Stent Graft Intervention in a Lumbar Artery Pseudoaneurysm Improved Cauda Equina Syndrome: A Case Report of Combined Embolization Strategy.

A lumbar artery pseudoaneurysm following a lumbar vertebral compression fraction is rare. Cauda equina syndrome due to spinal canal compression is a rarer complication of this pseudoaneurysm. Endovascular treatment, which involves embolization of the inflow and outflow vessels, is the first-line treatment for lumbar artery pseudoaneurysms. We report a case of a lumbar artery pseudoaneurysm with spinal canal compression that was treated with coil embolization of the outflow vessel and stent graft placement to occlude the inflow vessel. After treatment, the symptoms of cauda equina syndrome impr

The impact of prolapse to canal ratio (PCR) in cauda equina syndrome outcomes and operative management.

PURPOSE: This study aimed to evaluate the prolapse to canal ratio (PCR) and its long-term impact on outcomes in cauda equina syndrome (CES). METHODS: A 5-year retrospective cohort study was conducted across two neurosurgical centers, including adult patients diagnosed with CES due to herniated lumbar discs, treated with laminectomy and/or discectomy, with available radiological data and follow-up. PCR was calculated as the proportion of the disc prolapse cross-sectional area to the total spinal canal area. MRI scans were independently analyzed by two investigators, with random verification by

Integrative Korean medicine for recurrent lumbar disc herniation after coronavirus disease vaccination: A case report and literature review.

RATIONALE: Many side effects have been associated with the coronavirus disease (COVID-19) vaccine. While most adverse events (AEs) are mild, serious adverse events are occasionally observed in the neurological and musculoskeletal systems. Nevertheless, articles reporting such serious adverse events after COVID-19 vaccination are limited, and only few case reports with detailed descriptions are available in the literature. PATIENT CONCERNS: Herein, we report the case of a 41-year-old male office worker who developed symptoms of recurrent disc herniation 2 days after COVID-19 vaccination; the p

Cauda equina syndrome with urinary retention as a postoperative complication of lumbar spine surgery: A case report.

BACKGROUND: Cauda equina syndrome (CES) is characterized by a group of symptoms that may be caused by inflammation, spinal cord compression, venous congestion, or ischemia. This syndrome is commonly an indication for surgical intervention but has not been determined as a postoperative complication following surgery for lumbar spine disease. CASE SUMMARY: To report the case of a 54-year-old male patient who had CES following spinal surgery, with no obvious compression lesions found during re-exploration, suggesting that vascular insufficiency may have contributed to the condition. Furthermore,

Is out-of-hours decompression for acute lumbar disc herniation associated with greater peri-operative complications? A tertiary centre, retrospective cohort study.

PURPOSE: Timing of emergent surgery for lumbar disc herniations, such as cauda equina syndrome, remains a challenging aspect of best medical practice in spine surgery, with most authors recommending decompressive surgery as soon as feasible. The literature conflicts on the merit and potential risk of emergent, "out-of-hours" decompression. Our aim was to evaluate if there is a higher complication rate associated with out-of-hours decompressive surgery for emergent lumbar disc herniations. METHODS: This was a single-centre, retrospective cohort study in a tertiary referral spinal unit of patie

Surgical management of spinal intradural hematomas: illustrative cases.

BACKGROUND: Spinal intradural hematoma (SIH) is a rare condition with potential for permanent neurological deficit. SIH can be managed conservatively with serial imaging or surgically with lumbar drainage or open evacuation. We present 3 SIH cases managed with multiple surgical techniques, including a novel lumbar drainage-and-advancement technique, and review the literature. OBSERVATIONS: Patient 1 was a 35-year-old peripartum female with cauda equina syndrome after epidural analgesia. MRI revealed SIH at L4-5, necessitating urgent L3-S1 laminectomy and hematoma evacuation. Patient 2 was a 4

Assessment of urgent investigative images transfer effectiveness from trauma and orthopaedic to neurosurgery department: a quality improvement project.

BACKGROUND: Neurosurgical emergencies, including cauda equina syndrome, traumatic spinal cord injuries and traumatic brain injuries, require timely diagnosis and intervention to prevent severe morbidity and mortality. Delays in these cases often stem from inefficiencies in transferring investigational images from district hospitals to external neurosurgery units. AIMS: This quality improvement project aimed to reduce image transfer time at Sandwell and West Birmingham Hospitals Trust (SWBH) to the Queen Elizabeth Hospital Birmingham neurosurgery unit. It sought to empower Trauma and Orthopaed

Clinical Reasoning: A 62-Year-Old Man With Symmetric Saddle Hypoesthesia and Sphincter Dysfunction.

A 62-year-old man presented with acute onset of symmetric burning dysesthesias in the buttocks, accompanied by saddle hypoesthesia and bladder, bowel, and sexual dysfunction. Initially, the clinical presentation was suggestive of cauda equina syndrome. However, the initial contrast-enhanced lumbar MRI revealed no evidence of extrinsic compression or enhancement of the lumbosacral nerve roots or conus medullaris. Further diagnostic workup uncovered a rare etiology, highlighting the importance of considering uncommon causes in the case of acute conus medullaris syndrome.

No turning back: a long-term retrospective analysis of urgent magnetic resonance imaging (MRI) performed for suspected cauda equina compression in a tertiary referral centre.

AIM: Cauda equina syndrome is a rare but potentially serious condition for which diagnosis is time-critical for the best outcomes. Magnetic resonance imaging (MRI) is the imaging modality of choice, but the diagnostic pathway is challenging as availability of 24/7 urgent MRI is mostly limited to tertiary centres. We present a 13-year experience at one such tertiary centre. MATERIALS AND METHODS: All lumbar spine MRIs performed for suspected cauda equina between 2011 and 2023 were identified retrospectively from trust-wide information technology (IT) systems.
