[A case of Fisher syndrome showing pharyngeal-cervical-brachial weakness with an elevation of anti-GQ 1 b and anti-GT 1 a antibodies].

A 15-year-old boy developed ataxic gait, diplopia and hoarseness. Within 3 days after the onset, he had additional symptoms of dysphagia and dysarthria. He was admitted to our hospital 7 days after the onset of the disease. On admission, he had total ophthalmoplegia, ataxia, areflexia, facial diplegia, bulbar palsy and weakness of the neck and upper arms. Serum anti-GQ 1 b and anti-GT 1 a antibodies were significantly elevated. A diagnosis of Fisher syndrome associated with pharyngeal-cervical-brachial weakness was made. He was placed on a high dose of intravenous immunoglobins (12.5 g/day x 2

Construct validity of the In-Hand Manipulation Test: a discriminant analysis with children without disability and children with spastic diplegia.

OBJECTIVE: This study examined the construct validity of the In-Hand Manipulation Test (IMT) by assessing the test's ability to discriminate between samples of children with and without known fine motor problems. METHOD: The IMT was administered to 55 children without known fine motor problems and 24 children with spastic diplegia who had mild to moderate fine motor problems. Construct validity was estimated by evaluating how accurately the IMT classified the children as having or not having fine motor problems on the basis of total score. RESULTS: A discriminant analysis indicated that IMT

Brachial amyotrophic diplegia: a slowly progressive motor neuron disorder.

OBJECTIVE: To describe a sporadic motor neuron disorder that remains largely restricted to the upper limbs over time. BACKGROUND: Progressive amyotrophy that is isolated to the upper limbs in an adult often suggests ALS. The fact that weakness can remain largely confined to the arms for long periods of time in individuals presenting with this phenotype has not been emphasized. METHODS: We reviewed the records of patients who had a neurogenic "man-in-the-barrel" phenotype documented by examination at least 18 months after onset. These patients had severe bilateral upper-extremity neurogenic a

Selective dorsal rhizotomy and rates of orthopedic surgery in children with spastic cerebral palsy.

If the spasticity of cerebral palsy (CP) is reduced in children at a young age by selective dorsal rhizotomy, the incidence of lower-extremity deformities requiring orthopedic surgery may be reduced; however, this has never been investigated in detail. The authors examined the effects of selective dorsal rhizotomy on rates of lower-extremity orthopedic surgery in 178 children with CP. Age at selective dorsal rhizotomy ranged from 2 to 19.3 years (mean 5.5 years) with follow-up intervals ranging from 24 to 70 months (mean 44 months). Spastic CP was classified as quadriplegia (33%), diplegia (65

Cerebral palsy.

Cerebral palsy is caused by a static lesion to the cerebral motor cortex that is acquired before, at, or within 5 years of birth. Multiple causes for the condition exist and include cerebral anoxia, cerebral hemorrhage, infection, and genetic syndromes. Cerebral palsy is commonly classified according to the type of movement problem that is present (spastic or athetoid) or according to the body parts involved (hemiplegia, diplegia, or quadriplegia). To care for children with cerebral palsy, a team approach is most effective; the team should include the pediatrician and orthopedist, among others

Multidimensional assessment of motor function in a child with cerebral palsy following intrathecal administration of baclofen.

This case report describes an 11-year-old boy with spastic diplegia whose reflex status, range of motion (ROM), strength, and motor performance were measured before and after implantation of an indwelling system for delivery of intrathecally administered baclofen. Before baclofen use, the subject experienced clonus that interfered with walking, needed assistance with transfers, and was unable to independently put on underwear and socks. Measures of spasticity, kinematics and electromyographic activity during voluntary movements, ROM, Gross Motor Function Measure (GMFM) scores, and self-reports

Nager acrofacial dysostosis. An adult male with severe neurological deficit.

In this report we describe a 50-year old male with Nager Acrofacial Dysostosis. In addition to the typical maxillofacial dysostosis and the bilateral thumb hypoplasia, he presented a severe neurological syndrome with spastic diplegia, hemiparesis of the left arm, and agenesis of the corpus callosum on CT-scan. He died at the age of 50 years from congestive heart failure due to a complete AV-block.

[Schizencephaly and upper limb malformation].

BACKGROUND: Schizencephaly, a failure of the cerebral mantle to form is usually unilateral and not associated with anomalies of the controlateral limbs. We present two cases associated with such anomalies. CASE REPORTS: Case no 1. A boy born with a lobster claw deformity in the right arm. His IQ was 60 and he developed generalized seizures at the age of 4 years with spastic diplegia. MRI examination showed unilateral left schizencephaly also lined with pachygyric cortex. Case no 2. A girl was born with a right ectromelic-hand and developed left spastic hemiparesy. Her IQ was normal. She had a

Assessment of upper-extremity function in children with spastic diplegia before and after selective dorsal rhizotomy.

The upper-extremity function of 26 children (mean age 6.0 years) with spastic diplegia was assessed prior to and one year following selective dorsal rhizotomy (SDR). No significant change was found in upper extremity range of motion, muscle tone, or strength (MMT). A significant increase was noted in grasp strength bilaterally which could not be explained simply on the basis of maturation. Manipulation patterns were noted to improve significantly one year after SDR; however, this was proposed to be a maturation rather than intervention effect. Standardized test results revealed no significant

Transient brachial diplegia (crossed paralysis): etiopathogeny and differential diagnosis.

The present paper presents three short clinical observations of patients with cerebral trauma and concussion also showing transient motor deficits of upper limbs but no sensory disorders. The etiopathogenesis of motor brachial deficits is also discussed. The conclusion is that the sudden anterior flexion of the head determines an instantaneous hit of the pyramidal bulbar decussation (the upper part) to the odontoid apophysis or to the anterior margin of the occipital foramen. Recovery depends on the severity of trauma. This clinical entity is very rare. There are other pathologic causes which

Isolated man-in-the-barrel syndrome following cardiac surgery.

A 59-year-old chronically hypertensive patient developed "man-in-the-barrel syndrome" following urgent coronary artery bypass graft surgery (CABG). This syndrome describes a brachial diplegia with intact motor function of the legs. Magnetic resonance imaging confirmed areas of ischaemia consistent with hypoperfusion leading to border-zone infarction between the territories of the anterior and middle cerebral arteries. The patient made a complete functional recovery.

[Congenital facioscapulohumeral muscular dystrophy associated with tongue atrophy and sensorineural hearing disturbance].

An 18-year-old high-school boy presented facial muscle weakness since birth, and then developed wasting around the neck, shoulder girdle, upper arms, and thighs. He was born to un-consanguineous parents. His father had suffered from similar but milder muscle atrophy with predominance on the right side of the face and shoulder girdle since adolescence. His mother and his only sibling were clinically unaffected. Hearing disturbance was detected at the age of 6, and he also noted atrophy of the tongue and the bilateral thighs at the age of 10. The symptom progressed gradually. Neurological examin

[The relationship between severity of spastic diplegia and laterality of the cerebral function].

Seventeen children with clinical evidences of preterm birth and spastic diplegia (SD) were studied about the relationship between clinical severity and laterality of cerebral function. The result showed the patients with the left-handed SD were more serious than those with the right-handed in their intelligence and activity of daily living. As the severity increased, motor difficulty was also observed at upper limbs, and especially spasticity was detected at the right hand. The dichotic listening test showed the right ear dominance only in the patients with the right-handed SD. MRI examination

[Cryotherapy in the complex restorative treatment of children with infantile cerebral palsy].

Children with spastic diplegia, hemiparetic and hyperkinetic forms of infantile cerebral paralysis were treated with local cryoapplications onto the hands. During the course of cryotherapy the authors studied interference and needle EMGs of the forearm muscles. Local cryotherapy induced a reduction in the muscle tone by gradual switching off the activity of individual motor units. Cryotherapy diminished the intensity of hyperkineses and increased the functional possibilities of the hand so that writing became possible. A conclusion is made that a decrease in the level of afferentation alters t

Spontaneous remission of cerebral palsy.

Among 2100 children with a diagnosis of cerebral palsy (CP) twenty carried the diagnosis: Previous CP, now normalized. Seventeen patients could be traced and were reevaluated. Cerebral palsy was diagnosed in these seventeen children (ten boys, seven girls) between the ages of three months and three years (average eleven months). They were found to be normal when reexamined between the ages of one year and five years (average two years two months). Two patients had tetraplegia, three diplegia, nine paraplegia ("paraplegia" were cases of diplegia with minimal affection of the upper limbs - now c

Brachial diplegia in central pontine myelinolysis.

A patient developed weakness in the upper limbs, eventually causing brachial diplegia with only slight paresis of the legs after rapid correction of severe hyponatraemia. Pseudobulbar palsy, mental confusion and urinary incontinence were also present. CT scan showed a zone of lucency in the pons. Clinical recovery occurred and the zone of lucency had disappeared 12 months after the appearance of the neurological signs.
