads to difficulties in comprehension, diminishes students' interest in learning, and results in poor teaching outcomes. Incorporating typical case studies into instruction can make genetic theories more tangible, enhance students' intuitive understanding, facilitate their grasp of abstract concepts, and boost their interest in learning. Hereditary spastic paraplegia (HSP) is a group of hereditary neurological disor

[TRANSVAGINAL BLADDER NECK CLOSURE FOR PARAPLEGIC ADULTS - REPORT OF TWO CASES].

We report our initial experiences with transvaginal bladder neck closure for two females with a devastated bladder neck. In each, the bladder neck was closed following a circumferential dissection and the suture line covered using an anterior vaginal flap, with urine draining via a suprapubic cystostomy tube. (Case 1) A female in her twenties was presented with urinary incontinence. The patient had been paraplegic since childhood and initially managed with intermittent catheterization. Later, severe incontinence developed, and Foley catheter was placed. At presentation, the urethra and bladder

Juvenile Huntington Disease With Cerebellar-Predominant Atrophy Identified by Genome Sequencing.

Juvenile Huntington disease accounts for a small fraction of Huntington disease cases and is driven by the largest trinucleotide repeat expansions. We describe a 6-year-old boy with progressive global regression, generalized dystonia, and epilepsy, in whom serial imaging showed striking cerebellar-predominant atrophy with an initially normal-appearing striatum. Extensive metabolic testing, exome sequencing, and an ataxia repeat expansion panel were nondiagnostic. Research genome sequencing identified a large pathogenic huntingtin CAG repeat expansion that confirmatory testing sized beyond 180

A Rare Case of Fatty Acid Hydroxylase-Associated Neurodegeneration in a Pakistani Boy With a Homozygous FA2H Variant.

Fatty acid hydroxylase-associated neurodegeneration (FAHN) is an ultra-rare neurological disorder caused by a mutation in the FA2H gene. Defective production of this gene leads to abnormal myelin formation, which subsequently causes neurodegeneration and brain iron accumulation. This case report presents the case of a 7-year-old boy from Pakistan who was born to consanguineous parents. The initial symptom presented at two and a half years of age with seizures. His clinical course progressively worsened, manifesting as gait disturbances, dystonia, and regression of milestones. By the age of 7,

Two siblings presented with spastic paraplegia and psychomotor retardation with or without seizures with HACE1 variants: A case report and literature review.

RATIONALE: Spastic paraplegia and psychomotor retardation with or without seizures (SPPRS) is a rare autosomal recessive neurodevelopmental disorder caused by biallelic HACE1 variants. Its long-term clinical course and genotype-phenotype spectrum remain incompletely characterized. PATIENT CONCERNS: Two siblings from a nonconsanguineous Chinese family presented with infantile-onset hypotonia, global developmental delay, markedly delayed motor milestones, gait impairment, language delay, and dysarthria. The older sister experienced recurrent febrile seizures, whereas her younger brother had no

Hereditary spastic paraplegia: a practical approach.

Hereditary spastic paraplegia (HSP) comprises a clinically and genetically diverse group of inherited neurodegenerative disorders unified by slowly progressive, usually symmetrical lower-limb spasticity secondary to corticospinal tract degeneration. For practising neurologists, the first step is recognising when a presentation is consistent with HSP and when it is not, and then carefully excluding treatable causes. We outline a pragmatic approach to assessing HSP, highlighting the broad clinical spectrum, 'red flags' that should trigger re-evaluation and the common acquired and genetic mimics.

Generation of a human induced pluripotent stem cell line (PNUYHi004-A) from a patient with hereditary spastic paraplegia type 46 (SPG46) carrying compound heterozygous GBA2 variants.

SPG46 is a rare autosomal recessive form of hereditary spastic paraplegia (HSP) caused by biallelic pathogenic variants in GBA2, encoding the non-lysosomal glucosylceramidase. We generated an induced pluripotent stem cell (iPSC) line from peripheral blood mononuclear cells of a 40-year-old Korean patient with adult-onset complicated HSP carrying novel compound heterozygous GBA2 variants (NM_020944.3: c.2059_2072del, p.Tyr687ValfsTer23; c.2621C > T, p.Ser874Leu), using non-integrating Sendai virus vectors. The iPSC line showed a normal karyotype, expressed markers of undifferentiated hPSC, and

KIF1A c.773C > T impairs neural progenitor differentiation and axonal transport in association with dysregulated Wnt/β-catenin-BDNF signaling.

Pathogenic KIF1A variants are established cause neurodevelopmental and neurodegenerative disorders. However, the mechanisms by which they perturb early human neural development remains undefined. Here, we identified a heterozygous KIF1A c.773C > T (p.T258M) mutation in a Chinese family with hereditary spastic paraplegia and modeled its pathogenicity using patient-derived iPSCs and CRISPR-Cas9-corrected isogenic controls. We found that KIF1A-mutant neural stem cells (NSCs) exhibited hyperproliferation, impaired cell-cycle exit, and defective neuronal differentiation, accompanied by defective ne

Neuronal fatty acid bioenergetics: An emerging pathway and potential point in convergence in hereditary spastic paraplegias.

The human brain is an energetically demanding organ. Neurons have traditionally been considered to rely predominantly on glucose to sustain neuronal activity under normal physiological conditions. However, recent advances challenge this long-standing view, demonstrating that neurons can oxidise endogenous and exogenous long-chain fatty acids through mitochondrial β-oxidation to create energy. In this mini-review, we discuss the emerging role of neuronal fatty acid β-oxidation and how fatty acid metabolism is integrated with glycolysis, lipid storages, and mitochondrial energy production. We fu

Selective vulnerability of the spinal cord in a rat model of asphyxia-induced cardiac arrest.

Spinal cord injury after cardiac arrest (CA) can cause devastating neurological deficits, including paraplegia. Although lumbar spinal cord injury is clinically important, regional comparisons remain limited. In this study, we examined region-associated vulnerability of the thoracic and lumbar spinal cord using a rat model of asphyxial CA (ACA). Adult male Sprague-Dawley rats underwent sham surgery or ACA, and thoracic and lumbar spinal cord tissues were collected 24 h after return of spontaneous circulation (ROSC) from ACA survivors with hindlimb paralysis. Histopathological analysis demonstr

KCNA2-Associated Hereditary Spastic Paraplegia: Clinical Characteristics and Rationale for Targeted Treatment.

BACKGROUND: A single recurrent variant, NM_004974.4(KCNA2):c.881G>A:p.(Arg294His), causes hereditary spastic paraplegia (HSP) through a unique variant-specific mechanism. Data from paralogous variants suggest that this may be a druggable mechanism. OBJECTIVE: The objective of this work is to give an overview of the clinical spectrum and course of KCNA2-p.(Arg294His)-associated HSP (HSP-KCNA2) and report treatment with eplerenone, a potassium-sparing diuretic. METHODS: In this retrospective study, we recruited individuals with the variant of interest through international rare disease network

Rehabilitation management of children with hereditary spastic paraplegia: a retrospective cohort study.

BACKGROUND: Hereditary spastic paraplegia (HSP) is a group of rare, genetically heterogeneous neurodegenerative conditions characterized by progressive lower limb spasticity. In children, rehabilitation management of HSP is commonly extrapolated from cerebral palsy practice, despite differences in etiology and disease progression. There is limited condition-specific evidence to guide rehabilitation clinical decision-making in pediatric HSP. METHODS: A retrospective cohort study of children with HSP attending a tertiary hospital rehabilitation service (May 2018 - August 2024). Children were cl

Spontaneous dissolution of a renal pelvic stone with intrastone gas: A case report with serial CT findings.

Spontaneous dissolution of a large renal pelvic stone is extremely rare. We report the case of a 65-year-old man with paraplegia below Th6 who experienced recurrent febrile urinary tract infections beginning in May 2016. Noncontrast computed tomography (CT) in August 2016 revealed a right renal pelvic stone measuring 26 × 13 mm. During long-term follow-up, serial CT imaging demonstrated structural homogenization, central cavitation, progressive rim thinning associated with intrastone gas, and complete disappearance by November 2020. CT attenuation analysis suggested a uric acid-dominant core s

A De Novo ATP1A3 p.Arg995His Variant in a Patient With an Adult-Onset Primary Lateral Sclerosis-Like Syndrome.

Pathogenic variants in ATP1A3 are classically associated with alternating hemiplegia of childhood, CAPOS syndrome, and rapid-onset dystonia-parkinsonism. However, the phenotypic spectrum of ATP1A3-related disease has expanded considerably in recent years, including atypical presentations with spasticity and hereditary spastic paraplegia-like phenotypes. Hereditary Spastic Paraplegia Type 11: A Comprehensive Case Report.

Hereditary spastic paraplegia (HSP) is a heterogeneous group of genetically determined diseases, characterized by progressive spastic paraparesis of the lower limbs, associated with degeneration of the corticospinal tract and the posterior column of the spinal cord. Currently known to be about 70 genes are involved in HSP, though new potentially pathogenic variants are being reported regularly. Almost all patterns of inheritance like autosomal dominant, autosomal recessive, X-linked, and mitochondrial have been found in families of HSP patients. However, autosomal recessive form, hereditary sp
