Seven-day Accelerometer-derived Wrist Activity after Acute Stroke Stratified by Hemiparesis Severity: A Prospective Observational Pilot Study.

OBJECTIVES: This hypothesis-driven prospective pilot study characterized 7-day trajectories of bilateral wrist activity after acute stroke by baseline Fugl-Meyer Assessment-Upper Extremity (FMA-UE) severity. We hypothesized that mild impairment would be associated with greater paretic-side activity and more symmetric activity, whereas bilateral balance would change little despite clinical improvement. METHODS: Patients who began rehabilitation within 3 days after stroke wore bilateral wrist accelerometers for 7 days. Exploratory strata were mild (FMA-UE >45) and severe (≤45), based on an infl

Microsurgical Revascularization for Thrombosed Middle Cerebral Artery Dissecting Aneurysms: Classification Scheme and Surgical Strategies.

OBJECTIVE: Thrombosed dissecting aneurysms represent highly challenging lesions in neurosurgery. Among these, thrombosed middle cerebral artery (MCA) dissecting aneurysms are clinically rare and exceptionally difficult to manage. Their defining feature is the extensive organized thrombus within the aneurysmal lumen and the subsequent progressive mass effect, which presents a significant dilemma for traditional endovascular treatment and microsurgical clipping to balance safety and efficacy. To address this, the authors summarize the clinical experience of a single senior surgeon at our center

Sporadic Multiple Meningiomas Presenting With Drug-Resistant Seizures in a Young Adult: A Case Report.

Meningiomas represent the most common benign primary intracranial tumors. However, multiple meningiomas (MMs) are relatively rare and can be challenging to manage, often requiring repeated surgical interventions and long-term follow-up. Here, we present the case of a 28-year-old woman who was operated on in 2019 for a right parasagittal MM, WHO grade 1. Her initial presentation was characterized by seizures and left-sided hemiplegia. Seven years later, she presented with recurrent seizures that were resistant to medical treatment. Neurological examination revealed residual left-sided spastic p

Acute Cauda Equina Syndrome in Rapid Succession: A Case Series of Eight Consecutive Patients Undergoing Emergency Surgical Decompression Over One Week.

INTRODUCTION: Cauda equina syndrome (CES) is a rare but devastating spinal surgical emergency, most commonly caused by lumbar disc herniation. Delayed diagnosis or decompression may result in irreversible neurological and sphincter dysfunction. MATERIALS AND METHODS: This retrospective single-Center case series included eight consecutive patients with acute CES who underwent emergency surgical decompression over 1 week at a tertiary spine care center. Clinical presentation, radiological findings, surgical management, and 6-month post-operative outcomes were evaluated. RESULTS: The cohort inc

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

Ketogenic diet therapy in developmental and epileptic encephalopathies: toward syndrome- and gene-informed precision therapy.

BACKGROUND: Developmental and epileptic encephalopathies (DEEs) are severe, early-onset, frequently drug-resistant epilepsies in which recurrent seizures and epileptiform activity contribute to cognitive impairment beyond the underlying etiology. Ketogenic dietary therapies (KDTs) exert broad, multimodal antiseizure and neuroprotective effects and occupy a distinct position among nonpharmacologic treatments for DEEs. OBJECTIVE: To synthesize the evidence for KDT across the principal electroclinical DEE syndromes, an expanding spectrum of gene-specific DEEs, and acute presentations such as feb

Reconnecting the disconnected forebrain: multimodal resting-state bold dynamics underlying zolpidem-induced arousal in incomplete Locked-in syndrome.

PRIMARY OBJECTIVE: Zolpidem paradoxically restores behavioral responsiveness and communication in select patients with severe disorders of consciousness and profound motor deafferentation. While the anterior forebrain mesocircuit hypothesis attributes this awakening to central thalamic disinhibition, macroscopic network dynamics and lower brainstem gating remain under-characterized in locked-in syndrome (LIS). METHODS AND PROCEDURES: A 45-year-old male with ventral pontine necrosis initially misdiagnosed as vegetative state/unresponsive wakefulness syndrome (VS/UWS) was formally identified wi

Association of trunk and lower limb muscle thickness with sedentary behavior in patients with chronic post-stroke hemiplegia.

[Purpose] Individuals with chronic stroke often exhibit low physical activity and prolonged sedentary behavior, possibly due to muscle weakness and impaired postural control. This study investigated associations between trunk and lower limb muscle thickness and physical activity, including sedentary behavior, in ambulatory individuals with chronic post-stroke hemiplegia. [Participants and Methods] Twenty-six ambulatory individuals with chronic post-stroke hemiplegia were included. Physical activity was assessed for 7 consecutive days using a triaxial accelerometer and categorized as sedentary

A De Novo ATP1A3 p.Arg995His Variant in a Patient With an Adult-Onset Primary Lateral Sclerosis-Like Syndrome.

Pathogenic variants in ATP1A3 are classically associated with alternating hemiplegia of childhood, CAPOS syndrome, and rapid-onset dystonia-parkinsonism. However, the phenotypic spectrum of ATP1A3-related disease has expanded considerably in recent years, including atypical presentations with spasticity and hereditary spastic paraplegia-like phenotypes. We describe a 43-year-old woman who developed a progressive upper motor neuron syndrome characterized by spastic paraparesis, generalized hyperreflexia, bilateral Babinski signs, inexhaustible ankle clonus, spastic dysarthria, dysphagia, and mi

Bilateral Latent Synergy-Informed Residual sEMG Reconstruction for Incomplete Hand Gesture Recognition in Hemiparetic Patients.

Accurate decoding of incomplete hand gestures from residual paretic surface electromyography (sEMG) remains challenging after stroke because voluntary myoelectric activity is often weak, non-stationary, and contaminated by pathological co-activation. This study proposes a Latent Synergy-Informed Purification Network (LSP-Net) for reference-guided reconstruction of residual paretic sEMG. Specifically, LSP-Net jointly models synchronous paretic- and non-paretic-side sEMG, using the non-paretic signal as both bilateral reference information and a paired waveform-level supervision target for laten

Ischemic stroke as the first manifestation of acute promyelocytic.

Acute promyelocytic leukemia (APL) is a hematologic emergency characterized by a high risk of coagulopathy that can manifest with both severe bleeding and thrombotic events, including ischemic stroke. However, its initial presentation as large vessel arterial thrombosis is infrequent and potentially lethal. We report the case of a previously healthy 15-year-old female referred for suspected left frontal ischemic stroke with right spastic hemiplegia and expressive aphasia. One week earlier, she had presented with headache, nausea and transient episodes of paresthesias and weakness of the right

Inferior vena cava hyperechoic structure in a patient with cardiac myxoma: a surgical case report and literature review.

BACKGROUND: IVC hyperechoic structure detected by echocardiography is relatively uncommon and is usually associated with deep vein thrombosis and cancers. In most cases, the etiology can be clarified using computed tomography (CT) and magnetic resonance imaging (MRI). However, we encountered an IVC hyperechoic structure that was not confirmed by multimodality imaging in a patient with cardiac myxoma. CASE PRESENTATION: A 62-year-old woman presented with sudden-onset left hemiplegia and right homonymous hemianopsia. On arrival, her left arm was cold to the touch. Contrast-enhanced CT demonstra

Spinal Subdural Abscess in the Emergency Department: A Rare Diagnosis in an Immunocompetent Host.

Spinal subdural abscess is an exceptionally rare and potentially life-threatening spinal infection characterized by the accumulation of purulent material between the dura and arachnoid mater. In contrast to the more common spinal epidural abscess, spinal subdural abscesses have been reported in fewer than 80 cases, often in pediatric, postoperative, or immunocompromised patients. This case represents the fourth documented instance of a spinal subdural abscess in an immunocompetent adult without identifiable risk factors or a known source of infection and the first reported in emergency medicin
