A 48-year-old man, who had spastic tetraplegia and a marked atrophy of the medulla oblongata and upper cervical cord, was reported. He began to walk in spastic fashion at the age of 12 years, and was diagnosed as spastic paraplegia. His father, whose onset of the disorder was 35 years old and died of pneumonia at the age of 69 years, manifested symptoms and a course, both resembling those of the present patient. Generalized weakness progressed gradually, and the patient was confined to bed in his forties. On admission, he showed markedly increased tendon reflexes and pathological reflexes in a

We report serial clinical, radiological, and neurophysiological findings of a patient with late-infantile Krabbe disease. At age 13 months, the patient was hospitalized for sudden stiffness and irritability and a diagnosis of spastic diplegia was made. At age 24 months, he was readmitted because of further psychomotor deterioration; neurologically, he manifested severe spastic tetraplegia with optic atrophy. MRI disclosed diffuse high intensity in the cerebral white matter on T2-weighted images. Nerve conduction velocity and evoked potential studies were markedly abnormal, as were the EEG and

Severe degenerative features of the nervous system of a hitherto unknown kind, associated with a neuromuscular disorder with histopathological features of congenital muscular dystrophy, are reported in two female siblings. The clinical profile was characterized by generalized hypotonia followed by spastic tetraplegia, contractures, polyneuropathy, lack of cognitive development and progressive microcephaly. There as no involvement of the eyes. Neuropathological examination of the brain of one sibling, who died at the age of 30 months, revealed subtotal loss of neurons in the cerebral and cerebe

Two sisters with familial Alzheimer's disease developed spastic gait disturbance as an initial manifestation. Their gait disturbance progressed gradually, followed by dementia a few years later. Post-mortem examination of one of the patients disclosed degeneration of the thalamus and corticospinal tract in addition to numerous senile plaques and neurofibrillary tangles in the neocortex, both of which were confirmed by immunohistochemistry. This is the first report in which clinicopathological evaluation is sufficient to establish a new variant of Alzheimer's disease presenting initially as spa

STUDY DESIGN: Pseudohypoparathyroidism associated with disorders of the spine is rarely reported. In this report, the authors present a case of pseudohypoparathyroidism in a 41-year-old man who had narrow spinal canal and multiple disc herniation in the cervical spine.
SUMMARY OF BACKGROUND DATA: The patient had progressive spastic tetraplegia due to cervical cord compression caused by multiple disc herniations at the C3-C4, C4-5, and C5-C6 levels in the developmentally narrow spinal canal. Based on the features of Albright's osteodystrophy, serum laboratory results, and an Ellsworth-Howard t

We report a 21-year-old man with agammaglobulinemia and chronic progressive encephalopathy. The patient was diagnosed as having X-linked agammaglobulinemia at 6 months of age, and gamma globulin supplementation was initiated. He exhibited normal development until he was 11 years old, when he showed a decline in school performance and a personality change. Computed tomography images at that time disclosed diffuse cerebral atrophy. Several generalized tonic-clonic convulsions, myoclonus and spasticity appeared at the age of 13 years. He lost his ability to walk and speak at the age of 17 years o

A 35-year-old female with progressive systemic sclerosis (PSS) and Sjögren's syndrome developed consciousness disturbance and generalized seizure after the episode of fever and erythema lasting for 3 weeks. Neurological examination disclosed deep coma and spastic tetraplegia with pathological reflexes. Laboratory data showed mild anemia, severe hypoproteinemia, hypoalbuminemia and increase of protein content in the cerebrospinal fluid. Cranial CT scans obtained after convulsion revealed diffuse brain swelling and bilateral symmetrical hypodensity involving the thalami and posterior limbs of th

A syndrome of ocular and cutaneous hypopigmentation, severe mental retardation with spastic tetraplegia and athetosis was first observed by Cross in three siblings of an inbred Amish family. Since then, seven other patients, three sporadic and four with familial recurrence, have been reported in the literature, confirming the autosomal recessive inheritance. The clinical spectrum of the syndrome has been expanded to include true developmental defects of the CNS such as cystic malformation of the posterior fossa of the Dandy-Walker type. We report a new case of Cross syndrome.

Argininemia is caused by a hereditary deficiency of liver-type arginase (E.C.3.5.3.1) and is characterized by psychomotor retardation and spastic tetraplegia. We examined findings in three Japanese patients with argininemia, by using the PCR, cloning, and sequencing procedures. We found three different mutations--G-to-A-365 in exon 4, G-to-C-703 in exon 7, and C-del-842 in exon 8--thereby leading to mutant arginase proteins of W122X, G235R, and L282FS, respectively. Patient 1 was a compound heterozygote, inheriting the allele with G-to-A-365 from his mother and the allele with G-to-C-703 from

We re-examined 371 infants with birth weights less than 1501 g at a corrected age of 18-20 months. This sample amounted to 91% of such infants admitted to one of the six neonatal intensive care units in Hamburg between July 1983 and 1986. The neurological examination and a developmental evaluation using the Griffith Developmental Scale revealed higher rates of abnormalities than in most other studies. Fifty-five children (14.8%) suffered from cerebral palsy, classified in 45 as spastic diplegia, in 5 as spastic tetraplegia, in 1 as spastic hemiplegia and in 4 as dystonia. Of the children, 41 (

Spastic tetraplegia due to cervical medullary injury above the origin of the phrenic nerve neurons means complete respiratory paralysis. In this case a phrenic pacemaker can be considered as an alternative to long term mechanical ventilation. An implantable 8-channel stimulation device has been developed for functional electrical stimulation of the phrenic nerves. The "Karussellstimulation" provides fatigue free stimulation for 24 hours a day. We report the case of a 12 year old girl with complete ventilatory insufficiency who underwent implantation of a phrenic pacemaker. 18 month after impla

Between December 1985 and July 1986 a study on cerebral palsy was undertaken among the inpatients and outpatients of the department of Paediatrics and Child Health, Muhimbili Medical Centre Centre, Dar Es Salaam. The objective of the study was to determine the clinical pattern of cerebral palsy and its associated handicaps. During this period, 100 children with cerebral palsy 56 boys and 44 girls ranging in age between four months and 10 years were seen. The commonest type of cerebral palsy seen was spastic tetraplegia which occurred in 36 percent of the cases followed by spastic diplegia and

I identified forty-three patients with severe retardation aged 6-14 years in a defined community. More than half of the patients (23/43) had spastic tetraplegia and 9 had spastic diplegia. Only one case showed abnormal increase of muscle tone of dyskinetic type. The period when they had got underlying causes of their handicaps was as follows : congenital (acquired intrauterine, including intractable cases) 25, perinatal 6, and after the neonatal period 12. An increased proportion of congenital cases and a decreased proportion of perinatal cases are noted when the data were compared with previo

The incidence of inguinal hernia among 247 children with cerebral palsy was ascertained. During the first year of life, 20 of the 153 boys developed hernia, as did one of the 94 girls. Among boys with birthweights of 1000 to 2000g the incidence was 31 per cent, which is twice the rate for normal children. The incidence among boys with birthweights greater than 2000g was 8 per cent. A comparison group could not be found, but this incidence appears to be excessive, considering the reported normal incidence of 1 to 4 per cent in boys. The authors recommend routine examination for inguinal hernia,

The prevalence of cerebral palsy (CP) increased significantly from 1968-72 to 1978-82 in the region of the University Central Hospital of Turku, Finland, from 1.6/1,000 live births during the first five-year period to 2.5/1,000 live births during the second. The increase was due to an increase of CP in low birthweight children (p = 0.0002). The distribution of CP patterns did not change significantly. During the second period the diagnosis was made earlier in all groups (except that of spastic tetraplegia), though it reached statistical significance only in the hemi- and diplegic groups. While

A care-load study was performed on a homogeneous group of 51 children, adolescents and young adults with spastic tetraplegia, born in Sweden between 1959 and 1978. All were immobile and severely mentally retarded, none could speak and the majority had epilepsy, scoliosis and contractures. 16 were living with their families and 35 in care units. The mean time spent meeting basic care requirements was 24.7 hours/week (median 23.1), with feeding being the most time-consuming. 47 individuals went to training schools or day centres daily. Treatment by physiotherapists totalled a mean of 2.3 hours/w
