The Long Haul: Microtubule Motors as the Essential Supply Line for Neuronal Longevity.

The extreme morphology and polarised architecture of neurons require the highly sophisticated microtubule transport system for both construction and lifelong survival. Genomic evidence from an expanding landscape of human mutations supports the essential role of the microtubule transport machinery. During neurodevelopment, mutations disrupt the proliferation and migration of neuronal precursors, as well as the initial establishment of polarity. In the mature nervous system, the reliance on microtubule transport shifts to the long-term maintenance of axon integrity and synaptic proteostasis. Ac

Digital Motor Outcomes Capture Upper Limb and Mild Stage Impairment in Hereditary Spastic Paraplegia.

INTRODUCTION: Digital motor outcomes may surpass the sensitivity of clinician-reported outcomes in hereditary spastic paraplegia (HSP), particularly in cases of mild upper limb impairment and early disease stages. We thus validated a trial-ready quantitative motor (Q-Motor) assessment of rapid alternating limb movement tasks against clinician-reported, patient-focused, and performance outcomes in HSP. METHODS: Exploratory single-center cross-sectional assessment in 41 patients with different HSP genotypes (age: 48 ± 14 years), with speeded foot tapping, diadochokinesia, and finger tapping mea

Proteomic analysis reveals early pathological defects in corticospinal motor neurons of a spastin model of hereditary spastic paraplegia, which are improved by NU-9 treatment.

Upper motor neuron (UMN) degeneration is a characteristic feature of hereditary spastic paraplegia (HSP), a genetically heterogeneous heritable neurodegenerative disorder resulting from mutations in over ninety genes. The mutations in the SPAST gene, which encodes the microtubule-severing protein spastin, are responsible for about 40% of all HSP cases. To date, the cellular and molecular mechanisms linking mutant spastin protein to UMN vulnerability in HSP patients remain unknown and there are no disease modifying therapies. To address this knowledge gap, we isolated pure populations of cortic

Rescue of mitochondrial and neurite pathology in SPG7 hereditary spastic paraplegia patient-derived cortical neurons.

Biallelic pathogenic variants in SPG7 are a frequent cause of hereditary spastic paraplegia leading to progressive disability due to a length-dependent degeneration of cerebellar and cortical projection neurons. While underlying mechanisms have been linked to impaired mitochondrial function, no disease-modifying therapy is available. We generated induced pluripotent stem cell-derived cortical neurons from SPG7 patients with non-sense or truncating variants and from matched controls. We performed detailed phenotyping of neuronal differentiation, as well as mitochondrial and neuritic morphology

Hereditary spastic paraplegia (HSP) gene 11 (Spg11) attenuates lipid accumulation in myeloid cells and neuroinflammation in the midbrain without affecting α-synuclein pathology.

Hereditary spastic paraplegia type 11 (SPG11-HSP) is a neurodegenerative disorder caused by mutations in SPG11, which encodes the large scaffolding protein spatacsin, involved in lysosomal and autophagosomal trafficking. A portion of patients with SPG11 mutations present with parkinsonism features. While spatacsin dysfunction is linked to neurodegeneration, the underlying cellular mechanisms, especially in the midbrain, remain largely unclear. Here, we demonstrate that loss of Spg11 in mice results in neuroinflammation and lipid accumulation in myeloid cells. Bulk RNA sequencing revealed a str

Mutation-specific cellular mechanisms in Drosophila models of ATL1-associated hereditary spastic paraplegia.

Mutations in ATL1 are a frequent cause of autosomal dominant hereditary spastic paraplegia (HSP), yet patients display a wide range of clinical severity, from slowly progressive "pure" to severe, early-onset "complex" forms. The cellular mechanisms underlying this heterogeneity remain poorly defined, and mammalian model systems have thus far failed to distinguish between these subtypes. This study investigates the cellular basis of these clinically distinct forms of HSP using Drosophila models carrying homologous disease-causing mutations in the gene encoding Atlastin-1 (ATL1). Drosophila harb

Hereditary spastic paraplegia in three siblings with distinct genetic mutations.

Hereditary spastic paraplegia is characterized by progressive motor features, including spastic gait and lower limb weakness, and encompasses numerous genetic subtypes. We report a family of six siblings born to asymptomatic consanguineous parents, in which three siblings exhibited overlapping spastic paraplegia phenotypes with developmental delay. The index patient demonstrated significant lower limb weakness and spasticity despite multiple orthopedic interventions. Next-generation sequencing identified a homozygous SELENOI c.797C > T (p.Pro266Leu) variant of uncertain significance, interpret

Case Report: Novel homozygous pathogenic variant of the SPG20 gene causes the Troyer syndrome in China.

Hereditary spastic paraplegia (HSP) comprises a group of neurodegenerative disorders characterized by progressive spasticity of the lower limbs. Troyer syndrome (MIM #275900), an autosomal recessive form of complicated HSP, was initially described in the Old Order Amish population. The syndrome is associated with a spectrum of clinical manifestations, including spastic paraplegia, muscle atrophy, dysarthria, intellectual disability, and abnormal white matter on neuroimaging. The causative gene for Troyer syndrome has been identified as SPG20, which encodes the spartin protein. Spartin plays a

Clinical and Electrodiagnostic Characterization of Pediatric Triple A Syndrome: A Cohort of 17 Patients.

INTRODUCTION/AIMS: Triple A syndrome is an autosomal recessive disorder characterized by alacrima, achalasia, and adrenal insufficiency. Neurological involvement is common, but its electrodiagnostic (EDX) patterns are varied. The resulting phenotype, often marked by spasticity, can lead to diagnostic confusion with other motor neuron disorders. We aimed to define the neurological and EDX spectrum in a pediatric cohort and clarify their diagnostic relevance. METHODS: This was a cross-sectional study of 17 pediatric patients with genetically confirmed triple A syndrome. All patients underwent n

Unraveling SPG46: Clinical, Genetic, and Neuroimaging Features.

BACKGROUND: SPG46 is an autosomal recessive hereditary spastic paraplegia (HSP) caused by biallelic GBA2 mutations. As a rare and still poorly understood condition, information about its clinical and genetic spectrum is scarce. OBJECTIVE: This study aimed to delineate the clinical, neuroimaging, and genetic characteristics of a Brazilian SPG46 cohort. METHODS: We conducted a retrospective cross-sectional study at two referral centers, identifying nine patients from six unrelated families harboring pathogenic GBA2 variants through whole-exome sequencing. Demographic, clinical, neuroimaging, a

Position effect at the SOX3 locus by an interchromosomal insertion causes hereditary spastic paraplegia.

Pathogenic rewiring of the three-dimensional (3D) genome architecture is increasingly being identified as the cause of genetic diseases, but recognizing the cis-regulatory effects of structural variation remains a challenge. The Xq27.1 region contains a quasi-palindrome identified as a pleiotropic hotspot for disease-causing interchromosomal insertions. In a large Danish family affected by X-linked recessive complex spastic paraplegia, we identified the segregation of a 149-kb interchromosomal insertion at Xq27.1 originating from 4q24. To understand the disease mechanism, we generated induced

Neurodevelopmental alterations are key drivers of SPG56.

Biallelic pathological variants in CYP2U1 are associated with SPG56, a complex ultra-rare form of hereditary spastic paraplegia. CYP2U1 encodes a member of the cytochrome P450 family 2, which is highly expressed in the brain and involved in arachidonic acid metabolism and lipid hydroxylation. Little is known about the early stages of neurodevelopment in SPG56. To gain insight into the molecular mechanisms of the disease, we generated and characterized both a zebrafish knock-out model (cyp2u1-/-) and cortical neurons derived from induced pluripotent stem cells (iPSCs) of two patients affected b

Alu-mediated SPAST deletion impairs golgi zinc transport and reveals a druggable vulnerability.

Alu elements are primate-specific retrotransposons known to mediate structural genomic alterations associated with neurological disorders, yet their underlying disease mechanism remains incompletely understood. Here, we leveraged human pluripotent stem cell-derived cortical organoids and xenografts to model an Alu-mediated exon 17 deletion in the SPAST gene, a frequent cause of hereditary spastic paraplegia (SPG4) with dementia. This deletion generates fusion transcripts between SPAST and the downstream gene SLC30A6, resulting in reduced expression of ZnT6, the Golgi-resident zinc transporter

SPG4 Hereditary Spastic Paraplegia: From Etiology to Therapy.

Hereditary spastic paraplegias (HSPs) comprise a heterogeneous group of heritable neurodegenerative disorders resulting from mutations in a wide variety of genes. HSP locomotor symptoms include lower limb weakness and spasticity that arise from progressive degeneration of corticospinal axons projecting from the motor cortex to the distal spinal cord. Ensuing gait defects typically lead to wheelchair dependence. Degeneration of other tracts can also occur, expanding the constellation of symptoms to potentially include urinary, fecal, and speech problems, as well as intellectual disability. SPG4

Walking aids to improve gait in hereditary spastic paraplegia - effects and predictive parameters in a short-term sensor-based evaluation.

INTRODUCTION: Symptomatic management of hereditary spastic paraplegia (HSP) often involves assistive devices, yet neither the timing nor the choice of device is standardized, and their immediate impact on mobility in HSP remains poorly characterized. We aimed to quantify the immediate effects of classical walking aids in HSP, and to identify individualized responses. METHODS: HSP patients and healthy controls performed a standardized 4 × 10 meters walking test under three conditions: unaided, with bilateral walking sticks, and with a four-wheel walker. Mobile gait parameters were recorded for

Instrumented Three-Dimensional Gait Analysis in an Adult With KIF1A-Associated Neurological Disorder: A Case Report.

KIF1A-associated neurological disorder (KAND) encompasses recessive and dominant variants with wide clinical variability. Several de novo variants in the KIF1A gene have been reported to cause a complicated form of hereditary spastic paraplegia (HSP), frequently accompanied by peripheral neuropathy, cerebellar ataxia, and cognitive impairment. Instrumented three-dimensional (3D) gait analysis is underutilized in this population, and comparative data between KAND and pure HSP remain limited. We performed a comprehensive instrumented 3D gait analysis in a 19-year-old woman with KAND caused by a
