Clinical Decision Making, Red-Flag Recognition, and Scope-of-Practice Perspectives for Direct Access Physical Therapy in Saudi Arabia.

Background/Objectives: This survey examined referral decisions, the recognition and screening of warning features (red flags) of serious disease, regulatory interpretations, and barriers to direct-access physical therapy in Saudi Arabia. Methods: An English-language cross-sectional convenience survey (May-July 2026) used an investigator-developed questionnaire with 12 clinical vignettes (written patient cases) and 20 warning-feature items paired with screening questions. The primary outcome was choosing medical referral before physical therapy in all four urgent/critical vignettes: suspected g

Spinal neurocysticercosis: synthesizing evidence on clinical presentations, anatomical patterns, diagnostic approaches, and therapeutic strategies.

Spinal neurocysticercosis (NCC) is an uncommon manifestation of NCC involving the spinal cord, meninges, nerve roots, and subarachnoid spaces. Although less common than intracranial disease, spinal involvement can cause substantial neurological morbidity because even small lesions within the confined spinal canal may produce progressive myelopathy. Disease may result from hematogenous dissemination or cerebrospinal fluid (CSF) spread of Taenia solium larvae and may present as intramedullary, intradural extramedullary, subarachnoid, or, rarely, extradural lesions. Clinical manifestations are di

Leptomeningeal metastasis of lung adenocarcinoma presenting as cauda equina syndrome: a case report and literature review.

OBJECTIVE: This study aims to improve the understanding of the imaging characteristics of leptomeningeal metastasis, particularly in patients presenting with cauda equina syndrome as the initial manifestation, in order to improve diagnostic accuracy and reduce the misdiagnosis rate. METHODS: A 69-year-old male presented with a 4-month history of lower back pain accompanied by progressive lower limb numbness and gait instability. Non-contrast lumbar MRI performed at another hospital revealed punctate abnormal signal intensity in the cauda equina region, which was overlooked. The patient was tr

Acute Cauda Equina Syndrome in Rapid Succession: A Case Series of Eight Consecutive Patients Undergoing Emergency Surgical Decompression Over One Week.

INTRODUCTION: Cauda equina syndrome (CES) is a rare but devastating spinal surgical emergency, most commonly caused by lumbar disc herniation. Delayed diagnosis or decompression may result in irreversible neurological and sphincter dysfunction. MATERIALS AND METHODS: This retrospective single-Center case series included eight consecutive patients with acute CES who underwent emergency surgical decompression over 1 week at a tertiary spine care center. Clinical presentation, radiological findings, surgical management, and 6-month post-operative outcomes were evaluated. RESULTS: The cohort inc

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

Spinal Subdural Abscess in the Emergency Department: A Rare Diagnosis in an Immunocompetent Host.

Spinal subdural abscess is an exceptionally rare and potentially life-threatening spinal infection characterized by the accumulation of purulent material between the dura and arachnoid mater. In contrast to the more common spinal epidural abscess, spinal subdural abscesses have been reported in fewer than 80 cases, often in pediatric, postoperative, or immunocompromised patients. This case represents the fourth documented instance of a spinal subdural abscess in an immunocompetent adult without identifiable risk factors or a known source of infection and the first reported in emergency medicin

Malignant phyllodes tumour with spinal metastasis presenting as cauda equina syndrome.

Malignant phyllodes tumours (MPTs) are rare fibroepithelial breast neoplasms. While local recurrence is relatively common, distant metastasis is uncommon. Spinal metastases are exceptionally rare, with only a handful of cases in the literature. This case describes a female in her early 50s with a prior diagnosis of borderline phyllodes tumour status post right mastectomy and adjuvant radiotherapy. Two years post treatment, she presented with progressive lower back and bilateral leg pain, later identified as severe spinal canal compression causing cauda equina syndrome secondary to an L4 verteb

Percutaneous Endoscopic Interlaminar Lumbar Discectomy for Treating Acute Cauda Equina Syndrome: A Case Series of 25 Patients.

BACKGROUND: Cauda equina syndrome (CES) is caused by compression of nerves in the lumbar spine below the conus medullaris. Symptoms include progressive weakness in the lower extremities, back pain with sciatica, saddle anesthesia, and urinary or bowel dysfunction. These symptoms vary in intensity and onset, and can sometimes be an emergency. The standard surgical procedure for CES typically involves a microscope-assisted interlaminar decompression with laminotomy and sequestrectomy, with or without partial, rather than complete, discectomy. However, endoscopic spine surgery was introduced as a

Case Report: Successful treatment of mixed-phenotype acute leukemia with cauda equina syndrome as the initial manifestation.

BACKGROUND: Mixed-phenotype acute leukemia (MPAL) has a poor prognosis, and initial presentation with cauda equina syndrome (CES) is extremely rare. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) may be the only potentially curative modality. To our knowledge, this is the first reported case of MPAL presenting with CES at disease onset. METHODS: We reviewed the clinical data of a 21-year-old man with high-risk myeloid/B-lymphoid MPAL presenting with CES as the initial manifestation. The patient initially presented with lumbosacral radicular pain and lower limb numbness, which

Progressive Pseudorheumatoid Dysplasia Presenting with Cauda Equina Compression: A Case Report.

PURPOSE: Fresh vegetables are essential human dietary components but are increasingly implicated in foodborne infections caused by Escherichia coli. Contamination may occur during cultivation, irrigation, handling, and retail display, and the presence of pathogenic or antimicrobial-resistant E. coli poses significant public health risks. Despite widespread vegetable consumption in Ogun State, Nigeria, limited data exist on the resistance and virulence characteristics of E. coli in these foods. This study therefore assessed antimicrobial resistance profiles and virulence genes in E. coli isolat

Incidence of transient neurologic symptoms in patients receiving lidocaine spinal anesthesia for outpatient joint arthroplasty.

BACKGROUND: Spinal anesthesia is commonly administered for lower limb total joint arthroplasty, but the prolonged motor and sympathetic block associated with bupivacaine can delay recovery. In contrast, lidocaine, with its swift onset and intermediate duration, is an attractive alternative that is well-tailored for outpatient lower limb surgery. It has historically been associated with transient neurologic symptoms (TNS), a self-limiting but potentially distressing pain syndrome. The incidence of TNS reported in older studies varies widely, often exceeding 20%, which has led to a decline in th

Spinal Cord Stimulation for Painful Neuropathic Cauda Equina Syndrome Following Ballistic Penetrating Lumbar Spine Injury: Proof-of-Concept Case.

Modern warfare, as well as the rise of terrorist attacks on the mainland France, make military personnel deployed overseas but also civilians more prone to sustain wartime penetrating injury, the spine being involved in 6%-8% of the cases. In the long-term, some of these casualties may suffer from neuropathic pain of the lower limbs refractory to medical treatment. Thoracic spinal cord stimulation is a possible surgical treatment option for chronic neuropathic pain of the limbs caused by diabetic neuropathy, type 2 persistent spinal pain syndrome after back surgery, and complex regional pain s

Chiropractic spinal manipulative therapy versus physical therapist-led exercise and the risk of cauda equina syndrome in adults with lumbar disc herniation, stenosis, or radiculopathy.

BACKGROUND: Cauda equina syndrome is a surgical emergency often caused by lumbar disc herniation. Spinal manipulative therapy is commonly used for lumbar spine disorders, but case reports have raised concerns it may precipitate cauda equina syndrome. One cohort study suggested no increased risk, although it did not focus on patients with lumbar spine disorders pertinent to cauda equina syndrome, such as disc herniation, stenosis, or radiculopathy/sciatica. OBJECTIVE: To address this evidence gap, we tested the null hypothesis that there is no increased risk of cauda equina syndrome following

Symptom-only cauda equina syndrome: A medicolegal assessment of new national guidelines.

National guidelines and the legal standard of care for the management of cauda equina syndrome (CES) patients in primary care have recently changed. Any patient with a history of symptoms of CES must be referred to secondary care for urgent MR imaging (MRI) even in the absence of any objective signs of CES. This article reviews the prevalence and clinical diagnosis of CES, the new national guidelines and management in primary care. The standard of care for doctors, the legal basis of national guidelines and an illustrative judgement are discussed. This new standard of care for GPs will make it
