Brain intraparenchymal hemorrhage following urgent cesarean section: a case report.

BACKGROUND: Intracerebral hemorrhage in the postpartum period is an uncommon but potentially life-threatening condition. Hypertensive disorders of pregnancy remain the most frequent cause. However, rare case reports have described intracranial bleeding occurring after neuraxial anesthesia. However, the coexistence of multiple confounding factors, including hypertensive crises, individual cerebrovascular vulnerability, and family history of hemorrhagic events, complicates interpretation. Establishing causation is particularly challenging, as most available evidence consists of sporadic case des

Surgical Management of a Rare Ascending Aortic Mural Thrombus Using a Modified Brain Isolation Strategy: A Case Report.

INTRODUCTION: Ascending aortic mural thrombus (AMT) is an exceptionally rare condition. Diagnosis is often difficult because non-ECG-gated contrast-enhanced CT is susceptible to motion artifacts, and the distal ascending aorta can sometimes be challenging to evaluate using transesophageal echocardiography. A standardized surgical strategy for ascending AMT has not been established. CASE PRESENTATION: A 61-year-old man presented with transient left hemiplegia caused by embolic stroke. A non-ECG-gated contrast-enhanced CT scan revealed a thrombus-like structure in the ascending aorta; however,

Modulation of cerebral activation strategies by training mode in stratified stroke cohorts: an fNIRS study.

INTRODUCTION: Robot-assisted training (RAT) exhibits inconsistent efficacy in post-stroke upper limb rehabilitation, with its underlying neural mechanisms remaining unclear. This study aimed to investigate how different therapy modes modulate cerebral activation strategies in distinct subgroups of stroke patients. METHODS: We utilized functional near-infrared spectroscopy (fNIRS) to investigate differences in cortical activation strategies, specifically the response sensitivity to various robotic training modes, by stratifying forty-one patients based on functional level, disease chronicity,

Additive effects of mirror therapy and bilateral TENS on upper limb recovery in chronic stroke: a randomized controlled trial.

OBJECTIVE: To determine whether the addition of mirror therapy to bilateral transcutaneous electrical nerve stimulation (Bi-TENS), a supraspinally acting intervention, yields additional benefits in upper extremity motor recovery after chronic stroke. METHODS: Thirty individuals with chronic hemiparesis were randomized to receive either Bi-TENS combined with mirror therapy (n = 15) or Bi-TENS alone (n = 15). In both groups, Bi-TENS was applied bilaterally to the finger extensor muscles for 5 sessions per week over 4 weeks. Upper extremity motor function was assessed using the Fugl-Meyer Assess

Basal ganglia glioblastoma with lung adenocarcinoma tumor-to-tumor metastasis and atypical early imaging presentation: illustrative case.

BACKGROUND: Tumor-to-tumor metastasis is a rare oncological phenomenon in which one neoplasm serves as the recipient for metastatic seeding from another primary tumor. Glioblastoma is an exceptionally uncommon recipient, and no prior reports have described this process occurring in the setting of atypical early imaging presentation. OBSERVATIONS: The authors present an unusual case of a 75-year-old man with hemiplegia and hemianesthesia with initial brain imaging demonstrating a solitary right basal ganglia diffusion-restricting, nonenhancing lesion initially diagnosed as a subacute ischemic

D801N in ATP1A3-encoded Na/K-ATPase alpha 3 causes cardiac arrhythmogenesis through sodium-calcium exchanger-mediated calcium overload.

Short QT syndrome is a heritable arrhythmia disorder linked to sudden cardiac death. We recently identified that individuals with alternating hemiplegia of childhood (AHC), a rare neurodevelopmental disorder, can exhibit shortened corrected QT intervals and elevated risk for ventricular fibrillation. This is especially true for patients with AHC heterozygous for the recurrent ATP1A3-D801N variant, though the underlying cardiac mechanism remains unclear. We hypothesized that the D801N missense impairs Na+/K+-ATPase function, causing Ca2+ overload, shortened action potential duration (APD), and

Wrist Flexor Spasticity and Hemiplegic-Contralateral Median Nerve Latency Asymmetry After Stroke: A Bilateral Nerve Conduction and Ultrasound Study.

Background/Objectives: The paretic wrist after stroke may exhibit median nerve conduction abnormalities, but factors underlying hemiplegic-contralateral asymmetry remain uncertain. We compared electrodiagnostic and ultrasonographic wrist measures between sides and assessed predictors of side-to-side differences in distal motor latency (ΔDML) and distal sensory latency (ΔDSL). Methods: We retrospectively analyzed 85 patients with stroke. Distal motor latency (DML), distal sensory latency (DSL), wrist-to-forearm ratio (WFR), and median nerve inlet cross-sectional area (CSA) were measured bilater

A Silent Vasculitis With a Loud Presentation: Takayasu Arteritis Causing Intracerebral Hemorrhage.

Takayasu arteritis is a chronic large-vessel vasculitis affecting the aorta and its major branches, most commonly in young women. Neurological involvement is well recognized but typically manifests as ischemic stroke due to arterial stenosis or occlusion. Intracerebral hemorrhage as the initial presentation of Takayasu arteritis is rare and is usually related to severe secondary hypertension or extensive vascular disease. We describe a 27-year-old woman who presented with acute left-sided hemiplegia and facial asymmetry and was found to have a right gangliocapsular intracerebral hemorrhage. Sh

Reversible cerebral vasoconstriction syndrome following ciltacabtagene autoleucel therapy for relapsed multiple myeloma: a case report.

Ciltacabtagene autoleucel (cilta-cel), a BCMA-directed CAR-T therapy, is approved for relapsed/refractory multiple myeloma (RRMM). Cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS) are common, but reversible cerebral vasoconstriction syndrome (RCVS) is a previously unreported complication following cilta-cel. A 63-year-old female with RRMM (IgA lambda, t(14;16)), no vascular risk factors, received cilta-cel after two prior therapies, including autologous transplantation. On day +10 post-CAR-T infusion, she developed grade 1 CRS, managed with toc

Model-Free Adaptive Control-Based Electrical Stimulation Modulation System for Upper Limb Bi-Joint Function.

Functional Electrical Stimulation (FES) is a rehabilitation technique that helps restore or improve motor function by activating muscles through the distribution of electrical signals to nerves or muscles. However, because of the nonlinear and time-varying characteristics of muscle feedback to external electrical stimulation, achieving highly accurate real-time control is difficult. To precisely control the angles of the elbow joint and wrist joints of the upper limb simultaneously, this study adopted a model-free adaptive control (MFAC) algorithm to design the controller and improve the singl

International consensus recommendations for the diagnosis and treatment of Rasmussen syndrome: A modified Delphi procedure.

Rasmussen syndrome (RS) includes a well-described constellation of refractory focal seizures, often including epilepsia partialis continua, hemiplegia with progressive unilateral cortical atrophy, and cognitive/language decline. However, the precise early pathogenesis and reliable biomarkers remain elusive. In addition, we lack operational management guidelines, including diagnostic evaluation, disease-monitoring assessments, and medical and surgical treatment approaches. We aimed to create an expert consensus statement to guide and standardize the treatment of RS, with the goal of providing r

Apollo microcatheter-assisted transvenous embolization for brain arteriovenous malformation with a tortuous stenotic drainage vein and "Caput Medusa" sign: a case report.

INTRODUCTION AND IMPORTANCE: Transvenous embolization (TVE) for brain arteriovenous malformations (AVMs) typically requires two microcatheters or a dual-lumen mini-balloon catheter in the draining vein. A single Apollo microcatheter can replace these devices, offering a novel TVE alternative for AVMs with the "Caput Medusa" sign. This case report describes the technique, as such reports remain limited. CASE PRESENTATION: A 34-year-old male patient suffered intracranial hemorrhage and developed right-sided hemiplegia and aphasia. Angiography revealed a left corpus callosum AVM supplied by the

Luton cauda equina syndrome pathway: a pragmatic approach to achieve a national target of time to scan in UK district general hospitals.

AIMS: Cauda equina syndrome (CES) is a rare neurosurgical emergency where diagnosis and treatment by emergency spinal decompression is time critical. A national CES pathway from Getting It Right First Time (GIRFT) suggested development of 24/7 MRI services at district general hospitals (DGHs). It gave a national target of time to scan time between MRI scan request and actual scan of less than four hours. The Luton CES pathway was introduced in 2021 at Luton and Dunstable University Hospital, UK, to improve the time (in hours) to a MRI service, as well as to streamline patient care for suspecte
