Reconnecting the disconnected forebrain: multimodal resting-state bold dynamics underlying zolpidem-induced arousal in incomplete Locked-in syndrome.

PRIMARY OBJECTIVE: Zolpidem paradoxically restores behavioral responsiveness and communication in select patients with severe disorders of consciousness and profound motor deafferentation. While the anterior forebrain mesocircuit hypothesis attributes this awakening to central thalamic disinhibition, macroscopic network dynamics and lower brainstem gating remain under-characterized in locked-in syndrome (LIS). METHODS AND PROCEDURES: A 45-year-old male with ventral pontine necrosis initially misdiagnosed as vegetative state/unresponsive wakefulness syndrome (VS/UWS) was formally identified wi

A visual imagery paradigm for BCI strategies using imagined flickering patterns.

Steady state visually evoked potentials (SSVEPs) are a popular type of control signals in brain-computer interfaces (BCIs), in which they are typically elicited by observing a visual stimulus flashing at a specific frequency. For some patients, using SSVEP as control signal for a BCI can be difficult, for instance if they are unable to focus their gaze over the visual stimuli. To address this issue, some approaches were presented to design a gaze-independent SSVEP-controlled BCI but some difficulties have been reported, for instance for patients suffering from locked-in syndrome. In this work

Large-scale training data enhances silent speech decoding with around-ear EEG.

Objective. Silent speech decoding (SSD) offers a potential communication alternative for individuals with impaired vocalization. However, conventional multi-electrode electroencephalography (EEG) or facial electromyography (EMG) systems require cumbersome preparation and are unsuitable for daily use. This study evaluates the practicality of SSD using a wearable around-ear EEG device, focusing on data scaling, cross-subject transfer, vocabulary extensibility, and online decoding performance.Approach. We collected 72 h of around-ear EEG from 24 healthy participants and one individual with incomp

Brain death and the brainstem.

Brain death/death by neurologic criteria (BD/DNC) represents a distinct state after a catastrophic brain injury. In the 19th century, anatomists and physiologists progressively began to sustain the conception of the brainstem as a mediator of vital physiologic functions. A primary supratentorial brain lesion usually provokes a rostrocaudal transtentorial brain herniation syndrome, resulting in brain function impairment in both cerebral hemispheres and the brainstem. When a secondary brain lesion (i.e., cerebral hypoxia) is present, the brainstem, like the forebrain, is also affected. Neverthel

Brainstem control of sleep, wakefulness, alertness, and consciousness.

The brainstem is crucial for maintaining wakefulness, sleep regulation, and consciousness. It orchestrates alertness through the ascending reticular activating system, integrating sensory information and projecting to the cortex. Consciousness emerges from intricate interactions involving the brainstem, though the brainstem alone does not generate consciousness. Clinical syndromes resulting from brainstem lesions highlight distinctions between wakefulness, alertness, and consciousness. Disorders such as sleep paralysis, parasomnias, vegetative state, minimally conscious state, coma, and locked

Locked-in syndrome.

The prerequisites for the locked-in stste are the presence of two components: one negative-sufficient paralysis of structures that render usual communications (mouth and oral apparatus for speech, and limbs for writing and pointing accurately) and one positive- preservation of consciousness. Lesions of the pontine base cause the syndrome. The most common pontine lesions are infarction related to basilar artery occlusion and pontine hemorrhages. Occasionally midbrain vascular lesions can cause the syndrome.

Decoding intended speech with an intracortical brain-computer interface in a person with long-standing anarthria and locked-in syndrome.

Intracortical brain-computer interfaces (iBCIs) for decoding intended speech have provided individuals with ALS and severe dysarthria an intuitive method for high-throughput communication. These advances have been demonstrated in individuals who are still able to vocalize and move speech articulators. Here, we decoded intended speech from an individual with long-standing anarthria, locked-in syndrome, and ventilator dependence due to advanced symptoms of ALS. We found that phonemes, words, and higher order language units could be decoded well above chance. While sentence decoding accuracy was

Advancing EEG-based assessment of consciousness and cognition in prolonged disorders of consciousness.

BACKGROUND: Accurate assessment of residual awareness in patients with Prolonged Disorders of Consciousness (PDoC) remains a major clinical challenge, as conventional behavioural tools can underestimate covert cognition. This study evaluates whether a structured, multi-phase motor imagery Brain-Computer Interface (MI-BCI) protocol provides objective electroencephalography (EEG)-based indicators of awareness that complement behavioural assessments. METHODS: Forty-four participants (N = 44) completed repeated imagined-movement tasks using wearable EEG (PDoC: Unresponsive Wakefulness Syndrome (U

An intelligent EEG-based ensemble framework for communication assistance in Locked-In Syndrome patients.

Locked-In Syndrome (LIS) is a neurological condition in which patients experience complete motor paralysis while preserving cognitive function, severely restricting their ability to communicate. Electroencephalography (EEG)-based brain–computer interface (BCI) systems, particularly those utilizing the P300 event-related potential, offer a promising solution for enabling communication in such patients. Despite their potential, P300-based systems often suffer from low robustness due to noise, non-stationarity of EEG signals, and limitations of single-classifier approaches. This study addresses t

Design and Performance Evaluation of a Low-Cost High-SNR EOG Sensing System for Arabic Locked-In Syndrome Communication.

Locked-in Syndrome (LIS) is a neurological condition in which individuals remain conscious but experience complete paralysis of voluntary muscles, except for eye movements-highlighting the need for reliable assistive communication technologies. This study presents the design and evaluation of an Arabic electrooculogram (EOG)-based communication system with adaptive classification capabilities for LIS applications. A custom-designed EOG acquisition circuit incorporating filtering and amplification stages was implemented and compared with the OpenBCI Cyton board. The system employed a hybrid cla

Device for communication through foot movements in a patient with Locked-In Syndrome.

PURPOSE: Locked-in Syndrome (LIS) is a rare and severe condition characterised by tetraplegia, facial paralysis, and anarthria, with only vertical eye movements and blinking typically preserved. It usually results from brainstem stroke. While several devices enable communication through eye movements, no device existed that facilitated communication through foot movements when both ocular motility and eyelid function are completely impaired. MATERIALS AND METHODS: This report describes the creation, development, and adaptation of a communication device operated via foot movements in a patient

The anatomy of concealed awareness: lessons from partial locked-in syndrome.

Partial locked-in syndrome (PLIS) is a rare stroke presentation in which preserved consciousness is masked by severe motor impairment, potentially leading to misinterpretation as encephalopathy. We report a patient in her early seventies with vascular risk factors and a prior pontine infarct who presented with acute quadriplegia and mutism. Initial clinical impressions raised concern for altered mental status. However, subsequent neurological examination demonstrated preserved awareness, with consistent eye tracking and command following through head movements. Examination showed bifacial and

The current status of foundation models in decoding inner speech from non-invasive brain signals: a mini review.

Inner speech (IS), or imagined speech without overt articulation, is a promising target for brain-computer interfaces (BCIs) aimed at restoring communication in individuals with severe speech impairments, such as locked-in syndrome. Foundation models (FMs), typically trained using self-supervised learning (SSL) on large-scale datasets, offer new opportunities for learning transferable and robust representations from neural signals. This mini review provides an overview of FM-based approaches for IS decoding using non-invasive neuroimaging modalities, including functional magnetic resonance ima

Auditory Perception as a Surrogate for Auditory Imagery in EEG-Based BCI Training: Neural Evidence and a Feasibility Study.

Brain-computer interface (BCI) systems are commonly classified as reactive or active. Reactive BCIs rely on responses to external stimuli, simplifying decoding but limiting direct user control. In contrast, active BCIs enable intuitive control through spontaneous mental activity but impose greater signal-processing challenges. Speech BCIs, a subset of active systems, hold promise for individuals with severe neurological impairments such as locked-in syndrome (LIS), potentially enabling communication through imagined speech. However, their adoption is hindered by the demanding pre-training requ

Brain activity in an end-stage ALS patient suggests the presence of an unresponsive wakefulness syndrome.

OBJECTIVE: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disease primarily affecting motor neurons. It is widely assumed that cortical structures beyond motor neurons are relatively preserved, and patients in the end-stage ALS are regarded as being in complete locked-in syndrome (cLIS). However, emerging evidence suggests substantial heterogeneity in cognitive functioning among ALS patients, indicating possible extra-motor cortical involvement and impaired levels of consciousness. We report a case study assessing electrophysiological markers and auditory system integri

Neuropsychological assessment in low-resource versus high-resource contexts: A reply to comments on Stiers (2026).

OBJECTIVE: Stiers (2026) presented a case example of differential diagnosis in clinical neuropsychology wherein standardized testing was not possible, and discussed a structure and process for using history, behavioral observations, and disease base rates to develop a preliminary diagnosis. In response, Basso et al. (2026) state that these alone are not sufficient, and they emphasize the importance of standardized testing. METHODS: This article describes a clinical encounter where standardized norm-referenced testing was not possible because appropriate standardized tests or norms did not exi

Ethical challenges in treatment-goal transitions in invasively ventilated ALS: a case-based topical review.

INTRODUCTION: In advanced amyotrophic lateral sclerosis (ALS), eye movements often represent the last channel for intentional communication. While oculomotor function has traditionally been considered relatively preserved, emerging evidence indicates progressive impairment in long-term survivors on tracheostomy-invasive ventilation (TIV). As a result, eye-based communication may become increasingly unreliable before complete loss, challenging clinical decision-making and advance care planning (ACP). METHODS: We conducted a case-based topical review integrating clinical observation and literat

Fulminant Powassan Virus Encephalitis Presenting as Acute Ischemic Stroke: A Case Report.

Powassan virus (POWV) is a rare tick-borne flavivirus that can cause severe neuroinvasive disease with high morbidity and mortality. Early clinical and radiographic features are often nonspecific and may mimic acute ischemic stroke, leading to diagnostic delay. We report the case of a 77-year-old woman with recent tick exposure who presented with acute-onset focal neurologic deficits and fever. Initial neuroimaging demonstrated focal diffusion restriction, raising concern for acute ischemic stroke, and dual antiplatelet therapy was initiated. Over subsequent days, the patient developed progres
