Oxidative stress plays a critical role in many diseases, making it essential to study its impact on disease progression. However, clinical trials have many limitations and, in some cases, may not be possible at all. In this case, the development of in vivo models is highly anticipated. This is especially relevant for neurodegenerative diseases. Drosophila melanogaster models have a number of advantages over many other animal models, including the availability and costeffectiveness of breeding, the accumulated knowledge of the Drosophila genome, and the ability to manipulate a large number of i

Adaptor protein complex 4-associated hereditary spastic paraplegia (AP-4-HSP), a childhood-onset neurogenetic disorder and frequent mimic of cerebral palsy, is caused by biallelic variants in the adaptor protein complex 4 (AP-4) subunit genes (AP4B1 [for SPG47], AP4M1 [for SPG50], AP4E1 [for SPG51], and AP4S1 [for SPG52]). Diagnosis is often confounded by variants of uncertain significance. We evaluated the ATG9A ratio, a measure of ATG9A mislocalization in patient-derived fibroblasts, as a functional assay of AP-4 deficiency. In six of eight individuals with suspected AP-4-HSP, the assay demo

BACKGROUND: Hereditary spastic paraplegia (HSP) genes are emerging as new causes of neurodevelopmental disorders (NDDs). While the association of intellectual disability and autism spectrum disorder (ASD) with many complex forms of HSP is well established, little is known about a possible association with childhood-onset SPG7.
AIM: To add new data on the phenotypic spectrum and associated NDDs in childhood-onset HSP.
RESULTS: We report three patients with biallelic variants in SPG7. Consistent with previous reports, childhood-onset SPG7 manifests as a complex HSP phenotype, with clinical fea

BACKGROUND The most frequently mutated gene in hereditary spastic paraplegia (HSP) is SPAST. Only symptomatic treatment is available for this disease. Fampridine has been successfully used to treat gait disturbances in some patients with HSP. A positive effect of fampridine has not been previously reported in HSP4 caused by the c.683-2A>C variant in the SPAST gene. CASE REPORT We report the case of a 63-year-old woman with hypogeusia and hyposmia for several years, pollakiuria, gait disturbances, reduced walking speed, occasional dysphagia, constipation and delayed defecation, occasional memor

We report the case of a 27-year-old man with a history of speech delay and chronic, progressive movement disorder. He first developed gait difficulty at the age of 12. Given clinical signs of bradykinesia and resting tremor, he received a clinical diagnosis of childhood-onset parkinsonism. Treatment with oral levodopa initially improved symptoms, but after 2 years, he developed motor fluctuations and dyskinesias. Additional signs of spasticity and brain MRI showing a thin corpus callosum prompted genetic testing that identified a heterozygous pathogenic variant in the PRKN gene. However, he ex

OBJECTIVE: Hereditary spastic paraplegias (HSPs) comprise a heterogeneous group of rare, neurodegenerative disorders. The most prominent HSP features: spastic paraparesis, mild somatosensory deficits and bladder dysfunction may be accompanied by additional symptoms i.e.: neuropathy, epilepsy, dementia. We aimed to determine subclinical involvement of nonmotor or sensory brain structures in hereditary spastic paraplegias type 3 A (SPG3A) and type 4 (SPG4). METHODS: Visual evoked potentials (VEPs), brainstem evoked potentials (BAEPs) and electroencephalography (EEG) were performed in 28 SPG4 and

BACKGROUND: Most current studies on Hereditary Spastic Paraplegia (HSP) focus on adolescents and adults. There is a lack of research on the effectiveness of structured, evidence-based developmental physiotherapy during early childhood. While early intervention strategies for children with motor development delays offer a useful framework, no specific guidelines have been developed for early physiotherapy in HSP. This report aimed to describe the effects of a home-based, evidence-based developmental physiotherapy approach in a child diagnosed with HSP Type 47 (SPG47).

Hereditary spastic paraplegia (HSP) groups rare, clinically and genetically heterogeneous neurodegenerative disorders, characterized by progressive lower-limb spasticity and weakness. Over the past decades, diagnostic strategies have evolved from pure clinical assessment to the integration of molecular tools, with next-generation and long-read sequencing (LRS) substantially increasing diagnostic yield and refining genotype-phenotype correlations. Neuroimaging provides complementary information, especially in specific subtypes such as SPG11 and SPG15, supporting diagnosis and guiding testing. T

In a patient with genetically confirmed SPG4-linked hereditary spastic paraplegia, initiation of allopurinol for asymptomatic hyperuricemia led to rapid and sustained improvement in lower limb spasticity, allowing marked reduction of antispasmodic therapy. This serendipitous finding suggests a potential therapeutic role of xanthine oxidase inhibition in managing HSP-related spasticity.

BACKGROUND AND OBJECTIVES: Cognitive deficits often accompany brain structural changes, but the association between spinal cord atrophy and cognitive function remains unclear. Spastic paraplegia type 5 (SPG5) is a subtype of hereditary spastic paraplegias (HSPs), caused by mutations in cholesterol metabolism genes, with radiological features of spinal cord atrophy. Our study aims to explore cognitive impairment in SPG5 and its relation to spinal cord structure.
METHODS: We conducted a prospective cross-sectional study with 30 HSPs patients and 47 healthy controls (HC), performing brain and sp

BACKGROUND: 5,10-methylenetetrahydrofolate reductase (MTHFR) deficiency is an autosomal recessive metabolic disorder caused by biallelic pathogenic variants in the MTHFR gene. Although clinical heterogeneity is well recognized, the condition rarely presents as spastic paraplegia with onset in adulthood. We report a case associated with two novel MTHFR pathogenic variants manifesting this clinical phenotype.
METHODS: We describe a 45-year-old Chinese man who developed a spastic gait at age 38, followed by lower limb weakness at age 43. Brain magnetic resonance imaging revealed mild leukoenceph

Charcot-Marie-Tooth disease (CMT), an inherited neuropathy characterized by progressive distal muscle weakness and atrophy, is associated with axonal impairment. Although mutations in the TRK-fused gene (TFG) have been linked to both CMT and hereditary spastic paraplegia, their pathogenic mechanisms remain poorly understood. Previously, we have demonstrated that the TFG p.G269V mutation causes progressive muscle weakness in patients, impairs neurite outgrowth in primary cultured mouse neurons, and induces neuronal apoptosis in zebrafish, suggesting a conserved role in neurodevelopment. Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease marked by progressive degeneration of upper and lower motor neurons. Most forms of ALS associated with a suspected causal variant are inherited in an autosomal dominant manner. However, there is an important subset of autosomal recessive (AR) variants, often associated with early-onset or atypical clinical features. Advances in genetic sequencing have led to increased recognition of AR ALS. In this review, we focus on four key confirmed AR ALS-associated genes, which appear to be most common-ALS2, SPG11, OPTN, and the D90

BACKGROUND: Group-based exercise classes are popular ways to promote engagement in regular exercise. However, such opportunities are limited for individuals with neurological impairments, who often are more physically deconditioned, require disability-specific instructions and specialised, accessible equipment. OBJECTIVE: This study aimed to co-develop a group-based, community-delivered arm-crank exercise (ACE) programme with individuals with neurological impairments and to evaluate the feasibility of implementing the programme in community settings. METHODS: A pragmatic ACE programme was deve

SPG76 is a complicated form of hereditary spastic paraplegia (HSP) associated with mutations in the CAPN1 gene. The encoded protein, calpain 1, is a calcium-activated cysteine protease that catalyzes the proteolytic cleavage of a variety of cellular proteins and is involved in a wide range of biological processes. Calpain 1 and calpain 2 isoforms are highly expressed in various tissues and have opposite effects on survival: calpain 1 induces the activation of Akt and ERK pro-survival pathways, regulates autophagy and is neuroprotective, while calpain 2 induces neurodegeneration. We characteriz

We present the case of a male in his mid-30s with a progressive complex neurological phenotype primarily characterised by levodopa-responsive parkinsonism with motor fluctuations as well as gait ataxia, peripheral neuropathy and finally also spastic paraplegia. Genetic analysis identified a novel heterozygous variant in the KIF5A gene: c.937G>A (p.Glu313Lys). This variant is genetically classified as likely pathogenic. Other pathogenic mutations in the KIF5A gene are associated with hereditary spastic paraplegia type 10, Charcot-Marie-Tooth disease type 2 and amyotrophic lateral sclerosis. We

Spastin is a conserved microtubule-severing enzyme mutated in hereditary spastic paraplegia. The role that spastin plays in the cell biology of axon regeneration and degeneration has recently been investigated in Drosophila We show that the C. elegans spastin ortholog, spas-1, is expressed in GABA motor neurons, in addition to the known expression in touch receptor neurons (TRNs) and that it is required for axon regeneration in the GABA motor neurons after in vivo laser axotomy. We identified no neuronal developmental defects in the GABA motor neurons and only minor branching variations in the
