Paraspinal muscle impingement causing acute Brown-Sequard syndrome after posterior cervical decompression.

STUDY DESIGN: A case report. OBJECTIVE: To present a previously unreported cause of neurologic compromise after cervical spine surgery. SUMMARY OF BACKGROUND DATA: Several different causes of postoperative neurologic deficit have been reported in the literature. The authors present a case of acute postoperative paralysis after posterior cervical decompression by a mechanism that has not yet been reported in the literature. METHODS: A 54-year-old muscular, short-statured man underwent posterior cervical laminectomy from C3-C5 without instrumentation and left C5 foraminotomy. Within hours of

Thoracic disc herniation of the adjacent segment with acutely progressing myelopathy.

We report a case of a 66-year-old woman with progressing myelopathy. Her history revealed instrumented fusion from T10 to S1 for degenerative lumbar kyphosis and spinal stenosis. The plain radiographs showed narrowing of the intervertebral disc space with a gas shadow and sclerotic end-plate changes at T9-T10. Magnetic resonance imaging revealed a posterolateral mass compressing the spinal cord at the T9-T10 level. The patient was treated with a discectomy through the posterior approach combined with posterior instrumentation. The patient's symptoms and myelopathy resolved completely after the

[Diagnosis and treatment of Paget disease of the spinal vertebrae].

Paget disease, localized only on thoracic spine is extremly rare. It is a huge diagnostic problem and an equally big therapy challenge. Course of this disease is slow, it chronically worsens, thus demanding differentiation of various conditions, along with orthopaedic, neurological, radiographic and endocrine evaluation. Paget disease is followed by back pain, progressive weakening of legs and sphincter functions, due to pathological spine fractures and spinal stenosis. The treatment is surgical, in form of decompression and applying biphosphonates. Three patients are presented, all with worse

Rare cause of thoracic myelopathy: ossified ligamentum flavum.

Myelopathy due to ossification of ligamentum flavum was first described in 1938 by Anzai (J Jpn Orthop Assosc. 1938;13:305-316). This is a rare condition that has been described mostly in the far east, most commonly in Japan (Miyasaka et al, Am J Neuroradiol. 1983;4:629-632). Cases reported from the western hemisphere are rare, particularly in the African-American population (Wiseman et al, J Spinal Disord Tech. 2002;15:542-545). We report an African-American patient who presented with progressive back and chest pain of recent onset, numbness, tingling, and gait disturbance due to T3 paraplegi

[Imaging features of neurologic and orthopedic complications from severe trauma].

Cranial and spinal trauma are a frequent cause of disability in the general population. Post-traumatic paraplegia or quadriplegia or hemiplegia from vascular injury (CVA) can lead to early complications (respiratory, cardiovascular, urinary, cutaneous, infectious...) that may have an impact on the immediate prognosis. Neurologic and orthopedic complications occur later and further impair the quality of life of patients. Orthopedic complications include: neurogenic paraosteoarthropathy (NPOA) or neurogenic osteoma or myositis ossificans (NMO). The nomenclature currently in use is NMO; Osseous c

Symptomatic T2 vertebral hemangioma in a pregnant woman treated by one stage combination surgery; posterior stabilization and anterior subtotal tumor resection. Case report.

A 27-year-old woman presented with vertebral hemangioma manifesting as sudden onset of paraplegia, and bladder and bowel dysfunction during pregnancy. Magnetic resonance imaging revealed a mass lesion that had infiltrated into the entire T2 vertebral body and expanded to the vertebral canal. Laminectomy from T1 to T3 and biopsy of the lesion were performed. Biopsy confirmed the diagnosis of vertebral hemangioma, but laminectomy resulted in no neurological changes. The patient was transferred to our hospital, where radical treatment comprising embolization of the feeding arteries, posterior sta

Clinical care and evolution of paraplegic monkeys (Macaca mulatta) over fourteen months post-lesion.

We have generated a non-human primate model of complete spinal cord injury (SCI) with a protracted survival time. Two adult Macaca mulatta underwent complete spinal cord transection at T8-T9. We report the effective daily care protocol for over one year survival, the health problems we encountered and the treatments applied. The animals' cages were customized to maintain them in the best possible condition when paraplegic. Daily care, adapted from human care protocols, focused mainly on urinary bladder and skin care, and lower limb rehabilitation. The most important health problems we faced we

[Balloon kyphoplasty in the treatment of osteoporotic vertebral fractures: indications - treatment strategy - complications].

BACKGROUND: Considering the demographic changes in the populations of Germany and Europe as a whole, the field of geriatric traumatology is gaining more and more importance within the specialty of orthopedic and trauma surgery. The high prevalence of osteoporosis in this specific group of patients poses a special challenge, with vertebral compression fractures being the by far most common osteoporosis-related fractures. These fractures present with acute as well as chronic back pain leading to severe consequences for the affected patients. Mobility and quality of life are often heavily impaire

Acute paraplegia after chiropraxis.

Spinal manipulation is a form of back and other musculoskeletal pain treatment that often involves a high-velocity thrust, a technique in which the joints are adjusted rapidly. The main objective of chiropractors is to correct spinal malalignment and relieve the nerves, allowing them to function optimally (Ernst In: Expert Rev Neurother 7:1451-1452, 2007; Oppenheim et al. In: Spine J 5:660-666, 2005). The evidence for the effectiveness of this treatment based on randomized clinical trials still remains uncertain (Cassidy et al. In: Spine 33(4 suppl): S176-S183, 2008; Dupeyron et al. In: Ann Re

Anterior spinal artery syndrome following vertebroplasty: a case report.

STUDY DESIGN: Case report. OBJECTIVE: To report a rare case of anterior spinal cord syndrome caused by a cement embolism in the anterior spinal artery after vertebroplasty. SUMMARY OF BACKGROUND DATA: Vertebroplasty is commonly performed for the management of pain associated with benign compression fractures, multiple myelomas, lymphomas, vertebral metastatic lesions, and hemangiomas. Here, we describe a severe complication associated with this procedure; a similar complication has not been reported previously. METHODS: A 63-year-old woman suffered from persistent severe back pain that radi

Long-term follow-up of the surgical management of neuropathic arthropathy of the spine.

BACKGROUND CONTEXT: No studies have discussed the long-term surgical management and outcomes of Charcot arthropathy of the spine. This case series presents nine patients treated over 30 years. The study hypothesis was that surgery would reduce instability, pain, recurrence, and the need for revision surgery in the long-term, given previous study findings of successful fusion of Charcot spine in the short-term. PURPOSE: To evaluate the long-term outcomes of surgery for Charcot spine. STUDY DESIGN/SETTING: Retrospective case series. Cases took place at Stanford University Medical Center and Sa

Decubitus ulcers: pathophysiology and primary prevention.

BACKGROUND: Pressure sores are a serious complication of multimorbidity and lack of mobility. Decubitus ulcers have become rarer among bed-ridden patients because of the conscientious use of pressure-reducing measures and increased mobilization. Nonetheless, not all decubitus ulcers can be considered preventable or potentially curable, because poor circulation makes some patients more susceptible to them, and because cognitive impairment can make prophylactic measures difficult to apply. METHODS: A systematic literature search was performed in 2004 and 2005 in the setting of a health technolo

Solitary plasmacytoma of the thoracic vertebra presenting with sudden paraplegia and back pain: a pathologic case report.

Solitary plasmacytoma (SPC) accounts for 5% of plasma cell neoplasm. SPC of the spine is relatively rare, and SPC presenting with sudden paraplegia is very rare. A 53 year-old woman was admitted to our hospital complaining of sudden severe paraplegia and back pain. Emergency MRI revealed a tumor of the thoracic vertebra no. 10 (Th10). Pressure fracture of the Th10 was also recognized. The tumor was not osteolytic and invasion was recognized around the Th10. The tumor directly compressed the spinal cord. An excision of the tumor was performed under the clinical diagnosis of metastatic carcinoma

Paraplegia associated with spinal hydatid cyst: a case report.

STUDY DESIGN: Case report. OBJECTIVES: To report a case with paraplegia caused by spinal hydatid cyst. SUMMARY OF BACKGROUND DATA: Hydatid cyst is a disease caused by larval Echinococcus granulosus tapeworm. Spinal hydatid cyst rarely leads to severe neurologic problems. METHODS: A 34-year-old male patient was referred to our outpatient clinic due to back and low back pain, progressive weakness and numbness in both lower extremities, and a prediagnosis of lumbar disc hernia. He had spastic paraplegia, and thorax magnetic resonance imaging revealed a lobulated cystic lesion with extradural i

Medical treatment of primary extradural solitary lumbar hydatid disease.

Hydatid disease of the spine is rare and has a poor prognosis, presenting both diagnostic and therapeutic challenges. Paraplegia is the most serious complication of the disease and is caused by compression of the spinal cord by the cysts. We report a 30-year-old woman with an isolated primary hydatid cyst that responded to treatment with albendazole. She presented with back and right leg pain. MRI of the lumbar spine showed a solitary cyst measuring 2.2 x 2.7 cm, with an intraspinal extension at L4-L5. Physical examination did not reveal any focal deficit and the patient was treated with alben

Primary spinal extradural hydatid cyst in a 4-year-old child.

The authors report a case of 4-year-old boy with primary spinal extradural hydatid cyst of the thoracic spine. The location is extremely rare. However, the case is unique because the patient is the youngest reported in the English language medical literature. Surgical excision is the gold standard therapy.
