Acupuncture treatment of facial palsy.

BACKGROUND: Bell's palsy is an idiopathic, acute peripheral-nerve palsy involving the facial nerve which supplies all the muscles of facial expression. This study was conducted to evaluate the effects of electro-A=acupuncture on patients with facial palsy. METHODS: This study was conducted on patients with facial palsy at a private clinic at Peshawar during 1999-2009, and 49 cases were included in the study. All those cases that were within first two weeks of illness or who had related history of stroke or they had upper motor neuron lesion were not included in the study. Electroacupuncture w

SOD1 (A4V)-mediated ALS presenting with lower motor neuron facial diplegia and unilateral vocal cord paralysis.

Amyotrophic lateral sclerosis (ALS) is a progressive degenerative neuromuscular disease that presents with upper and lower motor neuron signs. Although the majority of ALS cases are sporadic, 10% are familial, of which 20%-25% result from mutations in the superoxide dismutase (SOD1) gene. We describe a novel case of SOD1 (A4V)-mediated ALS that presented with lower motor neuron facial diplegia and unilateral vocal cord paralysis. This case expands the phenotypic expression of the A4V mutation.

Facial paralysis: a critical review of accepted explanation.

Historically, paralysis of facial muscles has been divided into "upper motor neuron injury" and "lower motor neuron injury". Patients who experience a stroke in the cortex or internal capsule have UMN injury and cannot purse their lips or smile on command. They are, however, able to wrinkle their forehead, raise their eyebrows, and completely close their eyes. Patients with LMN injury, in addition to the aforementioned impairments cannot raise their eyebrows. The classical explanations for these clinical findings are that the upper facial muscles receive bilateral innervation from the cerebral

Brown-Vialetto-Van Laere syndrome: a rare syndrome in otology.

Brown-Vialetto-Van Laere syndrome, or pontobulbar palsy with deafness, is a rare disorder characterized by bilateral sensorineural deafness and a variety of cranial nerve disorders usually involving the motor components of the lower cranial nerves. Less commonly, spinal motor nerves and upper motor neurons are involved. Familial and sporadic cases have been reported. Based on evidences, this syndrome has been related to autosomal recessive, autosomal dominant and X-linked inheritance. Autoimmune origin has been considered as well. In this paper, we report the case of a 38-year-old female patie

Respiratory failure in a patient with antecedent poliomyelitis: amyotrophic lateral sclerosis or post-polio syndrome?

We report a 69-year-old man who developed paralytic poliomyelitis in childhood and then decades later suffered from fatal respiratory failure. Six months before this event, he had progressive weight loss and shortness of breath. He had severe muscular atrophy of the entire right leg as a sequela of the paralytic poliomyelitis. He showed mild weakness of the facial muscle and tongue, dysarthria, and severe muscle atrophy from the neck to proximal upper extremities and trunk, but no obvious pyramidal signs. Electromyogram revealed neurogenic changes in the right leg, and in the paraspinal, stern

[Application of a mathematical algorithm for the detection of electroneuromyographic results in the pathogenesis study of facial dyskinesia].

To carry out a differential diagnosis of two facial dyskinesia (FD) models--facial hemispasm (FH) and facial paraspasm (FP), a combined program of electroneuromyographic (ENMG) examination has been created, using statistical analyses, including that for objects identification based on hybrid neural network with the application of adaptive fuzzy logic method and standard statistics programs (Wilcoxon, Student statistics). In FH, a lesion of peripheral facial neuromotor apparatus with augmentation of functions of inter-neurons in segmental and upper segmental stem levels predominated. In FP, pri

Pure motor hemiplegia including the face induced by an infarct of the medullary pyramid.

Four autopsy cases of pure motor hemiparesis due to medullary pyramid infarcts have been previously reported. The deficits that were described included overt limb weakness and "slight facial weakness". According to current neurological teaching, the lesion responsible for an upper motor neuron facial palsy affects the corticobulbar tract at the level of the midpons or more rostrally.

[Chronic progressive spinobulbar spasticity (primary lateral sclerosis)].

This paper presents an account of chronic-progressive Spinobulbar Spasticity (SBS) or Primary Lateral Sclerosis (PLS), a rare syndrome involving degeneration of the upper motoneuron, on the basis of 6 clinically examined cases. Individuals of both sexes can be affected. Onset of the syndrome occurs around the age of 54, but may sometimes be before 50. Early symptoms of the disease are spasticity on one leg and disturbance of motor skills in one hand. The symptoms generalize within two to three years into tetraspasticity accentuated in the legs, accompanied by pseudo-bulbar dysarthria and dysph

Electronic integration of glottic closure and circopharyngeal relaxation for the control of aspiration: a canine study.

Aspiration can result from muscular weakness or paralysis of laryngopharyngeal muscles after lower motor neuron disorders (e.g., stroke) or unchecked gastroesophageal reflux. We submit that rehabilitation of the finely tuned swallowing mechanism should provide at least restoration of the normal dynamic relationships between glottic closure and cricopharyngeal relaxation. In three dogs under general endotracheal anesthesia, the recurrent laryngeal nerves and the pharyngeal musculature were exposed through a midline cervical incision. A tracheotomy was performed to allow unhindered laryngoscopic

Peripheral facial nerve paralysis after local upper dental anaesthesia.

A 50-year-old female patient developed a facial nerve paralysis after a local upper dental block. This complication is known to occur after an inferior dental block but has only sporadically been reported after local anaesthetic procedures in the upper jaw for dental treatment. Some possible pathogenetic mechanisms are discussed.

[Two autopsy cases of sporadic amyotrophic lateral sclerosis with 20-year-clinical course without respirators].

The present paper concerns the clinicopathological study of two patients with sporadic amyotrophic lateral sclerosis (ALS) with 21 or 22 years' clinical course without respirators. The two cases developed marked upper motor neuron signs and continually progressive paralysis. Involvement of the facial and bulbar muscles remained fairly mild. At autopsy, the anterior horns showed marked loss of motor neurons associated with numerous reactive astrocytes. On the other hand, the motor cortex and the pyramidal tract showed diminution of Betz cells and large myelinated fibers but no positive material

Extended neuralgic amyotrophy syndrome.

Neuralgic amyotrophy refers to an idiopathic syndrome where weakness and wasting occur in one limb, usually in the muscles innervated by the upper brachial plexus. Seven patients are presented who developed cranial nerve involvement (facial, spinal accessory) in the midst of a typical attack of neuralgic amyotrophy or who developed either recurrent brachial or brachial and lumbosacral plexopathies. An underlying demyelinating neuropathy was identified in one patient and two patients were herion addicts. These reports confirm that neuralgic amyotrophy may occasionally form part of a more extens

Organization of the facial nucleus and corticofacial projection in the monkey: a reconsideration of the upper motor neuron facial palsy.

The somatotopic organization of the facial nucleus and the distribution of the corticofacial projection in the monkey were studied by the use of retrograde and anterograde transport of horseradish peroxidase. Facial motor neurons innervating lower facial muscles were primarily found in the lateral part of the nucleus, those supplying upper facial muscles in the dorsal part of the nucleus, and those innervating the platysma and posterior auricular muscles in the medial part of the nucleus. Descending corticofacial fibers innervated the lower facial motor nuclear region bilaterally, although wit

Isolated facial palsy: a new lacunar syndrome.

Three cases of sudden isolated upper motor neuron facial palsy and two with associated pseudobulbar palsy have been seen. All were without significant limb weakness. Computed tomography demonstrated small deep infarcts in the internal capsular/corona radiata regions. Pure upper motor neuron facial palsy may be another lacunar syndrome, due to a lesion in the internal capsule or corona radiata.

Nonlanguage disorders of speech reflect complex neurologic apparatus.

The mute or nearly mute patient who is alert and has good understanding of speech and a right hemiparesis could have Broca's aphasia, akinesia of speech (transcortical motor aphasia), or aphemia. The patient who has Broca's aphasia does not write well, and his speech does not improve greatly with repetition. The speech of a patient with akinesia of speech improves with repetition. The aphemic patient writes normally, but his speech does not improve with repetition. The mute patient whose eyes are open but who is poorly responsive and moves little or not at all could be an akinetic mute (with e
