Decoding speech intent from non-frontal cortical areas.

Objective. Brain machine interfaces (BMIs) that can restore speech have predominantly focused on decoding speech signals from the speech motor cortices. A few studies have shown some information outside the speech motor cortices, such as in parietal and temporal lobes, that also may be useful for BMIs. The ability to use information from outside the frontal lobe could be useful not only for people with locked-in syndrome, but also to people with frontal lobe damage, which can cause nonfluent aphasia or apraxia of speech. However, temporal and parietal lobes are predominantly involved in percep

A novel non-invasive EEG-SSVEP diagnostic tool for color vision deficiency in individuals with locked-in syndrome.

INTRODUCTION: Color vision deficiency (CVD), a common visual impairment, affects individuals' ability to differentiate between various colors due to malfunctioning or absent color photoreceptors in the retina. Currently available diagnostic tests require a behavioral response, rendering them unsuitable for individuals with limited physical and communication abilities, such as those with locked-in syndrome. This study introduces a novel, non-invasive method that employs brain signals, specifically Steady-State Visually Evoked Potentials (SSVEPs), along with Ishihara plates to diagnose CVD. This

Pan-neurofascin autoimmune nodoparanodopathy: A case report and literature review.

RATIONALE: Locked-in syndrome (and its variant, completely locked-in state) generally has a high mortality rate in the acute setting; however, when induced by conditions such as acute inflammatory polyradiculoneuropathy, it may well be curable such that an attempt at cure should be systematically sought by clinicians. PATIENT CONCERNS: A 52-year-old man presented with acute tetraparesia and areflexia, initially diagnosed as Guillain-Barré syndrome. Despite appropriate treatment, his condition deteriorated, evolving into a completely locked-in state. DIAGNOSES: The detection of anti-pan-neuro

Designing an online BCI forum: insights from researchers and end-users.

Objective.Brain-computer interfaces (BCIs) can support non-muscular communication and device control for severely paralyzed people. However, efforts that directly involve potential or actual end-users and address their individual needs are scarce, demonstrating a translational gap. An online BCI forum supported by the BCI Society could initiate and sustainably strengthen interactions between BCI researchers and end-users to bridge this gap.Approach.We interviewed six severely paralyzed individuals and surveyed 121 BCI researchers to capture their opinions and wishes concerning an online BCI fo

Bilateral medial medullary and spinal cord infarctions presenting as locked-in syndrome in an adolescent with primary antiphospholipid syndrome.

Bilateral medial medullary infarction (BMMI) is a rare stroke subtype in the paediatric population characterised by quadriplegia, cranial nerve dysfunction and respiratory failure. MRI typically reveals a 'heart-shaped' sign in the medial medulla on diffusion-weighted imaging (DWI). Paediatric stroke aetiologies, including vasculopathy, thrombophilia and cardioembolic disease, differ from those in adults. Antiphospholipid syndrome (APS) is a rare but important cause of paediatric stroke. We report an adolescent male presenting with acute quadriplegia, bulbar dysfunction and respiratory failure

Simulating online and offline tasks using hybrid cheetah optimization algorithm for patients affected by neurodegenerative diseases.

Brain-Computer Interface (BCI) is a versatile technique to offer better communication system for people affected by the locked-in syndrome (LIS).In the current decade, there has been a growing demand for improved care and services for individuals with neurodegenerative diseases. To address this barrier, the current work is designed with four states of BCI for paralyzed persons using Welch Power Spectral Density (W-PSD). The features extracted from the signals were trained with a hybrid Feed Forward Neural Network Cheetah Optimization Algorithm (FFNNCOA) in both offline and online modes. Totall

Impact of communication modalities on autonomy and social participation of persons with locked-in syndrome.

Introduction Locked-in syndrome (LIS) is a complex neurological condition characterised by paralysis of all four limbs and anarthria. Persons with LIS may communicate via eye blinks/low technology Augmentative and Alternative Communication devices (low tech AAC devices) or may use high technology Augmentative and Alternative Communication devices (high tech AAC devices). Our three objectives were: (1) to describe the distribution of communication modalities used by persons with LIS, (2) to describe the satisfaction of persons with LIS with their communication modality and social participation,

The effectiveness of FOUR score versus GCS scale in predicting mortality and morbidity in traumatic brain injured patients in intensive care: A systematic review.

BACKGROUND: Traumatic brain-injury (TBI) is one of the leading causes of death and disability worldwide. In intensive care, the Glasgow coma scale (GCS) is widely used to assess severity of brain injury. An alternative tool is the Full Outline of UnResponsiveness (FOUR) score which assesses stages of locked-in syndrome and brain herniation. The purpose of this study is to evaluate the effectiveness of both assessment tools in predicting mortality and morbidity in critically ill patients with TBI. METHODOLOGY: Systematic review guided by the Joanna Briggs Institute (JBI) Manual for Evidence Sy

Legal personhood and legal capacity: the case of the locked-in syndrome.

The situation of persons diagnosed with locked-in syndrome (LIS) raises a significant legal challenge. As a consequence of a brainstem stroke, they are quadriplegic and lack articulate speech but have normal visual perception, bodily sensations, consciousness, and cognitive functions. It is only with human and technological assistance that they can communicate, express their will, make responsible decisions, and exert their civil rights. Insofar as they can communicate, there seems in principle to be no reason for restricting their legal capacity. This, however, has not always been recognized.

Don't put words in my mouth: speech perception can falsely activate a brain-computer interface.

BACKGROUND: Recent studies have demonstrated that speech can be decoded from brain activity which in turn can be used for brain-computer interface (BCI)-based communication. It is however also known that the area often used as a signal source for speech decoding BCIs, the sensorimotor cortex (SMC), is also engaged when people perceive speech, thus making speech perception a potential source of false positive activation of the BCI. The current study investigated if and how speech perception may interfere with reliable speech BCI control. METHODS: We recorded high-density electrocorticography (

Decoding intended speech with an intracortical brain-computer interface in a person with longstanding anarthria and locked-in syndrome.

Intracortical brain-computer interfaces (iBCIs) for decoding intended speech have provided individuals with ALS and severe dysarthria an intuitive method for high-throughput communication. These advances have been demonstrated in individuals who are still able to vocalize and move speech articulators. Here, we decoded intended speech from an individual with longstanding anarthria, locked-in syndrome, and ventilator dependence due to advanced symptoms of ALS. We found that phonemes, words, and higher-order language units could be decoded well above chance. While sentence decoding accuracy was b

Jahi McMath case: A comprehensive and updated narrative.

The Jahi McMath case represents a pivotal moment in the reevaluation of brain death or death by neurological criteria (BD/DNC) and disorders of consciousness. Declared brain dead on December 12, 2013, following surgical complications, Jahi's case became a landmark in challenging conventional BD/DNC definitions. As an independent consultant for the International Brain Research Foundation, I recommended advanced ancillary tests, including neuroimaging, electrophysiological, and heart rate variability assessments, conducted at Rutgers University Hospital and Jahi's apartment in New Jersey. My ana

Assessing Capacity and Exploring Goals of Care in Locked-In Syndrome: A Clinician's Guide.

Patients with locked-in syndrome (LIS) pose unique ethical challenges for decision making, given the complexities of communication. However, some of these patients may retain the ability to participate in decisions regarding their care, including whether to continue life-sustaining treatment or pursue comfort care only. Following a deidentified case, this article has 2 central aims. First, it examines the ethical foundations of decision making and the complexities of capacity assessments for this patient population. Second, it offers a practical guide for neurologists to use when making such e

Implantable Neural Speech Decoders: Recent Advances, Future Challenges.

The social life of locked-in syndrome (LIS) patients is significantly impacted by their difficulties to communicate. Consequently, researchers have started to explore how to decode intended speech from neural signals directly recorded from the cortex. The first studies in the late 2000s reported modest decoding accuracies. However, thanks to fast advances in machine learning, the most recent studies have reached decoding accuracies high enough to be optimistic about the clinical benefit of neural speech decoders in the near future. We first discuss the selection criteria for implanting a neura

Real-time detection of spoken speech from unlabeled ECoG signals: a pilot study with an ALS participant.

Objective. Brain-computer interfaces hold significant promise for restoring communication in individuals with partial or complete loss of the ability to speak due to paralysis from amyotrophic lateral sclerosis (ALS), brainstem stroke, and other neurological disorders. Many of the approaches to speech decoding reported in the BCI literature have required time-aligned target representations to allow successful training-a major challenge when translating such approaches to people who have already lost their voice.Approach. In this pilot study, we made a first step toward scenarios in which no gr

Covert consciousness: what's in a name?

Over the past decade, it has become apparent that up to 25% of behaviourally unresponsive patients with acute or chronic disorders of consciousness reveal high spatio-temporal complexity following a direct electrical or magnetic pulse to the brain, highly differentiated EEG responses or voluntary modulation of their brain activity on command, each of which has been interpreted, to varying degrees, as evidence of consciousness. Practitioners designate this phenomenon using a dizzying variety of terms. The realization that 'unresponsiveness' does not equate to 'unconsciousness' changes how patie

Assessing consciousness in patients with locked-in syndrome using their EEG.

Research indicates that locked-in syndrome (LIS) patients retain both consciousness and cognitive functions, despite their inability to perform voluntary muscle movements or communicate. Brain-Computer Interfaces (BCIs) provide a means for these patients to communicate, which is crucial, as the ability to interact with their environment has been shown to significantly enhance their wellbeing and quality of life. This paper presents an innovative approach to analyzing electroencephalogram (EEG) data from four LIS patients to assess their consciousness levels, referred to as normalized conscious

Disorder of consciousness rather than complete Locked-In Syndrome for end stage Amyotrophic Lateral Sclerosis: a case series.

BACKGROUND: The end-stage of amyotrophic lateral sclerosis (ALS) is commonly regarded as a complete Locked-In Syndrome (cLIS). Shifting the perspective from cLIS (assumed consciousness) to Cognitive Motor Dissociation (potentially demonstrable consciousness), we aimed to assess the preservation of covert awareness (internally preserved but externally inaccessible) using a multimodal battery. METHODS: We evaluate two end-stage ALS patients using neurophysiological testing, passive and active auditory oddball paradigms, an auditory Brain-Computer Interface (BCI), functional activation-task imag
