Cervical Myelitis Masquerading as Guillain-Barré Syndrome: A Case of Acute Flaccid Tetraplegia With a Sensory Level at the Third Cervical Dermatome.

Acute flaccid tetraplegia with areflexia and respiratory failure is frequently attributed to Guillain-Barré syndrome in emergency practice. However, high cervical inflammatory spinal cord lesions may present with a similar clinical picture, leading to potential diagnostic delays and inappropriate initial management. A 26-year-old woman presented with sudden interscapular pain followed within hours by rapidly progressive weakness of all four limbs and acute respiratory distress. Neurological examination revealed generalized areflexia, absence of pyramidal signs, and a clearly defined sensory le

First Ecuadorian Pediatric Case of Multisystem and Neurological Involvement Associated with Influenza A-H5N1 Virus-Case Report.

Influenza A (H5N1) is a highly pathogenic zoonotic virus with a human fatality rate of approximately 60%. Pediatric cases and associated neurological manifestations remain poorly documented in Latin America. This report describes the first confirmed Ecuadorian pediatric case of H5N1-associated encephalitis and multisystem organ failure in a previously healthy 9-year-old female following direct contact with infected poultry. The clinical course was characterized by an atypical initial presentation of bilateral periorbital edema and headache, progressing to acute encephalitis, cerebral ischemia,

Acute cervical myelomeningitis secondary to Lyme neuroborreliosis.

BACKGROUND: Lyme disease, caused by the bacterium Borrelia burgdorferi, can cause multi-systemic signs and symptoms, including peripheral and central nervous system disease. Lyme neuroborreliosis rarely presents with spinal cord involvement. CASE DESCRIPTION: We describe a case of a 62-year-old man presenting with fever, meningeal syndrome, rapidly progressive flaccid tetraplegia with preserved sensory function, and urinary and fecal incontinence. Cervical magnetic resonance imaging (MRI) demonstrated extensive intramedullary T2 hyperintensity associated with diffuse circumferential pachymeni

Tibialis Cranialis Extrafusal and Intrafusal Myofibers and Muscle Spindle Pathology in Canine Degenerative Myelopathy.

Canine degenerative myelopathy (DM) is a progressive, adult-onset neurodegenerative disease, with similarities to superoxide dismutase 1 (SOD1)-associated ALS. Clinical signs of DM begin with asymmetric pelvic limb general proprioceptive ataxia and spastic paresis that progresses to flaccid tetraplegia and respiratory failure. Although pelvic limb muscle atrophy and peripheral neuropathy have been reported in DM, intramuscular SOD1 accumulation and muscle spindle pathology remain incompletely characterized. This study evaluated pathological changes in the tibialis cranialis muscle of Pembroke

[Severe eosinophilic granulomatosis with polyangiitis-related peripheral neuropathy after the cessation of mepolizumab. A case report].

A 78-years-old man was treated for asthma and pansinusitis for >5 years, and mepolizumab was initiated two years previously. Two months after the cessation of mepolizumab treatment, the asthma symptoms worsened and acute progressive muscle weakness and sensory disturbance developed. On day 8 after the onset of weakness and hypoesthesia, the patient presented with complete flaccid tetraplegia and diffuse hypoesthesia of all extremities, without paresthesia or pain, and was admitted to our hospital. Blood tests revealed eosinophilia without anti-neutrophil cytoplasmic antibody elevation. Nerve c

Efgartigimod as Add-On Treatment in Refractory Guillain-Barré Syndrome: A Case Report.

BACKGROUND AND AIMS: Guillain-Barré Syndrome (GBS) is an autoimmune disease that can cause flaccid tetraplegia. Efgartigimod, a monoclonal antibody that inhibits the neonatal receptor (FcRn), increases IgG degradation. METHODS: A 60-year-old lady was admitted with acute flaccid tetraparesis, requiring mechanical ventilation within 12 h. Nerve conduction studies were compatible with acute inflammatory demyelinating polyneuropathy (AIDP). No anti-ganglioside or anti-nodal/paranodal antibody could be identified in serum. Despite receiving standard treatment with plasma exchange (PLEX), the patie

Neurogenic Heterotopic Ossification: A Rare Complication of Guillain-Barré Syndrome Exacerbated by Ankylosing Spondylitis and HLA-B27.

We report an exceptional case of neurogenic heterotopic ossification (NHO) developing in a 28-year-old female with pre-existing human leukocyte antigen B27 (HLA-B27) positive ankylosing spondylitis (AS), following a severe course of Guillain-Barré syndrome (GBS). Seven days postpartum, she developed acute GBS requiring three months of intensive care, intubation, mechanical ventilation, and plasmapheresis, leading to flaccid tetraplegia and prolonged immobilization. Four months post-GBS onset, she presented with bilateral hip pain and severely restricted range of motion. Radiographs and CT scan

Critical insights for intensivists on Guillain-Barré syndrome.

Guillain-Barré Syndrome (GBS) is a leading cause of acute flaccid tetraplegia worldwide, with an incidence of 1-2 cases per 100,000 people per year. Characterized by an immune-mediated polyneuropathy, GBS often follows infections or immunological triggers, including vaccinations. The syndrome is classified into three main subtypes based on electrophysiological findings: acute inflammatory demyelinating polyneuropathy (AIDP), acute motor axonal neuropathy (AMAN), and acute motor sensory axonal neuropathy (AMSAN). The pathophysiology of GBS involves molecular mimicry between microbial antigens a

Guillain-Barré Syndrome After a SARS-CoV-2 Vaccine.

The worldwide mass vaccination campaign against COVID-19 has been the largest one ever undertaken. Although the short-term safety profile of the different vaccines has been well established, neuroinflammatory complications have been described, including rare cases of acute demyelinating inflammatory polyneuropathy. We report a 63-year-old man who presented to the emergency department with proximal muscle weakness and paresthesia. He had received the first dose of the AZD1222 SARS-CoV-2 vaccine (Oxford, AstraZeneca) two weeks before presentation. The diagnosis of Guillain-Barré syndrome (GBS) w

Manual therapeutic plasma exchange for treatment of a dog with suspected acute canine polyradiculoneuritis.

BACKGROUND: Acute canine polyradiculoneuritis is one of the most common polyneuropathies occurring in dogs. The disease is very similar to the Guillain-Barré syndrome in humans. In veterinary medicine, there is no established treatment for this disease, while in human medicine, therapeutic plasma exchange and intravenous immunoglobulin administration are two main immunotherapy treatments of this syndrome. CASE PRESENTATION: A 12-year-old male Jack Russel Terrier was presented with a history of acute weakness of the pelvic limbs progressing to flaccid tetraplegia with respiratory compromise. C

The dangers of "Chasing the dragon": a fatal case of heroin-induced leukoencephalopathy.

Heroin-induced leukoencephalopathy (HLE) is a rare toxic encephalopathy associated primarily with heroin inhalation, commonly referred to as "chasing the dragon." This study presents a clinical case of a 27-year-old polydrug user diagnosed with HLE during hospitalization for rapidly progressive flaccid tetraplegia and aphasia. The clinical manifestations encompassed cerebellar and bulbar dysfunction, coupled with motor impairment and altered consciousness. Based on the clinical data and MRI results, HLE was identified as the most likely cause. This article aims to provide insights into the cli

The first pediatric anti-lactosylceramide antibody-positive encephalomyeloradiculoneuropathy.

INTRODUCTION: The anti-lactosylceramide (LacCer) antibody is an anti-neutral glycolipid antibody that is involved in the pathogenesis of encephalomyeloradiculoneuropathy (EMRN). It causes acute and subacute injuries to both the central and peripheral nerves. However, no pediatric cases of anti-LacCer antibody-positive EMRN have been reported so far. CASE: A 12-year-old girl presented with signs of meningitis. She subsequently showed disturbance of consciousness and flaccid tetraplegia and was placed on mechanical ventilation due to respiratory failure. MRI showed lesions in the cerebral white

Childhood-Onset Multifocal Motor Neuropathy with IgM Antibodies to Gangliosides GM1: A Case Report with Poor Outcome.

BACKGROUND: Multifocal motor neuropathy (MMN) is a slowly progressive motor neuropathy characterized by asymmetric muscle weakness without sensory involvement. Typically, MMN respond completely to treatment with intravenous immunoglobulin (IVIg). MMN is even rarer in the pediatric population, where only five patients have been reported up to now. CASE REPORT: We discuss the 3-year follow-up of a 13-year-old girl with MMN who was positive for IgM antibodies to gangliosides GM1. She was diagnosed with MMN in accordance with the European Federation of Neurological Societies/Peripheral Nerve Soci

Guillain-Barré syndrome as only manifestation of COVID-19 infection.

Post-infectious/immune mediated effects of COVID-19 infection include descriptions of Guillain-Barré syndrome (GBS) in patients usually with respiratory failure and after 1-2 weeks from the onset of viral illness. Asymptomatic cases for COVID-19 infection were rarely described. Herein, we studied a 62-year-old patient with progressive weakness of lower extremities, rapidly evolving to a severe, flaccid tetraplegia and dysphagia. Neurological symptoms weren't preceded by fever or pulmonary symptoms. Because of laboratory test abnormalities (thrombocytopenia, lymphocytopenia, high inflammation i
