tDCS synchronous robotic training on lower-limb with post-stroke hemiplegia: a comparative study between synchronous and sequential group.

BACKGROUND: This study was aimed to compare the effects of tDCS synchronized with robotic training(RT)and tDCS sequential followed by RT on lower-limb with post-stroke hemiplegia, thus to inform clinical decision-making between these delivery schedules. METHODS: 64 patients who met the inclusion and exclusion criteria were allocated into a synchronous group and a sequential group. The synchronous group underwent tDCS synchronous RT, while the sequential group received RT after tDCS ended. Both group received 40 minutes for treatment, five times a week, for a continuous two weeks. Gait Watch t

Adaptive Sliding-Mode Controller with Grey Wolf Optimization and Interval Type-2 Fuzzy Logic System for Rehabilitation Lower-Limb Exoskeletons.

In recent years, the potential of exoskeletons to enhance human capabilities has attracted significant research interest. Nevertheless, the control of Rehabilitation Lower-Limb Exoskeletons (RLLEs) is challenging because of their strong nonlinear behaviour. In the paper, a Grey Fuzzy Sliding-Mode (GFSM) controller, which is designed based on the optimization accuracy and estimation capability of the fuzzy logic system, was used for trajectory tracking of a RLLE's joints. This paper presents the tuning of the controller parameters optimally using Grey Wolf Optimization (GWO) integrated with an

Changes in upper limb kinematics in children with cerebral palsy after lower limb surgery: a retrospective comparative study.

BACKGROUND: Cerebral palsy (CP), which is characterized by movement and posture disorders, is a neurological disorder that affects the movement of both the lower and upper extremities. Current research on gait analysis in children with cerebral palsy is mostly focused on the lower extremities, whereas research on the upper extremities is limited to three-dimensional gait analysis. However, in many countries, video-based gait analysis is used instead of three-dimensional gait analysis owing to its high cost. The primary aim of this study was to determine whether upper extremity kinematics chang

Personalized combined reconstructive surgical protocols for spastic foot and ankle deformities in adult stroke: a retrospective case series.

PURPOSE: Spastic foot and ankle deformities following stroke severely impair lower limb function in adults, with conservative treatments often yielding limited and short-lived results. Drawing on established experience from pediatric cerebral palsy orthopedics, we designed personalized combined surgical protocols tailored to different deformity presentations, applied them to adult stroke, and aimed to observe the feasibility and safety of the protocol for treating such complex deformities and report preliminary functional outcomes. METHODS: This single-center retrospective observational case

Comparing the real-world effectiveness of botulinum toxin type A injections across distinct poststroke muscle hyper-resistance patterns.

BACKGROUND: Post-stroke muscle hyper-resistance is produced by both neurogenic (spasticity) and non-neurogenic (contracture) factors. BoNT-A is the most effective intervention for post-stroke spasticity, yet whether concomitant contracture alters its therapeutic benefit remains unclear. AIMS: To compare BoNT-A effectiveness in plantar-flexor hyper-resistance stratified by contracture. METHODS: We retrospectively reviewed stroke survivors with spastic hemiplegia and ankle plantar-flexor hyper-resistance who received BoNT-A injections. Patients were stratified into two groups according to the

Plantar Needle-pricking for Early Lower-Limb Recovery After Stroke via the Nociceptive Withdrawal Reflex: Assessor-Blinded Randomized Trial.

This assessor-blinded randomized controlled trial evaluated whether a standardized plantar needle-pricking protocol designed to evoke the nociceptive withdrawal reflex (NWR) improves early lower-limb motor recovery in post-stroke flaccid hemiplegia. Eighty patients were randomized (1:1) to receive either NWR-evoking plantar needle-pricking plus standardized rehabilitation or standardized rehabilitation alone for 6 consecutive days; 76 participants completed outcome assessments (38 per group). Primary outcomes were Brunnstrom stage and the Fugl-Meyer Assessment of the Lower Extremity (FMA-LE).

Clinimetric evaluation of the 1-minute sit-to-stand test as a functional performance measure associated with quadriceps strength in children with spastic hemiplegic cerebral palsy.

PURPOSE: To evaluate the construct validity, test-retest reliability, and clinical applicability of the 1-minute sit-to-stand test (1-MSTST) as a functional performance-based measure associated with quadriceps strength in ambulatory children with spastic hemiplegic cerebral palsy (GMFCS I-II). Despite its validity, instrumented strength assessment is not always feasible in routine pediatric rehabilitation, highlighting the need for simple functional alternatives. METHODS: In this cross-sectional methodological study with a test-retest design, 78 children aged 12-18 years were assessed. Quadri

Combined Gastrocnemius Release with Gracilis and Toe Flexor Release Offers a Targeted Multi-Level Approach within Single Event Multi-Level Surgery for Correcting Unilateral Lower Limb Deformities in Spastic Hemiplegic Cerebral Palsy.

INTRODUCTION: Spastic hemiplegic cerebral palsy (CP) often causes multi-level lower limb deformities, impairing gait and mobility. Single-level surgeries yield suboptimal results due to persistent spasticity elsewhere. CASE SERIES: We report outcomes in 15 ambulatory patients (aged 4-18 years, gross motor function classification system [GMFCS] I-III) undergoing combined gastrocnemius release, gracilis release, and toe flexor lengthening as part of single-event multi-level surgery (SEMLS) in spastic hemiplegic CP. RESULTS: Ankle dorsiflexion improved from -10° to +10° (P < 0.05), knee flexion

Acute aortic dissection presenting as crossed neurological symptoms: a case report.

Aortic dissection can present with a wide variety of symptoms depending on the transient arterial occlusion during propagation of the dissection. Neurological manifestation, including hemiplegia, paralysis, paraplegia, and monoplegia, have been reported previously. However, crossed neuropathy has never been found. In this case report, we present the case of a 14-year-old girl who presented with right upper limb numbness and weakness together with persistent left lower extremity weakness, which mimicked peripheral polyneuropathy. The patient was finally diagnosed with arterial dissection but un

Multiterritory Acute Ischemic Stroke with Bilateral Middle Cerebellar Peduncle T2/FLAIR Hyperintensities in the Setting of a Fatal Systemic Arterial Thrombotic Process: A Case Report.

Ischemic stroke under 55 years presents a distinct etiological challenge with higher prevalence of non-atherosclerotic causes. We report a fatal case of multiterritory acute cerebral infarction with bilateral middle cerebellar peduncle (MCP) T2/FLAIR hyperintensities in a 50-year-old woman, pursuing two educational objectives: (1) provide a radiological framework for interpreting bilateral MCP T2/FLAIR hyperintensities without diffusion restriction in the context of basilar occlusion; and (2) illustrate a diagnostic approach to multiterritory arterial thrombosis concurrent with systemic infect

Stroke in Young Female as a Presenting Feature of Systemic Lupus Erythematosus with Central Nervous System Vasculitis and Its Management.

Systemic lupus erythematosus (SLE) is a chronic, inflammatory, autoimmune disorder involving multiple systems. A 18-year-old female patient presented with complaints of headache, weakness, and tingling sensation in the left upper limb and lower limb. On magnetic resonance imaging (MRI), acute nonhemorrhagic infarct in right thalamo-capsular region was seen. The patient gave a history of malar rash and intermittent joint pains for which she had taken nonspecific treatment. Antineutrophilic antibody (ANA) by innumoflourescence and ANA blot was suggestive of SLE. Hence, the patient had central ne

Deep Temporal Clustering of Pathological Gait Patterns in Post-Stroke Patients Using Joint Angle Trajectories: A Cross-Sectional Study.

Rehabilitation of gait function in post-stroke hemiplegic patients is critical for improving mobility and quality of life, requiring a comprehensive understanding of individual gait patterns. Previous studies on gait analysis using unsupervised clustering often involve manual feature extraction, which introduces limitations such as low accuracy, low consistency, and potential bias due to human intervention. This cross-sectional study aimed to identify and cluster gait patterns using an end-to-end deep learning approach that autonomously extracts features from joint angle trajectories for a gai

Bilateral lower limb symmetry during sit-to-stand and stand-to-sit tasks in stroke patients with hemiplegia.

INTRODUCTION: Stroke patients with hemiplegia are at an increased risk of falling during sit-to-stand (Si-St) and stand-to-sit (St-Si) tasks, partly due to impaired bilateral lower limb symmetry. Maintaining symmetrical movement between the limbs in these tasks can help reduce fall incidence. METHOD: This study aimed to investigate bilateral lower limb symmetry during Si-St and St-Si tasks in stroke patients with hemiplegia to compare their performance with healthy controls. Thirteen stroke patients and 13 healthy controls participated in the study. Participants were instructed to perform the
