A multi-ancestry genetic reference for the Quebec population.

While international efforts have characterized genetic variation in millions of individuals, the interplay of environmental, social, cultural and genetic factors is poorly understood for most worldwide populations. The province of Quebec in Canada has been the site of numerous genetic studies, often focusing on Mendelian diseases in founder sub-populations. Here, we analyze genome-wide genotyped variation in 29,337 Quebec residents from the multi-ancestry population-based cohort CARTaGENE (CaG) who provided DNA samples. We also sequence the whole-genome of 2,173 CaG participants with four gran

Plasma neurofilament light chain in pediatric hereditary spastic paraplegia.

BACKGROUND: Plasma neurofilament light chain (pNfL) is increasingly investigated as a biomarker of axonal damage in several neurological disorders, including hereditary spastic paraplegias (HSPs). Currently, very few studies are focused on pediatric HSPs. METHODS: Plasma NfL levels were measured in 40 pediatric (e.g., <18 years) subjects affected by genetically solved or unsolved HSPs. For 31 subjects, longitudinal NfL evaluation was also available. Potential correlation between pNfL and disease-specific or non-specific features were explored. RESULTS: Median age at enrollment was 11.53 year

Identification of an additional deep intronic splice variant prompts critical evaluation of SPG7 inheritance.

SPG7-related hereditary spastic paraplegia (SPG7-HSP) is one of the most common forms of autosomal recessive HSP. There is a growing number of reports of affected individuals found to be heterozygous carriers for the recurrent pathogenic coding variants in SPG7, most notably p.Ala510Val, and this has further led to the suggestion of SPG7-HSP having both recessive and dominant forms. Here, we report a proband with pure HSP initially found to carry a heterozygous pathogenic stop-gain variant in SPG7 (NM_ 003119.4:c.1672 A > T; p.Lys558Ter). Subsequent short-read genome sequencing (GS) identified

MK4 Repositioning for IAHSP: Overcoming In Vivo Data Gaps through In Silico Refinement and In Vitro Validation.

Infantile-onset Ascending Hereditary Spastic Paralysis (IAHSP) is an ultrarare, autosomal recessive form of Hereditary Spastic Paraplegia (HSP), caused by mutations in the ALS2 gene, which encodes the protein ALSIN. In a previous study, we proposed a personalized therapeutic strategy for an Italian IAHSP patient (AO), aiming to correct the aberrant function of the R1611W mutant ALSIN using Menatetrenone (MK4). While our results supported compassionate-use approval for a patient-specific therapeutic regimen, further investigation was needed to highlight the treatment's benefits in the absence o

Two pathogenetic intronic variants in SPG4/SPAST and expansion of the clinical presentation.

Hereditary Spastic Paraplegia (HSP) is a group of inherited neurodegenerative disorders primarily characterized by progressive lower limb spasticity and weakness. Among the various genetic causes of HSP, pathogenetic variants in SPG4/SPAST are the most frequently identified, making them the leading molecular cause of autosomal dominant HSP. The SPAST gene encodes spastin, a protein involved in microtubule dynamics. In this study, we focused on the functional characterization of two specific intronic variants in SPAST absent or present at a very low frequency in GnomAD database and with conflic

Structural analysis of the plant glycoside hydrolase family 116 glucosylceramidase AtGCD3 by cryogenic electron microscopy.

Glucosylceramide (GlcCer) is a common glycosphingolipid that accumulates in cells in response to Gaucher disease, diabetes, and skin disorders in humans and is also found in plants. In animals, GlcCer is catabolized by glucosylcerebrosidase 1 and 2 (GBA1 and GBA2). GBA1 is a lysosomal enzyme in glycoside hydrolase (GH) family GH30, while GBA2 is a membrane-associated cytoplasmic protein in family GH116. Currently, there are no experimental structures of eukaryotic GH116 homologues. Although the bacterial TxGH116 β-glucosidase structure was determined by X-ray crystallography, TxGH116 does not

Loss of function variants in HPDL impair human cortical development via alterations of mitochondrial function.

Human brain development is highly regulated by several spatiotemporal processes, which disruption can result in severe neurological disorders. Emerging evidence highlights the pivotal role of mitochondrial function as one of these fundamental pathways involved in neurodevelopment. Our study investigates the role of 4-hydroxyphenylpyruvate dioxygenase-like (HPDL) protein in cortical neurogenesis and mitochondrial activity, since mutations in the HPDL gene are associated with a childhood-onset form of hereditary spastic paraplegia characterized by corticospinal tract degeneration and cortical ab

MDSGene Systematic Review of Common Forms of Dominant Hereditary Spastic Paraplegia: Novel Insights.

BACKGROUND: Hereditary spastic paraplegia (HSP) is a neurodegenerative disorder characterized by progressive spasticity and lower limb weakness. The most common forms of autosomal dominant HSP are caused by pathogenic variants in SPAST (SPG4 or HSP-SPAST), ATL1 (SPG3A or HSP-ATL1), and REEP1 (SPG31 or HSP-REEP1). OBJECTIVES: We performed an MDSGene Systematic Review to determine in-depth genotype-phenotype associations and to estimate longitudinal progression, to inform prognostication and clinical trial stratification. METHODS: We systematically collected demographic, phenotypic, and geneti

Modeling spastic paraplegia 4 with corticospinal motor neuron-enriched cortical organoids reveals genotype-phenotype and HDAC6-targetable pathology.

Spastic paraplegia 4 (SPG4), the most common form of hereditary spastic paraplegia, causes progressive gait deficiency due to corticospinal tract degeneration. SPG4 results from mutations in the SPAST gene, which encodes spastin, a microtubule-severing AAA-ATPase. To dissect genotype-phenotype relationships, we generated isogenic human induced pluripotent stem cell lines carrying either an SPAST missense (SPASTWT/C448Y) or truncation (SPASTWT/S245X) mutation and differentiated them into corticospinal motor neuron-enriched cortical organoids. These models revealed mutation-specific patterns of

A GJA1 Variant Triggers Earlier SPG4 Onset by Destabilizing Deubiquitinase VCPIP1 to Lower SPASTIN Levels.

BACKGROUND: SPG4, the most common hereditary spastic paraplegia caused by SPAST variants, exhibits extreme age-at-onset variability, indicative of uncharacterized genetic modifiers. The GJA1R148Q variant (linked to oculodentodigital dysplasia) may modulate SPG4 pathogenesis, yet its underlying molecular mechanism remains undefined. OBJECTIVE: We investigated the molecular mechanism by which GJA1R148Q accelerates early SPG4 onset in a proband with dual SPAST and GJA1 variants, and validated its function as a disease modifier for SPG4. METHODS: We performed comprehensive clinical phenotyping,

Children with suspected hereditary spastic paraplegia clearly benefit from whole exome analysis.

BACKGROUND: Establish a genetic diagnosis in patients referred for clinical suspicion of hereditary spastic paraplegia (HSP). The cohort of 134 paediatric patients was divided into three groups according to age at symptom onset: group A (0–5 years, n = 82), group B (5–10 years, n = 32), and group C (10–18 years, n = 20). Exome sequencing was followed by virtual panel evaluation of HSP associated genes. In patients with a negative result, full exome evaluation was subsequently performed. RESULTS: We established the genetic diagnosis in 31% of patients across all the groups, representing 41 inde

Beyond motor impairment: investigating cognitive aspects of hereditary spastic paraplegia in a Greek case series.

Hereditary spastic paraplegia (HSP) is a heterogeneous group of inherited disorders primarily characterized by progressive spasticity and lower extremity weakness. Traditionally associated with corticospinal tract dysfunction, HSP was long presumed to leave cognitive function largely unaffected. However, growing evidence increasingly recognizes cognitive impairment as a relevant feature of the condition. The primary aim of this study is to broaden the understanding of the cognitive profile in HSP, by examining cognitive impairments in a Greek cohort. A detailed neuropsychological assessment wa

Beyond mobility: A prospective study on diet and metabolism in hereditary spastic paraplegia.

Metabolism plays an important role in neurodegenerative diseases. Hereditary spastic paraplegias (HSP) are a heterogeneous group of rare genetic neurodegenerative disorders, commonly characterized by the clinical syndrome of progressive lower limb spasticity and mobility loss. Obesity has been linked to distinct genotypes, but the role of metabolism and nutrition in HSP remains unclear. We aimed to To evaluate metabolism and nutrition in specific HSP genotypes and to assess the impact of nutritional counseling on disease progression and body composition. Inhibition of FicD-mediated AMPylation and deAMPylation by isoprenoid diphosphates.

FicD regulates Unfolded Protein Response (UPR) through reversible AMPylation and deAMPylation of BiP, an HSP70 chaperone and master regulator of the UPR. FicD activity is regulated by endoplasmic reticulum-stress, catalyzing BiP AMPylation under low stress conditions to hold inactive chaperone in reserve. In stressed cells, FicD deAMPylates BiP, acutely increasing its active pool to assist in protein folding. Variants in UPR machinery, including those in the FicD gene, are linked to hereditary diseases. Despite the known role of FicD in UPR, in-vivo regulation of its activity remains elusive,

Phosphatidylethanolamine: Its Biological Significance and Responses to Nutritional Factors for Neurohealth.

Phosphatidylethanolamine (PE) is a major component of biological membranes, pivotal to numerous life processes. Inhibition of its synthesis via either the cytidine diphosphate-ethanolamine (Kennedy) pathway or the phosphatidylserine decarboxylation pathway can be lethal. PEs derived from different organelles have different functions and biological significance, so they are not fully interchangeable. From a biological perspective, PE is involved in constructing membranes and plays a crucial part in mitochondrial biogenesis, ferroptosis, cell autophagy, cell division, synthesis of biomolecules,

Functional validation of the novel KIF5A p.R17Q VUS reveals defective axonal transport in iPSC-motoneurons from a SPG10 patient.

Cytoskeletal alterations and axonal transport deficits are key factors in many neurodegenerative disorders. The neuronal kinesin family member 5A (KIF5A) is a microtubule-based motor protein critical for anterograde transport of RNA granules, organelles, and neurofilaments along axons and dendrites. Heterozygous missense and nonsense mutations in the N-terminal motor and stalk domains are associated with hereditary spastic paraplegia 10 (SPG10) and Charcot-Marie-Tooth disease type 2 (CMT2), while frameshift mutations in KIF5A C-terminal cargo-binding domain are linked to amyotrophic lateral sc

An MSA-P patient presenting with preserved glucose metabolism in the putamen, cerebellar hypometabolism and pronounced loss of presynaptic dopamine transporter in the striatum.

Positron emission tomography (PET) of the brain using [18F]fluorodeoxyglucose (FDG) is becoming increasingly important for the diagnosis and differential diagnosis of atypical parkinsonian syndrome such as multiple system atrophy (MSA), which is characterized by hypometabolism of the putamen, pons, and cerebellum. We report on a patient with clinically established MSA based on a rapidly progressive, poorly levodopa-responsive parkinsonian syndrome, multidomain autonomic failure, and imaging findings where hereditary spastic paraplegia was discussed as a differential diagnosis. X-linked adrenoleukodystrophy as an etiological cause of progressive spastic paraplegia: A case report.

X-linked adrenoleukodystrophy is a rare peroxisomal disorder caused by mutations in ABCD1, thereby resulting in impaired β-oxidation of very long-chain fatty acids. Its adult-onset form, adrenomyeloneuropathy, typically presents as progressive spastic paraparesis that mimics hereditary spastic paraplegia. Brain and spinal magnetic resonance imaging findings are often unremarkable, contributing to diagnostic delays and misdiagnosis. Herein, we report the case of a 60-year-old woman who presented with a 10-year history of progressive lower limb stiffness, weakness, and gait disturbance. She was
