Extensive thoracic spontaneous epidural hematoma presenting with position-induced transient paraplegia: successful conservative management in the resource limited settings.

BACKGROUND: Spontaneous spinal epidural hematoma (SSEH) is a rare condition with an estimated incidence of 0.1 per 100,000 individuals. It is usually characterized by sudden spinal pain followed by rapidly progressive neurological deficits. Surgical treatment is generally indicated if a patient presents with neurological deficits. We report a case of SSEH presenting with neurological deficits relieved after position change, recovering without surgery. CASE PRESENTATION: The patient was a 73-year-old woman developed sudden, severe pain in the neck, back, lumbar region, and abdomen while sitting

Bladder and Bowel Dysfunction, Cognitive Appraisals of Disability, and Life Satisfaction in Individuals With Pediatric-Onset Spinal Cord Injury.

BACKGROUND: Individuals with pediatric-onset spinal cord injury (SCI) represent a distinct group; management can be more complex than in adult-onset SCI. Changing medical needs, transfer of responsibility, and cognitive appraisals of disability may all impact life satisfaction in pediatric-onset SCI. Further, bladder/bowel management over many years may be time-consuming and socially disruptive. Cognitive appraisal of disability has been linked to life satisfaction and functioning in pediatric-onset SCI. OBJECTIVES: This study examines associations between bladder/bowel accidents, bowel manag

Spinal Epidural Abscess Complicated by Saddle Pulmonary Embolism: A Case Report.

CASE: A 36-year-old woman with intravenous drug use presented with back pain, paraplegia, and urinary incontinence. MRI revealed a ventral cervical spinal epidural abscess from C5 to T1 with cord signal changes. CT angiography showed a large saddle pulmonary embolism with right heart strain. Multidisciplinary management included initiation of venoarterial extracorporeal membrane oxygenation (VA-ECMO) without anticoagulation, followed by urgent C6 corpectomy and C5 to T1 anterior decompression and fusion. Anticoagulation and thrombectomy were performed postoperatively. CONCLUSION: This rare ca

Safety and efficacy of delayed-release diameter reduction in constructing physician-modified endografts for complex aortic aneurysms.

BACKGROUND: Physician-modified endografts (PMEGs) remain an option to treat complex aortic aneurysms with fenestrated endovascular aortic repair (FEVAR) if they are unsuitable for commercial endografts or open surgery. FEVAR traditionally involves uniform diameter reduction for graft alignment and target vessel cannulation, after which diameter reduction is released before the delivery of bridging stent grafts. We describe the results of an advanced PMEG design using nonuniform constraints to mold the graft to the inner shape of the visceral aorta with delayed-release diameter reduction. This

Understanding Vascular Risks in Lumbar Transforaminal Epidural Injections: Insights From Anatomy and Clinical Evidence.

Transforaminal epidural steroid injections (TFESIs) are frequently used to treat lumbar and radicular pain. Although ischemic complications are extremely rare, their severity-often resulting in irreversible paraplegia-necessitates a thorough understanding of the anatomical and procedural risk factors. This review carefully examines the vascular anatomy of the lumbar intervertebral foramen, the distribution and risks associated with radiculomedullary arteries, and reported cases of severe complications related to lumbar TFESIs. Anatomical and radiological data indicate that radicular arteries a

Real-time brain-computer interface control of walking exoskeleton with bilateral sensory feedback.

PURPOSE: Brain-computer interfaces (BCIs) offer a pathway to restore ambulation in individuals with spinal cord injury (SCI). However, existing BCI systems for gait are unidirectional and lack sensory feedback. This study aimed to demonstrate that a bidirectional brain-computer interface (BDBCI) can simultaneously enable real-time brain-controlled walking and artificial leg sensation via electrical stimulation of the sensory cortex. METHODS: Epilepsy patients undergoing bilateral interhemispheric subdural electrocorticography (ECoG) implantation were recruited for this proof-of-concept study.

Fatal arachnoiditis following accidental intradural injection of chlorhexidine: a case report and literature review.

BACKGROUND: Chlorhexidine is the preferred antiseptic for skin preparation owing to its superior antimicrobial efficacy compared with povidone-iodine. However, its supposed neurotoxicity-particularly in combination with alcohol-can cause catastrophic damage if inadvertently introduced into the neuraxial space during epidural or spinal anesthesia. CASE PRESENTATION: We describe a fatal case of arachnoiditis following unintentional intradural injection of alcohol-based chlorhexidine during epidural catheter placement for labor analgesia and report a literature review. Among 114 records, five ca

Cortical Brain Activation During Robot-Assisted Gait in Humans With Acute and Chronic Spinal Cord Injury: A Functional Near-Infrared Spectroscopy Study.

Most treatments being developed to regain motor function following spinal cord injury (SCI) presuppose that brain motor functions remain intact. To examine this assumption, this study aims to analyze residual neurological functions during assisted robotic gait in individuals with SCI comparing blocks (gait × resting), time after SCI (acute × chronic), injury level (paraplegic × tetraplegic), and ASIA scale (ASIA C × D). The hemodynamic functions were analyzed using functional near-infrared spectroscopy (fNIRS) in 23 individuals (11 acute, 12 chronic; ASIA Impairment Scale grade C: 10, D: 13; p

Life after the hospital: Lived experience of people with traumatic spinal cord injury in a resource-constrained setting - a qualitative study.

BACKGROUND: When a traumatic spinal cord injury (TSCI) happens to healthy individuals, it requires an adjustment to life situations in the community. Exploring the lived experience of spinal cord injury survivors is important because it forms a foundation for designing strategies to improve their reintegration back into the community. There are limited studies in the East and Central African region regarding the experience of community reintegration following a TSCI. OBJECTIVES: This study explored lived experiences of persons with TSCI in Rwanda. Specifically, it sought to identify and under

Silently Gradually Progressing Paraparesis Pott's Spine Lumbar Vertebra with Bilateral Psoas Abscess.

INTRODUCTION: Extrapulmonary tuberculosis (TB) can involve the spine, presenting as Pott's disease. Patients typically develop back pain, and complications, such as psoas abscess and paraplegia may occur if diagnosis is delayed. A high index of suspicion and gadolinium-enhanced magnetic resonance imaging (MRI) are essential for early detection. Timely treatment helps prevent neurological deficits. We report the case of a 55-year-old afebrile man with progressive lower-limb weakness, unexplained weight loss, and an atraumatic vertebral fracture. MRI revealed classic cold abscesses consistent wit

Machine learning models for predicting postoperative paraplegia in acute type A aortic dissection patients.

BACKGROUND: Acute type A aortic dissection (ATAAD) is an extremely life-threatening cardiovascular emergency characterized by a critical clinical course and high mortality rate. Once the diagnosis is confirmed, emergency surgical intervention is required to save the patient's life. Postoperative paraplegia is a relatively rare but severe surgical complication, typically manifesting as transient or permanent loss of sensory and motor functions in both lower extremities after surgery. It severely impairs patients' survival and prognosis, imposing a heavy burden on both patients' families and the

Integrated Clinical Workflow for Preoperative Planning and Resection of Giant Iliofemoral Heterotopic Ossification Using Three-Dimensional Technologies.

Background/Objectives: Neurogenic heterotopic ossification (HO) is an abnormal formation of lamellar bone in soft tissues, frequently developing near major joints in patients with spinal cord injury. While imaging provides valuable diagnostic insights, large and anatomically complex HO often requires advanced preoperative planning to minimize surgical risks. This study presents the development and clinical application of a structured six-stage workflow integrating three-dimensional (3D) technologies for the preoperative planning and surgical resection of giant iliofemoral HO. Stopping an outbreak of Pseudomonas aeruginosa ST111 by temporarily blocking the siphon to disinfect it before the transition to the wastewater drain.

INTRODUCTION: Resulting from an analysis of an outbreak of carbapenem-resistant Pseudomonas (P.) aeruginosa, the siphons of washbasins in patient rooms were identified as source of colonizations and/or infections. To avoid installing new siphons, a solution was needed to temporarily block the siphon before the transition to the wastewater drain. RESULT: By temporarily blocking the drain before the transition to the wastewater drain with a balloon, it was possible to disinfect the traps without disassembly and stop the outbreak. DISCUSSION: Temporarily blocking the drain can stop outbreaks or
