Complicated Spastic Paraparesis: Study of a Patient With a De Novo Pathogenic Variant in ELOVL1.

Very long-chain fatty acid elongase-1 (ELOVL1) is essential for fatty acid elongation and is widely expressed in numerous tissues, including the central nervous system, being involved in the elongation of very long-chain fatty acids (VLCFAs), which are essential for biological processes such as myelin formation. Monoallelic pathogenic variants in ELOVL1 have been described in association to a condition characterized by ichthyosis, spasticity, nystagmus and cerebral hypomyelination, although a similar but more severe presentation has been reported in patients with biallelic variants. We report

Surgical Management of Pott's Paraplegia in a 27-Week Pregnant Woman with Continuation of Pregnancy: A Case Report.

INTRODUCTION: Spinal tuberculosis is primarily managed with antitubercular therapy, while surgical intervention is reserved for cases with neurological compromise or spinal instability. The occurrence of Pott's paraplegia during pregnancy is rare and presents unique diagnostic and therapeutic challenges due to concerns regarding fetal safety, imaging limitations, and surgical risks. Management during the second trimester is particularly complex, as options, such as medical termination or delivery are not feasible. Only a limited number of case reports describing Pott's paraplegia during pregna

Identification of a putative founder effect involving the HACE1:c.1396C>T variant in two Iranian families and review of reported cases.

BACKGROUND: Biallelic pathogenic variants in the HACE1 gene cause the ultra-rare neurodevelopmental disorder, Spastic Paraplegia and Psychomotor Retardation with or without Seizures (SPPRS). We identified a recurrent HACE1 variant in two unrelated, consanguineous Iranian families and investigated its potential founder origin, alongside a review of all reported HACE1-related cases. METHODS: Whole exome sequencing (WES) was performed on the probands, and the variant was confirmed and segregated by Sanger sequencing. To assess a shared ancestral origin, we conducted runs of homozygosity (ROH) ma

Genomic Landscape and Phenotypic Spectrum of Autosomal Recessive Cerebellar Ataxia in China.

BACKGROUND: Autosomal recessive cerebellar ataxias (ARCA) are clinically and genetically heterogeneous, often presenting as sporadic cases that pose a significant diagnostic challenge. OBJECTIVES: The aim was to characterize the clinical and mutational landscape of ARCA in the largest Chinese cohort using whole-genome sequencing (WGS). METHODS: We performed WGS on a cohort of 187 patients with suspected hereditary ataxia. Clinical severity was assessed using the Scale for the Assessment and Rating of Ataxia and the International Cooperative Ataxia Rating Scale. Pathogenicity was determined a

Prevalence, Disease Onset and Clinical Outcome in Arginase 1 Deficiency: Cross-Border Surveillance in Germany, Austria, and Switzerland.

Arginase 1 deficiency (ARG1-D) is an ultra-rare urea cycle disorder characterized by progressive spastic paraplegia, developmental delay, epilepsy, and episodic hyperammonemia. Evidence on prevalence and clinical presentation is scarce. Therefore, epidemiology and the phenotypical spectrum were assessed in Germany, Austria, and Switzerland (DACH region). We conducted a questionnaire-based, cross-sectional study of confirmed ARG1-D patients in the DACH region. Patients were stratified into early-diagnosed (newborn screening [NBS] or high-risk family screening [HR]) and diagnosed after symptom o

Assessment of balance control mechanisms in patient populations using perturbation-based modeling: a narrative review with clinical implications.

Clinical assessments of balance often rely on indirect measures that offer limited insights into the underlying sensorimotor control mechanisms. Consequently, there is a need for standardized and objective quantification of balance control mechanisms. Research over the past 20 years has demonstrated that Central Sensorimotor Integration (CSMI)-type tests can reliably identify functionally relevant parameters accounting for experimentally observed dynamics of the balance control system. However, the complexity and the lack of standardization of this approach have led to heterogeneous methodolog

Retinal Dystrophy and Leukodystrophy Caused by ACBD5 Deficiency in Five Omani Patients: A Case Series.

Acyl-coenzyme A-binding domain-containing protein 5 (ACBD5) is an acyl-CoA-binding peroxisomal membrane protein. Its deficiency impairs peroxisomal beta-oxidation of very long-chain fatty acids and causes an autosomal recessive disorder that manifests as retinal dystrophy and leukodystrophy. We report five Omani patients with ages ranging between 4 and 30 years. First presentation was in infancy with nystagmus and photophobia and progressed to legal blindness by 10 years of age. Electroretinogram confirmed severe cone-rod dystrophy. Motor neuroregression with variable ages of onset and signs o

New variant in PLP1 gene associated with X-linked spastic paraplegia type 2: First report of a family in Colombia.

Hereditary spastic paraplegias are genetic disorders characterized by spasticity in the lower limbs, weakness, and sensory disturbances. Global prevalence ranges from 1.27 to 9.6 per 100 000 individuals. Mutations in the PLP1 gene cause various X-linked hereditary spastic paraplegias phenotypes, including Pelizaeus-Merzbacher disease and spastic paraplegia type 2. A 53-year-old male presented with chronic lower limb weakness since childhood, requiring a wheelchair at age 47 and exhibiting zero strength in the lower limbs. Magnetic resonance imaging revealed periventricular leukodystrophy; simi

Structural divergence in N-terminal domains of AAA proteases paraplegin (SPG7) and FtsH indicates a key structural function in complex formation.

AAA proteases are hexameric ATP-dependent metallopeptidases that perform crucial proteolytic activities within prokaryotic and eukaryotic membranes. Structurally, protomers are comprised of catalytically active C-terminal domains that are anchored to the membrane by an N-terminal autonomous folding unit. In this study, we determined the fold, stability, and oligomeric state of the N-terminal intermembrane domains of human spastic paraplegia type 7 (SPG7)/ paraplegin protein and its bacterial orthologue FtsH using circular dichroism (CD), small-angle X-ray scattering (SAXS), small-angle neutron

BHLHE22 monoallelic and biallelic variants cause a neurodevelopmental disorder with agenesis of the corpus callosum, intellectual disability, abnormal muscle tone and movement abnormalities.

BACKGROUND: BHLHE22 encodes a basic helix-loop-helix transcription factor expressed exclusively in the retina and central nervous system and functions as an important regulator of neuronal differentiation. However, BHLHE22 has not yet been associated with a Mendelian neurodevelopmental or neurological disorder. METHODS: 15 individuals from 13 unrelated families carrying BHLHE22 variants identified by exome sequencing were collected through an international collaboration. RESULTS: De novo missense variants located in the highly conserved helix-loop-helix domain of the protein were found in si

From PLP1 Misfolding to Oligodendrocyte Degeneration: A Proteostasis-Centered Framework for Pelizaeus-Merzbacher Disease.

Oligodendrocytes (OLs) are the myelinating cells of the central nervous system (CNS). The PLP1 gene, predominantly expressed in OLs, encodes proteolipid protein (PLP), a major structural component of CNS myelin that also regulates oligodendrocyte precursor cell (OPC) proliferation, differentiation, and maturation. Pelizaeus-Merzbacher disease (PMD) is a rare X-linked leukodystrophy caused by PLP1 mutations and characterized by defective myelination. Clinical manifestations range from severe connatal PMD to classic PMD and the milder spastic paraplegia type 2 (SPG2), reflecting substantial phen

Splice effect of a synonymous variant in AP4B1: multiomics approach establishes the diagnosis in two sisters with spastic paraplegia.

Patients with suspected monogenic disorders often remain undiagnosed after exome sequencing. We report a family with two sisters affected by a complex spastic paraplegia. Initial exome sequencing had identified monoallelic pathogenic nonsense variants in AP4S1 and AP4B1, subunits of the adaptor protein complex 4 (AP-4), suggesting digenic inheritance. As digenic inheritance has not been established for AP-4-associated disorders, we applied a multiomics approach including genome sequencing, RNA sequencing and proteomics to clarify the genetic cause. By RNA sequencing a predicted synonymous vari

Pedigree Case Report of adult-onset phenotypically heterogeneous Krabbe disease.

BACKGROUND: Krabbe disease is a rare autosomal recessive leukodystrophy, typically considered a fatal disorder of infancy. Adult-onset forms are uncommon and diagnostically challenging due to their nonspecific presentations. CASE PRESENTATION: We report a consanguineous Han Chinese pedigree comprising two siblings diagnosed with adult-onset Krabbe disease, both carrying the identical homozygous GALC c.1048T>G (p.Phe350Val) mutation. Notably, the age at onset differed by 26 years between the two siblings (49 years vs. 23 years), with markedly distinct clinical trajectories-one presenting with

A novel ABCD1 frameshift mutation detected in a Chinese male with adrenomyeloneuropathy.

BACKGROUND: Adrenomyeloneuropathy (AMN), an adult-onset form of X-linked adrenoleukodystrophy (X-ALD), is caused by pathogenic mutations in ABCD1 gene. CASE: We reported a 60-year-old Chinese male with 15-year chronic progressive spastic paraplegia without symptoms suggesting adrenal insufficiency. RESULTS: Critical findings included abnormal plasma very long-chain fatty acids (VLCFAs), slightly elevated ACTH, and a novel pathogenic ABCD1 mutation (c.172_198delinsACGCAGGA, p.Ser58Thrfs*4) confirmed by genetic testing, which established the AMN diagnosis. CONCLUSION: Our case underscores tha

Hereditary neurodegenerative disorders with spastic-ataxic manifestations: genetic and clinical insights into previously reported variants.

Hereditary neurodegenerative disorders comprise a heterogeneous group of genetic conditions that affect the central and/or peripheral nervous system, often presenting with hallmark clinical features including ataxia, spasticity, dystonia and neuropathy. Our study aims to investigate the underlying cause of rare and overlapping phenotypes of inherited neurodegenerative disorders in the four familial cases of the Pakistani population. Four Pakistani families with a wide range of spastic-ataxic features were evaluated using exome sequencing, homozygosity mapping, and Sanger sequencing. These mole

The pharmacotherapy for pediatric obesity: insights and challenges from adult weight-loss medications.

Obesity significantly increases the risk of developing other chronic diseases and shortens the life expectancy by 5 to 8 years. Childhood obesity is particularly prominent as 80% of teenagers with obesity have shown it to persist into adulthood. With reference to the development of adult medication, the present review summarises the recent research progress in the development of pediatric anti-obesity drugs. The current management guidelines support the use of medication for adolescents aged 12 and above when lifestyle measures are ineffective. Early weight-loss drugs like the serotonergic ago

VRK1-Related Motor Neuropathy With Upper Motor Neuron Signs and Selective Muscle Involvement.

INTRODUCTION: Hereditary motor neuropathies (HMN) represent a heterogeneous group of disorders with wide clinical and genetic variability. Despite advances in molecular diagnostics, approximately 50% of cases remain genetically unresolved, particularly those where distinguishing length-dependent motor neuropathy from motor neuron disorder with disproportionate segmental involvement is a challenge. Variants in the VRK1 gene, originally described in association with pontocerebellar hypoplasia, are now known to produce a broad clinical spectrum, including amyotrophic lateral sclerosis, dHMN, and
