n-Hexane-Toxic Neuropathy Presenting with Cauda Equina Inflammation.

BACKGROUND: Habitual exposure to n-hexane can cause polyneuropathy through axonal degeneration and secondary demyelination. To our knowledge, n-hexane-toxic neuropathy presenting with a cauda equina-like syndrome has not previously been reported. CASE PRESENTATION: A 55-year-old man developed sensory and motor polyneuropathy over a 3-month period, with nerve conduction studies indicating demyelination. Initial treatment for suspected chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) with steroids and plasma exchange was ineffective, leading to worsening symptoms. The patient and

Transmural collaboration in lifestyle care: views of healthcare professionals and individuals with spinal cord injury, cauda equina syndrome, or spina bifida.

PURPOSE: Individuals with spinal cord injury, cauda equina syndrome or spina bifida often face challenges in maintaining a healthy lifestyle. A coordinated lifestyle care pathway beginning in the rehabilitation center and continuing into primary care may support the conditions needed for sustainable lifestyle behavior. This study aimed to identify determinants to inform the development and implementation of a lifestyle care pathway. METHODS: We conducted semi-structured interviews with 19 healthcare professionals and 24 individuals with spinal cord disorders. Separate topic guides were develo

Updated perspective: STAT surgery for significant cauda equina syndromes.

BACKGROUND: For over a decade, STAT surgery has been the standard of care (SOC) for treating significant cauda equina syndromes (CES). Nevertheless, too many spinal surgeons, physician extenders, and other medical professionals still wrongly believe they have up to 24-48 hours to treat CES. METHODS: To counter this misconception, we reviewed the evolution of treatment for significant CES from the outdated dominant protocols of < 48 h, to 24 - < 48 h, to < 24 h, to 0-< 24 h, to the present SOC; STAT, "prompt", surgery performed "the sooner the better". RESULTS: A 2014 study found that there i

Efficacy and Risks of Intradiscal Electrothermal Therapy (IDET) for Treatment of Low Back Pain: A Narrative Review.

PURPOSE OF REVIEW: Chronic low back pain, particularly discogenic pain, remains challenging to treat effectively due to complex underlying mechanisms and variability in patient response. Intradiscal Electrothermal Therapy (IDET) has emerged as a minimally invasive intervention to alleviate discogenic back pain through targeted thermal denaturation of nociceptive nerve fibers and collagen remodeling within the intervertebral disc. The present investigation critically examines existing literature to assess efficacy, safety profile, optimal patient selection criteria, and relative effectiveness c

Dural-Invasive spinal metalloma following titanium internal fixation: a rare case report and systematic literature review.

BACKGROUND: Spinal metalloma is a rare, space-occupying inflammatory pseudotumor caused by wear or corrosion of metallic spinal implants, which can lead to severe neurological compression. Although previously reported, cases arising from a biocompatible titanium alloy construct without evidence of implant failure or instability, and exhibiting direct dural invasion, are exceptionally rare, and their pathophysiology remains to be elucidated. CASE PRESENTATION: We report a 75-year-old male who presented with progressive low back pain, left lower limb radiculopathy, and cauda equina syndrome four

[A case of slowly progressive cauda equina syndrome caused by invasive lobular carcinoma of the left breast with leptomeningeal carcinomatosis].

Herein, we present the case of a 67-year-old woman who presented with a 14-month history of progressive lower limb paresthesia and muscle weakness. Bilateral sensorineural hearing loss also developed during the disease course. Six months prior to admission, cerebrospinal fluid cytology revealed findings suspect of malignancy. Although tumor screening was performed, the cause could not be identified. Due to symptom progression, the patient was re-admitted after which further examination revealed invasive lobular carcinoma of the left breast as the primary tumor, with further metastasis to the l

Long-term recovery of cauda equina syndrome and sexual dysfunction following delayed decompression of a giant lumbar disc herniation: illustrative case.

BACKGROUND: Cauda equina syndrome (CES) with sexual dysfunction is an uncommon but serious complication of lumbar disc herniation. Giant lumbar disc herniations, defined as occupying more than 50% of the spinal canal, can cause severe neural compression. Delayed decompression in CES is traditionally associated with poor neurological and autonomic recovery. OBSERVATIONS: A 36-year-old man presented with a 1-year history of low back pain and progressive radiculopathy, followed by 3 months of incomplete CES manifested by urinary dysfunction, constipation, saddle hypoesthesia, sexual dysfunction,

Neoplastic Cauda Equina Syndrome: When Do We Not Operate?

BACKGROUND AND IMPORTANCE: Burkitt lymphoma is a high-grade non-Hodgkin lymphoma that can cause spinal cord compression leading to neurological compromise. Prompt interventions are critical to prevent permanent deficits. Through this case report, we highlight the rapid resolution of acute cauda equina syndrome in an adolescent with Burkitt lymphoma in the setting of tandem spinal cord and cauda equina compression. CLINICAL PRESENTATION: An 18-year-old man developed cervical lymphadenopathy and weight loss 2 months prior and was diagnosed with Burkitt lymphoma 1 month earlier. He developed bil

Reassessing the clock in cauda equina syndrome: a systematic review and meta-analysis of surgical timing and outcomes.

BACKGROUND CONTEXT: Despite universal advocacy for urgent decompression in cauda equina syndrome (CES), the literature remains inconsistent regarding the optimal surgical timing. Heterogeneous diagnostic criteria, variable timing definitions, and a mix of clinical cohorts and national datasets have contributed to ongoing debate over 24- and 48-hour thresholds. PURPOSE: To evaluate whether timing of decompression influences urinary, neurological, functional, and inpatient outcomes in CES, and to identify clinically meaningful temporal thresholds. STUDY DESIGN/SETTING: Systematic review and me

Applications of pelvic neurophysiology testing in clinical practice.

Pelvic neurophysiology testing uniquely provides an evaluation of the sacral S2, 3 and 4 segmental somatic innervation. Since the tests are involved and some are invasive, a clear clinical question is essential in order to tailor the tests appropriately. Results should always be interpreted in conjunction with the clinical presentation and other investigation findings. The tests provide objective quantitative evidence for nerve injury and there are specific indications where these tests become of paramount importance such as the evaluation of suspected cauda equina syndrome, suspected MSA, chr

Spontaneous L4/L5 Dislocation Associated With Diffuse Idiopathic Skeletal Hyperostosis in a Young Patient With Severe Obesity: A Case Report.

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by extensive ossification of the spinal ligaments and entheses, resulting in a rigid spine that behaves biomechanically like a long bone. Although spinal fractures and dislocations after minor trauma are well recognized in ankylosed spines, non-traumatic spontaneous instability is rare. We report a rare case of spontaneous L4/L5 dislocation in a young patient with severe obesity and DISH. A 40-year-old man presented with progressive low back pain, bilateral lower extremity weakness, and bladder-bowel dysfunction. He had initially

Strategic endovascular approach for a rare type IVa perimedullary spinal arteriovenous fistula.

A 74-year-old man with a history of lumbar decompression for cauda equina syndrome presented with 1 month of progressive weakness and numbness of his legs and urinary incontinence. MRI revealed lower thoracic spinal cord edema and serpiginous perimedullary flow voids, prompting suspicion for an underlying vascular lesion. Spinal angiography identified a rare type IV (perimedullary) spinal arteriovenous fistula (AVF) supplied by the anterior spinal artery corresponding with the artery of Ademkiewickz arising from the left L1 segmental artery. A 3.7 mm venous varix was noted just beyond the fist

Cauda Equina Syndrome-A 2025 Narrative Review.

Back pain is a common presentation in healthcare settings. The most serious cause is cauda equina syndrome (CES), a surgical emergency caused by acute compression of the lumbosacral nerve roots. In the UK, diagnosis requires a thorough clinical examination and emergency magnetic resonance imaging (MRI). Once confirmed on imaging, definitive treatment is urgent surgical decompression usually within 24 hours. Delays in diagnosis or treatment can lead to life-changing consequences including bowel and bladder incontinence, sexual dysfunction, lower limb paralysis and chronic pain. This narrative r

A case of myelin oligodendrocyte glycoprotein antibody-associated disease presenting with radiculopathies.

Myelin oligodendrocyte glycoprotein (MOG) antibody-associated disease (MOGAD) is a demyelinating disease of the central nervous system (CNS). Recent reports suggest the peripheral nervous system (PNS) could also be involved, although the pathophysiology remains unclear. We report a 50-year-old woman who presented with acute cauda equina syndrome as the initial neurological manifestation of MOGAD. MOG antibodies were detected only in the cerebrospinal fluid (CSF). MRI showed a peripherally located small intramedullary lesion in the ventral spinal cord at the twelfth thoracic level. Leptomeninge

WITHDRAWN: Multiple myeloma revealed by a pulmonary embolism, with more anodal migration of λ IgG in the β2 region on serum protein electrophoresis.

Источник пометил публикацию как отозванную

INTRODUCTION: Serum protein electrophoresis (SPE) is widely used to detect monoclonal gammopathies and other hematologic or inflammatory disorders. IgG monoclonal proteins typically migrate in the gamma region, while IgA frequently migrates in the beta region. However, IgG migration in the β2 region is an exceptionally rare occurrence that can pose significant diagnostic challenges. CASE PRESENTATION: We present the case of a 76-year-old woman who was initially diagnosed with acute bilateral pulmonary embolism and deep vein thrombosis as the first clinical manifestation of multiple myeloma. Lumbar Facet Joint Septic Arthritis Complicated by Spinal Epidural Abscess in Adolescents: Two Case Reports and a Literature Review.

Septic arthritis of the spinal facet joint (SAFJ) is a rare spinal infection predominantly affecting adults and remains exceptional in pediatric and adolescent populations. Spinal epidural abscess (SEA) may occur as a complication and can lead to severe neurological deficits if diagnosis and treatment are delayed. We herein report two adolescent patients with lumbar SAFJ complicated by SEA. The first case involved a 14-year-old boy presenting with acute low back pain, fever, and rapid neurological deterioration culminating in cauda equina syndrome, requiring urgent surgical decompression and p

Brucellar spondylodiscitis mimicking lumbar discectomy with cauda equina syndrome: A case report.

Brucellar spondylodiscitis frequently poses diagnostic challenges due to its atypical early clinical manifestations, which may mimic degenerative spinal diseases. This report described a 42-year-old male patient who initially presented with severe low back pain and acute cauda equina syndrome and was preoperatively diagnosed with lumbar disc herniation. Emergency lumbar decompression was performed; however, the patient later developed fever and systemic symptoms. Repeated serological testing confirmed brucellosis. The early clinical, laboratory, and imaging features were nonspecific, and initi
