[Central nervous system leukemia mimicking rapidly progressive HTLV-1 associated myelopathy].

A 79-year-old woman was suffered from rapidly progressive paresthesia of lower limbs and gait disturbance. After one month, she showed flaccid paraplegia and hyperreflexia in the lower limbs with positive Babinski signs. Anti-HTLV-1 antibody titer was elevated in the serum, but negative in the cerebrospinal fluid (CSF). CSF examination showed mild pleocytosis, elevated protein, and normal glucose content. Adult T cell lymphoma (ATL)-like cells were seen in the CSF. MRI showed no abnormal intensity in the spinal cord and brain. Two months later, she showed rapid worsening of the paraplegia and

Chronic cervicothoracic spinal subdural hematoma.

A 33-year-old female presented with an extremely rare case of chronic spinal subdural hematoma in the cervicothoracic region manifesting as sudden onset of flaccid paraplegia and urinary retention. She was receiving warfarin medication for an episode of superior sagittal sinus thrombosis 2 years earlier. Two months previously, she had undergone a minilaparotomy under general anesthesia following unsuccessful spinal anesthesia. Magnetic resonance imaging showed a large cervicothoracic extramedullary mass causing cord compression. However, laminectomy for excision of mass revealed a purely subdu

Intramedullary melanotic schwannoma of the conus medullaris: a case report.

STUDY DESIGN: A case of a very rare type of schwannoma is reported. It is the sixth reported case of intramedullary melanotic schwannoma and the only one localized in the conus. METHODS: A 56-year-old woman was treated in this department for a C5-C6 spondylodiscitis. After 6 months her arms showed a rapid recovery, but her incomplete flaccid paraplegia remained stable. Magnetic resonance imaging (MRI) with gadolinium enhancement of the lumbar tract revealed an intramedullary lesion at the level of Th12-L1. During surgery, an intramedullary, poorly vascularized, dark gray lesion was detected a

[A case of hematomyelia caused by coagulation--fibrinolysis abnormality accompanied with colon cancer and its metastasis].

A 76-year-old woman developed weakness and sensory loss in the lower limbs and urinary disturbance in four days. She had a history of operation for the ascending colon cancer and lung metastasis one year ago. Neurological examination revealed flaccid paraplegia, absent Achilles tendon reflex, severe disturbance of superficial and deep sensation below the L3 level, and vesicorectal abnormality. Magnetic resonance imaging (MRI) studies showed an intramedullary T1-iso, T2-low lesion with Gd-DTPA contrast enhancement in conus medullaris at LI level. The laboratory examination revealed the elevated

Acute toxemic schistosomiasis complicated by acute flaccid paraplegia due to schistosomal myeloradiculopathy in Sudan.

A 55-year old Sudanese physician presented with one month history of diarrhea, loss of weight (10 kg) and low grade nocturnal fever. Following colonoscopy, he rapidly developed paraparesis and retention of urine. Magnetic resonance imaging (MRI) of the spinal cord showed low cord lesion suggestive of transverse myelitis. We present a detailed account of diagnostic and management challenges and a literature review of the final diagnosis of acute toxemic schistosomiasis, complicated by acute flaccid paraplegia due to schistosomal myeloradiculopathy. We are reporting this case to increase the awa

Bilateral oculomotor nerve palsy in Guillain-Barre syndrome.

Guillain-Barre syndrome (GBS) is an acquired immune-mediated inflammatory disorder of the peripheral nervous system. GBS is also called acute idiopathic polyradiculoneuritis. Cranial nerves are affected in over 50% of all cases, with the facial nerves being affected the most. Otherwise, oculomotor nerves affection is rare and might occur in about 10% of cases. In this case report we present 61 years old female with GBS (acute motor and sensory axonal neuropathy subtype) associated with bilateral oculomotor nerve palsy. At the admittance in the neurological status were flaccid paraplegia, tendo

Restoration of muscle volume and shape induced by electrical stimulation of denervated degenerated muscles: qualitative and quantitative measurement of changes in rectus femoris using computer tomography and image segmentation.

This study demonstrates in a novel way how volume and shape are restored to denervated degenerated muscles due to a special pattern of electrical stimulation. To this purpose, Spiral Computer Tomography (CT) and special image processing tools were used to develop a method to isolate the rectus femoris from other muscle bellies in the thigh and monitor growth and morphology changes very accurately. During 4 years of electrical stimulation, three-dimensional (3D) reconstructions of the rectus femoris muscles from patients with long-term flaccid paraplegia were made at different points in time. Open surgical repair of descending thoracic aortic aneurysms in the endovascular era: a 9-year single-center study.

The purpose of this study was to present a single center's experience with elective treatment of descending thoracic aortic aneurysms (DTAAs) in the endovascular era. From July 1997 to May 2005, we operated on 173 patients for DTAA. A total of 52 patients (30.1%) underwent endovascular stent-graft repair (group I). Endovascular repair was carried out exclusively in high-surgical risk patients in whom preoperative spinal cord arteriography usually demonstrated that the origin of the Adamkiewicz artery was located outside the zone to be covered by the stent graft. The remaining 121 patients (69.

Acute paraplegia after the initiation of anti-tumour necrosis factor-alpha therapy for Crohn's disease.

Therapies aimed at inhibiting tumour necrosis factor are currently successfully administered to an increasing number of patients with autoimmune diseases. Infliximab has been approved to induce and maintain remission in Crohn's disease and fistulizing Crohn's disease. We report a case of acute-onset flaccid paraplegia after the initiation of anti-tumour necrosis factor therapy (infliximab) for Crohn's disease. Neuroimaging findings revealed an extensive longitudinal myelopathy. Two months later, no abnormal signal intensity was observed in the spinal cord and after 4 months, the patient presen

Spinal cord retrograde perfusion: review of the literature and experimental observations.

BACKGROUND: Spinal cord damage represents a devastating complication of thoracic and thoracoabdominal aortic surgery. Retrograde perfusion as an alternative route to protect the spinal cord has recently been investigated with controversial results. We reviewed the literature and analyzed additional experimental observations. METHODS: Ten juvenile pigs were divided into control and study groups (A and B, respectively). Through a lateral thoracotomy the distal aortic arch was cannulated and connected to a cardiotomy reservoir. All animals underwent 40-minute single cross-clamping of the proxima

Inflammatory mass of an intrathecal catheter in patients receiving baclofen as a sole agent: a report of two cases and a review of the identification and treatment of the complication.

INTRODUCTION: Intrathecal inflammatory masses or granuloma have been described extensively in the literature in patients receiving chronic spinal infusions for pain. After an extensive literature review, no reported cases of baclofen causing this disorder when administered as a sole agent were identified. Intrathecal baclofen has been used to treat spasticity secondary to stroke, multiple sclerosis, cerebral palsy, spinal cord injury, and other neurological disease. CASES: Two patients who received intrathecal infusions of baclofen to treat spasticity developed catheter failure. Magnetic reso

[Spontaneous spinal epidural haematoma causing rapid flaccid paraplegia in a healthy 25-year-old patient].

Although clinical presentation of a spinal epidural compressive haematoma is well recognized, causing acute radicular pain shortly followed by cord compression syndrome, its aetiology may pose a quandary. Rare and most commonly seen after trauma, spinal surgery, epidural anaesthesia, anticoagulation therapy, vascular malformation or coagulopathy (haemophilia), spinal epidural haematoma (SHE) can be spontaneous. Surgical decompression remains the mainstay treatment especially when the prognosis depends on the interval to surgery and the severity of preoperative neurological deficit. We report t

Neurological complications of Schistosoma infection.

Schistosomiasis is a parasitic disease caused by blood flukes of the genus Schistosoma. Currently more than 200 million people worldwide are affected. Neuroschistosomiasis constitutes a severe presentation of the disease. Neurological symptoms result from the inflammatory response of the host to egg deposition in the brain and spinal cord. Neurological complications of cerebral schistosomiasis include delirium, loss of consciousness, seizures, dysphasia, visual field impairment, focal motor deficits and ataxia. Cerebral and cerebellar tumour-like neuroschistosomiasis can present with increased

Giant terminal lipomyelocystocele.

Terminal myelocystocele is a rare form of occult spinal dysraphism in which the hydromyelic caudal spinal cord and the subarachnoid space are herniated through a posterior spina bifida. A 1-year-old female child presented with a large lumbosacral mass (30 x 20 x 10 cm), flaccid paraplegia and urinary incontinence since birth. Magnetic resonance imaging revealed a low-lying conus (with associated conus lipoma) and a dilated central canal surrounded by a meningocele suggestive of terminal lipomyelocystocele and was operated on successfully. In our experience, this was a giant terminal lipomyeloc
