Primary intradural extramedullary anaplastic ependymoma of the cauda equina: a rare and challenging case.

Intradural extramedullary anaplastic ependymomas in the cauda equina are extremely rare. The latent period from the beginning of symptoms until diagnosis is notably shorter compared to other ependymoma subtypes. In some cases, severe neurological deficit may develop rapidly due to intratumoral hemorrhage. We present the case of a 36-year-old male who experienced sudden numbness in his legs, followed by an inability to move his feet. In the following days, independent walking became impossible, and he stopped urinating. At the time of admission, bilateral hypesthesia was observed across the are

Diagnostic Dilemmas and Perioperative Challenges in Lumbar Spinal Chondroma: A Narrative Review with Case Presentation.

The occurrence of lumbar spinal chondromas is exceptionally rare, with fewer than 30 cases reported. Due to their rarity and similar presentation to more common spinal pathologies, lumbar spinal chondromas often pose diagnostic challenges. A 19-year-old patient was diagnosed with lumbar spinal chondroma at the L5-S1 level. The tumor was initially misinterpreted as a case of lumbar disc prolapse, and surgical decompression was planned. During surgery, a whitish firm, cartilaginous tumor compressing the S1 nerve root was discovered and excised. A narrative review of the literature was conducted,

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Lumbosacral transitional vertebrae in small-breed dogs: Prevalence, classification, and characteristics.

The lumbosacral transitional vertebra (LTV) is an abnormally formed vertebra that can be associated with cauda equina syndrome, lumbosacral (LS) degeneration, and pelvic rotation. However, there is little information on LTV in small-breed dogs, and no studies have investigated the association between LTV and LS diseases or degenerative changes in the coxofemoral joints. The purpose of this multicenter, retrospective, and cross-sectional study was to determine the prevalence of LTV in small-breed dogs (<10 kg), classify the type of LTV, and identify its association with degenerative changes in

Cauda Equina Syndrome Without Perineal Sensory Changes or Lower Extremity Neurological Deficits Following Postoperative Spinal Epidural Hematoma: A Case Report and Literature Review.

BACKGROUND: Postoperative spinal epidural hematoma (SEH) is a rare but serious complication following lumbar surgery, with cauda equina syndrome (CES) being one of its most devastating outcomes. While CES typically presents with a combination of bladder and/or bowel dysfunction, diminished sensation in the saddle area, and motor or sensory changes in the lower limbs, atypical cases with isolated urinary symptoms are less recognized and pose significant diagnostic challenges. CASE PRESENTATION: We report the case of a 46-year-old male who developed CES following lumbar microdiscectomy, present

Drug-Induced Cauda Equina Syndrome in an 8-Year-Old Boy With Acute Lymphoblastic Leukemia: An Uncommon Case Report.

Intrathecal methotrexate can cause cauda equina syndrome in pediatric ALL patients, as demonstrated in this rare case of an 8-year-old boy. Symptoms included progressive limb weakness and urinary retention. Early recognition, prompt discontinuation of the offending agent, and multidisciplinary management are crucial. Vigilant neurological monitoring is essential for pediatric acute lymphoblastic leukemia patients receiving intrathecal chemotherapy.

Development of a Machine-Learning Algorithm to Identify Cauda Equina Compression on Magnetic Resonance Imaging Scans.

OBJECTIVE: Cauda equina syndrome (CES) poses significant neurological risks if untreated. Diagnosis relies on clinical and radiological features. As the symptoms are often nonspecific and common, the diagnosis is usually made after a magnetic resonance imaging (MRI) scan. A huge number of MRI scans are done to exclude CES but nearly 80% of them will not have CES. This study aimed to develop and validate a machine-learning model for automated CES detection from MRI scans to enable faster triage of patients presenting with CES like clinical features. METHODS: MRI scans from suspected CES patien

Cauda Equina Syndrome: A Review of Classification, Diagnosis, Treatment, and Best Practices.

BACKGROUND: Cauda equina syndrome (CES) is a rare but life-altering disease resulting from compression of the nerve roots at the spinal cord's terminus. CES typically presents with low back pain, sciatica, sensorimotor deficits, and bowel and bladder dysfunction. Owing to its rarity, the condition is often missed, leading to significant morbidity and potential legal implications for physicians. METHODS: This review synthesizes the current literature on CES, including its epidemiology, pathophysiology, classifications, and management strategies. Emphasis is placed on the diagnosis and treatmen

Transitional lumbosacral vertebrae in black Norwegian elkhound and Brittany dogs: Clinical findings and its association with degenerative lumbosacral stenosis.

BACKGROUND: Lumbosacral transitional vertebra (LTV) is a congenital anomaly in dogs and have been proposed to be associated with cauda equina syndrome (CES) in German Shepherd dogs. This prospective study, including 32 dogs from two breeds, aims to investigate if LTV is associated with lower back pain in dogs. The study compared owners' assessment of their dogs' well-being and clinical evaluation with advanced diagnostic imaging to identify changes that might explain clinical findings. RESULTS: Dogs with LTV type 2 (57.0%) and with LTV type 3 (70.0%) exhibited mild lower back pain, which was

A Novel, Joint Sparing Surgical Approach to the Management of Giant Cell Tumor of the Distal Radius in a Pediatric Patient: A Case Report and Comprehensive Literature Review.

INTRODUCTION: The "Whoops Procedure" first described by Giuliano and Eilber in 1985 refers to an unplanned sarcoma resection, where a lesion presumed to be benign is inadvertently excised, only to later be diagnosed as malignant. This leads to higher risks of local recurrence, lower limb salvage rates, and poor functional outcomes. Osteosarcoma, the most common primary bone cancer in adolescents, requires meticulous pre-operative evaluation to prevent inadvertent contamination and ensure optimal treatment. CASE REPORT: We present a case of a 13-year-old girl who initially underwent curettage

Assessment and early investigation of cauda equina syndrome- a systematic review of existing international guidelines and summary of the current evidence.

BACKGROUND CONTEXT: Cauda Equina Syndrome (CES) is an emergency clinical syndrome with correlating radiological evidence of spinal compression. Urgent recognition and treatment is key. In many units, it lacks a clear pathway of investigation and referral, despite a constantly expanding evidence base for its management. PURPOSE: The aim of this article is to provide a systematic review of currently available guidelines internationally, and also an up-to-date review of current key evidence on topics central to the accurate assessment and investigation of CES. STUDY DESIGN: A systematic review

Causal Association Between Inflammatory Proteins, Inflammatory Cells, and Cauda Equina Syndrome: A Two-Sample Mendelian Randomization.

BACKGROUND: Recent studies have shown that inflammation plays a crucial role in the progression of cauda equina syndrome (CES). However, the exact cause-and-effect relationship between them is still unclear. METHODS: We used CES data from the FinnGen genome-wide association study (GWAS), containing 329 cases and 408,351 control patients. Inflammatory proteins data were obtained from a large scale GWAS of 14,828 European ancestry participants, and inflammatory cells data were obtained from a GWAS summary of 3757 Sardinians. We chose inverse variance weighted as the main method and the Cochrane

Incidence and long-term patient reported outcome measures of patients with radiological cauda equina compression but without clinical features of cauda equina syndrome.

BACKGROUND: Cauda equina syndrome (CES) is a rare spinal emergency. Paradoxically, in the British NHS, suspected CES requiring an emergency magnetic resonance imaging constitutes one of the commonest reasons for acute referrals to neurosurgery/spine surgery. Further acute referrals also occur with the reverse scenario, when radiological evidence of cauda equina compression is not accompanied by the clinical red flags of the syndrome. The understanding of CES, particularly in cases with radiological compression but lacking clinical symptoms, remains limited. This study aims to explore the incid

Neurogenic bladder pathophysiology, assessment and management after lumbar diseases.

Neurogenic bladder (NB) is a group of bladder and/or urethral dysfunctions caused by neurological lesions, commonly seen in patients with lumbar spine diseases, manifesting as urinary storage and voiding dysfunction, significantly affecting patients' quality of life. Degenerative changes or trauma to the lumbar spine can lead to narrowing of the dural sac, compressing the sacral nerve roots, cauda equina or blood vessels, causing bladder dysfunction and leading to NB. Diagnostic methods for NB include history taking, physical examination and noninvasive and invasive tests, such as urodynamic t
