From January 1960 to March 1979 25 children with spinal cord injury were admitted to our hospital (10 newborns with birth injury to the spinal cord were excluded). Among 12 patients with complete thoracic lesions four remained permanently flaccid. Three clinico-pathological cases of necrotic myelopathies with a distant malignancy are presented. Two cases had a lymphosarcoma and one case a prostatic carcinoma. A 35 years old male began to show, 16 years after a transient paraplegia, the neurological signs of a multifocal damage of the spinal cord, which progressed slowly and irregularly over a period of several months. The diagnosis of spinal cord angioma was suspected on the basis of an increase of cerebrospinal fluid white cells and proteins, and of opaque myelography, and proved by spinal angiography. After surgical excision a transient flaccid paraplegia regressed in two weeks, substituted by a mild spastic paraparesis.

Five patients with acontractile bladders were observed during lateral voiding cystourethrography in the resting and straining states. Two abnormalities were noted: (1) posterior displacement of the bladder with closure of the posterior vesicourethral angle on straining, and (2) plication of the bladder over the symphysis pubis, resulting in the formation of an anterior pseudodiverticulum. Two other abnormalities are discussed: closure of the anterior vesicourethral angle, and descent of the bladder and prostate in flaccid paraplegia, causing plication at the prostatic apex. During the past 10 years, 15 traumatic ruptures of the aorta isthmus have been diagnosed. The most likely mechanism is that of a brutal deceleration. Three patients developed a complete mid-thoracic flaccid paraplegia. This electromyographic study was undertaken to assess the peripheral nervous system in patients with various types of paraplegia. Twelve patients were studied. In 8 patients the paraplegia was of the spastic type while 4 had flaccid paraplegia. Electromyographic abnormalities consisting of denervation potentials, slowed conduction and inexcitability of nerve were demonstrated in the lower limbs of all patients. Severe flaccid paraplegia accompanied by extensive sensory loss after epidural anesthesia administered for labor and delivery occurred in a previously healthy 24-year-old primagravida. Recovery was virtually complete after 16 months. The authors have treated two hydatic cysts, three plasmocytomas, one chondrosarcoma, one osteogenic sarcoma and four metastasis at the level of the spine. A large number of target fibres is observed in a muscle biopsy from a patient with an acute flaccid paraplegia of the lower limbs due to a polyneuroradiculopathy of unknown origin. With the electron microscope up to 4 structural zones are seen in the target fibers. The histochemical and ultrastructural observations in this case, suggest that the "target fiber" phenomenon is due to a disturbed trophic influence of the nervous system.

Swelling of the knee joint occurred in 8 of 35 patients with spinal cord injury; in seven of the eight patients the swelling developed within the first eight months after injury. Six of the eight patients had flaccid paraplegia. None of the patients showed any constitutional or local changes other than swelling. No laboratory evidence of inflammation could be detected. Swelling disappeared in two to six weeks and no sequelae were noted. Although trauma appeared to be the most likely cause of the swelling. Only one of four joints showed hemarthrosis on aspiration.

The case is described of a 52 year old patient admitted for acauda equina compression syndrome (flaccid paraplegia of the lower limbs, bilateral lumbar and sciatic pains, urinary incontinence) of rapid onset and due to narrowing of the lumbar canal from tabetic arthropathy. These vertebral manifestations occur in a low percentage of tabetic arthropathies, which are relatively rare, and are not often accompanied by severe neurological disturbances. Decompressive laminectomy afforded rapid and lasting relief.

In a patient with Holmes-Adie syndrome, and in another with tabes dorsalis, a transverse cord lesion resulted in a severe, but flaccid paraplegia with absent tendon reflexes. Flexor spasms were severe in both patients, but spasticity was absent. The significance of these observations is discussed in relation to the functional and anatomical disorder in these two syndromes.
