During a period of 4 years (December 1983-December 1987) 250 children were operated upon by the authors. At the outpatient clinic of the New Children's Hospital 313 infants and children suffering a myelodysraphic lesion were seen, 63 of whom were not treated surgically owing to the presence of severe hydrocephalus in 16 cases and complete flaccid paraplegia with marked sphincteric disturbances in 47. All of the patients came from poor families. Malnutrition, many siblings per family, poor maternal care, and repeated pregnancies and abortions have added to the large incidence of such a handicap

Appropriate diagnostic procedure for spinal epidural hematomas has not been established yet. The authors reported a case of spontaneous epidural hematomas at the thoracic level, in which correct diagnosis was made with MRI and good results were obtained by surgery. A 63-year-old female experienced a severe back pain which appeared suddenly during a walk and was followed by motor weakness in both legs deteriorating quickly to paraplegia. The patient had no history of hypertension, trauma or bleeding tendency. The laboratory data were normal. On admission, neurological examination revealed flacc

The neuropathological effects of intrathecally administered hypertonic saline have not been described previously. We report a patient who, within a day of receiving 20 ml of hypertonic (7.5%) saline intrathecally, developed flaccid paraplegia and complete sensory loss below the umbilicus. Subsequently, there was some transitory return of motor and coarse sensory function. The patient died 16 months after the injection. At autopsy, there was peripherally accentuated loss of myelinated fibers within the spinal cord from T12 downward, as well as dense collagenous thickening of the dorsal leptomen

The authors report a case of a 71-year-old man, who had thoracic pain, flaccid paraplegia, sensory loss below the level of Th10, and disturbance of the sphincter control. CT myelography disclosed an isodensity epidural mass lesion from Th10 to L2, which compressed the spinal cord ventrolaterally. Laminectomy was immediately performed and an epidural hematoma was verified. The literatures of spinal epidural hematoma located in the thoraco-lumber region were reviewed.

A case of flaccid paraplegia due to a metastatic spinal epidural melanoma is reported. Symptoms occurred approximately 9 years after the enucleation of the left eye. A solid melanotic mass was removed almost totally. Three years after the operation, the patient had not regained the motility of the lower limbs. The need for long term follow-up and management of patients with such problems is discussed, and the pertinent literature is reviewed.

Intractable lower extremity spasms after spinal cord injury is a significant source of morbidity. A case of refractory spasticity in paraplegia was successfully converted to flaccid paraplegia by intrathecal injection of phenol and glycerin in metrizamide. This chemical rhizolysis is simple and effective, and the presence of metrizamide allows both fluoroscopic guidance for accurate intrathecal phenol placement and good miscibility with cerebrospinal fluid. A brief comparative review of alternative therapeutic modalities is presented.

31 control subjects and 14 patients with dorso-lumbar spinal cord lesions were studied using the spEP (spinal evoked response). The presented responses were recorded by subcutaneous needle electrodes following stimulation of the peroneal nerve. Results from control subjects are presented first (triphasic negative potential, latency increasing with level of recording). The 14 patients were grouped according to their clinical symptoms. The prognostic value of the spEP response was considered. Results were as follow: flaccid paraplegia with no motor response to stimulation: no spEP was recorded;

Flaccid paralysis with spinal cord compression led to discovery of Wilm's tumor with multiple subcutaneous and bone metastases in a 12 year-old child. Intraspinal seeding of Wilm's tumor by hematogenous route or direct extension is extremely rare and usually appears late in the course of therapy. Bone metastases are also rare and are frequently seen in the sarcomatous form of the tumor which involves the vertebral column and differential diagnosis with bone metastasizing renal tumor of childhood (BMRTC) should be considered.

Previous clinical studies have shown that some men with spinal cord injury (SCI) have aspermatogenesis and germinal epithelial atrophy. Additionally, an altered response to a luteinizing hormone releasing hormone (LHRH) challenge has been observed in men having SCI with abnormal testicular biopsy. In the present study, an LHRH challenge was given to 13 SCI and able-bodied men, and serum levels of follicular stimulating hormone (FSH), luteinizing hormone (LH), prolactin, and testosterone was determined by RIA. Based on physical examinations, the 13 subjects were divided into four groups: (A) me

A review of reports of confirmed cases of involvement of the CNS in Schistosoma mansoni (SM) and S. haematobium (SH) infection was undertaken. Deposition of ova in the brain has been reported in 17 studies of SM infection and 4 studies of SH infection. In uncomplicated schistosomiasis, SH ova are more likely than SM ova to be deposited in the brain and may be carried there by the vertebral venous plexus. The deposition of SM ova in the brain and meninges is more frequent in hepatosplenic schistosomiasis, especially with cor pulmonale, and the route to the brain may be through pulmonary arterio

Myeloma may be complicated or revealed by spinal cord compression. Out of 105 cases of myeloma admitted to this Department, 6 cases of spinal cord compression were observed, with a favourable outcome after treatment by laminectomy combined with radiotherapy. In 5 cases out of 6, spinal cord compression was either the presenting sign or occurred within the first months after diagnosis. Compression occurred in the thoracic cord in 5 cases, and in the lumbar cord in 1 case. The interval between the first symptom and diagnosis varied greatly (from a few hours to 1 year), as did the degree of parap

A 71-year-old man experienced gradually progressive leg weakness, urinary retention, and mild loss of sensation in dermatomes T8 through T12 bilaterally. After 5 to 6 weeks of illness, he developed flaccid paraplegia and sensory loss below T8. He died 16 weeks after onset of neurological symptoms. Neuropathologically, there was widespread, subtotal necrosis of the spinal cord, largely of nonhemorrhagic character, from T8 downward. Dorsal and anterior median spinal veins were occluded by a partially organized thrombus. Comparison of this case with 19 previously recorded examples of venous infar

Two cases of paraplegia caused by head injuries sustained in recent military conflicts are presented. The underlying pathological disturbances could be related to the CT scan appearances and are discussed in the light of previously reported series. Both cases were remarkable for an early and prolonged paraplegia which was flaccid, rather than spastic, and for the sensory impairment produced.

Two cases of a very rare complication of rubella i.e. a strictly spinal cord lesion developing during the course of the viral infection, are reported. In the first case, there was a flaccid paraplegia which incompletely regressed after two months. In the second case there were only sensory disorders and sphincter disturbances and recovery was complete after one month. The post-eruption encephalomyelitis lesions are of uncertain paphogenesis but the most common hypothesis suggested is that of a delayed hypersensitivity mechanism involving a cell-mediated immunologic reaction. The prognosis of t

Clinicopathologic findings are reported in the case of an unusual combined degeneration of the cerebellum and spinal cord associated with bronchial carcinoma. Clinical features were sudden sensory onset, rapid and complete flaccid paraplegia with ascending course, sphincter paralysis, rapid impairment of general condition, and high protein content in CSF without inflammatory cell reaction. The neuropathologic findings included massive transverse necrosis at the thoracic level and degeneration of the ascending and descending tracts. The necrotic areas involved mostly white matter without any va

We studied a case of flaccid paraplegia that immediately followed intrathecal injection of cytosine arabinoside diluted in 1.5% bacteriostatic water (1.5% benzyl alcohol in H2O). The complication was reversed by rinsing the CSF space with saline. Autopsy showed fibrosis and remyelination of lumbosacral nerve roots. In acute and chronic animal experiments, we showed that benzyl alcohol in commercially used concentrations can have a local anesthetic and an irreversible toxic effect on nerve fibers. This provides a sufficient explanation for many cases of paraparesis following intrathecal chemoth

A term male infant with pneumonia was receiving fluid and antibiotics through an intravenous line in the scalp. Air was accidentally infused in this line. Subsequently, discoloration of the skin, a flaccid paraplegia, hematuria, and gastrointestinal bleeding occurred as the presumed effect of venous air entering the systemic arteries. The umbilical vessels were never catheterized. Absence of any apparent cerebral air embolism is interpreted as being due to the infant's position (head down) at the time of air infusion. The pathogenesis and prevention of this tragic complication are discussed.

Spinal cord infarction in association with pneumococcal meningitis has not been previously recognized. The case is reported of a 5 1/2-year-old boy who had Streptococcus pneumoniae meningitis complicated by the sudden onset of flaccid paraplegia and loss of all sensory modalities below the level of T-2. At operation, the spinal cord was pale and enveloped by dense adhesions, suggesting compromise of the arterial vasculature with concomitant infarction.
