Cauda equina syndrome (CES) is a severe neurosurgical emergency typically caused by lumbar disc herniation. Its diverse clinical manifestations include radiating lower limb pain, numbness, decreased muscle strength, saddle anesthesia or reduced perineal sensation, bladder and bowel dysfunction (such as urinary retention and constipation), and sexual dysfunction. Early diagnosis and timely decompression surgery are crucial for improving prognosis. Currently, literature reporting the use of unilateral biportal endoscopy (UBE) for treating radiologically atypical CES remains scarce. We report a 2

CASE: A 25-year-old man presented with cauda equina syndrome after a fall from height. Imaging revealed an isolated transverse sacral fracture with contiguous S1 to S4 fracture lines, creating a distinctive "stepladder" morphology without significant canal compromise. The fracture was managed conservatively. Neurological status improved from ASIA Grade C to D, with recovery of perianal sensation and bladder function. At 12 months, the patient demonstrated clinical recovery, with improved functional outcome.
CONCLUSION: This unique "stepladder pattern" transverse sacral fracture demonstrated e

BACKGROUND CONTEXT: Cannabis use has increased substantially in the United States and is commonly perceived to improve pain control and reduce opioid requirements. However, its perioperative impact in spine surgery remains unclear, particularly when isolated from nicotine use.
PURPOSE: To evaluate the independent and combined associations of cannabis and nicotine use with postoperative outcomes following cervical and lumbar interbody fusion.
STUDY DESIGN/SETTING: Retrospective cohort study using the TriNetX Research Network, a federated national database of deidentified electronic health rec

BACKGROUND AND PURPOSE: Cauda Equina Syndrome (CES) is a rare but serious spinal condition requiring urgent diagnosis and management. Physiotherapists in UK musculoskeletal (MSK) services increasingly encounter suspected CES cases, but little is known about their clinical decision-making and referral practices. This study aimed to examine the clinical decision-making processes of UK physiotherapists when presented with three clinical vignettes of suspected CES and to identify themes in referral handover methods when emergency care is deemed necessary.
METHODS: An online survey featuring three

A 7-year-old male intact German shorthaired pointer was referred to a board-certified veterinary neurologist for further evaluation of a chronic (> 6 months) history of gradually progressive paraparesis, limp tail, and spinal pain despite medical management of these clinical signs and presumptive neurologic disease with the family veterinarian. Radiographs revealed evidence of intervertebral disc space collapse, endplate sclerosis, and bridging spondylosis at L7-S1, and the patient was presumptively diagnosed with degenerative lumbosacral stenosis (DLSS). Initial medical management included or

A patient presented with progressive lower limb weakness, sensory disturbance and bladder and bowel dysfunction, initially raising concern for cauda equina syndrome. Lumbar MRI excluded compressive pathology, and further imaging revealed a long-segment thoracic cord T2 hyperintensity suggestive of transverse myelitis (TM). Cerebrospinal fluid analysis supported an inflammatory cause. CT imaging identified mediastinal and supraclavicular lymphadenopathy, and lymph node biopsy confirmed non-necrotising granulomatous lymphadenitis, consistent with sarcoidosis. The diagnosis was TM secondary to ne

Spinal echinococcosis is a rare manifestation of hydatid disease with potentially debilitating neurological consequences. We present two cases of spinal echinococcosis: one depicting residual disease postsurgical decompression and the other representing recurrence after prior spinal hydatid surgery. Case 1, an early 60s male, underwent posterior decompression and laminectomy for cauda equina compression due to multicompartment extradural cysts. Despite surgical clearance, a single residual cyst was noted on follow-up MRI and managed medically. Case 2, a mid-20s female, presented 3 years after

BACKGROUND: Conus medullaris intramedullary schistosomiasis is a rare disease, and may present acutely with serious neurological consequences. In its early stages, making a prompt diagnosis, and instituting appropriate treatment results in a good outcome. We share our experience of the diagnostic challenge, surgical treatment and outcome of care of this rare disease entity.
CASE DESCRIPTION: A 7-year-old female patient, of Mada ethnicity, presented with 4 days history of bi-sphincteric dysfunction, severe low back pain, and paraparesis. Examination of the patient revealed a bilateral muscle p

Elsberg syndrome is a rare infectious neurologic condition characterized by acute or subacute lumbosacral radiculitis, with or without associated myelitis, most commonly linked to herpes simplex virus type 2 (HSV-2) or varicella zoster virus (VZV). Patients present with cauda equina-like symptoms, including urinary retention, saddle anesthesia, and neuropathic pain. Due to its variable diagnostic findings, the condition is often underrecognized. We present the case of a 32-year-old woman with no prior neurologic history who developed progressive urinary retention, saddle anesthesia, genital se

A 46-year-old male initially presented with multifocal neurological symptoms including leptomeningeal involvement, multiple cranial nerve palsies, myeloradiculopathy, myelopathy, and cauda equina syndrome. On admission, the patient tested positive for the human immunodeficiency virus (HIV). Based on the initial physical examination, which suggested Guillain-Barré syndrome, plasma exchange was initiated; however, clinical improvement was limited. Further evaluation, including histopathological analysis of the ileocecal mass and flow cytometry of the cerebrospinal fluid, revealed Burkitt lymphom

BACKGROUND: In recent times due to the advancement of haemodialysis techniques, there is increase in the life span of the patients with end-stage renal disease, which tends to increase the risk of complications of bacteraemia due to skin penetration for blood access and leading to propensity for more skeletal and spinal complications. Hence, early diagnosis of spine infection and early initiation of appropriate therapy are important in ensuring successful treatment and preventing further morbidity.
MATERIALS AND METHODS: The study included 22 males and 12 females who were known case of end-st

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

BACKGROUND: Campylobacter jejuni (C. jejuni) is a common cause of self-limiting gastroenteritis but rarely causes extraintestinal infections. Vertebral osteomyelitis due to this pathogen is exceptionally uncommon, and cases presenting without gastrointestinal symptoms are even rarer. To our knowledge, no previous case has described lumbar osteomyelitis with a spinal extradural abscess leading to cauda equina syndrome in the absence of enteric symptoms. Diagnosing and managing this infection remains clinically challenging.
CASE PRESENTATION: A 65-year-old man presented with a two-month history

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or
