Prolonged Agitation and Hypotonia due to Baclofen and Tizanidine Overdose Followed by Withdrawal: A Case Report.

We describe a unique case of combined baclofen and tizanidine overdose followed by dual withdrawal syndromes, highlighting overlapping and contrasting clinical features that may complicate diagnosis and management. A 26-year-old man with paraplegia from cervical spinal cord injury, prescribed baclofen and tizanidine for spasticity, was found unresponsive at home next to partially empty medication bottles. Clinical evaluation included laboratory testing, neuroimaging, continuous monitoring, and serial neurologic and cardiopulmonary assessments. On admission, the patient exhibited CNS depression

Exploring the Integration of Immersive Virtual Reality With Adapted Neuromuscular Stimulation Rowing for Individuals With Spinal Cord Injury.

People with a spinal cord injury (SCI) are more susceptible to adopting a sedentary lifestyle, which increases the risk for further health complications such as cardiovascular disease. Exercise has proven beneficial for the improvement of overall health; however, options can be limited following SCI, especially for engaging paralyzed muscles. Using neuromuscular electrical stimulation (NMES) to produce contractions of the paralyzed musculature following SCI is one strategy for increasing physical work during exercise, but NMES can be disengaging. Previous reports indicate that the introduction

Characterizing Neurological Status and Outcomes During Inpatient Rehabilitation in Persons With Nontraumatic Spinal Cord Injury: Early Findings From a Spinal Cord Injury Model Systems Feasibility Study.

BACKGROUND: The National SCI Model Systems (SCIMS) Database was established in 1973 to study longitudinal data from persons with traumatic SCI (TSCI). The SCIMS provides an ideal platform to explore the feasibility of a parallel data collection effort to understand the characteristics of individuals with nontraumatic SCI (NTSCI). In the current data collection cycle (2021-2026), all SCIMS centers gathered data specific to NTSCI. OBJECTIVES: To (1) describe the demographic and injury-specific characteristics of participants with NTSCI and (2) assess neurological changes from admission to disch

Giant Thoracic Meningioma: Missed Diagnosis and Challenging Management in a Resource-Limited Setting.

Intradural extramedullary spinal tumors are surgically treatable lesions, but delayed diagnosis, particularly in low-resource settings, can result in irreversible neurological deficits. We present a case of a 37-year-old woman with a five-year history of progressive paraplegia due to a thoracic T4-T5 intradural extramedullary tumor, whose diagnosis and management were significantly delayed due to system-level limitations. This case illustrates the severe consequences of missed or delayed diagnosis of surgically treatable spinal tumors in resource-limited settings. Early MRI interpretation by t

Predictors of Complications and Recurrence After Pressure Injury Reconstruction in Spinal Cord Injury: A Systematic Review and Meta-Analysis.

Spinal cord injury (SCI) predisposes patients to pressure injuries (PIs) often requiring surgery. Multiple factors influence postoperative complications and recurrence, though their associations remain unclear. We performed a PRISMA-guided systematic review and meta-analysis to assess predictors of acute postoperative complications and PI recurrence in SCI patients undergoing surgery. PubMed, Scopus and Embase were searched. Studies with ≥ 15 participants were eligible; 24 (n = 2566 subjects) were included, with 15 (n = 1976 subjects) quantitatively synthesized. Risk of bias was assessed with

Early Recurrence of an Adult Desmoplastic Medulloblastoma With Intramedullary Spinal and Suprasellar Metastases.

Medulloblastoma is a highly aggressive embryonal tumor of the central nervous system with a strong tendency for dissemination through cerebrospinal fluid. While it occurs predominantly in pediatric populations, it is uncommon in adults. Intramedullary spinal metastases and suprasellar involvement are exceptionally rare patterns of dissemination in adults. We report the case of a 21-year-old male diagnosed with desmoplastic medulloblastoma of the cerebellar vermis who underwent ventriculoperitoneal shunting, gross total resection, and craniospinal radiotherapy. Despite receiving standard multim

Esophageal squamous cell carcinoma in a patient with spinal cord injury complicated by achalasia: a case report.

The progression from spinal cord injury(SCI) to secondary achalasia and subsequent esophageal squamous cell carcinoma (ESCC) is exceptionally rare. We report the case of a 51-year-old female who developed progressive dysphagia seven years after a T6-level spinal injury. Barium swallow and endoscopic examinations revealed the classic "bird's beak sign" and a suspicious esophageal lesion. Given the patient's paraplegic condition and associated anesthetic risks, endoscopic intervention was contraindicated. Following evaluation by a multidisciplinary team, a combined thoracoscopic-laparoscopic eso

Protective effects of ceria nanoclusters against delayed paraplegia after spinal cord ischemia.

Spinal cord ischemia reperfusion injury (SCIRI) induced delayed paraplegia is a complex complication of thoracoabdominal aortic surgery. Reactive oxygen species (ROS) play a critical role in the pathogenesis of delayed paraplegia, suggesting that antioxidants could be a promising therapeutic strategy. Ceria nanomaterials have demonstrated potent antioxidant properties and therapeutic potential in various central nervous system disorders. Herein, the preventive effects of ceria nanoclusters (NCs) against delayed paraplegia were investigated. NCs were synthesized using a facile and green method,

Combined Dorsal and Ventral Spinal Cord Stimulation for Treatment of Refractory Neuropathic Pain After Spinal Cord Injury: A Case Report.

BACKGROUND: Central neuropathic pain that follows spinal cord injury is often refractory to medical therapy. Spinal cord stimulation (SCS) is an established treatment for chronic neuropathic pain, but conventional dorsal column placement may not capture all pain regions. CASE REPORT: A 63-year-old man with C6-T2 spinal cord infarction and C7 ASIA C paraplegia experienced severe refractory pain in both feet and the left leg for over a decade. Two dorsal leads provided coverage for the feet but not the leg. An additional ventral lead at T12 captured the buttock and hamstring, reducing pain from

Spinal Cord Injury as a Socially Lived Disability: A Phenomenological Study of Rehabilitation and Everyday Life Among Community-Dwelling Individuals.

Background/Objectives: Spinal cord injury (SCI) leads to long-term changes in mobility, bodily function, and everyday participation, extending beyond physical impairment to affect autonomy, identity, and social inclusion. In Greece, limited community-based rehabilitation services, environmental inaccessibility, and fragmented follow-up care further shape the lived experience of individuals with SCI. This study aimed to explore the lived experiences and perceived rehabilitation needs of people with paraplegia living in the community, adopting a phenomenological approach to understand rehabilita

Surgical Management of Giant Cervico-Dorsal Aneurysmal Bone Cyst: Case Report with Literature-Review.

Aneurysmal bone cysts (ABC) are benign, vascular, osseous lesions accounting for 1% of all bony tumors. Giant cervicodorsal ABC are rare. Surgical excision is the treatment of choice. Incomplete excision has a high chance of recurrence. Giant lesions of the spinal column require staged surgeries and circumferential instrumentation to avoid instability. An 18-year-old male presenting with acute onset paraplegia and urinary incontinence with features of myelopathy. On evaluation, he was diagnosed with multi-level giant ABC of the cervico-dorsal spine. He underwent tumor excision and instrumentat
