Seven-day Accelerometer-derived Wrist Activity after Acute Stroke Stratified by Hemiparesis Severity: A Prospective Observational Pilot Study.

OBJECTIVES: This hypothesis-driven prospective pilot study characterized 7-day trajectories of bilateral wrist activity after acute stroke by baseline Fugl-Meyer Assessment-Upper Extremity (FMA-UE) severity. We hypothesized that mild impairment would be associated with greater paretic-side activity and more symmetric activity, whereas bilateral balance would change little despite clinical improvement. METHODS: Patients who began rehabilitation within 3 days after stroke wore bilateral wrist accelerometers for 7 days. Exploratory strata were mild (FMA-UE >45) and severe (≤45), based on an infl

Assessment of Neuro-Musculo-Vascular Activity in Hemiplegic Shoulder Pain: A Multimodal Study With HD-sEMG, Ultrasonography, and Laser Speckle Imaging.

Hemiplegic shoulder pain (HSP) is a common complication following stroke, significantly affecting upper limb recovery and quality of life. However, the underlying pathophysiological mechanisms of HSP remain poorly understood, which poses a major obstacle to the development of effective therapeutic strategies. This study aims to investigate the underlying mechanism of HSP by evaluating neuro-musculo-vascular activities using high-density surface electromyography (HD-sEMG), musculoskeletal ultrasound, and laser speckle contrast imaging (LSCI). A total of 12 HSP patients and 12 hemiplegic control

Novel lower limb robot-assisted gait phase stimulation enhances walking and balance in stroke patients: a pilot trial.

BACKGROUND: Hemiplegic patients often experience weakness in key muscle groups during gait, significantly impairing walking and balance. To address this, we developed a novel gait phase-specific functional electrical stimulation (FES) delivered through a lower limb rehabilitation robot. This pilot study investigated the effects of combining gait phase-specific FES with robot-assisted gait training (RAGT) in subacute stroke patients. METHODS: Fifty-one hemiplegic stroke patients were randomized into experimental, control, and placebo groups (17 each). The experimental group received convention

Lower limb functional electrical stimulation improves stair stepping in an individual with stroke: a case report.

This case report details the potential benefits of an implanted neural stimulation system for walking to assist stair climbing after stroke. A 12-channel stimulation system was implanted in a stroke survivor for walking. Despite biweekly gait training with functional electrical stimulation (FES) that yielded locomotor improvements, the participant did not feel comfortable attempting stair climbing due to the lack of stability and fear of falling. During physical therapy, he previously limited his attempts at stair climbing to a step-to strategy with the paretic limb. The participant agreed to

Low-output lymphatic leakage after contralateral seventh cervical nerve cross transfer: a clinical and anatomical observation.

BACKGROUND: Contralateral seventh cervical nerve (CC7) cross transfer restores upper limb function in hemiplegia after brain injury. As a cervical procedure involving dissection near major lymphatic structures, CC7 transfer carries a potential risk of lymphatic leakage, a complication well described in other neck surgeries but not systematically studied in this context. METHODS: This study aims to evaluate lymphatic leakage after CC7 transfer through a combined clinical analysis and cadaveric anatomical study. We retrospectively reviewed 507 patients who underwent CC7 transfer from May 2023 to

Impact of adjustable dorsiflexion range and stiffness in articulated ankle-foot orthosis on center of pressure progression in post-stroke gait.

A multi-function articulated ankle-foot orthosis (AFO) allows independent adjustment of dorsiflexion and plantarflexion range of motion (ROM) and stiffness to optimize gait performance in individuals post-stroke. This study investigated the effects of varying two parameters of an articulated AFO (dorsiflexion ROM and stiffness) on center of pressure (CoP) trajectories, plantar forces, and spatiotemporal parameters during treadmill walking in 10 individuals post-stroke. After individualized tuning of the plantarflexion settings, dorsiflexion ROM was set at 0°, 5°, and 10° using the Triple Actio

Effect of comprehensive bilateral training protocol on motor recovery in post-stroke hemiplegia- A randomized controlled trial.

BACKGROUND AND PURPOSE: Most physiotherapists focus on the affected (hemiparetic) side when developing a rehabilitation plan for a hemiplegic patient. The less-affected side is left inactive and begins to lose its qualities, which deconditions the patient's overall rehabilitation. The objective of the study was to compare between extensive bilateral training and traditional rehabilitation methods for motor recovery. METHODS: Forty hemiplegic patients were selected for this randomized controlled trial. Patients were randomly assigned to two groups: Group A (Conventional Physiotherapy) and Grou
