[Thyrotoxic periodic paralysis: two new cases in Blacks].

INTRODUCTION: Thyreotoxic hypokalemic periodic paralysis (THPP) has mainly been described in Asian people. It is rare in Caucasians, and only 12 cases were reported in Blacks. EXEGESIS: We report two cases of THPP in black patients. Hypokalemia was important in case number 1, leading to severe flaccid tetraplegia with respiratory and cardiac complications, but was only mild in case number 2. Complete relief of paralysis was observed under potassic treatment in the first case and spontaneously in the second one. Hyperthyroidism was established only on the basis of biological tests in case numb

Polyradiculoneuropathy revealing a solitary plasmacytoma of the ilium. A new case-report.

Neurological manifestations are uncommon in myeloma patients, and subacute polyradiculoneuropathy as the inaugural manifestations of solitary plasmacytoma of bone is exceedingly rare. We report the case of a 52-year-old man who was evaluated for a three-month history of flaccid tetraplegia with a gradually ascending onset and for a deterioration in general health. Electromyography findings were consistent with polyradiculoneuropathy. Laboratory tests showed a moderate amount of a monoclonal IgG-lambda antibody. Findings were normal from a radiographic bone survey and a radionuclide bone scan.

Secondary hyperkalaemic paralysis.

Besides the hereditary hyperkalaemic paralysis, a secondary form exists which often mimicks Guillain-Barre syndrome. A 62 year old patient is reported on who developed severe hyperkalaemic paralysis on the basis of mild renal failure and additive spironolactone intake. Neurophysiological examinations disclosed normal muscle fibre activity but delayed nerve conduction velocities indicating that the mechanism underlying secondary hyperkalaemic paralysis is different from channelopathies. Haemodialysis led to complete recovery. Review of the medical literature showed that spironolactone intake is

[A case of acute polyradiculoneuritis with multiple cranial nerve palsy and cerebral lesion--possible evidence of encephalo-myelo-radiculo-neuropathy].

We report a case of acute polyradiculoneuritis with multiple cranial nerve palsy and cerebral lesions. A boy, born on July 26, 1987, developed unusual sensation on the extremities, backache and sleep disturbance on June 23, 1996. On July 2, following a complaint of blindness he developed a convulsion and was admitted to our hospital. Neurological examination revealed intact consciousness, severe external ophthalmoplegia, bifacial palsy and generalized areflexia. On the next day, flaccid tetraplegia and respiratory dysfunction developed and progressed without disturbance of consciousness. After

[Automatic movements of extremities induced in primary massive brain lesion with apneic coma].

In three cases of primary massive brain lesion with apneic coma, various automatic movements of the extremities were elicited by physical or sensory stimulation. In each case, these movements appeared after a period of cessation of spontaneous respiration followed by flaccid tetraplegia. Brainstem reflexes were absent throughout in all cases. The movements were induced mainly by ventroflexion of the neck, and each case showed movements as described below: in the first case, the patient flexed her elbows and raised both arms slowly, a typical Lazarus sign; in the second case, the patient raised

Paralytic illness resembling inflammatory polyradiculoneuropathy in a chimpanzee.

An adult male chimpanzee housed in an outdoor corral with a group of other chimpanzees had an acute onset of ascending motor paresis that progressed to flaccid tetraplegia over 3 days. Tendon reflexes were weak, and CSF protein concentration was high. The chimpanzee regained normal mobility over several months. This chimpanzee's ascending, symmetrical, monophasic, flaccid paralytic illness, with albuminocytologic dissociation in CSF, and recovery following supportive treatment, was characteristic of inflammatory polyradiculoneuropathy, known as Guillain-Barré syndrome in human beings. Coonhoun

[Meningeal seeding of spinal cord glioblastoma multiforme without any signs of myelopathy].

An autopsy case of meningeal spreading of glioblastoma multiforme (GBM) probably originating in the cervical cord was reported. In contrast to autopsy findings, main symptoms were similar to subacute meningitis, and any signs of myelopathy could not be detected during the clinical course. The patient was a 22-year-old man who was hospitalized because of a 2-week history of progressive headache following cough and slight fever. Vomiting and somnolence, developing 5 days before admission, were improved the day after a lumbar puncture performed at another hospital. On admission, meningeal signs,

[Successful treatment with hyperbaric oxygen following severe cerebro-arterial gas embolism].

We report on a patient suffering from a severe cerebral arterial gas embolism associated with decompression from a simulated high pressure chamber dive. Treatment with hyperbaric oxygen (HBO) commenced immediately after the accident and was continued subsequently for 8 weeks with a total of 49 HBO-sessions. Despite initial transitory amaurosis and flaccid tetraplegia lasting for two weeks the patient made a near complete recovery except for circumscript numbness and paraesthesia confined to the left tibia and palm. This case underscores the need to consider patients with cerebral arterial gas

Treatment of hypertensive cerebellar hemorrhage--surgical or conservative management?

One hundred and one consecutive patients with hypertensive cerebellar hemorrhage were analyzed to determine the criteria for surgery. New criteria based on the patient's Glasgow Coma Scale score at admission and the maximum diameter of the hematoma, as disclosed by computed tomography, are proposed from a retrospective analysis of 52 earlier cases. The criteria are as follows: 1) patients with Glasgow Coma Scale scores of 14 or 15 and with a hematoma of less than 40 mm in maximum diameter are treated conservatively; 2) for the patients with Glasgow Coma Scale scores of 13 or less at admission

Multiple deletions of mitochondrial DNA in sporadic and atypical cases of encephalomyopathy.

Multiple deletions of mitochondrial DNA (mtDNA) were first identified in patients with mitochondrial encephalomyopathy with a clear mendelian inheritance. We found this genetic alteration in four atypical and sporadic cases of mitochondrial encephalomyopathy, characterized by RRF and partial COX deficiency. One patient was affected by essential hyperCPKemia, 1 by subacute onset flaccid tetraplegia and 2 by parkinsonism. Southern blot and PCR revealed mtDNA multiple deletions in muscle tissue of these patients. These findings indicate that these alterations are not confined to the families with

[Tetraplegia after colonoscopy: benzodiazepine withdrawal syndrome induced by flumazenil?].

A case is reported of a 68-year-old female patient who developed a flaccid tetraplegia on recovering from a colonoscopy, carried out under general anaesthesia (3 mg of midazolam, 20 mg of etomidate). During the preanaesthetic visit, she omitted to report a 15-year-addiction to lorazepam 2.5 mg every night. At the end of the procedure, 0.2 mg of flumazenil were injected. Recovery was immediate but followed by neurological signs associating general weakness, paraesthesia and tetraplegia. These symptoms discontinued spontaneously two hours later. She was discharged the same day but complained of

Bilateral medial medullary infarction: magnetic resonance imaging and correlative histopathologic findings.

Bilateral medial medullary infarction is a rare event which clinically presents with flaccid tetraplegia sparing the face, bilateral disturbance of deep sensation, hypoglossal nerve palsy and respiratory failure. We here report a patient with such symptoms in whom magnetic resonance imaging enabled the detection of signal abnormalities in the lower brainstem as soon as 9 h after onset. Results of a control study 3 weeks later correlated well with the extent of infarction that was seen at autopsy. Early lesion detection in the lower medulla by magnetic resonance imaging and the unfavorable prog

X-linked ataxia, weakness, deafness, and loss of vision in early childhood with a fatal course.

An X-linked recessive disease with, in almost all patients, a fatal course in early childhood, occurring in a five-generation family is described. The 12 affected boys had early-onset floppiness, ataxia, liability to infections especially of the upper respiratory tract, deafness, and later, a flaccid tetraplegia and areflexia. Eleven boys died before the age of 5 years. One boy is still alive at the age of 12 years, but in addition to the above-mentioned signs, he must be ventilated at night and is nearly blind due to optic atrophy. In the only patient whose central nervous system could be exa

[Meningoencephalitis associated with polyradiculoneuropathy with increased HSV IgG antibody--report of three cases].

We report three patients presented with meningoencephalitis associated with polyradiculoneuropathy with increased HSV IgG antibody titer. The first patient was a 27-year-old woman with meningoencephalitis who developed status epilepticus. The CSF showed pleocytosis and increase in HSV IgG antibody titer. Herpes encephalitis was suspected, and she was treated with acyclovir. The symptoms of meningoencephalitis improved, but she developed flaccid tetraplegia. The NCV study was compatible with polyradiculoneuropathy. About two months later from the onset, muscle atrophy appeared in her all limbs.

[Recurrent peripheral neuropathy caused by thallium poisoning].

A 28-years old woman, working as a secretary, developed a rapidly progressive paraplegia with burning pain in the lower limbs. She recovered within 4 months but relapsed shortly after she was discharged. There was flaccid tetraplegia, associated with hyperalgesia of the limbs, diffuse muscle atrophy, blindness and alopecia. The patient needed respiratory assistance for 2 months. Bilateral optic neuropathy and paraparesis persisted after a 17 months follow-up. The clinical picture suggested thallium poisoning, which was confirmed by high thallium concentration in plasma and urine. The most like

Good recovery after sublethal ethylene glycol intoxication: serial EEG and CT findings.

The case of a young man with sublethal ethylene glycol poisoning is reported. Complete recovery occurred after 12 days of deep coma and a prolonged flaccid tetraplegia. The lesions of gross degree in the CNS, demonstrated by serial CT, resolved and electrophysiological testing as well as neuromuscular signs improved. The recovery, clinically and radiographically, over 35 days was dramatic. Ethylene glycol intoxication should always be considered in the differential diagnosis of acute severe encephalopathy because complete recovery is possible with correct therapy.

[A case of traumatic anterior dislocation of C4 recovered from complete tetraplegia].

A case of traumatic anterior dislocation of C4 is presented. A 65-year-old man who was beastly drunken fell down backward and severely struck occipital region against the door and immediately developed tetraplegia. Neurological examination 12 hours after the trauma revealed complete flaccid tetraplegia, abdominal respiration, bladder-bowel disturbance, anesthesia below C5 and hyperpathia in C3 and C4 dermatomes. Plain films of the cervical spine disclosed anterior dislocation of C4 upon C5 approximately 6 mm and possible disc herniation of C4/5. On Amipaque cervical myelography via C1C2 latera

[Apropos of a case of thyrotoxic periodic paralysis (TPP): electromyographic study (EMG)].

The authors report on a case of thyrotoxic periodic paralysis characterized by attacks of flaccid tetraplegia with hypokalemia in a patient with Basedow disease, the cure of which led to the disappearance of the neurological symptomatology. The EMG investigations, performed in the course of a spontaneous paralytic attack, showed the same abnormalities as in the few similar published cases: discordance between the rich interferential recordings and the weakness of the movements; low amplitude of the electrodetection tracings; reduced amplitude of the evoked muscle action potentials on nerve sti

[An operated case of cervical spontaneous hematomyelia].

We have reported a case of cervical spontaneous hematomyelia caused by cavernous hemangioma. A 47-year-old woman experienced a sudden onset of pain in the neck on 5 December 1979. One week after the onset she began to have tetraparesis. There was a rapidly increasing weakness of the extremities and she was referred to our department on 25 December 1979. At the time, the positive neurological findings were flaccid tetraplegia, bilateral loss of all sensory perception below the C4 level and urinary retention. Chest x-ray films demonstrated an elevation of right diaphragma. Roentgenograms of the

[Acute subdural hygroma in the posterior fossa (author's transl)].

A 2-year-old girl was admitted to Chugoku Rosai Hospital one hour after a car accident on July 16, 1978. On admission she was conscious and no physical or neurologic abnormality was found. However, 2 hours and a half after the accident, she was attacked suddenly by generalized convulsion, and then became comatose with apnea. Bilateral horizontal nystagmus to the left, rhythmic constriction and dilation of the pupils reacting to light, and flaccid tetraplegia were recognized. Plain skull films revealed a linear fracture in the right posterior fossa, which crossed the transverse this region. A b
