Facial-onset SOD1 amyotrophic lateral sclerosis: A case report and systematic review.

BACKGROUND: Facial-onset weakness is an exceptionally rare presentation of SOD1-associated amyotrophic lateral sclerosis (ALS), and its natural history, anatomical spread, and prognostic implications remain unclear. METHODS: We report a woman carrying a heterozygous SOD1 A5T variant who presented with isolated, rapidly progressive bilateral facial palsy, and we performed a PRISMA-compliant systematic review of MEDLINE, Scopus, and Web of Science to identify genetically confirmed SOD1-positive ALS with facial-onset weakness. Case-level demographic, genetic, clinical, neurophysiological, and ou

Neuromyelitis optica spectrum disorder in an 11-year-old: a case report in a pediatric patient from Nepal.

INTRODUCTION AND IMPORTANCE: Neuromyelitis optica spectrum disorder (NMOSD) is an auto-immune inflammatory disorder of the central nervous system mediated by auto-antibodies against Aquaporin-4 (AQP4). It causes vision impairment, vomiting, and paralysis. CASE PRESENTATION: We report a case of an 11-year-old Nepalese girl who initially presented with persistent vomiting for 2 months, followed by progressive limb weakness and facial asymmetry. She was initially misdiagnosed as gastroduodenitis with mesenteric lymphadenitis and later with major depressive illness and dissociative motor disorder

Acute otitis media complicated by peripheral facial nerve palsy in an adult: A case report.

Acute otitis media (AOM) is a common infection, whereas peripheral facial nerve palsy is an uncommon extracranial complication in the post-antibiotic era. We report the case of a 53-year-old immunocompetent man who developed acute suppurative otitis media complicated by ipsilateral peripheral facial nerve palsy following an upper respiratory tract infection. The illness began with sore throat and cough during Hajj and progressed to severe right-sided otalgia, followed by complete right lower motor neuron facial weakness despite sequential empirical oral antibiotics. On return to Pakistan, otos

Botulinum Neurotoxin for Postoperative Facial Nerve Paralysis: A Technical Note.

Excision of a vestibular schwannoma or any lesion in close proximity to the seventh cranial nerve (facial nerve) can compromise the nerve due to traction or, less commonly, neuropraxia. We retrospectively evaluated the effect of botulinum neurotoxin (BoNT) on preventing exposure keratitis in patients with postoperative facial nerve paralysis. We included 18 patients with postoperative lower motor neuron (LMN) facial nerve paralysis. All these patients had received BoNT injections into the upper eyelid in the postoperative period as an alternative to tarsorrhaphy. BoNT was effective in inducing

The Diagnostic Pitfalls and Clinical Challenges of Unilateral Facial Paralysis in Acute Demyelinating Disorders: A Case Report and Literature Review.

Guillain-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy typically presenting with progressive limb weakness and areflexia, while bilateral facial nerve involvement is a well-recognized feature. However, unilateral facial palsy is exceedingly rare and can closely mimic Bell's palsy, complicating early diagnosis. We report the case of a previously healthy 32-year-old man whose illness began with subtle bilateral fingertip numbness ascending to his elbows, followed by the acute onset of right-sided facial weakness, perioral numbness, slurred speech, and inability to close

Peripheral Nervous System Complications after COVID-19 Vaccination.

While vaccination against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) remains crucial, neurological complications have been detected following the coronavirus disease 2019 (COVID-19) vaccination. The neurological complications of COVID-19 vaccination can be seen in both the central nervous system (CNS) and the peripheral nervous system (PNS). In this study, we reviewed PNS complications after COVID-19 vaccination, their underlying mechanisms, diagnosis, and management. Inflammatory polyneuropathy, small fiber neuropathy, Parsonage-Turner syndrome (PTS), cranial mononeuropathie

Facial onset sensory and motor neuronopathy (FOSMN syndrome): Cases series and systematic review.

OBJECTIVE: To provide new and comprehensive evidence for diagnosis and management of FOSMN syndrome. METHODS: We reviewed our database to identify patients with FOSMN syndrome. Online database including PubMed, EMBASE, and OVID were also searched for relevant cases. RESULTS: We identified a total of 71 cases, including 4 cases from our database and 67 ones from online searching. A predominance of male was observed [44 (62.0%)] with median onset age of 53 (range: 7-75) years old. The median (range) disease duration was 60 (3-552) months at the time of the visit. The initial symptoms could be

An interesting case of crossed syndrome: ipsilateral facial paralysis with contralateral glossoplegia.

BACKGROUND: Stroke is rarely accompanied with peripheral facial paralysis and supranuclear palsy of the hypoglossal nerve. Both sides of the motor cortex innervate the hypoglossal nucleus; therefore, unilateral lesions of the upper motor neurons rarely result in contralateral lingual paresis. We report a rare case of crossed syndrome with associated hyperacute peripheral hemifacial paralysis and contralateral lingual paresis after a lower pontine tegmentum ischemic stroke. CASE PRESENTATION: A 73-year-old man presented with symptoms of hyperacute peripheral hemifacial paralysis. Upon protrusi

Corticobasal syndrome mimicking Foix-Chavany-Marie syndrome with suggested 4-repeat tauopathy by tau PET.

BACKGROUND: Corticobasal syndrome (CBS) is a neurodegenerative disease diagnosed based on clinical manifestations such as asymmetrical parkinsonism, limb apraxia, and speech and language impairment. The background pathology of CBS is commonly a variety of proteinopathies, but association with cerebrovascular disease has also been reported. Foix-Chavany-Marie syndrome (FCMS) is a rare neurological disorder characterized by facio-pharyngo-glossal diplegia with automatic-voluntary movement dissociation presenting with bilateral paresis of the facial, lingual, pharyngeal and masticatory muscles. Bell's Palsy: A Review.

Bell's palsy, also known as "acute facial palsy of unknown cause", is a common cranial neuropathy leading to facial muscle paresis or complete paralysis characteristically on one side, occurring suddenly and may progress over 48 hours. It results from facial nerve dysfunction due to trauma or inflammation of the 7th cranial nerve or facial nerve or its branches along its course, primarily in the bony canal. Both sexes are equally affected, and though no age is immune, its incidence rises with increasing age. The risk is high in diabetics, hypertensives, women who are pregnant, obese, and peopl

A case of aphemia following non-dominant sub-insular stroke: unveiling the Foix-Chavany-Marie phenomenon.

Aphemia refers to the clinical syndrome of inability to orally produce speech with intact comprehension and written expression. Aphemia has been primarily reported in dominant frontal lobe strokes resulting in apraxia of speech (AoS), and in Foix-Chavany-Marie (FCM) syndrome where bilateral opercular or sub-opercular lesions result in anarthria due to deafferentation of brainstem nuclei supplying the oro-facio-lingual and pharyngeal musculature. Aphemia is not reported in non-dominant sub-insular strokes. Here, we present a case of aphemia following non-dominant sub-insular stroke in a patient

Guillain-Barré syndrome with associated unilateral ptosis without ophthalmoplegia - a rare presentation: a case report and review of the literature.

BACKGROUND: Guillain-Barré syndrome is an acute inflammatory polyradiculoneuropathy. Nearly half of patients with Guillain-Barré syndrome have cranial nerve involvement. However, isolated bilateral ptosis without ophthalmoplegia is a rare manifestation, and isolated unilateral ptosis without ophthalmoplegia in Guillain-Barré syndrome has not previously been reported in the literature. Furthermore, only few cases of Guillain-Barré syndrome with cranial nerve enhancement visualized by gadolinium-enhanced magnetic resonance imaging have previously been reported. We describe the first reported cas

Spinobulbar muscular atrophy combined with atypical hereditary neuropathy with liability to pressure palsy.

Spinobulbar muscular atrophy (SBMA) is an X-linked recessive disease, presenting motor weakness and wasting of facial, bulbar and limb muscles. Hereditary neuropathy with liability to pressure palsy (HNPP) is autosomal dominant disorder characterized by recurrent neuropathies at common entrapment sites. We report a case of co-existence of SBMA and atypical HNPP with genetic confirmation of CAG expansion in the androgen receptor (AR) gene and deletion of the peripheral myelin protein 22 (PMP22) gene. A 62-year-old man presented with progressive muscle weakness, fasciculations in upper and lower

Facial nerve palsy in a child: Bell's palsy? Think again!

INTRODUCTION: Half of facial paralysis in children is idiopathic at origin. However, dismissing facial paralysis as being idiopathic without a thorough history and meticulous examination could be disastrous as illustrated by this case. CASE REPORT: We report a case of sphenoid wing meningioma in a 4-year-old girl. She first presented with only facial asymmetry that was noticed by her mother. Examination suggested a left upper motor neuron facial nerve palsy. A sphenoid wing meningioma was found on magnetic resonance imaging (MRI) of her brain. She underwent craniotomy and total tumour excisio

Recurrent upper motor neuron facial weakness of possible epileptic etiology: a case report.

We report on an otherwise healthy infant who presented with clusters of alternating central facial nerve paresis. At 11 months of age, the patient showed intermittent facial asymmetry compatible with right-sided upper motor neuron facial paresis and accompanying incomplete upper motor neuron hypoglossal paresis. Laboratory work-up and imaging studies did not reveal signs of infection, infarction or structural lesions, and after one week, symptoms spontaneously resolved. Similar episodes affecting alternate sides were noted at ages of 17, 27 and 49 months lasting for 4-14 days. At 49 months, EE

Facial animation with free-muscle transfer innervated by the masseter motor nerve in unilateral facial paralysis.

PURPOSE: Facial paralysis is a congenital or acquired disorder of varying severity leading to an asymmetric or absent facial expression. It represents an important debilitation from both esthetic and functional points of view. In this article we report our experience with patients treated with gracilis muscle transplantation innervated by the motor nerve to the masseter muscle. We discuss the surgical technique and the functional and esthetic results and evaluate the effectiveness of this donor nerve in providing adequate innervation to the muscle transfer for lower facial reanimation. Neuro-ophthalmologic complications and manifestations of upper and lower motor neuron facial paresis.

The facial nerve (cranial nerve VII) courses a long pathway beginning in the precentral gyrus and ending at the facial muscles, lacrimal and salivary glands, and structures of the inner ear. Lesions along this pathway, clinically divided into upper and lower motor neuron lesions, present with unique characteristics that assist the physician in identifying the lesion site. The sequelae particularly of peripheral CN VII palsies, may result in significant and chronic damage to the cornea that may be challenging for the physician and patient.
