Thoracic intramedullary spinal cord astrocytoma associated with syringomyelia: a case report of gross total resection.

INTRODUCTION AND IMPORTANCE: Intramedullary spinal cord tumors (IMSCTs) are rare, accounting for 2-4% of central nervous system neoplasms. Astrocytomas comprise about 30% of these tumors and are typically infiltrative. The coexistence of astrocytoma with syringomyelia - a fluid-filled cavity within the spinal cord - is uncommon but clinically significant, as it can worsen neurological deficits. Understanding and managing this association remain challenging, especially in elderly patients. CASE PRESENTATION: A 73-year-old male farmer presented with a 6-month history of progressive lower limb w

Thigh Compartment Syndrome Following Physician-Modified Fenestrated Endograft Aneurysm Repair.

BACKGROUND: Thigh compartment syndrome (TCS), a rare but serious complication of vascular procedures, often leads to severe morbidity. Unlike leg compartment syndrome, TCS can present insidiously, delaying diagnosis and treatment. While most cases of TCS are trauma-related, vascular causes such as ischemia-reperfusion injury have been reported. CASE REPORT: A 67-year-old male with coronary artery disease, chronic obstructive pulmonary disease, and a 50 pack-year smoking history underwent physician-modified fenestrated endograft repair of an unruptured thoracoabdominal aortic aneurysm. The pro

Interventional spine therapy for neuropathic pain in individuals with paraplegia.

BACKGROUND: Infiltrations of the spine can be used to treat nociceptive or neuropathic pain. There is little data in the literature on the use of spinal injections in patients with paraplegia. OBJECTIVE: The aim of this study is to determine whether patients with spinal cord injury experienced improvement in pain and spasticity following spinal injection. METHODS: 19 (9 female, 10 male) patients (5 patients AIS A, 5 patients AIS C, 9 patients AIS D), including 9 tetraplegic and 10 paraplegic patients, with 22 injection techniques and a follow-up of 4.2 months (1-12 months) were retrospective

Long-Term Survival following TEVAR for Blunt Thoracic Aortic Injury.

BACKGROUND: Blunt thoracic aortic injury (BTAI) is a leading cause of death in blunt chest trauma. Thoracic endovascular aortic repair (TEVAR) has emerged as the preferred treatment, offering reduced morbidity and improved survival compared with open repair. This study evaluates long-term outcomes of TEVAR for BTAI at a single Canadian center. METHODS: We retrospectively reviewed 46 patients with BTAI treated with TEVAR at McGill University from 2002 to 2025. Data collected included demographics, associated injuries, Society for Vascular Surgery (SVS) grade, aortic zone, procedural details, a

Quadrilateral Pinwheel Flap Reconstruction for a Complex Colocutaneous Fistula-Associated Flank Wound in a Paraplegic Patient: A Case Report.

Background/Objectives: Chronic wounds are a major source of morbidity in patients with paraplegia, often resulting in repeated treatment, prolonged hospitalization, and reduced quality of life. Reconstruction becomes particularly challenging when a wound arises in a scarred trunk region and is further complicated by deep infection, osteomyelitis, or enteric fistula. We describe the staged management of a complex left flank wound in a paraplegic patient, initially reconstructed with a quadrilateral pinwheel flap and later requiring multidisciplinary salvage for recurrence associated with rib os

Placebo Effect on Pain-Related Autonomic Responses in a State of Experimentally-Induced Sensitization.

BACKGROUND: Pain-related autonomic responses are increased after experimentally-induced secondary mechanical hyperalgesia (SMH), as well as in a variety of chronic pain cohorts. In this state of sensitization, negative expectations further amplify these enhanced responses. Whether these pain-related autonomic responses can also be modulated by positive expectations, i.e., placebo, was investigated in this experimental study. METHODS: Forty healthy participants (20 females) were recruited and assigned to either a PLACEBO or a CONTROL group. Pain ratings and skin conductance responses (SCR) wer

In silico modelling of changes in spinal cord blood flow after endovascular aortic aneurysm repair.

AIMS: To develop an in-silico model of the aorta and its spinal cord-supplying branches, using it to characterise haemodynamic changes following aortic aneurysm (AA) repair. The work is motivated by the risk of spinal cord ischaemia (SCI) and paraplegia, serious complications that can arise from disruption of spinal cord perfusion during AA surgery. An objective, patient-specific tool capable of predicting changes in spinal cord blood flow pre-intervention would address a critical unmet clinical need. METHODS: SimVascular was used to retrospectively model a 76-year-old female patient's aorta

Modeling and validation of crutch-supported exoskeleton gait using a double-inverted pendulum approach.

PURPOSE: Exoskeleton-assisted walking for individuals with paraplegia presents unique biomechanical challenges due to minimal lower-limb actuation and substantial reliance on upper-body support via crutches. However, quantitative models rarely elucidate the dynamic interplay between exoskeleton actuation and crutch assistance. METHODS: We introduce a double-inverted pendulum model that captures the essential dynamics of crutch-supported exoskeleton gait. The model extends the canonical inverted pendulum framework by directly encoding gravitationally induced hip torques and ground reaction forc
