Navigation-Guided Resection of a Cystic Schwannoma with Spinopelvic Fixation in Acute Cauda Equina Syndrome: A Case Report.

CASE: A 45-year-old female patient presented with acute perianal numbness, severe back pain, and bilateral radiculopathy. Magnetic resonance imaging revealed a large cystic lesion involving the L5-S1 vertebrae with compression of the thecal sac. Given the erosion of the L5 pedicle and sacral involvement, navigation-guided tumor resection with L4-S2 alar-iliac fixation and L4-5 transforaminal lumbar interbody fusion was performed. Intraoperative O-arm navigation ensured accurate resection and safe instrumentation despite distorted anatomy. Postoperatively, the patient had significant pain relie

Bizarre astrocytes with cytoplasmic/intranuclear inclusions in an individual with alternating hemiplegia, migraine, and brain swelling associated with a GGC repeat expansion in NOTCH2NLC.

The aberrant GGC repeat expansion in the 5'-untranslated region of the NOTCH2NLC gene causes neuronal intranuclear inclusion disease (NIID), a progressive neurodegenerative disorder. The clinical features of NIID are highly variable and include cognitive dysfunction, peripheral neuropathy, and episodic neurogenic symptoms. The pathogenesis of episodic symptoms in NIIDs remains unknown, and histopathological studies are limited. Here, we report an autopsy case of NIID in a 32-year-old Japanese female who developed severe episodic symptoms, including hemiplegic migraine, seizures, and impaired c

The prevalence of ENPP1 deficiency in humans with Ossification of the Posterior Longitudinal Ligament and the preclinical efficacy of ENPP1 enzyme therapy in murine Ossification of the Posterior Longitudinal Ligament.

Ossification of the posterior longitudinal ligament (OPLL) and diffuse idiopathic skeletal hyperostosis are debilitating conditions characterized by pain, stiffness, myelopathy, and impaired mobility due to progressive enthesopathies and spinal fractures. These disorders worsen with age and may lead to hemiplegia. The underlying mechanisms of these diseases remain poorly understood, and effective treatments are currently lacking. To elucidate the pathogenesis of OPLL, we conducted a prospective study involving plasma analyte measurement in 50 consecutive OPLL and 25 consecutive cervical osteoa

Quantitative Assessment of Upper Limb Multi-Modal Feature Fusion Under Task-Oriented Movement.

Assessment of motor function is an important component of a post-stroke rehabilitation program. The traditional assessment process mainly relies on clinical experience and lacks quantitative analysis. To objectively assess the upper limb motor status of post-stroke hemiplegic patients, this study proposes a novel assessment method based on multi-modal feature fusion of the upper limb for task-oriented movement. Features are extracted from each modal data and input into the corresponding base classifiers. The kinematic and muscle synergy are quantified by singular value decomposition (SVD) and

Attention-Adaptive BCI-AOT System Enhances Motor Recovery and Neural Engagement After Stroke.

Stroke frequently results in long-term motor deficits that impair quality of life. Action observation therapy (AOT) has shown promise for motor recovery through engagement of the mirror neuron system (MNS), yet its passive nature and lack of attentional tracking limit its neuroplasticity efficacy. To address these limitations, we developed a closed-loop Brain-Computer Interface-integrated AOT (BCI-AOT) system employing real-time Steady-State Visual Evoked Potential (SSVEP)-based attention monitoring to dynamically control therapy delivery. In a within-subject crossover study, 22 individuals wi

Predicting rehabilitation outcomes of unilateral stroke after brain-computer interface training based on magnetic resonance imaging data.

Stroke remains a significant cause of disability globally, with a noticeable prevalence in China. Post-stroke rehabilitation, particularly through brain-computer interface (BCI) methods, plays a vital role in enhancing motor function recovery. However, the efficacy of BCI rehabilitation might be hindered by challenges in individualized program of prognosis prediction. This study aimed to develop prognostic prediction models for unilateral hemiplegia after BCI rehabilitation, utilizing both clinical and functional magnetic resonance imaging (fMRI) data, in order to enhance treatment efficiency

Machine Learning-driven Probability Calculators Can Accurately Predict 1-year Mortality After Proximal Humerus Fractures in Patients Over the Age of 65 Years.

BACKGROUND: Proximal humerus fractures (PHFs) in patients ≥ 65 years of age are associated with increased risk of death in the months after injury. Controversy exists regarding the preferred treatment strategy in these patients, and operative treatment is associated with high complication and reoperation rates. Machine learning (ML)-driven probability calculators for mortality prediction therefore may be valuable during shared decision-making for surgeons and patients. QUESTIONS/PURPOSES: (1) To develop ML algorithms to predict 1-year mortality in patients ≥ 65 years of age. (2) To externally

Normal pressure hydrocephalus plus atypical presentation with symptomatic resolution following the restoration of CSF flow: illustrative case.

BACKGROUND: Normal pressure hydrocephalus (NPH) is a condition characterized by enlarged intracerebral ventricles with normal intracranial pressure. The typical clinical presentation includes cognitive decline, gait disturbances, and urinary incontinence. While the classic triad of symptoms is well documented, atypical presentations, including focal deficits and aphasia, pose significant diagnostic challenges. OBSERVATIONS: This is the case of a previously healthy 70-year-old patient presenting with rapidly progressive gait disturbances, right hemiplegia, cognitive decline, and mutism. The fo

Targeted and Sequential Cryoneurolysis Improves Gait After Botulinum-Toxin Unresponsiveness in Post-Stroke Spasticity: A Laboratory-Verified Case.

Background: Chronic post-stroke spasticity often limits gait despite best-practice botulinum-toxin intramuscular injections (BTIs), whose benefit is constrained by short duration, dose ceilings, and tachyphylaxis. Cryoneurolysis (CNL) induces a reversible axonotmesis with preserved endoneurium, potentially providing longer tone reduction with fewer adverse effects, but its impact on whole-gait quality and its compatibility with implanted functional electrical stimulation (FES) remain poorly documented. Case presentation: A 43-year-old man, 12 years after right middle cerebral artery stroke, wa
