Reliability of upper and lower limb three-dimensional kinematics in children with hemiplegia.

The repeatability of both 3D kinematic measurements of arm movement during simple upper limb tasks and lower limb movement during gait analysis was evaluated in 10 children with hemiplegic cerebral palsy. All tasks were completed on two separate occasions, 1 week apart. The 3D lower limb gait analysis showed high levels of repeatability in the sagittal plane measures, with mean coefficient of multiple correlations (CMCs) greater than 0.92 between sessions. Transverse and frontal plane measures had mean CMCs greater than 0.7 between sessions. A 3D analysis of shoulder and elbow flexion/extensio

Electromyographic evaluation of the sound and involved side during gait of spastic hemiplegic children with cerebral palsy.

The purpose of this study was to investigate the surface electromyogram (EMG) of the lower limbs of hemiplegic children with spastic cerebral palsy during gait. The EMG of seven muscles was analyzed for 17 children (5-12 years old). The EMG of the involved side was decreased for the examined shank muscles and increased for the hamstrings, during certain phases of the gait cycle, compared with the sound side. The rectus femoris of the involved side showed prolonged activation during the swing phase. The enhanced activation of the hamstrings may be beneficial/compensatory, preventing knee hypere

Immediate effect of percutaneous intramuscular stimulation during gait in children with cerebral palsy: a feasibility study.

The feasibility of percutaneous intramuscular functional electrical stimulation (P-FES) in children with cerebral palsy (CP) for immediate improvement of ankle kinematics during gait has not previously been reported. Eight children with CP (six with diplegia, two with hemiplegia; mean age 9 years 1 month [SD 1 y 4 mo; range 7 y 11 mo to 11 y 10 mo]) had percutaneous intramuscular electrodes implanted into the gastrocnemius (GA) and tibialis anterior (TA) muscles of their involved limbs. Stimulation was provided during appropriate phases of the gait cycle in three conditions (GA only, TA only,

Factors predicting the efficacy of botulinum toxin-A treatment of the lower limb in children with cerebral palsy.

Botulinum toxin A is widely used for spasticity management in children with cerebral palsy, although outcomes are unpredictable. The aim of this study was to identify criteria for selecting patients most likely to benefit from botulinum toxin A treatment. Fifty-five subjects, aged 2.5 to 18 years, were recruited. The assessment covered measures of spasticity (Modified Ashworth Scale), function (using the Gross Motor Function Measure and the Physician's Rating Scale), selective motor control, static range of motion at the ankle with knee extended and flexed, range of motion of the knee flexors,

Quantitative segmental analysis of weight-bearing radiographs of the foot and ankle for children: normal alignment.

Clinical decision-making for the management of foot deformities in children is primarily based upon the analysis of weight-bearing radiographs of the foot and ankle. However, a comprehensive quantitative technique for the analysis of such radiographs has not been described. Ten radiographic measurements were developed and applied to the foot and ankle radiographs of a normal foot and ankle in 60 children (mean age 10 years, range 5-17 years). Intraobserver variability and interobserver variability were determined for 10 cases. Mean values for the 10 measurements were calculated from the entire

Concurrent cerebral and axillary artery occlusion: a possible source of cerebral embolization from peripheral artery thrombosis.

Intracranial embolization usually arises from heart, precerebral artery or aorta but rarely the peripheral artery as retrograde upstream of embolus should be overcome. We encountered a woman who experienced a transient right acronumbness followed by a sudden onset of left hemiplegia and conscious change concurrently. Cranial computerized tomography revealed a hemorrhagic infarct at right lentiform nucleus. Angiography disclosed right axillary artery occlusion at the proximal portion without collateral circulation. Cardioaortic survey and coagulation profile were normal. Thrombectomy was done a

Validation of a visual gait assessment scale for children with hemiplegic cerebral palsy.

This study reports the experience of reliability testing and validation of a visual assessment of gait based on the Physician Rating Scale in children with hemiplegic cerebral palsy. Comparison with three-dimensional gait analysis showed excellent correlation between observers and full gait analysis for sagittal plane motion at the knee in stance. Inter and intra-rater reliability showed moderate to almost perfect agreement for foot contact characteristics and ankle in stance. Use of this scale may be helpful in assessment of children where frequent analysis of these characteristics is needed.

Bilateral foot center of pressure measures predict hemiparetic gait velocity.

Stroke is a major cause of disability in the US, leaving most survivors with abnormal motor function, often resulting in hemiparetic gait. Quality of gait in this population has been well characterized by measures of velocity, though velocity alone provides only a general index of functional mobility. Researchers and clinicians should continue to develop a complete characterization of behavioral alterations, but should also work to identify the neuromechanical processes underlying the loss and recovery of locomotor function. We hypothesized that selected foot center of pressure (CoP) measures

Stepping over obstacles of different heights: kinematic and kinetic strategies of leading limb in hemiplegic children.

Twelve hemiplegic children (HC) and five normally developing children (NDC) were instructed to walk barefoot over a level surface and to cross obstacles of various heights (0%, 10%, 15% and 20% of their leg lengths). Kinematic and kinetic strategies of their leading limb were mainly examined. For the starting position of the trials HC was adjusted so that the affected limb was the leading one, while NDC step over the obstacle with their preferred selection of leading and trailing limb. The results demonstrated that the crossing speed was decreased and the crossing swing phase was increased in

[Reflex responses from the sural nerve to tibialis anterior muscle in hemiplegic patients: the relation between the responses and Babinski sign].

Stimulation of the sural nerve of healthy subjects induced short latency inhibition in the ipsilateral tibialis anterior muscle and facilitation in peroneal muscle. We examined lower limb muscle responses after stimulation of the sural nerve in 19 patients with hemiplegia caused by cerebro-vascular disease and compared them with the control responses. The sural nerve was stimulated electrically (3 or 5 square wave pulses of 0.5 ms repeated at 250 Hz) during weak tonic contraction. Stimulation was triggered to average the rectified surface electromyography (EMG) of the test muscle. Usually 100

[Refractory Evans syndrome complicated with transverse sinus thrombosis].

A 29-year-old woman was diagnosed as having Evans syndrome in 2002 and underwent a splenectomy for the refractory status of the disorder in May 2004. One and a half months after the operation, her platelet count again decreased due to relapse, and she was then prescribed with high dose dexamethasone (38 mg/day x 4 days). Five days after the medication, she complained of a severe headache and then fell into coma, even though her platelet count had risen to 8 x 10(4)/mm3. Computer tomography scan of the brain showed severe edema with a massive hemorrhage in left temporooccipital lobe, which was

Macrophageal/microglial cell activation and cerebral injury induced by excretory-secretory products secreted by Paragonimus westermani.

Cerebral paragonimiasis causes various neurological disorders including seizures, visual impairment and hemiplegia. The excretory-secretory product (ESP) released by Paragonimus westermani has a cysteine protease activity and plays important roles in its migration in the host tissue and modulation of host immune responses. To gain more insight into the pathogenesis of ESP in the brain, we investigated the inflammatory reaction and cerebral injury following microinjection of ESP into rat striatum. The size of injury was maximally observed 3 days after microinjection of ESP and then declined to
