Alternating hemiplegia of childhood (AHC) represents a severe and complex pediatric neurodevelopmental disorder, predominantly characterized by the occurrence of paroxysmal episodes of transient unilateral or bilateral paresis prior to 18 months of age. It belongs to a group of ultra-rare neurological disorders with a prevalence from 1:100,000 to 1:1,000,000. Even though the majority of AHC patients harbor pathogenic variants in the ATP1A3 gene, recent studies have pinpointed to other causative genes in ATP1A3-negative patients, including RHOBTB2. In this review, we report the case of a patien

OBJECTIVES: To investigate current Canadian physicians' practice patterns of treating upper limb post-stroke spasticity (PSS) and hemiplegic shoulder pain (HSP) acutely after a stroke. In addition, by examining Canadian physicians' diagnostic capabilities, time till treatment, minimum criteria to begin treatment, mechanisms of treatment, targeting of muscles, and benefits and adverse effects of treatment, we aim to learn about areas of improvement to optimize PSS management for Canadians.
DESIGN: The present study was a cross-sectional survey, polling practicing Canadian physicians.
RESULTS:

Background: Familial hemiplegic migraine (FHM) is a rare and severe form of migraine disorder featuring aura symptoms that include hemiplegia during attacks. While pathogenic missense variants in CACNA1A, ATP1A2, and SCN1A can cause FHM or its sporadic form, they explain less than 20% of suspected hemiplegic migraine cases, suggesting the involvement of other genes or genetic variations, potentially including copy number variations (CNVs). PPRT2 gene variants including CNVs have also been implicated in hemiplegic migraine. Methods: Multiplex ligation-dependent probe amplification (MLPA) assays

Hemiplegia following stroke is characterized by disrupted neuromuscular interactions, yet the central-peripheral dynamics remain unclear. This study investigated dynamic causal interactions between electroencephalography (EEG) and electromyography (EMG) using the adaptive directed transfer function (ADTF) during a thumb-pressing task in hemiplegic patients and explored the central-peripheral balance between central motor commands and peripheral sensory feedback. Results suggested that patients with better motor functions may exhibit a dynamic transition from relatively balanced bidirectional i

This case report describes a rare case of a giant right atrial aneurysm (RAA) in a 7-month-old male infant admitted to Children's Hospital Affiliated to Zhengzhou University in July 2025. A RAA was detected during a physical examination at 2 months of age with no clinical symptoms present. Follow-up reexamination at 7 months of age revealed progressive enlargement of the RAA, and the infant thus underwent aneurysm resection plus atrial septal defect repair under cardiopulmonary bypass. Postoperative histopathological examination confirmed the diagnosis of RAA. On the second postoperative day,

BACKGROUND: Stroke increases the risk of developing depression and anxiety. While psychological therapies are a recommended first-line treatment, outcomes vary between stroke survivors. Identifying those at risk of poorer treatment outcomes could improve treatment selection and patient outcomes. This study aimed to identify classes of stroke survivors based on stroke-related clinical characteristics and examine their psychological treatment outcomes.
METHODS: NHS Talking Therapies for anxiety and depression (NHS TTad) data, linked to secondary care health records, were used to identify stroke

We report a rare case of Nocardia farcinica brain abscess in the basal ganglia, detailing its diagnosis, management, and rehabilitation. Diagnosing brain abscess based solely on clinical and imaging findings remains extremely challenging. Fortunately, metagenomic next-generation sequencing (mNGS) proved valuable in this case by rapidly identifying the pathogen, thereby facilitating targeted antibiotic therapy. This case highlights the importance of differentiating brain abscess from ischemic stroke and intracranial tumors. After completing a full course of anti-infective therapy and comprehens

BACKGROUND AND PURPOSE: Hemiplegic shoulder pain (HSP) represents a common and disabling complication following stroke, which has multiple causes and important functional consequences. Physical modalities are frequently used to manage HSP, but evidence on their effectiveness remains inconsistent. Our aim is to systematically review the evidence from randomized controlled trials (RCTs) on the efficacy and safety of physical modalities, compared with sham or standard care, for improving pain and function in adults with poststroke HSP.
METHODS: This systematic review was conducted following the

BACKGROUND: Hemiplegia is a common motor impairment following ischemic stroke. Post-stroke recovery involves extensive reorganization of structural and functional brain networks, which correlates with clinical deficits and recovery. While acupuncture has demonstrated safety and efficacy in improving motor function, robust neuroimaging evidence elucidating its central mechanisms remains limited. This study aimed to investigate the specific neuroplastic effects of acupuncture by focusing on its modulation of cortical gray matter volume (GMV) and multilayer dynamic functional network topology.

BACKGROUND: Tenecteplase (TNK) is a novel thrombolytic agent with improved fibrin specificity and longer half-life than alteplase. It is increasingly being adopted worldwide for the treatment of acute ischemic stroke (AIS). Although well tolerated, rare adverse reactions can occur, such as angioedema, which can rapidly progress and potentially cause airway compromise.
CASE DESCRIPTION: We report a 60-year-old female presenting to the neurology emergency department with acute right-sided hemiplegia and motor aphasia who received intravenous TNK approximately 2 h after symptom onset. Shortly af

AIMS: Cauda equina syndrome (CES) is a rare but critical spinal emergency. However, over 80% of urgent MRIs for suspected CES show no cauda equina compression. This diagnostic uncertainty places strain on imaging services and delays care. We aimed to develop and validate a clinical triage tool, the Suspected Cauda Equina Syndrome Score (SuCESS), to safely exclude CES and improve the accuracy of triage.
METHODS: We retrospectively developed the SuCESS score using the records of 259 patients who had been referred to a UK tertiary spinal unit between 2016 and 2018. Six clinical variables - saddl

Hemodialysis is a life-sustaining but procedurally complex intervention. Pneumocephalus, a rare complication of hemodialysis defined as the presence of air within the cranial vault, can range from small asymptomatic collections to life-threatening tension pneumocephalus. We report a case of acute pneumocephalus in a 65-year-old woman undergoing routine hemodialysis for end stage renal disease secondary to polycystic kidney disease. The patient collapsed within minutes of initiating dialysis with sudden alteration of consciousness. Emergency non-contrast brain CT revealed pneumocephalus. She wa

INTRODUCTION: Failed back surgery syndrome (FBSS) is a well-documented yet underinvestigated phenomenon, with prevalence estimates ranging from 10 to 40% among all spine surgeries. FBSS contributes significantly to chronic pain and disability, leading to socioeconomic burdens. Studies have emphasized the multifactorial etiology of FBSS, including inadequate decompression, misdiagnosis, iatrogenic injury, and psychosocial contributors. However, there is a scarcity of large-scale, real-world clinical datasets offering multidimensional analysis of FBSS.
MATERIALS AND METHODS: This retrospective

BACKGROUND: Adjunctive dural sealants such as BioGlue® are widely used to reinforce dural closure in cranial and spinal surgery. While generally effective, BioGlue® has been associated with inflammatory foreign-body reactions that may lead to delayed mass formation and neural compression. These delayed complications can mimic the original pathology. Here, a 66-year-old male underwent primary dural closure that included the use of BioGlue, which, 6 months later, caused the delayed onset of a cauda equina syndrome.

Obesity is a clinical condition characterized by weight gain and a progressively increasing prevalence that poses a significant threat to human health. It may also predispose individuals to numerous chronic diseases. Physical limitations such as musculoskeletal problems (e.g., osteoarthritis and low back pain), spinal cord injuries, hemiplegia, and other comorbidities both increase the development of obesity and make exercise participation more difficult in obese patients. This review summarizes current evidence regarding exercise approaches used in obese individuals with physical limitations

BACKGROUND: The 180° turning movement can be divided into the approach phase, turning phase, and departure phase. Compared with integral turning research, phase‑specific analysis is capable of revealing more detailed and comprehensive turning characteristics. To date, few studies have focused on the phase‑specific characteristics of turning movements in patients with post‑stroke hemiplegia.
RESEARCH QUESTION: What are the phase‑specific characteristics and differences in 180° turning movements between patients with post‑stroke hemiplegia and healthy older adults?

Intradural migration of a bullet within the spinal canal represents an exceptionally uncommon clinical scenario and carries the potential for progressive or delayed neurological decline. Although such fragments may initially remain silent, their subsequent movement within the spinal canal can create significant diagnostic and therapeutic challenges. In this report, we present an unusual and instructive case of cauda equina syndrome that developed secondary to the delayed caudal migration of an intradural bullet fragment. We further provide a detailed discussion of the diagnostic evaluation, su
